A 3-year-old female Connemara pony was referred to Jeju National University Equine Hospital because of weight loss and prolonged anorexia. On admission, heart and respiratory rates were slightly elevated while body temperature was within the normal range. The color of the conjunctival and oral mucosa was yellowish pink. The blood chemistry results showed that total bilirubin, aspartate aminotransferase, and gamma-glutamyl transferase levels were remarkably elevated while blood urea nitrogen was within the reference range. Transcutaneous abdominal ultrasound revealed an enlarged right lobe of the liver with prominently increased parenchymal echogenicity, reduced hepatic vessels, and rounded caudal border. The pony was tentatively diagnosed with chronic hepatitis and icterus; rest and supportive treatments were provided. Clinical signs aggravated on day 2 with hind quarter paresis and cranial nerve signs such as circling, drooping, jerking, and head pressing against walls. Recumbency and generalized ataxia (grade 5/5) were shown on day 3. Plasma ammonia concentration on day 3 was as high as 656 µmol/L. Necropsy and histopathologic examinations strongly supported a diagnosis of hepatic encephalopathy. This case of hepatic encephalopathy exhibited rapid progress from low to terminal grade within 4 days in a Connemara pony. The results provide well-established clinical and pathological data for future application.
The existing concrete bridges are time-varying working systems, where the maintenance strategy should be planned according to the time-varying performance of the bridge. This work proposes a time-dependent residual capacity assessment procedure, which considers the non-stationary bridge load effects under growing traffic and non-stationary structural deterioration owing to material degradations. Lifetime bridge load effects under traffic growth are predicated by the non-stationary peaks-over-threshold (POT) method using time-dependent generalized Pareto distribution (GPD) models. The non-stationary structural resistance owing to material degradation is modeled by incorporating the Gamma deterioration process and field inspection data. A three-span continuous box-girder bridge is illustrated as an example to demonstrate the application of the proposed procedure, and the time-varying reliability indexes of the bridge girder are calculated. The accuracy of the proposed non-stationary POT method is verified through numerical examples, where the shape parameter of the time-varying GPD model is constant but the threshold and scale parameters are polynomial functions increasing with time. The case study illustrates that the residual flexural capacities show a degradation trend from a slow decrease to an accelerated decrease under traffic growth and material degradation. The reliability index for the mid-span cross-section reduces from 4.91 to 4.55 after being in service for 100 years, and the value is from 4.96 to 4.75 for the mid-support cross-section. The studied bridge shows no safety risk under traffic growth and structural deterioration owing to its high design safety reserve. However, applying the proposed numerical approach to analyze the degradation of residual bearing capacity for bridge structures with low safety reserves is of great significance for management and maintenance.
벼 수량 추정에 대한 기존의 국내 연구는 주로 저해상도인 MODIS 위성영상을 사용하여 우리나라 전역을 대상으로 시군 단위에서 수행되었다. 기존 연구와 달리, 본 연구는 전북 김제시를 사례로 중해상도인 Sentinel-2 위성영상과 강우 및 토양자료를 활용하여 읍면동 단위에서 벼 수량을 추정하고 그 정확성을 평가하였다. 전북 김제시를 대상으로 2018년 8월 1일에 촬영된 Sentinel-2 영상으로부터 산출된 NDVI, LAI, EVI2, MCARI1, MCARI2의 다섯 가지 식생지수와 강우량 및 논 토양 유형 자료를 읍면동별로 집계하고 종속변수의 비정규성 문제를 해결하기 위해 다중회귀분석을 확장한 감마 일반화 선형모형으로 벼 수량을 추정하였다. 벼 수량 추정 모형에서 EVI2, 9월 강우일수, 염해답 비율이 유의한 독립변수로 선정되었다. 모형의 적합도를 나타내는 결정계수는 0.68이었고, 모형의 정확성을 나타내는 RMSE는 62.29kg/10a였다. 이 모형으로 2018년 김제시 전역의 쌀 생산량을 추정한 결과는 96,914.6M/T으로 통계연보의 94,470.3M/T과 비교해 0.46%의 오차를 보여 매우 근접한 결과가 도출되었다. 또한, 김제시의 단위면적당 쌀 생산량은 552kg/10a로 도출되어 통계자료의 550kg/10a와 거의 일치하였다. 이러한 결과는 기존 연구들과 유사한 결과로 국내에서 시군 이하 단위에서 Sentinel-2 위성영상을 활용하여 벼 수량을 추정하는 것이 가능하다는 것을 입증하였다.
