Stopping Power Ratio Estimation Method Based on Dual-energy Computed Tomography Denoising Images for Proton Radiotherapy Planning (양성자치료계획을 위한 이중에너지 전산화단층촬영 잡음 제거 영상 기반 저지능비 추정 방법)

  • Computed tomography (CT) images are used as the basis for proton Bragg peak position estimation and treatment plan simulation. During the Hounsfield Unit (HU) based proton stopping power ratio (SPR) estimation, small differences in the patient's density and elemental composition lead to uncertainty in the Bragg peak positions along the path of the proton beam. In this study, we investigated the potential of dual-energy computed tomography image-based proton SPRs prediction accuracy to reduce the uncertainty of Bragg peak position prediction. Single- and dual-energy images of an electron density phantom (CIRS Model 062M electron density phantom, CIRS Inc., Norfolk, VA, USA) were acquired using a computed tomography system (Somatom Definition AS, Siemens Health Care, Forchheim, Germany) to estimate the SPRs of the proton beam. To validate the method, it was compared to the SPRs estimated from standard data provided by the National Institute of Standards and Technology (NIST). The results show that the dual-energy image-based method has the potential to improve accuracy in predicting the SPRs of proton beams, and it is expected that further improvements in predicting the position of the proton's Bragg peak will be possible if a wider variety of substitutes with different densities and elemental compositions of the human body are used to predict the SPRs.

  • Background: Patients with autism spectrum disorder (ASD) present challenges in dental treatment cooperation owing to deficits in communication skills and social interaction. Behavioral guidance, sedation, and general anesthesia may be employed to ensure the quality of dental care for individuals with ASD. This study aimed to examine the trends in dental treatment for patients with ASD who visited the Department of Pediatric Dentistry at Dankook University Jukjeon Dental Hospital, an oral health center for the disabled in the Gyeonggi region, over the past 10 years. Methods: This study utilized the order communication system to gather data on sex, age, cooperation level, number of quadrants treated, and administration of sedation or general anesthesia for patients with ASD who visited the Department of Pediatric Dentistry at Dankook University Jukjeon Dental Hospital between January 2013 and December 2022. Results: The total number of patients with ASD increased annually, possibly due to an increase in ASD prevalence and the hospital's designation as a center for disabled oral health. General anesthesia was predominant before 2017, with a shift towards N2O-O2 sedation. The most common age group for sedation or general anesthesia was 6-9 years, with a higher prevalence in males than in females. Notably, N2O-O2 and midazolam sedation resulted in better cooperation and fewer treated teeth than general anesthesia. Conclusion: This study highlights the evolving trends in dental treatment for individuals with ASD, indicating a shift towards outpatient methods, particularly N2O-O2 sedation. The sex distribution aligns with national statistics, emphasizing a higher prevalence of ASD in males than in females. These findings underscore the need for further research to establish evidence-based guidelines for optimal dental care strategies tailored to the unique needs of individuals with ASD.

