Objectives: The aim of this study is to describe the effects of traditional Korean medicine treatment on a patient with vascular dementia caused by an intracerebral hemorrhage. Methods: The patient was treated with herbal medicine (modified Guibi-tang) and acupuncture. The effects of these treatments were evaluated using the Korean Mini Mental Status Examination (K-MMSE-2), Cognitive Dementia Rating (CDR), Manual Muscle Test (MMT), Korean version of Modified Barthel Index (K-MBI), and Videofluoroscopic Swallow Study (VFSS). Results: After treatment, the K-MMSE-2 score, CDR score, MMT, K-MBI, and VFSS improved. Conclusion: The results suggest that traditional Korean medicine may be effective for the symptoms of vascular dementia caused by intracerebral hemorrhage.
Objectives : To establish the fundaments for EBM of Traditional Korean Medicine, the papers on Guibi-tang(Guipitang) which were frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international literatures. Methods : The papers were classified by the year of publishment, by experimental methods, by laboratory animals used in biological experiment and by the kinds of studies on biological efficacy. Results : Of total 39 papers were registered in domestic journals. Since 1980, publishments of papers have continuously increased. The papers on biological studies were 34 volumes, clinical studies were 5 volumes. Biological studies mainly showed the effects of anti-stress, anti-oxidant, hemostasis, anti-osteoporosis, gastroprotection, reproductive ability, brain cell activity, radioprotection. And clinical studies showed improvements of fearful throbbing, memory and orientation, insomnia, vericose vein, schizophrenia. Conclusions : Guibi-tang(Guipitang) could be used to fortify the spleen, nourish the heart and then control the blood. However biological studies need to be conducted in accordance with clinical study and further clinical studies on randomized controlled trials should be proceeded.
Objective: This study evaluates the effects of traditional Korean medicine (including Guibi-tang) on a psychogenic urinary incontinence patient with hypersomnia disorder.Method: Herbal medicine, acupuncture, and moxibustion were carried out. The effect of the treatments was then evaluated via the Beck depression inventory (BDI), the Beck anxiety inventory (BAI), the symptom checklist-90-revised (SCL 90-R), the Epworth sleepiness scale (ESS), and the revised urinary incontinence scale (RUIS), as well as the number of urinations, incontinence levels, and sleeping times during the day.Results: After the treatments, both the urinary incontinence and hypersomnia symptoms showed improvement.Conclusion: This study suggests that traditional Korean medicine treatments are effective in caring for patients who suffer from psychogenic urinary incontinence with hypersomnia disorder.
The purpose of this study is to investigate the protective effects of 25 herbal formulas on acetaminophen (APAP) or D-galactosamine (D-GalN)-induced hepatotoxicity in primary rat hepatocytes. Cell viability was measured using by Cell Counting Kit-8. 15 kinds of herbal formulas significantly reversed the cell viabilities of D-GalN-treated rat hepatocytes compared with D-GalN alone (p<0.05). In particular, 9 herbal formulas (Bangpungtongseong-san, Bojungikgi-tang, Galgeun-tang, Gumiganghwal-tang, Guibi-tang, Sagunja-tang, Samsoeum, Pyeongwi-san and Yijin-tang) showed the potent protective effects. However, 8 herbal formula exerted weak protective effects and 2 herbal formula did not exert effects on hepatotoxicity by D-GalN. On APAP-induced hepatotoxicity, 7 kinds of herbal formulas increased the viabilities of hepatocytes compare with APAP alone (p<0.05). These results could be provide a valuable information for the future in vivo or clinical studies to predict the hepatoprotective effects of herbal formulas.
Objectives : The objective of present study was to investigate the vasorelaxant effects of 10 traditional Korean Herbal Prescriptions (KHP) on isolated rat thoracic aorta precontracted with potassium chloride (KCl). Methods : An electric extractor was used to extract KHP in distilled water for 3h. Rat aorta rings were isolated and were precontracted using KCl in organ chambers containing 10 ml Krebs Henseleit (KH) buffer. KHP extracts were added in increasing concentrations (10-1000 ㎍/㎖) to investigate vasorelaxant effects. The vasorelaxant responses induced by KHP were expressed as a percentage in response to contraction generated by KCl. Results : Among the 10 KHP, Gamisoyo-san, Galgeun-tang, Gyeji-tang, Gwakhyangjeonggi-san, Daeyoung-jeon, and Socheongryong-tang showed significant vasorelaxant effect at high concentration. In contrast, Gyejibokryeong-hwan constricted more the aorta rings precontracted by KCl. And Gumiganghwal-tang, Guibi-tang, Saengmaek-san showed no significant effect. Also, rat aorta rings treated with Gyejibokryeong-hwan or Gyeji-tang after pre-relaxation by amlodipine did not cause any significant change. Conclusion : Thus, these results provide the experimental evidence as useful herbal prescriptions for the treatment of hypertension and suggest guidelines in conjunction with other western drugs, including amlodipine.
