ICRP 호흡기 및 생체역동학적 모델을 이용한 우라늄 생물분석 결과의 해석 (Interpretation of Uranium Bioassay Results with the ICRP Respiratory Track and Biokinetic Model)

  • 본 연구는 호흡을 통해 우라늄을 만성 또는 급성섭취한 경우 생물분석 결과의 해석을 통해 예탁유효선량을 평가하는 실질적인 방법을 기술하고 있다. 인체에서의 우라늄 거동의 해석을 위해 인체의 장기를 ICRP에서 권고하는 소화기 모델, 호흡기 모델 그리고 생체역동학적 모델에 따라 일련의 수학적 격실로 구성하였다. Birchall의 알고리듬을 이용하여 각 격실에서의 균형방정식의 해석적인 해를 얻었으며 우라늄의 소변 배설함수와 폐 잔류함수를 획득하였다. 소변 중 우라늄 농도와 폐 계수기로 측정된 폐 부하량에 각각 배설 및 잔류함수를 적용하여 섭취모드에 따른 초기 섭취량 또는 총 섭취량을 계산하였다. 예탁유효선량은 ICRP 78에서 제공하는 선량 환산계수를 계산된 섭취량에 적용함으로써 평가된다.

  • 무선 캡슐 내시경 (WCE)은 세기 초에 개발 될 수 있는 가장 영향력 있는 바이오 의료 전자 기술 중 하나였다. 기존의 내시경 진단과 비교하여,이 응용 프로그램은 이에 훨씬 더 사용자 친화적인 방식으로 전체 위장관 (GI) 트랙을 검사하기 위한 새로운 대안을 가진 외과 의사를 제공하는 비 침습 및 저 위험이 특징이다. 그 이외에도 일반 하드웨어 업그레이드에서 WCE 연구의 국경은 기본적으로 캡슐의 소형화 및 활성화 운동에 있다. 운동은 일반적으로 자연적인 연동 운동의 함수이다 현재 상용화 WCE 제품의 본질적인 결점을 극복하기 위해, 활성 운동력 효과적으로 다른 장기에 장치를 탐색 대상으로 인간 내 치료 기능을 수행하기 위해 사용되는 전략의 시리즈로서 제안 조직과 연구의 측면에 대한 몇 가지 새로운 디자인의 리뷰는 이 문서에서 논의 될 것이다.

Prebiotics in the Infant Microbiome: The Past, Present, and Future

  • The latest definition of a prebiotic is "a substrate that is selectively utilized by host microorganisms conferring a health benefit"; it now includes non-food elements and is applicable to extra-intestinal tissues. Prebiotics are recognized as a promising tool in the promotion of general health and in the prevention and treatment of numerous juvenile diseases. Prebiotics are considered an immunoactive agent, with the potential for long-lasting effects extending past active administration of the prebiotic. Because of its extremely low risk of serious adverse effects, ease of administration, and strong potential for influencing the composition and function of the microbiota in the gut and beyond, the beneficial clinical applications of prebiotics are expanding. Prebiotics are the third largest component of human breast milk. Preparations including galactooligosaccharides (GOS), fructooligosaccharides (FOS), 2'-fucosyllactose, lacto-N-neo-tetraose are examples of commonly used and studied products for supplementation in baby formula. In particular, the GOS/FOS combination is the most studied. Maintaining a healthy microbiome is essential to promote homeostasis of the gut and other organs. With more than 1,000 different microbial species in the gut, it is likely more feasible to modify the gut microbiota through the use of certain prebiotic mixtures rather than supplementing with a particular probiotic strain. In this review, we discuss the latest clinical evidence regarding prebiotics and its role in gut immunity, allergy, infections, inflammation, and functional gastrointestinal disorders.