  • Based on the questionnaires sent to 270 nurses of public health centers in kyungnam during the period of March 19 to April 11 in 1992, this study was written for the of finding out the grade of satisfaction, obstacles in carrying out duties concerned with nursing services and the change of nurses role needed according to the change of the local public health administration. The first-ranking tasks carried by nurses of public health center are believed to have been family planning activities before the 1970's, nursing services during the 1970's, mother-child health activities during the 1980's, and nursing services during the period of 1990 to 1992. As far as the priority order of all the family planning activities is concerned, the counseling of the insertion of intrauterine contraceptive device, the use of oral pill or the distribution of condom was placed emphasis on before 1970, and publicity activities of family planning after that time. The first priority order of mother-child health activities has been put on the registration of pregnant women since 1970, with prenatal examination and vaccination ranking next to it. The priority order for activities against tuberculosis was laid on finding out and registration of new T.B. patients every year, with patients' control, and medication or injection ranking next to it. As for the priority order of nursing services, traveling medical examination and treatment ranked the first-stressed activity before 1970, with medication and injection ranking next to it. The first priority order management activity of communicable diseases was put on vaccination before 1970, with medication and injection. ranking next. And consultation and education ranked second to it during 1990 to 1992. As for the health services of the aged, traveling examination and treatment ranked the order, with the assistance of medical examination ranking next to it. As far as troubles and obstacles shown in case of family planning, the rate of residents' lack understanding was 28.8%, that of lacking budget 13.6%, and the imperfection of public health administration system 11.9%. In the case of tuberculosis control, residents' lacking understanding was 32.5%, the deficiency of public health administration system 18.2%, over-duty(shortage of hands) 15.6%, and the insufficiency skill and know-how 13.0%. In the case of nursing services, the deficiency of public health administration system was 18.2%, each over-duty(the shortage of hands) and the shortage of facilities and equipment 15.6% respective, and residents' lacking understanding 13.0%. The rate of dissatisfaction with the chance or possibility of promotion for his or her career or capability was shown to be 49.2%, and 65.9% of the health nurses expressed their complaints of the deficiency of the chance of the promotion to a professional or expert. when the public health nurses were asked in the questionably whether they were satisfied or not with current state of equipment and facilities needed for public health service, 49.6% of them answered in the negative. The grade of the satisfaction with the current individual position was shown to be low as much as the status of his or her position was now. 37.6% of those asked in the research answered to have the readiness to switch jobs for the reasons of dissatisfaction and so on with lacking promotion chance as well as bad working condition. Significant correlation between the grade of job satisfaction and the current status of the po as found to be in this research, which showed that the lower the status of position was, the lower the grade of job satisfaction was. But little correlation between the grade of job satisfaction and his or her schooling and career was found. In order to carry out primary health care successfully, it can be said that more education and publicity activities to make public health nurses and residents see it in a new light are requested. In addition to it, it is suggested that the improvement of promotion system for public health nurses and the enlargement of job province should also be taken in consideration of the high dissatisfaction with and complaints of the chance of promotion and the system of position. In order words, it is important that considerations for system improvement enough to make nursing services pleasant and satisfactory should be taken into.

The Differences in Quality Perceptions, Expectations, Evaluation, and Satisfaction for Nursing Service between Patients and Nurses: Small-medium Sized General Hospitals (중소 종합병원 입원환자와 간호사의 간호서비스에 대한 기대와 지각, 질 평가와 만족도 차이)

  • Purpose: This study was performed to give direction to quality improvement strategies of nursing services by comparing the differences in quality perceptions and satisfaction for nursing services between patients and nurses in small-medium sized general hospitals with 200 beds. Method: The subjects, who were 150 inpatients and 162 nurses of 4 general hospitals in a community, answered a self-report questionnaire with a SERVQUAL scale. Result: There were differences between patients' and nurses' expectations and perceptions of nursing service and satisfaction. In the service expectation, the highest factor was 'the responsiveness', and in the perceived performance, the highest was the 'assurance'. In addition, overall patients' perceptions on nursing services showed higher than nurses'. There were positive correlations among the expectations and perceptions on nursing service, and satisfaction. The correlation between perception and satisfaction was higher than the correlation between expectations and satisfaction. Conclusion: To improve the nursing service quality at small-medium hospitals, strengthening the 'assurance' factor and improving the nursing service support system is needed. Also, this study on nurses' perceived nursing service at small-medium sized hospitals should be duplicated.

