Study on Value Conceptions in fundamentals of Nursing (임상간호 가치관에 관한 일 조사연구)

  • This study was designed to investigate differences in some aspects of value conceptions in fundamentals of nursing functions between nursing service personnel and nurse-educators. The purpose of the study was to identify the difference of value conceptions between the hospital nurses and school instructors, to contribute as a reference in strengthening the educational program in establishing nay hinder professional growth. The questionaries of this study were focused in the following aspects: 1) establishment of nursing service management system, 2) strengthening of the professionalization of nursing service, 3) hindrance in accomplishment of independent nursing functions, 4) communication skills in nurse-patient relationship, 5) activities directly related to nursing care planning, 6) communication skills in nurse-doctor relationship, 7) attitude towards taking responsibilities for patient- centered nursing approach, and 8) nursing educational approach towards preparation of professional competence in practicing independent nursing functions. 265 graduate nurses from 14 general hospitals and 88 nursing instructors from 18 schools, collegiate and diploma, were sampled. The main findings of the study were as follows; 1. Both groups responded highly in the lack of public recognition of nursing profession and hospital administrators support in establishment of nursing service management system. Further investigation is needed to define some relationship between the intrinsic and extrinsic factors which might give influence to the professional development. 2. while hospital nursing personnel responded on environmental pressure such as a heavy nursing load as the factor giving hindrance to independent nursing functions, the nurse educators responded highly on lack of nursing competence of individual nurses. An emphasis should be placed on the development of nursing interaction through professional education. If the professional model is not well established, nursing function will be limited to medical assistance. 3. 3. The patient-centered approach for nurse-patient communication had given positive respond for both group, tut lacks in team concept in problem solving process. There exist a social distance between nurses and other professional co-workers in the hospital hierarchy. 4. It was indicated that, as an intrinsic factor in the development of nursing service and nursing education, building up a philosophical basis is an utmost importance. This question is opened for further and extensive study to clarify whether existence of philosophical absence or philosophical stasis in nursing profession hinders the development.

Analysis of Factors Related to Mortality in Adult ICU Patients: Focusing on Nurse Staffing Level (성인중환자실 이용 환자의 사망률 관련 요인 분석: 간호등급을 중심으로)

  • Purpose: The purpose of this study was to assess the influence of nurses staffing level on patient health outcomes in intensive care units (ICUs) in Korea. Methods: The study was retrospective in nature. Information on patients and their outcomes, as well as nurse cohort data, were obtained from Korea's National Health Insurance Service Database. The observation period was from January 1, 2008 to December 31, 2018, and data for 2,964,991 patients were analyzed. Independent variables included patient' age and sex and hospital type, intensivist, and nurses staffing level. Results: The mortality rate in ICUs was significantly higher at tertiary hospitals with a level 3~4 (HR, 1.21; 95% confidence interval, 1.19~1.22) or level 5~9 nurse staffing (HR, 1.31; 95% confidence interval, 1.27~1.34) compare to that of tertiary hospitals with a 1~2 level. 28-day mortality rate was also higher at general hospitals with a level 3~4 (HR, 1.13; 95% confidence interval, 1.12~1.14), level 5~6 (HR. 1.34; 95% confidence interval, 1.32~1.36), level 7~9 nurse staffing (HR, 1.38; 95% confidence interval, 1.38~1.42), using level 1~2 as reference. Conclusion: Nurses staffing level is a key determinant of healthcare-associated mortality in critically ICUs patients. Policies to achieve adequate nurse staffing levels are therefore required to enhance patient outcomes.

The Effect of Emotional Labor and Resilience on Organizational Commitment of General Hospital Nurses (종합병원 간호사의 감정노동과 회복 탄력성이 조직몰입에 미치는 영향)

  • The purpose of this study was to investigate emotional labor, resilience and organizational commitment and to identify affecting factors of organizational commitment on hospital nurses. The subjects consisted of 180 general hospital nurse in the G city. The variables were measured using questionnaires, analyzed using t-test, ANOVA, Pearson's Correlation Coefficient and multiple regression analysis. The average score of emotional labor was 3.21±0.06, resilience was 3.32±0.48, and organizational commitment was 3.16±0.46. The factors influencing on organizational commitment were the satisfaction of the nursing job, total work experience, and resilience. The explanation of organizational commitment was 39%. These results suggest that it is necessary to develop promotion program and stratiges to improve nursing job satisfaction and resilience at the nursing organization and hospital level.

