Effects of Project Perception of Research Nurses from Research-driven Hospitals, Research-relevant Performance: Focusing on the Mediating Effects of Research Capacity and Job Satisfaction (연구간호사의 연구중심병원사업 인지도가 연구성과에 미치는 영향: 연구역량 및 직무만족의 매개효과를 중심으로)

  • Purpose: The purpose of this study was to identify the level of project perception for those nurses from research-driven hospitals and to analyze the effect of research-relevant performance in the health care field focusing on the mediated effect of research capacity and job satisfaction. Methods: Data were collected from June, 2014 to July, 2014, and participants were 106 research nurses in Research-driven hospitals. Descriptive statistics, Independent t-test, One-way ANOVA, structural equation modeling (SEM). Results: As a result, Research-relevant performance according to project perception of research nurses from Research-driven Hospitals was not statistically significant, but research capacity and job satisfaction had a mediating role. Evaluation System Perception was significantly different from Research Capacity (p<.001), Research Capacity was significantly different from Job Satisfaction (p<.001), Job Satisfaction was significantly different from Research Performance (p<.001) Conclusion: The results indicate that research capacity building and job security research nurses are able to contribute to improving research performance of research-driven hospitals.

  • Hepatocellular carcinoma (HCC) is the most frequent type of malignant liver tumor and a high impact health problem worldwide. The prevalence of HCC is particularly high in many Asian and African countries. Some HCC patients have no symptoms prior to diagnosis and many of them therefore present at late stage and have a grave prognosis. The well-established causes of HCC are chronic hepatitis B virus (HBV) or chronic hepatitis C virus (HCV) infection or alcoholic cirrhosis and nonalcoholic steatohepatitis. The Barcelona Clinic Liver Cancer (BCLC) Staging System remains the most widely used for HCC management guidelines. To date, the treatments for HCC are still very challenging for physicians due to limited resources in many parts of the world, but many options of management have been proposed, including hepatic resection, liver transplantation, ablative therapy, chemoembolization, sorafnib and best supportive care. This review article describes the current evidence-based management of HCC with focus on early to advance stages that impact on patient overall survival.

  • 최근 국내 유제품 수요는 인구의 고령화, 학교급식 소비의 감소, 대체음료 시장의 성장 그리고 출산율 저하라는 여러 가지 영향에 기인하여 줄고 있다. 반대로 수입 유제품의 수요, 특히 치즈에 있어서는 눈에 띄는 성장을 보여 국내 낙농업 시장은 안타까운 실정이다. 뿐만 아니라 공급에 있어서는 계절적인 원유 수급불안 현상, 수입증가, 농가이탈에 의한 생산기반의 위축으로 인해 안정적으로 국내 낙농업 시장의 활성화가 필요한 시점이다. 이 논문에서는 낙농가에서 가장 중요한 젖소의 건강 관리(발정기, 맥박감지)를 통합해 유량을 늘리는 방안을 제시하여 낙농업계의 활성화를 목표로 한다.

An Investigation of Current Status of the Clinical Practice Guidelines in Korea (국내 임상진료지침의 개발 현황)

  • Objective: Clinical practice guidelines (CPGs) are systematically developed statements aimed at helping optimal care of the patient in a given clinical circumstance. Because of the increasing evidence that active implementation of CPGs improve health outcomes, there is a growing awareness of the importance of guideline development and dissemination. The objective of this study was to investigate the status of CPG development and availability of the CPGs in Korea. Method: We searched in the Web sites of 180 organizations to identify CPGs which were developed and/or published in Korea until 1 July 2014. The data of titles, published year, publisher, distributer, and accessibility at the internet web of all CPGs were collected and analyzed. Results: A total of 172 CPGs were developed and 80% had been released since 2009. Most (51.2%) were developed for management of 4 diseases: 28 for digestive system disease; 27 for infectious disease; 18 for endocrine and metabolic diseases; and 15 for neoplasms. Of the 172 CPGs, 150 CPGs were publicly available. Among the 150 CPGs, 78.7% (118/150) were developed by only one organization. Conclusion: To ensure the production of high-quality CPGs, it is necessary to collaborate with other relevant professional societies in guideline development process. In addition, stronger efforts on wider dissemination of CPGs must be employed at the country levels to promote implementation of CPGs in clinical settings.

  • Healthcare industry is one of the rapidly increasing sphere from high technology IT products and wireless communication. In particular, increase in aging society korea and whole world peoples want to treat whenever and wherever rather than go to the hospital. Accordingly, and also brand new companies which has creative ideas are being founded nowadays. As the development of healthcare industry, many different kind of health care devices is being developed use internet of things. Therefore we are conducted a surveying aimed at healthcare customer and analyse the healthcare use trend and development direction in this dissertation.

