  • Objectives : This study examined the association between depression and survival time in terminal cancer patients admitted to the palliative care unit. Emotional problems are important for terminal cancer patients in the palliative care unit, and evaluation of patients' depression plays an important role in treatment planning. Methods : From October 2015 to August 2018, we conducted a retrospective study of 291 terminal cancer patients admitted to a palliative care unit at a university hospital and evaluated depression with PHQ-9 at admission. Of the 291 patients, 146 (50.2%) completed PHQ-9 but 145 (49.8%) were not evaluated due to loss of consciousness or rejection. Results : 4-week survival rate in the Kaplan-Meier survival analysis were 45.4% in the non-depressed group (PHQ-9<10) and 18.7% in the depressed group (PHQ-9≥10). According to the severity of depression, in the Cox proportional hazard model, the risk of mortality in moderate, moderately sever and severe group was 2.778, 1.882 and 3.423 times higher than minimal group, respectively. Conclusions : Of the patients with terminal cancer who were admitted to the palliative care unit, the survival time was shorter in the depressed group than in the non-depressed group. Further research is needed to determine if treatment of depression increase the survival in terminal cancer patients.

  • A study on field bioassay test using four species of commercially important shellfish was carried out to assess the effect of pollutants and determine the extent of marine environmental pollution of the Masan Bay from 9 to 15 August 1978. Water quality analysis and planktological examination of sea water were made during the experiment and the examination gave the following results. The water temperature was so high at 31.7$^{\circ}C$ in maximum and rather subject to change on weather condition of the land than on the effect of the water mass from outer bay. The range of DO,COD and SS at the stations were 0.3-7.08cc/l, 0.07-3.31ppm and 5.5-117ppm, respectively with the high values of COD and SS at the stations 7 and 1. The concentrations of the dissolved inorganic nitrogen in sea water, NH$\sub$4/-N, NO$\sub$2/-N, NO$\sub$3/-N, and PO$\sub$4/-P were 18.90-99.80, 2.48- 19.60, 13.00-39.00 and 1.04-14.0$\mu\textrm{g}$ at/l, respectively with decrease of their values in the outer part of the Bay. The high values mentioned above were caused by the sewage and industrial activities. The effects of organic waste are increased oxygen demand, nutrient concentration, turbidity and a higher input of pathogens, leading to structural changes in the marine ecosystems and to a considerable hazard to public health. The percentage composition of phytoplankton standing crop between diatom and dinoflagellate was characterized by making a difference between the two groups in respect of location: a decrease of diatom and a increase of dinoflagellate in numerical abundance toward inner part from outer part of the bay. Namely phytoplankton organisms were composed of 80% of diatom and 20% of dinoflagellate in outer bay, on the contrary, only 4% of diatom and 96% of dinoflagellate occupied by 94% of prorocentrum micans known as tolerant species to polluted reaas in the inner bay. On the occurrence and composition of zooplankton, there are two significant communities in the bay:one is characterized by the predominance of Oithona nana and the other by Favella sp They were composed of a range from 84% to 90% of the total organisms and monotonously constituted of themselves only at most inner station 3 even small numbers. From the results mentioned above, Oithona nana, Favella and prorocentrum micans recommed themselves as valuable indicators for judging the extent of the marine pollution.During the period of the biossays Mytilus edulis showed the highest mortality and Tapes japonica the lowest one between the four test species. The highest death rate by stations was found at most inner stations 3'and 4near Masan Free Export Zone with the most sensitive response and the lowest one occured at outer station 13 where no death specimen of oyster and arkshell was found during the whole test period. As for mussel,85 percent death rate appeared after 72 hours and 100percent rate after 120hours at station 4. It was found that the significant high mortality of the test shellfish mentioned above was caused by severe pollution with mainly organic pollutants from domestic sewage and industrial wastes from the results of too much higher concentrations of dissolved inorganic nitrogen especially ammonia-N,COD,SS and lack of dissolved oxygen, and furthermore occurrence and abundance composition of Prorcentrum,Favella and Oithona nana by stations, valuable indicator species of coastal pollution by orgnic and boilogical pollutants.

  • This study evalutated the risk of foodborne illness from Vibrio spp. (Vibrio vulnificus and Vibrio cholerae) through sea squirt consumption. The prevalence of V. vulnificus and V. cholerae in sea squirt was evaluated, and the predictive models to describe the kinetic behavior of the Vibrio in sea squirt were developed. Distribution temperatures and times were collected, and they were fitted to probabilistic distributions to determine the appropriate distributions. The raw data from the Korea National Health and Nutrition Examination Survey 2016 were used to estimate the consumption rates and amount of sea squirt. In the hazard characterization, the Beta-Poisson model for V. vulnificus and V. cholerae infection was used. With the collected data, a simulation model was prepared and it was run with @RISK to estimate probabilities of foodborne illness by pathogenic Vibrio spp. through sea squirt consumption. Among 101 sea squirt samples, there were no V. vulnificus positive samples, but V. cholerae was detected in one sample. The developed predictive models described the fates of Vibrio spp. in sea squirt during distribution and storage, appropriately shown as 0.815-0.907 of R2 and 0.28 of RMSE. The consumption rate of sea squirt was 0.26%, and the daily consumption amount was 68.84 g per person. The Beta-Poisson model [P=1-(1+Dose/β)] was selected as a dose-response model. With these data, a simulation model was developed, and the risks of V. vulnificus and V. cholerae foodborne illness from sea squirt consumption were 2.66×10-15, and 1.02×10-12, respectively. These results suggest that the risk of pathogenic Vibrio spp. in sea squirt could be considered low in Korea.