다이옥신은 단일 화학물질이 아니라 비슷한 화학적 구조와 독성작용을 갖는 화학물질들의 그룹이다. 다이옥신 중에 가장 많은 연구가 이루어졌고 독성도 강한 물질이 2,3,7,8-tetrachlorodibenzo-p-dioxin(TCDD)인데, 다이옥신 혼합체의 독성을 평가할 때는 각 물질 별로 TCDD에 대한 상대독성을 고려하여 독성등가계수(TEQ)라는 지표를 만들어 사용한다. 일반인들의 평균적인 혈중 다이옥신에 대한 기준값은 명확하게 제시되지는 않고 있으나 관련 연구들을 종합할 때 대략 10-20 ppt $I-TEQ_{DF}$ 범위를 일반인의 평균적인 수준으로 볼 수 있다. 오염 가능지역의 주민들의 혈중 다이옥신 값은 지역에 따라 차이가 있으나 대부분 20ppt $I-TEQ_{DF}$ 이상의 값을 나타내고 있으며 특히 오염이 심한 지역의 주민들의 경우에는 직업적인 노출이 없는 일반 주민들임에도 불구하고 혈중 다이옥신 농도가 약 50 ppt $I-TEQ_{DF}$까지 상승할 수 있다. 다이옥신의 건강영향에 관해서는 수많은 역학적 연구들이 수행되었지만 혈액내에서 TCDD를 직접 측정하여 인체 노출을 비교적 정확하게 평가한 사업장 코호트나 세베소 지역 주민 코호트에서 나온 결과들이 보다 많은 신뢰를 받고 있다. 다이옥신 노출에 따른 건강영향은 크게 암발생과 암을 제외한 건강영향으로 구분할 수 있다. 국제암연구기구(IARC)나 미국 환경보호청(EPA) 모두 TCDD를 인체 발암물질로 분류하고 있으나 역학적 증거들이 충분한 상태는 아니다. TCDD를 제외한 나머지 다이옥신에 대해서는 IARC에서 발암성의 증거가 불충분한 물질(Group 3)로 분류하고 있다. 대부분의 장기에서 다이옥신의 건강영향에 대한 조사가 이루어져 왔다. 현재까지 비교적 다이옥신 노출과 관련성이 높은 것으로 평가되고 있는 것은 염소성여드름, 간 효소의 하나인 GGT의 상승, 그리고 생식호르몬의 변화 등이다. 그밖에 지질농도, 당뇨, 면역학적 영향, 심혈관계에 대한 영향, 갑상선기능, 신경계에 대한 영향, 소아 발달에 대한 영향 등에 대해서는 역학 연구들이 일관된 결과를 보여주지 못하고 있기 때문에 계속적인 연구를 필요로 하고 있다. 다이옥신의 독성은 수많은 동물실험을 통해 증명이 되었으나 인간을 대상으로 한 역학적 연구는 아직 기대만큼 명료한 결과들을 제시해주지 못하고 있다. 그러나 IARC나 EPA모두 인체발암물질로 분류하였고 다양한 건강영향에 대한 증거들이 있는 것은 분명한 만큼 다이옥신 노출을 최소화하려는 노력을 게을리 해서는 안 될 것이다.
가와사끼병은 주로 소아기에 발생하는 급성 전신 혈관염으로, 원인은 아직 명확히 알려져 있지 않다. EBV는 전염성 단핵구증의 원인으로 잘 알려져 있으며, 또한 가와사키병의 원인으로도 보고된 바 있다. 가와사끼병으로 인한 합병증은 관상동맥류를 포함한 심혈관계 합병증이 주를 이루지만, 다양한 종류의 합병증들이 보고 되고 있다. 저자들은 EBV 감염과 연관된 전염성 단핵구증, 거대 관상동맥류, 심낭삼출, 비경색증이 동반된 난치성 가와사끼병 환자 1예를 경험하였기에 보고하는 바이다. 환자는 3번의 면역글로불린 치료에도 반응이 없었지만, pulse methylprednisolone 치료 후에 임상 증상은 급격히 호전되었고, 거대 관상동맥류만 지금까지 지속되고 있다.
