Dietary Quality and Self-Management Status according to the Glycemic Control in the Elderly with Type 2 Diabetes (노인 당뇨환자에서 혈당조절에 따른 식사의 질과 당뇨자가관리상태)

  • Evidence for the effects of different health behaviors, including diet, in elderly diabetes is currently limited. The aim of this study was to compare the quality of diet and health behaviors in Korean elderly T2DM patients, using a glycemic control. T2DM elders(>65 yr, n=48) were recruited and categorized by the concentration of glycated-hemoglobin HbA1c; subjects with HbA1c<7% were the good control(GC) group, and subjects with $HbA1c{\geq}8%$ constituted the poor control(PC) group. General characteristics, self-management behavior questionnaires, and 3-d diet records were all collected and assessed. No significant differences in general characteristics between GC and PC were detected, with the exception of a higher level of education in GC(p<0.05). A twofold longer duration of diabetes was observed in PC as compared to GC(p<0.01). The GC group did exercise for a longer time(p<0.001), and had an earlier beginning of diabetes self-management education (DSME) by healthcare practitioners using a team teach as compared with the PC group(p<0.05). The total dietary quality index(p<0.001) and individual index for carbohydrate(p<0.001) or vegetables and fruit(p<0.05) were better in GC than in PC. Therefore, the earlier DSME including intensive exercise and balanced diet selection should be expected to improve glycemic control in diabetic Korean elders.

A Clinical Study on the Effect of 41 Obesity Patients through Yu$\'{e}$biji$\~{a}$sh$\`{u}$-t$\~{a}$ng with Low-Glycemic Diet (월비가출탕(越婢加朮湯)과 저당지수식이요법을 병행한 비만환자 41례에 관한 임상관찰)

  • Objective : The purpose of this study is to evaluate the effect of Yuebijiashu-tang with Low-Glycemic Diet on the obesity and to improve the oriental medical methods of treatment in obesity patients. Methods : Forty one patients were classified into four groups(Normal, Overweight, 1st Obesity, 2nd Obesity) by Body Mass Index(BMI). Weight, Body Fat Mass, Body Fat Rate, Waist Hip Rate, BMI and Obesity Index were compared in each group by t-test. Results : In distribution of sex, male was 7(17.1%), female was 34(82.9%). Mean age was 37${\pm}$11 and the patients in twenties were the most. Among four groups, the number of 1st obesity groups was the most. In the change of body composition, the reduction of weight was 6.82${\pm}$2.67kg, the reduction of body fat mass was 5.53${\pm}$2.30kg, the reduction of BMI was $2.51{\pm}0.85kg/m^2$, the reduction of body fat rate was 4.61${\pm}$2.17%, the reduction of waist hip rate was 0.019${\pm}$0.013. Conclusion : This study shows that Yu$\'{e}$biji$\~{a}$sh$\`{u}$-t$\~{a}$ng with low-glycemic diet is effective on weight reduction.

Effects of macronutrients in mixed meals on postprandial glycemic response (식품 및 음식의 다량영양소 구성 성분에 따른 혈당 반응 연구)

  • Purpose: The aim of study was to determine the effects of carbohydrate, fat, protein, and fiber contents on glycemic responses in a single food item or meal. Methods: Glycemic responses were measured in 30 healthy young adults (17 males and 13 females) with various test foods, including rice, egg whites, bean sprouts, olive oil, noodles, prune, broccoli, Korean dishes, Western dishes, and salad dishes, etc. Test foods were designed to contain various carbohydrate, fat, protein, and fiber contents in single or mixed foods or dishes. After 12 hours of fasting, participants consumed test foods, and the glycemic response was measured for a subsequent 120 min (0, 15, 30, 60, 90, and 120 min). Three hundred and fifty three glycemic responses from 62 foods were collected. The incremental area under the curve (AUC) was calculated for each test food for each subject to examine glycemic responses. Statistical analysis was conducted to identify which macronutrient (carbohydrate, fat, protein and fiber) affected the AUC using a mixed model. Results: Carbohydrates (${\beta}=37.18$, p < 0.0001) significantly increased while fat (${\beta}=-32.70$, p = 0.0054) and fiber (${\beta}=-32.01$, p = 0.0486) significantly reduced the glycemic response. Conclusion: It can be concluded that the glycemic response of a meal can be modified depending on the fat and fiber contents of ingredient foods, even though carbohydrate content is maintained.