Research Trend Analysis on Job Stress of Hospital Nurses (병원 간호사의 직무스트레스에 관한 연구 동향 분석)

  • 1) Purpose : The purpose of this research is to identify the trend of job stress of hospital nurses based on last 10 years of domestic research and to suggest directions for systematic future research. 2) Methodology : 101 studies between 2006 and 2016 were selected using key words such as registered nurse and job stress from Academic Information Service(Riss4u), the National Assembly Library and Korean studies Information Service System(KISS). 3) Findings : The researches on job stress of hospital nurses were increasing. The result showed that job stress increased for shift nurses. Especially, nurses of small-medium sized hospital showed remarkable effects to organization such as turnover intention. In addition, although the importance of approach to management is emphasized, there is little research on these aspects. 4) Practical Implications : It is necessary to develop systematic research on job stress directly effecting turnover intention of hospital nurses from hospital management aspect. The development of job stress measuring tools reflecting the characteristics of the medical institution, and the human resources management plan to resolve the imbalance in manpower for nurses should be required.

Impact of Medical Service Quality by Attribute on Overall Satisfaction -Focused on Out-patient and In-patient in High-level general Hospital- (의료서비스 속성에 따른 품질이 전체만족도에 미치는 영향 -일개 상급종합병원의 외래 및 입원환자를 중심으로-)

  • This study was aimed to identify the current status of medical service quality of medical institutes; propose improvements; and find out the impact of medical service quality by its attribute on overall customer satisfaction. The research was conducted on the out-patients and in-patients in a high-level general hospital located in A city. To examine the research, the IPA was utilized to identify the priority requirements for improving the medical service quality. The study also applied an extended theory to the analysis on mismatch between the level of satisfaction and importance recognized by the clients, to examine the impact of the above mentioned factors on the overall satisfaction and intention to revisit and recommend the hospital. For out-patients, the result showed that "hospital hygiene" had a positive and negative impact on the clients' intention to revisit the hospital in priority improvements. In terms of the sustainable improvements, "doctor's skill" had a negative impact on the overall service satisfaction, whereas "state-of-the-art facility" and "nurse's instruction" had a positive impact on the intention to revisit the hospital. In long-term improvements,"complaint remedy" had a positive impact on the intention to recommend the hospital but there were no relevant factors in excessive investment. On the other hand, for in-patients, the result demonstrated that there were no relevant factors in priority improvements and sustainable improvements. The factor of"service procedure speed"had a positive as well as negative impact on the intention to revisit the hospital. In excessive investment,"nurse's empathy"had a positive impact on the intention to recommend the hospital.

The Relationship among Nurse-Doctor Collaboration, Job Autonomy and Organizational Commitment (간호사-의사 협력, 직무자율성과 조직몰입의 관계)

  • Purpose: This study aimed to investigate and analyze the state of the relationship among nurse-doctor collaboration, job autonomy and organizational commitment. Method: The 304 participants were obtained who were working at a General ward, Intensive care unit and Operation room in three university hospitals located in Seoul and Kyunggi-do. The data were collected using a structured questionnaire from March 2d to April 10th, 2009. The collected data were analyzed with t-test, ANOVA, Scheff$\acute{e}$ test and Pearson's correlation on SPSS Win 16.0. Result: There was a significant relationship among nurse-doctor collaboration, job autonomy and organizational commitment. The level of appointment, clinical experience and current hospital experience of nurses affected significantly nurse-doctor collaboration, job autonomy and organizational commitment. The age of nurse had the relation nurse-doctor collaboration and organizational commitment. The relationship between the nurse's working area and job autonomy had positive correlation. Conclusions: The findings of study suggest that the program enhancing the collaborated relationship between nurses and doctors is important to improve nurse's job autonomy and organizational commitment under the situation of citizen's demanding more advanced medical service.