Gabapentin, [1-(aminomethyl) cyclohexaneacetic acid], a structural analog of $\gamma$-aminobutyric acid (GABA), is being developed for the treatment of epilepsy. Unlike GABA, gabapentin crosses the blood-brain barrier after systemic administration. Gabapentin is an effective antiepileptic drug in patients with partial and secondarily generalized seizures who are uncontrolled with use of existing anticonvulsant drug therapy. The purpose of the present study was to evaluate the bioequivalence of two gabapentin 400 mg capsules, $Neurontin^{(R)}$ capsule 400 mg (Pfizer Inc.) and Gabatin capsule 400 mg (Korean Drug Co. Ltd), according to the guidelines of the Korea Food and Drug Administration (KFDA). The release of gabapentin from the two gabapentin formulations in vitro was tested using KP VIII Apparatus II method with various dissolution media (pH 1.2, 4.0, 6.8 buffer solution and water). Twenty six healthy male subjects, 23.58$\pm$1.50 years in age and 66.74$\pm$8.31 kg in body weight, were divided into two groups and a randomized 2$\times$2 cross-over study was employed. After one capsule containing 400 mg as gabapentin were orally administered, blood was taken at predetermined time intervals and the concentrations of gabapentin in serum were determined using HPLC with fluorescence detector. The dissolution profiles of two formulations were similar at all dissolution media. In addition, the pharmacokinetic parameters such as $AUC_t$, $C_{max}$ and $T_{max}$ were calculated and ANOVA test was utilized for the statistical analysis of the parameters using logarithmically transformed $AUC_t$, $C_{max}$ and untransformed $T_{max}$. The results showed that the differences between two formulations based on the reference drug, $Neurontin^{(R)}$ capsule 400 mg, were 2.04, -3.68 and 16.79% for $AUC_t$, $C_{max}$ and $T_{max}$, respectively. There were no sequence effects between two formulations in these parameters. The 90% confidence intervals using logarithmically transformed data were within the acceptance range of log 0.8 to log 1.25 (e.g., log 0.91$\sim$log 1.16 and log 0.87$\sim$log 1.11 for $AUC_t$ and $C_{max}$, respectively). Thus, the criteria of the KFDA bioequivalence guideline were satisfied, indicating Gabatin capsule 400 mg was bioequivalent to $Neurontin^{(R)}$ capsule 400 mg.
Background: Environmental exposure is characterized by low concentration, chronic, and complex exposure. Traditional epidemiological studies show limitations in reflecting these characteristics since they usually focus on a single or very limited number of exposure factors at a time. In this study, we adopted the methodology of environment-wide association study (EWAS) to figure out the association of human liver function with various environmentally hazardous substances. Methods: We analyzed 2,961 participants from the Korean National Environmental Health Survey Cycle 4 (2018-2020). Using generalized linear model (GLM) analysis, we analyzed the association of 72 variables with 3 liver function indices (aspartate aminotransferase [AST], alanine aminotransferase [ALT], and gamma glutamyl transferase [GGT]). Finally, we visualized our results with Manhattan plot. Results: In GLM analysis, perfluorooctanesulfonate were positively associated with ALT (odds ratio [OR]: 2.2; 95% confidence interval [CI]: 1.39-3.46; padjusted = 0.0147) and perfluorodecanoic acid showed positive association with GGT (OR: 2.73; 95% CI: 1.36-5.5; padjusted = 0.0256). Plasma mercury showed positive association with GGT (OR: 1.45; 95% CI: 1.14-1.84; padjusted = 0.0315). Using a plastic container while keeping food in the refrigerator was associated with elevated GGT compared to using a glass container (OR: 1.51; 95% CI: 1.16-1.95; padjusted = 0.0153). 2-ethyl-5-oxohexyl phthalate, showed a negative trend with all 3 indices, with AST (OR: 0.54; 95% CI: 0.39-0.73; padjusted = 0.00357), ALT (OR: 0.5; 95% CI: 0.34-0.75; padjusted = 0.036), GGT (OR: 0.55; 95% CI: 0.4-0.76; padjusted = 0.00697). Bisphenol S and frequent use of sunblock cream showed negative association with ALT (OR: 0.77; 95% CI: 0.66-0.89), and GGT (OR: 0.25; 95% CI: 0.11-0.55), respectively. Conclusions: We conducted an exploratory study on environmental exposure and human liver function. By using EWAS methodology, we identified 7 factors that could have potential association with liver function.