A Genotoxicological Safety Evaluation of Crude Antifungal Compounds Produced by Lactobacillus Plantarum AF1 and Lactobacillus Plantarum HD1 (Lactobacillus plantarum AF1와 Lactobacillus plantarum HD1이 생성한 조항균 물질의 유전학적 독성평가)

  • This study investigates the genotoxicity of crude antifungal compounds produced by Lactobacillus plantarum AF1 (L.plantarum AF1) and Lactobacillus plantarum HD1 (L. plantarum HD1) isolated from kimchi. The genetic toxicity of crude antifungal compounds was evaluated in bacterial reverse mutation in Salmonella and Escherichia spp., chromosome aberrations in Chinese hamster lung cells, and micronucleous formations in mice. In bacterial reversion assays with Salmonella Typhimurium TA98, TA100, TA1535, TA1537, and WP2uvrA, crude antifungal compounds did not increase the number of revertant colonies in both the absence and presence of the 59 metabolic activation system. In the chromosome aberration test with Chinese hamster lung cells, crude antifungal compounds showed no increase in the frequency of chromosome aberrations in the short-period test with/without the S9 mix or in the continuos test. In the in vivo mouse micronucleus assay, crude antifungal compounds showed no increase in the frequency of polychromatic erythrocytes with micronuclei. The results show that crude antifungal compounds produced by L. plantarum AF1 and L. plantarum HD1 did not induce any genotoxicity.

  • Foreign body ingestions pose a significant health risk in children. Neodymium magnets are high-powered, rare-earth magnets that is a serious issue in the pediatric population due to their strong magnetic force and high rate of complications. When multiple magnets are ingested, there is potential for morbidity and mortality, including gastrointestinal fistula formation, obstruction, bleeding, perforation, and death. Many cases require surgical intervention for removal of the magnets and management of subsequent complications. However, we report a case of multiple magnet ingestion in a 19-month-old child complicated by gastroduodenal fistula that was successfully treated by endoscopic removal and supportive care avoiding the need for surgical intervention. At two-week follow-up, the child was asymptomatic and upper gastrointestinal series obtained six months later demonstrated resolution of the fistula.

  • Clinical research is a necessity, not an option, for developing better and new medicines and therapeutic modalities. But in the course of clinical research, there are rules and guidelines that should be followed to ensure the due respect for persons, beneficence, and justice for persons who voluntarily participate in the research as described in the Belmont Report. Good Clinical Practice (GCP) is an "international scientific and ethical quality standard for designing, conducting, recording, and reporting" clinical trials. The main purposes of GCP would be to protect rights, safety, and well-being of trial subjects, in compliance with the principles of Declaration of Helsinki, and to assure that the data obtained from clinical trials are credible. In order to achieve these, investigators must be fully aware of the meanings as well as actual procedures involved in the research and should make the best effort to comply with GCP. For those individuals who belong to vulnerable populations, such as neonates, in addition to the general principles of GCP, further measures to ensure added protection should be implemented. It is our duty to develop and provide better care through clinical research even for neonates. But in doing so, we have to make sure that the importance of protecting the rights, safety, and well-being of the subjects supersede the interests of science and society.

Immune Checkpoint Inhibitors: Therapeutic Tools for Breast Cancer

  • Breast cancer is one of the major threats to female health, and its incidence is rapidly increasing in many countries. Currently, breast cancer is treated with surgery, followed by chemotherapy or radiation therapy, or both. However, a substantial proportion of breast cancer patients might have a risk for local relapse that leads to recurrence of their disease and/or metastatic breast cancer. Therefore searching for new and potential strategies for breast cancer treatment remains necessary. Immunotherapy is an attractive and promising approach that can exploit the ability of the immune system to identify and destroy tumors and thus prevent recurrence and metastatic lesions. The most promising and attractive approach of immunotherapeutic research in cancer is the blockade of immune checkpoints. In this review, we discuss the potential of certain inhibitors of immune checkpoints, such as antibodies targeting cytotoxic T-lymphocyte antigen 4 (CTLA-4), programmed death 1 (PD-1) and lymphocyte activation gene-3 (LAG-3), in breast cancer therapeutics. Immune checkpoint inhibitors may represent future standards of care for breast cancer as monotherapy or combined with standard therapies.

User identification based Health care managing system (사용자인식 기반의 헬스케어 관리시스템)

  • 본 논문에서는 최근 자신의 몸과 건강에 대한 관심이 증가함에 따라 WSN과 ZIgbee를 이용하여 언제 어디서나 운동하는 사람(이하 사용자)의 건강상태를 체크할 수 있는 헬스케어시스템을 구현하였다. 구현된 시스템은 사용자를 인식하는 RFID리더기와 운동량을 측정하기 위한 센서그룹과 사용자의 혈압, 심전도, 체온, 맥박 등을 수집하는 바이오센서, 센싱 된 정보를 수집하여 전달하는 게이트웨이, 사용자정보, 수집정보와, 측정정보를 저장하는 헬스케어관리서버 그리고 측정결과를 사용자가 모니터할 수 있는 모니터프로그램으로 구성된다. 구현된 센서노드는 Zigbee 프로토콜을 이용하여 센서네트워크를 구성한다. 그리고 리눅스 기반의 임베디드 시스템을 윈도우 기반에서 사용하기 위해 Cygwin을 이용한다. 수집된 정보는 USN과 WSN 로 실시간으로 전송된다. 이 전송된 결과 값을 헬스케어관리서버에서 저장, 관리하며 긴급 상황 발생 시 모니터를 통해 경보음을 내보내는 동시에, 사용자의 상태를 병원이나 소방서와 같은 응급처리 센터에 전송하도록 설계되었다. 구현결과 Zigbee 프로토콜과 WSN을 이용하여 사용자인식기반의 헬스케어시스템의 구현됨을 확인할 수 있었다.