Use of Complementary and Alternative Medicine and its Affecting Factors in Women with Breast Cancer (유방암 여성의 대체요법 이용과 영향요인)

  • Purpose: This exploratory study was to identify the use of complementary and alternative medicine(CAM) in women with breast cancer and to analyze which factor influence CAM use. Methods: 114 patients with breast cancer were identified and approached. The instrument used to gather data were the common types, main reasons and effects as well as stress, mood states and fatigue. Results: 42.7% of all participants reported the use of CAM. The most common types of CAM used were Phellinus linteus, elm tree, and vitamin C. The main reasons for CAM use were to boost the immune system and to prevent recurrence. Prayer, massage, Shitake mushroom, and Ganoderma lucidum were, identified as being more effective than others. 93.6% of the respondents informed their physicians of their use of CAM. Logistic regression analysis determined that stress, mood states, and duration after diagnosis were factors significantly associated with CAM use. Conclusion: CAM use is increasing among patients with breast cancer. Health care professionals are in the main position to identify what treatments patients are using and implement CAM therapies that can be helpful to relieve patient symptoms related to treatment and psychological distress.

Characteristics of Cardiovascular Adverse Drug Reactions Reported to KFDA (식품의약품안전청에 보고된 심혈관계 약물유해반응의 특성 분석)

  • Adverse drug reaction (ADR) is a global problem of major concern in health care. ADRs can be accrued in any organs or systems. However, cardiovascular ADRs could be a more serious problem if they are irreversible or severe. For this reason, this study was conducted to analyze pattern and severity of cardiovascular ADRs, and suspicious medication. Total 646 reports including cardiovascular ADRs reported to the KFDA between January and June 2010 were analyzed. Amlodipine besylate (36 reports, 3.3%), iopromide (29 reports, 2.7%), tramadol HCl (28 reports, 2.6%) were most suspicious drugs that occurred cardiovascular ADRs. The most common cardiovascular ADRs were hypotension( 236 reports, 33.1%), palpitation (134 reports, 18.8%), and hypertension (89 reports, 12.5%). The most frequent ADRs were occurred in the age group of more than 60. This result could be of help to prescribers and other healthcare providers to predict and prevent cardiovascular ADRs. Also this study suggested that patients with cardiovascular ADR risk factors should be intensively monitored during the medications.

  • Hemodialysis is a way of treatment for the patients with acute and chronic renal failure. "Marginal man" depicts the enormous amount of stress that hemodialytic patients suffer. It is known that family with chronic illness get stressed along with the patients and they need nursing care also. This qualitative study was conducted in order to identify the family adaptation process in patients with hemodialysis. 6 dyads of hemodialytic patients and caregivers were interviewed for this research. By way of grounded theoretical strategy the data was collected from October 20, 1999 to November 20, 1999. at a hemodialytic unit that was located in Iksan, Chonbuk by the researcher. The main research questions were "What's the adaptation experience of a patient since after being received with hemodialysis?" to the patients, and "What's the family adaptation experience of hemodialysis?" to the caregivers. The collected data was analyzed by grounded theoretical strategy built by Glaser & Strauss in 1967. Results were as follows; 7 grounded concepts were observed from the patients with hemodialysis and 4 grounded concepts were found from the caregivers. Two categories were derived ; 1) Holding hope that is related with treatment 2) Changing a value system.

  • As the smartphone-based devices are diffused and developed rapidly, they provide the convenience to the users. The abovementioned sentence technologies are being used not only in the existing sensor and wireless network technology but also in the application services of the diverse fields application services such as smart appliance, smart car, smart health care, etc. and the new fusion paradigm from the industry is presented by undertaking the researches in diverse area by the enterprises and research institutions. However, the smart environment exposes its weaknesses in the mobile terminal area, existing wireless network and IT security area. In addition, due to new and variant ways of attack, not only the critical information are disclosed However also the financial damages occur. This paper proposed the protocol to perform the smartphone-based safe device monitoring and safe communication. The proposed protocol designed the management procedure of registration, identification, communication protocol and device update management protocol and the safety against the attack techniques such as the an-in-the-middle-attack, impersonation attack, credential threat, information leaks and privacy invasion was analyzed. It was observed that the proposed protocol showed the performance improved by approximately 52% in the communication process than the existing system.