Effect of varying levels of xylobiose in sugar on glycemic index and blood glucose response in healthy adults (자일로바이오스 첨가 비율이 다른 설탕이 건강한 성인의 혈당지수와 혈당반응에 미치는 영향)

  • Purpose: The objective of this study was to compare the effects of three different levels of xylobiose containing sucrose on glycemic indices based on oral glucose tolerance test (OGTT) and blood glucose response in healthy adults. Methods: Healthy adults (six male and five female participants, n = 11) underwent 14~16 hr of fasting. Subsequently, all participants took 50 g of available carbohydrates from glucose, sucrose containing 7% xylobiose (XB 7), sucrose containing 10% xylobiose (XB 10), or sucrose containing 14% xylobiose (XB 14) every week on the same day for 8 weeks. Finger prick blood was taken before and 15, 30, 45, 60, 90, and 120 min after starting to eat. Results: We observed reduction of the glycemic response to sucrose containing xylobiose. The glycemic indices of XB 7, XB 10, and XB 14 were 57.0, 53.6, and 49.7, respectively. The GI values of XB 7 were similar to those of foods with medium GI, and the GI values of XB 10 and XB 14 were similar to those of foods with low GI. The postprandial maximum blood glucose rise (Cmax) of XB 14 was the lowest among the test foods. XB 7, XB 10, and XB 14 showed significantly lower areas under the glucose curve (AUC) for 0~30 min, 0~60 min, 0~90 min and 0~120 min compared to glucose. Conclusion: The results of this study suggest that sucrose containing xylobiose has an acute suppressive effect on GI and postprandial maximum blood glucose rise. In addition, levels of xylobiose in sugar may allow more precise assessment of carbohydrate tolerance despite lower glycemic responses in a dose-dependent manner.

Study on Quality and Blood Glucose Response of Roll Breads for Diabetes Mellitus (당뇨환자를 위한 롤빵의 품질과 혈당반응에 관한 연구)

  • In order to develop the roll bread for diabetes mellitus patients, the roll bread mixed with soybean and barley flour to wheat flour was prepared and followed investigation of physical properties, sensory evaluation and blood glucose response of subjects. When soybean and barley flour was added more than 30%, hardness of mixed roll bread increased more than two times comparing to wheat flour. The roll bread mixed with 10% soybean flour was same in physical properties but was superior in sensory evaluation to wheat flour. While the blood glucose response after intake of roll bread mixed with soybean flour increased more slowly than that after intake of barley or wheat flour, glycemic index of soybean flour decreased. When roll bread was mixed with 10 to 30% soybean flour. the preference was almost same but control ability of blood glucose has improved comparing to adding barley or wheat alone.

The Effect of Dietary Fiber Content of Rice on the Postprandial Serum Glucose Response in Normal Subject (쌀의 식이섬유함량이 정상인의 혈당에 미치는 영향에 관한 연구)

  • The purpose of this study is to investigate the effect of dietary fiber content of rice on the postprandial serum glucose and insulin responses in normal subject. Two rice varieties, Ilpum and Suwon 464 which are different in dietary fiber content, were cooked in pressure cooker and used for the test. The rice with a higher dietary fiber content gave a significantly lower glucose level (p<0.01) and insulin level (p<0.05) than did the normal rice variety. After a meal, the 60-min glucose levels of Suwon 464 and Ilpum were 90.3${\pm}$4.8mg/dl and 111.6${\pm}$2.7mg/dl, respectively. The glycemic index (GI) of Suwon 464 shows 64.5%, which was apparently lower than that of Ilpum. These results indicate that Suwon 464 high in dietary fiber can be useful in low-GI diets.

