A Case Report of Progressive Hemifacial Atrophy (진행성 반안면위축환자의 치험례)

  • A progressive hemifacial atrophy is characterized by progressive atrophy of subcutaneous fat and rarely muscle and bone. Its contour follows the underlying muscle. Unilateral involvement is common. The treatment goal has been focused on the augmentation of the soft tissue. Many materials such as implants, collagen, fat graft, fat injection, dermal fat graft, filler and vascualized autogenous graft have been used. Although these materials have been used, the best treatment hasn't been achived. In severe cases underlying soft tissue, muscle and bone may be atrophied and massive soft tissue graft, implant and orthognathic surgery must be used. The author used the dermal-fat tissue for the pupose of soft tissue augmentation. We can get the massive soft tissue by the dermolipectomy procedure through the mini-abdominoplsty. The facial augmentation was done by augmentation of the dermal-fat tissue. The progressive hemifacial atrophy is hard to treat by only one procedure and many modalites must be considered.

TREATMENT OF DENTAL CARIES BY ER:YAG LASER IN CHILDREN (소아 환자에서 Er:YAG Laser를 이용한 우식 병소의 처치)

  • The lasers have been used in dentistry for more than 30 years and the application of lasers for drilling dental hard tissue has been investigated since the early developement of lasers. Recently, the Er:YAG laser was invented for hard tissue ablation. The Er:YAG laser, having a wavelength of 2.94um, is highly absorbed in both water and hydroxiapatite, leading to a very effective material for hard tissue removal by bursting off the solid tissue component that is, enamel and dentin are removed by the Er :YAG laser by water vaporization and microexplosion, without any melting of inorganic tissues. Therefore, the Er:YAG laser produced round craters with well defined margins and the surrounding tissues had no cracks and no charring. When used for cavity preparation, pulpal damage should not occur if hear buildup is minimized by careful selection of exposure parameters and by use of a water spray. The present study demonstrated that the Er:YAG laser cut the tooth substance adequately for composite resin restoration, without having undesirable side effects such as harmful effects on the pulp, discoloration or cracking etc. Also, the child patients were well cooperative during laser treatment mainly because of little noise, lesser vibration and minimal pain compared to conventional means of cavity preparation.

Pattern of lip retraction according to the presence of lip incompetence in patients with Class II malocclusion

  • Objective: The aim of this retrospective study was to compare changes in hard tissue and soft tissue after the four first premolars were extracted with anterior teeth retraction according to the presence or absence of lip incompetence. Methods: Patients who underwent the four first premolars were extracted with anterior teeth retraction were divided into competent (n = 20) and incompetent lip (n = 20) groups. Cephalometric measurements for hard tissue and soft tissue changes were performed pre-treatment and post-treatment. Results: In the competent group, the upper and lower lips retreated by 2.88 mm and 4.28 mm, respectively, and in the incompetent group by 4.13 mm and 5.57 mm, respectively; the differences between the two groups were significant (p < 0.05). A strong positive correlation between retraction of the upper lip and upper incisors was observed in both groups (p < 0.05), whereas a correlation between retraction of the lower lip and lower incisors was only found in the incompetent group. A simple linear regression analysis showed that the pattern of lip retraction following the retraction of the anterior teeth was more predictable in the incompetent group than in the competent group. Conclusions: These findings suggest that the initial evaluation of lip incompetence in patients with skeletal Class II is essential for the accurate prediction of the soft tissue changes following retraction of the anterior teeth in premolar extraction treatment. Therefore, sufficient explanation should be provided during patient consultations.

The effect of hard-type crosslinked hyaluronic acid with particulate bone substitute on bone regeneration: positive or negative?

