Methods: A nonequivalent control group pretest-posttest design study was conducted on 568 children and 85 child care teacher at 12 child care facilities for 12 weeks from July to October 2012. The program was consist of management, education, screening under concepts of traditional preventive medicine, Yangsaeng and Chimibyeong. Children's medical utilization due to infectious disease and attendance means functional status were measured by reports from parents. The Difference in difference(DID) estimator was applied data analysis, and added Zero-inflated negative binomial regression model. Also, attitudes on the infection of teacher was measured and analyzed through t-test. Results: After the intervention, the total medical utilization due to infectious disease decreased, but not significantly. Total absence, early leave and lateness decreased significantly. But, Attitude on the infection of child care teacher was not changed. The parent's satisfaction showed positive overall. Conclusions: The intervention program may be effective in preventing infectious disease and managing health in child-care center partially. To measure long-term effect, long-term study improved is requested.

  • Purpose: To describe the performance in the health-promoting lifestyle and to identify the major factors affecting the health-promoting relationships between self care behaviors and health promoting lifestyle profile in patients with pulmonary tuberculosis. Method: A convenience sample for this study was 172 pulmonary tuberculosis patients who have taken TB medications in urban city. The HPLP-II was selected to measure the concept of health-promoting lifestyle because of the number of research studies conducted using both the original HPLP and the revised HPLP-II. The statistical methods used in this study were t-test, ANOVA, Pearson correlations, and multiple regression. Result: The differences of the HPLP-II were found to have a significance of age, marital status, education level, and health service center. The level of self care behaviors was related positively to the level of health promoting lifestyle and their subcategories. Based on stepwise multiple regression analysis, the model that predicted factors included self care behaviors, age, health service center and education. Conclusion: After decades of decreasing rates, TB has reemerged as a serious national problem in Korea. The careful clinical management and more national concern of TB may help to improve the outcomes of many patients. The findings of this study suggest that TB patients who are more fulfilled in health-promoting lifestyles and self-care behaviors may be able to make better decisions regarding positive health-promoting behaviors.

  • Purpose: To identify the characteristics and health-care needs of high-risk pregnant women in maternal-fetal intensive care units (MFICU). Methods: A mixed-methods design was adopted. Data were collected from 78 high-risk pregnant women admitted to the MFICU. Qualitative data included ten participants' experiences with hospitalization and childbirth, which were analyzed using mixed content analysis. Quantitative data were analyzed using at-test and one-way ANOVA testing. Results: The average score for pregnancy and childbirth health-care needs was 3.54 points. Average score by area was before-admission health care (3.70), health care of baby (3.67), health of childbirth (3.61), postpartum health (3.51), and pregnancy health care during hospitalization (3.48). Qualitative results showed diverse feelings and experiences of high-risk pregnant women and their need for health care, which was expressed in three themes and 11 sub-themes. Conclusion: Nurses should recognize high-risk mothers' feelings and needs for pregnancy and childbirth-focused health care to help patients accept their vulnerability and cope positively.

  • The purpose of this study was to develop a home health care model in the public health system and to test the effectiveness of the model. Seven com-munity health practitioners in Yon- Cheon county. Kyunggi province, carried out home health care service for this research. The subjects of the home health care were a total of 111 community residents with chronic health problems and risk-prone infants and children; 29 persons with hypertension, 18 persons with diabetes, 12 persons with neurologic problems, 12 elderly, and 40 infants and children. During the period of study, from December, 1993 to March, 1995, a demonstrative home health care model was developed in the Yon-Cheon County community health centers with the cooperation of the Yon-Cheon Medical Center and Yon-Cheon Public Health Center for the first six months. A home care practice manual and recording system for home visits were also co-developed by the researchers and community health practitioners. Four workshops and monthly conferences were held for this purpose. Actual home care practice took place for two months, and on-going evaluation and replanning accompanied this process. The result of the evaluation of home care service were as follows. 1) For persons with hypertension, diabetes, neurologic problems, there was significant improvement in knowledge of disease and care, but no significant difference was seen in health behavior or symptoms after home care service. 2) No significant difference was seen in level of self esteem or depression after reminiscence therapy among 12 elderly subjects. 3) There were significant differences in satis-faction toward child rearing and parental sup-port, but no significant difference In education needs for parental role after home care service among parents of infants and children. 4) There was significant improvement in the quality of life among the subjects after the home care service. 5) Subjects responded that they were highly satisfied with the home care service given by the community health practitioners. Although, the actual implementation period was very short, and not all of the evaluation outcomes showed significant improvement, the home health care model of community health practitioners was, in general, positively evaluated. Through this re-search, the possibility of community health practitioners working as active home care personnel in the public health care system is supported. Further research with an expanded area and subjects for a longer period is recommended. Cost effectiveness research is also needed.

  • Purpose: This study was done to verify the effects of a self-regulation program for management of hypertension. Method: Thirty patients with hypertension registered in a community health center were selected as the experiment group, and control group were patients in another community health center, matched for age and gender. The self-regulation program included daily blood pressure checks, periodic counseling, and health education. A self-check digital device with instructions was provided for self-monitoring of blood pressure, and the participants were interviewed before they took part in the program. Results: The first hypothesis was supported: There will be a greater reduction in both systolic and diastolic blood pressure for patients with hypertension who participate in the self-regulation program compared to patients in the control group. The second hypothesis was also supported: Patients with hypertension who participate in the self-regulation program will perform self-care activities better than those in the control group. Conclusion: The findings indicate that a self-regulation program reduces systolic and diastolic blood pressure and improves self-care in patients with hypertension. It is recommended that this self-regulation program be used in community health clinics for management of hypertension and prevention of complications.

Methods: The study subjects were 96 participants who had been diagnosed with hypertension or had high risk factors for hypertension among those registered at a public health center in Seoul. The education program was individualized and delivered to enhance the management of hypertension including life-style modification, medication, and complication managements. Data were collected using questionnaires from May to December 2006 and analyzed using descriptive analysis, t-test, paired t-test, and ANOVA. Results: The blood pressure of the participants after the education was significantly decreased compared to before the education. Education also significantly affected the increases of knowledge of the disease and self-care capability. However, the blood pressure and self care were not significantly improved for the age group over 85 years. Conclusions: This study proves that the individualized health education for visiting nursing clients was effective in improving blood pressure, knowledge, and self-care of hypertension. However, it is necessary to develop strategies that are specifically targeted at particular age groups in order to improve the capability of self-care of hypertension.