  • The potato tuber is known as a rich source of essential nutrients, used throughout the world. Although potato-breeding programs share some priorities, the major objective is to increase the genetic potential for yield through breeding or to eliminate hazards that reduce yield. Glycoalkaloids, which are considered a serious hazard to human health, accumulate naturally in potatoes during growth, harvesting, transportation, and storage. Here, we used the AMMI (additive main effects and multiplicative interaction) and GGE (Genotype main effect and genotype by environment interaction) biplot model, to evaluate tuber yield stability and glycoalkaloid content in six potato cultivars across three locations during 2012/2013. The environment on tuber yield had the greatest effect and accounted for 33.0% of the total sum squares; genotypes accounted for 3.8% and $G{\times}E$ interaction accounted for 11.1% which is the nest highest contribution. Conversely, the genotype on glycoalkaloid had the greatest effect and accounted for 82.4% of the total sum squares), whereas environment and $G{\times}E$ effects on this trait accounted for only 0.4% and 3.7%, respectively. Furthermore, potato genotype 'Superior', which covers most of the cultivated area, exhibited high yield performance with stability. 'Goun', which showed lower glycoalkaloid content, was the most suitable and desirable genotype. Results showed that, while tuber yield was more affected by the environment, glycoalkaloid content was more dependent on genotype. Further, the use of the AMMI and GGE biplot model generated more interactive visuals, facilitated the identification of superior genotypes, and suggested decisions on a variety of recommendations for specific environments.

  • Purpose: This study examined the associations of Korean fermented cabbage kimchi consumption with the incidence risk of metabolic syndrome and its components in Korean adults. Methods: We used the community-based cohort data from the 2001 ~ 2012 Korean Genome and Epidemiology Study (KoGES). General characteristics, food group frequencies and nutrient intakes at baseline from 3,560 healthy individuals aged 40 ~ 69 years and the incidence of metabolic syndrome and its component from 2,259 participants, after excluding the data with incomplete anthropometric and blood test, during 10-year follow-up were analyzed. The participants were classified into three groups according to their daily consumption frequency of Korean fermented cabbage kimchi: 'less than once (< 1/day)', 'once or twice (1 ~ 2/day)', and 'three times (3/day)'. Results: After controlling for potential confounders such as age, education, income, residence area, alcohol drinking and energy intake, the consumption frequencies of rice and legumes were significantly higher, and the consumption frequency of meat was significantly lower in the 3/day group compared to that of the other two groups in men and women. The average intakes of energy and most nutrients, except fat and cholesterol, were higher in the 3/day group compared to those of the other two groups in men and women. Frequent consumption of kimchi was associated with a lower incidence of metabolic syndrome in all the models (unadjusted, age-adjusted, and multivariable-adjusted models) in women. When examining the multivariable-adjusted model, the hazard ratio for metabolic syndrome was 0.63 (95% CI: 0.47 ~ 0.86) for the 3/day group compared to that of the < 1/day group in women. However, there was no significant association between kimchi consumption and the incidence of metabolic syndrome in men. Conclusion: Our results show that consumption of kimchi at every meal was significantly associated with a lower incidence of metabolic syndrome in women.

  • The objective of this study was to determine microbiological risk factors in hot pepper farms for the application of good agricultural practices (GAP). Samples were collected from cultivation environments and utensils, plants, workers, and air at 3 hot pepper farms located in Cheongsong, Korea and were tested to detect sanitary indications [aerobic plate bacteria (APC), coliform, and Escherichia coli], foodborne pathogens, and fungi. APC, coliform, and fungi were detected at the levels of 0.7~6.2, 0.2~4.7, and 0.4~4.3 log CFU, respectively, in the three farms. Four (4.4%; l leaf, l irrigation water, and 2 soil) of 90 samples collected were revealed to be E. coli positives. For foodborne pathogens, Staphylococcus aureus was only detected at $1.0log\;CFU/100cm^2$ in the worker's cloth of B farm, and Bacillus cereus was detected at the levels 1.0~2.5 log CFU in the cultivation environments and utensils and worker of B and C farms. However, other pathogens were not detected. The results demonstrated potential microbiological risks for hot pepper cultivated in the farms. Therefore, a management system to minimize the microbial risk such as GAP is required to ensure the safety of hot pepper.