  • Purpose: The role of hard-type crosslinked hyaluronic acid (HA) with particulate bone substitutes in bone regeneration for combined inlay-onlay grafts has not been fully investigated. We aimed to evaluate the effect of hard-type crosslinked HA used with bone substitute in terms of new bone formation and space maintenance. Methods: A 15-mm-diameter round defect was formed in the calvaria of 30 New Zealand White rabbits. All animals were randomly assigned to 1 of 3 groups: the control group (spontaneous healing without material, n=10), the biphasic calcium phosphate (BCP) graft group (BCP, n=10), and the BCP graft with HA group (BCP/HA, n=10). The animals were evaluated 4 and 12 weeks after surgery. Half of the animals from each group were sacrificed at 4 and 12 weeks after surgery. Samples were evaluated using micro-computed tomography, histology, and histomorphometry. Results: The BCP group showed higher bone volume/tissue volume (BV/TV) values than the control and BCP/HA groups at both 4 and 12 weeks. The BCP and BCP/HA groups showed higher bone surface/tissue volume (BS/TV) values than the control group at both 4 and 12 weeks. The BCP group showed higher BS/TV values than the control and BCP/HA groups at both 4 and 12 weeks. No statistically significant difference in newly formed bone was found among the 3 groups at 4 weeks. The BCP group showed significantly higher new bone formation than the BCP/HA group at 12 weeks. Conclusions: Hard-type crosslinked HA did not show a positive effect on new bone formation and space maintenance. The negative effect of hard-type crosslinked HA may be due to the physical properties of HA that impede osteogenic potential.

Simultaneous Repair of Unilateral Cleft Lip and Hard Palate with Vomer Flap (편측성 구순구개열 환자에 있어 구순성형술과 동반한 서골피판법)

  • Vomer flap is used to repair anterior hard palate in complete cleft lip and palate patients. As the midline structure located in between the two cleft segments of hard palate, the vomer flap is very useful because of its vicinity to cleft site and their ease of execution when it is done with primary cheiloplasty simultaneously. In addition, the quality of tissue is very similar to that of the nasal mucosa with good vascularity. In cases of simultaneous repair of cleft lip with anterior palate using vomer flap, the hard palate can be repaired at the same time with primary cheiloplasty which is earlier period than other techniques. With simultaneous close of cleft lip and cleft hard palate by vomer flap, subsequent palatoplasty does not require wide dissection, and consequently chance of oronasal fistula formation will be minimized. Additionally, surgical time will be reduced and, the harmful effects on mid-facial growth will be diminished. In this article, we will introduce the comprehensive vomer flap technique with primary lip closure and review the comparative studies of the outcome of simultaneous repair of cleft lip and cleft hard palate with vomer flap.

A STUDY ON SOFT TISSUE FACIAL PROFILE CHANCES IN ORTHODONTIC TREATMENT WITH FOUR PREMOLAR EXTRACTIONS (4개 소구치 발치를 통한 교정치료시 나타나는 안모 연조직 변화에 관한 연구)

  • The purpose of this study was to evaluate the differences of soft tissue profile changes between the growing patients and the adult who had passed the growth peak, in orthodontic treatment with four premolar extractions. The results which was taken by correlating the soft tissue changes with hard tissue changes, lip thickness, molar relationship and arch length discrepancy in both groups was like the followings. 1. Significant hard tissue changes were decrease of VIs, VIi, UlPP, LlMP, HIi and increase of HPog'in adults and decrease of VIs, VIi and increase of VA, VPog'and all the vertical measurements in adolescents. 2. Significant soft tissue changes were decrease of VLs, VLi, and VILS in adults and increase of VSn, VSLS, VLs, VPog' and almost all vertical measurements in adolescents, and ${\Delta}LsE,\;{\Denta}LiE$ in adults and ${\Delta}LsE,\;{\Denta}LiE,\;{\Delta}LiSP\;and\;{\Delta}Mang$ was also significant 3. Correlation coefficient between ${\Delta}VIs\;and\;{\Delta}VLs$ was the greatest in adults and the next was ${\Delta}Ii/{\Delta}Li,\;{\Delta}Ii/{\Delta}ILS,\;{\Delta}ID/{\Delta}Li\;and\;{\Delta}ID/{\Delta}ILS$. In contrast all the vertical and horizontal measurements of hard and soft tissue in adolescents showed statistically significant corerrlation. 4. There were differences in correlation between soft tissue changes and incisor inclination and retraction at both groups, but the lower lip, nasolabial angle and mentolabial angle were commonly less influenced by the hard tissue changes in both groups. 5. The thinner the upper lip was, the more the ${\Delta}LsSP$ was in both groups, and the thinner the lower lip was, the more the ${\Delta}LiE\;and\;{\Denta}LiSP$ was in adolescents. 6. Molar relationship didn't influence the soft tissue profile changes. 7. Arch length discrepancy didn't influence the soft tissue profile changes.