Glycemic Index and Oral Glucose Tolerance Test of Takju (Korean turbid rice wine) Lees Extract (막걸리 박 열수추출물의 혈당지수 및 제2형 당뇨 모델 동물에서 경구혈당내성에 미치는 영향에 관한 연구)

  • Takju lees extract is expected to be a promising material for functional food. This study was conducted in order to assess the glycemic index (GI) of Takju lees extract and its effects in an oral glucose tolerance test (OGTT). The GI for Takju lees extract was evaluated with 10 healthy young adults (Male 5, Female 5). OGTT was conducted in 20 male db/db mice, fed on a diet containing 2% Takju lees extract for 4 weeks. Those animals were subjected to OGTT after one oral administration of Takju lees extract at 2 g/kg BW. The GI of the Takju lees extract was measured at 97.97. The effects of the Takju lees extract on the oral glucose tolerance test in db/db mice evidenced no differences as compared to the control group. In conclusion, Takju lees extract is a high GI material, and it has no effect on blood glucose levels in a type II diabetic animal model. Further studies will be required to confirm its anti-diabetic effects.

Effects of xylooligosaccharide-sugar mixture on glycemic index (GI) and blood glucose response in healthy adults (자일로올리고당을 함유한 설탕이 건강한 성인의 Glycemic Index (GI)와 혈당에 미치는 효과)

  • Purpose: The objective of this study was to investigate the effects of xyloologosaccharide (XOS)-sugar mixture on glycemic index (GI) and blood glucose in human subjects. Methods: Randomized double-blind cross-over studies were conducted to examine the effect of sucrose with 14% xyloologosaccharide powder (Xylo 14) and sucrose with 20% xylooligosaccharide powder (Xylo 20) on GI and postprandial glucose response at 15, 30, 45, 60, 90, and 120 min. Results: GIs of Xylo 14 and Xylo 20 were $60.0{\pm}23.5$ classified within medium GI range, and $54.3{\pm}17.7$ within low GI range, respectively. Xylo 14 and Xylo 20 showed significantly lower area under the glucose curve (AUC) for 0-15 min (p = 0.0113), 0-30 min (p = 0.0004), 0-45 min (p < 0.0001), 0-60 min (p < 0.0001), 0-90 min (p < 0.0001), and 0-120 min (p = 0.0001). In particular, compared with glucose, the blood glucose levels of Xylo 14 and Xylo 20 were significantly lower at every time point between 15 and 120 min. Conclusion: The results of this study suggested that Xylo 14 and Xylo 20 had an acute suppressive effect on GI and the postprandial glucose surge.

Glycemic Index Recognition and Practice of Low-Glycemic-Index Diet by Adults with Chronic Diseases in Some Rural Areas (일부 농촌 지역 당뇨병, 고혈압, 고지혈증을 가진 성인들의 당지수에 대한 인식도)

  • Background: The glycemic index (GI) indicates the rise in blood glucose caused by carbohydrate-containing foods. In Korea, there have been few studies on the understanding adults have of GI. Thus, this study investigated differences GI knowledge among the korean adults. Methods: A questionnaire on the perspective of GI and experience in GI education, participational intent to learn, general knowledge level, and dietary practice was conducted among those living in agricultural areas. Respondents were visitors to the health promotion center of a university hospital. Results: When asked about the GI familiarity, the standard of education, the relationships between diseases and GI, the diabetes group was better able to answer correctly than the other groups. However, the diabetes group showed either no difference or less correct responses for general knowledge of GI. With respect to their usual consideration and low GI dietary practice, the diabetes group provided higher responses than the other groups. On the whole, the diabetes group was better than the other groups for GI relative factor but none of the groups showed high levels for perspective, education, dietary practice of GI. Conclusions: The groups other than the diabetes group had a low perspective of GI, a lack of correct knowledge of GI, and did not follow a proper diet without considering GI. Even in the diabetes group the perspective, knowledge, and proper dietary practices were not adequate. Accordingly, further education of GI is necessary for diabetic patients, patients with chronic diseases and also people without diseases.