  • Background/Aims: The impact of left ventricular (LV) diastolic function and filling pressure on clinical outcomes in young patients with acute myocardial infarction (AMI) has been poorly studied. Therefore, the aim of this study was to investigate the impact of LV diastolic function and LV filling pressure on major adverse cardiac events (MACEs) in young patients with AMI. Methods: A total of 200 young patients (males < 45 year, females < 55 year) with AMI were divided into two groups according to the diastolic function; normal (n = 46, $39.5{\pm}5.3$ years) versus abnormal (n = 154, $43.5{\pm}5.1$ years). Results: Despite regional wall motion abnormalities, normal LV diastolic function was not uncommon in young AMI patients (23.0%). During the 40 months of clinical follow-up, MACEs developed in 26 patients (13.0%); 14 re-percutaneous coronary intervention (7.0%), 8 recurrent MI (4.0%), and 4 deaths (2.0%). MACEs did not differ between the normal and abnormal diastolic function group (13.6% vs. 10.9%, p = 0.810), but MACEs were significantly higher in the high LV filling pressure group than the normal LV filling pressure group (36.8% vs. 10.5%, p < 0.001). On multivariate analysis, high LV filling pressure was an independent predictor of MACEs (hazard ratio 3.022, 95% confidence interval 1.200-7.612, p = 0.019). Conclusions: This study suggested that measurement of the LV filling pressure (E/e' ratio) would be useful in the risk stratification of young patients with AMI. However, it would be necessary to monitor this category of patient more carefully.

  • Purpose: The objective of this study is to suggest desirable direction of Korean accident investigation organization by analyzing the operation status and way of overseas developed countries' investigation agency. Method: To accomplish the objective of this study, we were examined four main characteristics of accident investigation agencies of the U.S., Japan, and Sweden, focusing on (1); the background of the establishment, (2);organizational structure, (3);major tasks and functions, (4); accident investigation procedures. Result: First, the purpose of its establishment and task is to prevent recurrence of disasters and accidents, at the same time, administrating and researching duties such as legal system, policy, recommending improvement and conducting scientific disaster-cause analysis to contribute safety for the government. Second, it is operated as an independent organization under the president, not belonging to the ministry, in order to enable fair investigation in an impartial position. Third, it has the authority to be recognized for its expertise in the results of investigation. In other words, it is operated as a permanent organization with professional personnel, and secures authority through the accident research with indepth investigation and high-quality recommendations. Conclusion: The overseas investigation agencies rapidly manage and coordinate their operational practices in order to resolve national requirements and social conflicts with fairness, accuracy and expertise in accident investigations. In order to prevent the recurrence of similar events, Korea needs to efficiently reconstruct its investigative functions distributed by each government department. In addition, institutional improvement is needed to make general adjustments at the national level, organize and operate control tower for when the accident has happened.

  • Purpose: Tegafur/gimeracil/oteracil (S-1) and capecitabine plus oxaliplatin (CAPOX) are standard adjuvant chemotherapies (ACs) administered after gastrectomy to patients with stage II or III gastric cancer. However, the efficacy of AC in elderly patients remains unclear. The objective of this retrospective multicenter cohort study was to compare the efficacies of S-1 and CAPOX AC in patients aged ≥70 years. Materials and Methods: Nine hundred eighty-three patients who were treated with AC using S-1 (768 patients) or CAPOX (215 patients) were enrolled in this study. Each patient underwent AC after curative gastrectomy for stage II or III gastric cancer at one of 27 hospitals in the Republic of Korea between January 2012 and December 2013. Relapse-free survival (RFS) and overall survival (OS) were analyzed according to AC regimen and age group. Results: Of the 983 patients, 254 (25.8%) were elderly. This group had a similar RFS (P=0.099) but significantly poorer OS (p=0.003) compared with the non-elderly group. Subgroup analysis of the non-elderly group revealed no AC-associated differences in survival. Subgroup analysis of the elderly group revealed significantly better survival in the S-1 group than in the CAPOX group (RFS, P<0.001; OS, P<0.001). Multivariate analysis revealed that the CAPOX regimen was an independent poor prognostic factor for RFS (hazard ratio [HR], 1.891; 95% confidence interval [CI], 1.072-3.333; P=0.028) and OS (HR, 2.970; 95% CI, 1.550-5.692; P=0.001). Conclusions: This multicenter observational cohort study found significant differences in RFS and OS between S-1 and CAPOX AC among patients with gastric cancer aged ≥70 years.

  • Over the last decade, avian influenza (AI) has been considered an emerging disease that would become the next pandemic, particularly in countries like South Korea, with continuous animal outbreaks. In this situation, risk assessment is highly needed to prevent and prepare for human infection with AI. Thus, we developed the risk assessment matrix for a high-risk area of human infection with AI in South Korea based on the notion that risk is the multiplication of hazards with vulnerability. This matrix consisted of highly pathogenic avian influenza (HPAI) in poultry farms and the number of poultry-associated production facilities assumed as hazards of avian influenza and vulnerability, respectively. The average number of HPAI in poultry farms at the 229-municipal level as the hazard axis of the matrix was predicted using a negative binomial regression with nationwide outbreaks data from 2003 to 2018. The two components of the matrix were classified into five groups using the K-means clustering algorithm and multiplied, consequently producing the area-specific risk level of human infection. As a result, Naju-si, Jeongeup-si, and Namwon-si were categorized as high-risk areas for human infection with AI. These findings would contribute to designing the policies for human infection to minimize socio-economic damages.