  • Titanium and titanium alloys are the most common materials used for dental and biomedical implants, owing to their biocompatibility and favourable mechanical properties. However infection of the region surrounding a dental implant by pathogenic microorganisms is a significant factor in implant failure. Prevention and control of microbial colonization of implant surfaces is considerable interest to the biomedical community. One important strategy is to render the implant surface antibacterial by impeding the formation of biofilm. A number of approaches have been proposed for this purpose. Therefore, we reviewed the researches of antibacterial coatings on titanium implants in this articles.

Multiple idiopathic external and internal resorption: Case report with cone-beam computed tomography findings

  • Root resorption is loss of dental hard tissue as a result of clastic activities. The dental hard tissue of permanent teeth does not normally undergo resorption, except in cases of inflammation or trauma. However, there are rare cases of tooth resorption of an unknown cause, known as "idiopathic root resorption". This report would discuss a rare case of multiple idiopathic resorption in the permanent maxillary and mandibular teeth of an otherwise healthy 36-year-old male patient. In addition to a clinical examination, the patient was imaged using conventional radiography and cone-beam computed tomography (CBCT). The examinations revealed multiple external and internal resorption of the teeth in all four quadrants of the jaws with an unknown cause. Multiple root resorption is a rare clinical phenomenon that should be examined using different radiographic modalities. Cross-sectional CBCT is useful in the diagnosis and examination of such lesions.

RIDGE AUGMENTATION USING OF HARD TISSUE $REPLACEMENT(HTP^{TM})$;A CASE REPORT (치조제 증강을 위한 $HTR^{TM}$ 중합체 이식후 치유과정;증례보고)

  • The purpose of this study is to evaluate the effect of $HTR^{TM}$ (hard tissue replacement, Bioplant Inc, U.S.A) polymer on short-term healing as a grafting material for alveolar ridge augmentation. A 48-year-old female presented insufficient bone height and width for the placement of implants. $HTR^{TM}$ polymer was used for ridge augmentation. Bone biopsy was harvested 8 months after the ridge augmentation procedure. $HTR^{TM}$ polymer displayed rapid bone regeneration and mature lamellar and trabecular bone redevelopment. Clinical and histologic observation from the treatment of the patient presented suggest that $HTR^{TM}$ polymer seems to be a appropriate material for alveolar ridge augmentation.

A Study of Blood Group Identification with Teeth Left Standing at a High Temperature (온도변화가 치아경조직에서 혈형물질 검출에 미치는 영향에 관한 실험적 연구)

  • Identification of blood group from dental hard tissue for the purpose of individual identification of a highly burned corpse would play a significant role in a practical legal medicine. The author conducted a study of blood group with teeth left stading at a high temperature by the method of elution test. The following results were obtained. 1. The blood identifcation from heated dental hard tissue proved to be possible. 2. In cases of heat-treated theeth at $100^{\circ}C$ for 120 minutes, at $150^{\circ}C$ for 120 minutes and at $200^{\circ}C$ for 45 minutes for A.B.O(H) blood group, the identification of blood group was possible. 3. In case of heat-treated teeth, thermostability of blood group was found to be $150^{\circ}C$. 4. The adequate surface area for the detection of blood group was 40-80 meshes.