Objectives: This study evaluated the effects of Guibi-tang (GBT) on neuronal apoptosis and cognitive impairment induced by beta amyloid ($A{\beta}$), (1-42) injection in the hippocampus of ICR mice. Methods: $A{\beta}$ (1-42) was injected unilaterally into the lateral ventricle using a Hamilton syringe and micropump ($2{\mu}g/3{\mu}{\ell}$, $0.6{\mu}{\ell}/min$). Water extract of GBT was administered orally once a day (500 mg/kg) for 3 weeks after the $A{\beta}$ (1-42) injection. Acquisition of learning and retention of memory were tested using the Morris water maze. Neuronal damage and $A{\beta}$ accumulation in the hippocampus was observed using cresyl violet and Congo red staining. The anti-apoptotic effect of GBT was evaluated using TUNEL labeling in the hippocampus. Results: GBT significantly shortened the escape latencies during acquisition training trials. GBT significantly increased the number of target headings to the platform site, the swimming time spent in the target quadrant, and significantly shortened the time for the 1st target heading during the retention test trial. GBT significantly attenuated the reduction in thickness and number of CA1 neurons, and $A{\beta}$ accumulation in the hippocampus produced by $A{\beta}$ (1-42) injection. GBT significantly reduced the number of TUNEL-labeled neurons in the hippocampus. Conclusion: These results suggest that GBT improved cognitive impairment by reducing neuronal apoptosis and $A{\beta}$ accumulation in the hippocampus. GBT may be a beneficial herbal formulation in treating cognitive impairment including Alzheimer's disease.
Background: Alzheimer's disease (AD) is a chronic neurodegenerative disease that causes disorientation, mood swings, problems with language, and difficulty remembering recent events. Acetylcholinesterase inhibitors (AchEIs) and memantine have been used to slow the course of the disease, but they can neither modify its progression nor prevent disease onset. Previous studies have suggested that Kami-guibi-tang (KGT) could be beneficial for supporting cognitive function in AD patients, but few clinical trials have been published. This pilot study aimed to evaluate the effect of KGT in improving cognitive function in AD patients. Methods: The study will be a randomized, placebo-controlled, double-blind, single-center trial conducted using subjects diagnosed with mild AD by neurologists. Study subjects will be randomly assigned to either a treatment or control group. The treatment group will receive KGT granules for 24 weeks, while the control group will receive placebo granules. AchEI administration will be maintained in both groups during the entirety of the study. Subjects will be assessed using the following exams: the Seoul Neuropsychologic Screening Battery (SNSB) for cognitive function; brain magnetic resonance imaging (MRI) for brain metabolite, neurotransmitter, and cerebral blood flow (CBF) measurements; the Korean version of Quality of Life-Alzheimer's Disease (KQol-AD) for quality of life; the Caregiver-Administered Neuropsychiatric Inventory (CGA-NPI) for neurobehavioral symptoms; blood tests for amyloid and tau proteins and general blood parameters; and electrocardiography (ECG) before and after taking the medication. Discussion: Our findings will provide insight into the feasibility of large-scale trials to consolidate evidence for the efficacy of KGT for dementia treatment. Objectives : The purpose of this study was to investigate the clinical application of Korean Traditional treatment to psychogenic torticollis patient who has panic disorder as underlying disease. Methods : Herb medicine used in this study are Heosu-san(Huishou-san) which is previously used for torticollis in several studies, Guibi-tang(Guipi-tang) and Gamiondam-tang(Jiaweiwendan-tang) which are frequently used for phychological disease. Also, physical therapy and aroma therapy were practiced which basically stretches and relaxes muscles around cervical muscles. Self exercise of cervical muscles was carried out during acupuncture. Results : After the comprehensive treatment, the clinical symptoms of the patient were improved. Conclusions : It is suggested that oriental medical treatment are effective in psychogenic torticollis.
Objectives: The purpose of our study was to verify whether the Seven Emotions evaluated by Guibi-Tang questionnaire (GTQ) were predictive of self-awareness level of one's quality of life (QOL). Methods: Eighty volunteers participated in this study and completed the GTQ. In addition, the volunteers were asked to answer one's self-awareness of the QOL using numerical scale. Factor analysis was conducted for extraction of GTQ factors. Structural Equation Model (SEM) was conducted to verify path structure. Results: The estimates of the four factors on the level of one's QOL demonstrated that Chest Factor (=-0.330) showed the highest level of correlation followed by the seven emotions (=-0.213); Deficient fire (=0.141); and Qi Deficiency (=-0.023). Model fit is $X^2$=34.740 (df=36), GFI=0.931, AGFI=0.873, NFI=0.867, RFI=0.797, IFI=0.975, TLI=1.006, CFI=1.009, RMSEA=0.000. Therefore, considering these values, SEM could be accepted as a workable model. Conclusions: Our study results suggest that According to SEM, The Seven Emotions evaluated by GTQ are predictable for self-awareness of one's QOL.
Objectives: The coronavirus disease 2019 (COVID-19) has become a global pandemic. Mental sequelae occurring in patients with COVID-19 and the general population are important concerns. In Korea, herbal medicine is used nationwide to respond to this pandemic. It can be prescribed by COVID-19 telemedicine center of Korean medicine (KM). Among some herbal medicines, Gamiguibi-tang is the only herbal medicine prescribed for individuals with mental health, especially for those with insomnia. In this mini-review, the objective of this study was to summarize the evidence of some promising herbal medicines available for treating primary insomnia based on existing clinical and preclinical studies. Methods: A research team was formed for KM clinical practice guidelines for insomnia (version 1.0). Team members were provided with a list of references of relevant herbal medicines for insomnia. To gather evidence from clinical studies with appropriate sample sizes, among the list of references, randomized controlled trials for primary insomnia that included 50 subjects or more per arm and used herbal medicine were included in the final analysis. Moreover, pre-clinical studies examining the mechanism of action of each herbal medicine and studies on herb-drug interactions, were searched and summarized. Results: Four herbal medicines (Ondam-tang, Sanjoin-tang, Guibi-tang, and Hyeolbuchugeo-tang) were reviewed based on existing clinical and preclinical studies. Based on findings of existing studies, some suggestions of herbal medicines for insomnia in the COVID-19 era in Korea were suggested. Conclusions: Data of this study could be used to prepare a future revision of the manual of COVID-19 telemedicine center of KM.