Induction of Cytokines and Nitric Oxide in Murine Macrophages Stimulated with Enzymatically Digested Lactobacillus Strains

  • Based on observations that lactic acid bacteria have the ability to activate macrophages, we assessed the potential effects of eight different Lactobacillus strains treated with gastrointestinal enzymes on the production of nitric oxide and various cytokines in macrophages. RAW 264.7 murine macrophage cells were cultured with either precipitates or supernatants of Lactobacillus strains digested with pepsin followed by pancreatin. The increased production of nitric oxide and interleukin $(IL)-1{\beta}$, IL-6, IL-12 and tumour necrosis factor $(TNF)-{\alpha}$ were observed when cultured with precipitates, and this effect was largely strain-dependent. In contrast, the exposure of RAW 264.7 cells to supernatants produced weaker or nearly undetectable effects in comparison to the effects of exposure to precipitates. The induction of nitric oxide appeared to be unaffected. These results demonstrate that nitric oxide and cytokines were effectively induced when the bacterial precipitate was treated with macrophages. The results of the present study also indicate that Lactobacillus strains treated with digestive enzymes are capable of stimulating the production of nitric oxide and cytokines in macrophages, which may modulate the gastrointestinal immune function of the host when it is given as a feed additive.

산약이 흰쥐의 급성 위궤양에 의한 위장 장애에 미치는 효과 (Effect of Dioscorea Rhizoma on Gastritis by Acute Gastric Ulcer in Rats)

  • 본 연구는 산약(Dioscorea Rhizoma)이 급성 위궤양을 유발한 흰쥐의 위장 장애에 미치는 효과를 조사하였다. 산약 분말사료를 섭이한 실험군과 산약 분말액을 투여한 실험군의 궤양성 출혈은 1 일 이상 진행되었으나 시간이 경과함에 따라 서서히 호전되었으며, 특히 산약즙을 처리한 실험군의 경우 급성 위궤양 유발직후에 국소적인 위점막 울혈만이 관찰되었다. 또한 산약분말액과 산약즙을 투여한 실험군에서는 급성 위괘양 유발 후 증가되는 gastrin과 histamine의 농도가 감소되어 산약이 위산의 과다 분비를 억제하는 것으로 조사되었으며, 산약을 투여한 모든 실험군에서 과도한 지질과산화물의 생성이 감소되었다. 이와 더불어 산약의 투여는 급성 위궤양에 의해 급격하게 증가된 SOD와 GPX의 활성을 감소시키는 반면, CAT의 활성은 높여주어 조직 내 항산화효소 활성을 정상 수준으로 회복시켜주는 것으로 나타났다. 따라서 산약은 급성 위궤양에 의한 위 조직의 손상을 감소시키고 혈장 성분과 항산화효소의 활성을 정상수준으로 회복시켜 위궤양으로부터 위장기능을 개선하는데 높은 효과가 있을 것으로 사료된다.

소화기질환 진단의 최신지견 (Recent Advances in Diagnosis of Gastrointestinal Disease)

  • Objectives : If patients notice a symptom indicating inveterate dyspepsia but they don't have any problem around gastroscope, they get diagnosed as a functional dyspepsia or an imaginary stomach disease, but to overcome the limitations of these diagnoses, we are analyzing them for the common feature and are looking for a new diagnostics for them. Methods : Based on our survey with 122 patients with inveterate dyspepsia, we analyzed the period of onset, eating habits, the main symptoms, and observations on the gastroscope. We also analyzed the function of the stomach and intestines by EAV examinations, and the outer walls of the stomach and intestines by subdividing the level of coagulation into six with abdominal palpation. Results : We figured out that people who appeals about inveterate dyspepsia have had long period of onset, and that they had bad eating habits, shoulder stiffness, neck stiffness, headache, dizziness, etc. These are all the similar symptoms beside dyspepsia, which indicates that it is a syndrome. From about 70%, they didn't had particular problem in gastroscope, and as to be seen from the result of EAV examination, their stomachs and intestines had become functionless. Also, we were able to feel a stiffened tissue through abdominal palpation. Conclusion : Through this investigation, we found out that what the gastroscope can not find so that gets diagnosed as a functional dyspepsia or an imaginary stomach disease can be diagnosed as a syndrome called damjeok by overcoming the limitation utilizing the survey, EAV examinations, and abdominal palpation. We can find a mighty significance from the fact that it can be diagnosed as a syndrome.