A Study on the Subjective Musculoskeletal Symptoms Associated with Tasks of Operating Room Nurses (수술실 간호사의 간호업무에 따른 근골격계 자각증상에 관한 연구)

  • Purpose: To find the factors of subjective musculoskeletal symptoms associated with the general characteristics and tasks of operating room nurses (OR nurses) and then to use the factors as basic information for preventing and managing musculoskeletal symptoms in OR nurses. Method: This study was an exploratory research. Data were collected using a self-report questionnaire from OR nurses working at 8 polyclinics in Korea during the period from July 26 2004 to August 19 2004. The questionnaire contains 7 questions on general characteristics and 18 questions on tasks in the operating room. Result: 187 (75.1%) nurses said they 'had' subjective musculoskeletal symptoms. Statistically significant differences were observed according to clinical career, career as an OR nurse, skillfulness, satisfaction with tasks in the operating room, perception on suitable treatment, care of symptoms by the hospital. weight of hospital linen and weight of a basic operation set (p<.05). Conclusion: Musculoskeletal symptoms complained of by OR nurses were significant. This may cause difficulties in nursing tasks in the operating room. Thus, various arrangements should be made for OR nurse with subjective musculoskeletal symptoms at the early stage.

Nurses' Perception of Organizational Commitment, Nursing Work Environment, and Social Support in a General Hospital (일개 종합병원 간호사의 조직몰입, 간호업무환경, 사회적 지지)

  • Purpose: This study was done to identify the influence of nursing work environment and social support on multidimensional organizational commitment among nurses. Methods: The survey was conducted in August 2012 with self-report questionnaire. Participants were 480 registered nurses working in one general hospital. Data were analyzed with descriptive statistics, t-test, ANOVA, Pearson correlation, and multiple regression using SPSS/WIN(16.0). Results: The average scores were; for affective commitment 2.84, for continuance commitment 2.48, and for normative commitment 2.57. There were significant relationships between affective, continuance, normative commitment and sub-factors of nursing work environment and social support. The affective, continuance, and normative commitment were all influenced by 'staffing and resource adequacy' and 'nursing foundations for quality of care', sub-factors of nursing work environment. Distinctively, affective commitment was influenced by 'staffing and resource adequacy', nurses' age, 'nurse-doctor relations', 'supervisor's emotional support', 'nurse participation in hospital affairs', 'nursing foundations for quality of care', and 'supervisor's informational support', which explained 30.9% of variance in affective commitment(F=31.57, p<.001). Conclusion: The findings show that programs which promote supervisors' emotional and informational support are important to enhance nurses' affective commitment. Also, it is necessary to improve nursing work environment to improve nurses' organizational commitment.

A Study of Job Satisfaction of the Nurses in a University Hospital (일대학 부속병원 간호사의 직무만족도에 관한 연구)

  • Purpose: This study was conducted to describe and analyze about the degree of job satisfaction of nurses in a university hospital for basic data to develop the professionals and improve productivities, effectiveness in nursing practice. Methods: Design was the descriptive survey study. Sample were 97 nurses in K university hospital, and convenient sampling was used. Measures were job satisfaction of nurse for job satisfaction tool modified by Kim, S.N.(1998) and developed by Slavitt(1978). Data were collected from April 2005 to June 2005, and analyzed using SPSS PC+12 program. Descriptive statistics was used for general characteristics and characteristics related to job. F-test, t-test, and $X^2$ test were used for verification of differences. Results: 1. Scores in categories of job satisfaction were ranked for professional position(3.25), interpersonal relationship(3.19), autonomy(3.08), necessary job(2.88), payment and promotion(2.79), and administration(2.50) in order. 2. There were significant differences in age(F=3.343, p=.040) and satisfaction for current job(F=10.331, p=.000) in differences of job satisfaction by general characteristics and characteristics related to job. Conclusions: On the findings, authors suggest that more opportunities for direct nursing practice, appropriate time for the practice, and the intervention improving satisfaction for current job are needed to improve job satisfaction.

SATISFACTION OF PATIENTS WITH DISABILITIES ABOUT OUTPATIENT GENERAL ANESTHESIA AND NURSING CARE FOR DENTAL TREATMENT (외래전신마취 하 치과치료를 시행한 성인 치과장애인의 치과마취과 간호사에 대한 만족도 조사)

  • The purpose of this study is to survey the status of oral care of adult patients with disabilities and satisfaction degree of outpatient general anesthesia for dental treatment. 30 adult patients who underwent general anesthesia for dental treatment at clinic for the disabled in Seoul National University Dental Hospital enrolled for telephone survey. Many patients (83.3%) had mental problems to do social communication. And someone of family members (mainly mother) had cared for the patient but there was only one patient who got regular oral care. The satisfaction level of outpatient general anesthesia and nursing care for dental treatment was very high(100%).