Short-Term Changes in Gut Microflora and Intestinal Epithelium in X-Ray Exposed Mice

  • Background: Gut microflora contributes to the nutritional metabolism of the host and to strengthen its immune system. However, if the intestinal barrier function of the living body is destroyed by radiation exposure, the intestinal bacteria harm the health of the host and cause sepsis. Therefore, this study aims to trace short-term radiation-induced changes in the mouse gut microflora-dominant bacterial genus, and analyze the degree of intestinal epithelial damage. Materials and Methods: Mice were irradiated with 0, 2, 4, 8 Gy X-rays, and the gut microflora and intestinal epithelial changes were analyzed 72 hours later. Five representative genera of Actinobacteria, Firmicutes, and Bacteroidetes were analyzed in fecal samples, and the intestine was pathologically analyzed by Hematoxylin-Eosin and Alcian blue staining. In addition, DNA fragmentation was evaluated by the TdT-mediated dUTP nick-end labeling (TUNEL) assay. Results and Discussion: The small intestine showed shortened villi and reduced number of goblet cells upon 8 Gy irradiation. The large intestine epithelium showed no significant morphological changes, but the number of goblet cells were reduced in a radiation dose-dependent manner. Moreover, the small intestinal epithelium of 8 Gy-irradiated mice showed significant DNA damaged, whereas the large intestine epithelium was damaged in a dose-dependent manner. Overall, the large intestine epithelium showed less recovery potential upon radiation exposure than the small intestinal epithelium. Analysis of the intestinal flora revealed fluctuations in lactic acid bacteria excretion after irradiation regardless of the morphological changes of intestinal epithelium. Altogether, it became clear that radiation exposure could cause an immediate change of their excretion. Conclusion: This study revealed changes in the intestinal epithelium and intestinal microbiota that may pave the way for the identification of novel biomarkers of radiation-induced gastrointestinal disorders and develop new therapeutic strategies to treat patients with acute radiation syndrome.

Comparing Pre- and Post-Operative Findings in Patients Who Underwent Laparoscopic Proximal Gastrectomy With a Double-Flap Technique: A Study on High-Resolution Manometry, Impedance pH Monitoring, and Esophagogastroduodenoscopy Findings

  • Purpose: Laparoscopic proximal gastrectomy (LPG) is a viable choice for treating proximal gastric lesions. However, the occurrence of severe reflux has limited its widespread adoption. To address this issue, the double flap technique (DFT), which incorporates artificial lower esophageal sphincteroplasty, has been developed to prevent reflux problems after proximal gastrectomy. In this study, we aimed to investigate the usefulness of this technique using high-resolution manometry (HRM), impedance pH monitoring, and esophagogastroduodenoscopy (EGD). Materials and Methods: The findings of pre- and postoperative 6-month HRM, pH monitoring, and EGD were compared for 9 patients who underwent LPG with DFT for various proximal gastric lesions at Incheon St. Mary's Hospital from January 2021 to December. Results: A total of 9 patients underwent proximal gastrectomy. Approximately half of the patients had Hill's grade under II preoperatively, whereas all patients had Hill's grades I and II in EGD findings. In the HRM test, there was no significant difference between distal contractile integral (1,412.46±1,168.51 vs. 852.66±495.62 mmHg·cm·s, P=0.087) and integrated relaxation pressure (12.54±8.97 vs. 8.33±11.30 mmHg, P=0.27). The average lower esophageal sphincter (LES) pressure was 29.19±14.51 mmHg preoperatively, which did not differ from 19.97±18.03 mmHg after the surgery (P=0.17). DeMeester score (7.02±6.36 vs. 21.92±36.17, P=0.21) and total acid exposure time (1.49±1.48 vs. 5.61±10.17, P=0.24) were slightly higher, but the differences were not statistically significant. Conclusions: There is no significant functional difference in HRM and impedance pH monitoring tests after DFT. DFT appears to be useful in preserving LES function following proximal gastrectomy.

Increasing correlation between oral and gastric microbiota during gastric carcinogenesis

  • Background/Aims: Recent research has increasingly focused on the role of the gastric microbiome in the development of gastric cancer. We aimed to investigate the changes in the microbiome during gastric carcinogenesis in structural and functional aspects, with a specific focus on the association between oral and gastric microbiomes. Methods: We collected saliva, gastric juice, and gastric tissue samples from 141 patients at different stages of gastric carcinogenesis and processed them for microbiome analysis using 16S rRNA gene profiling. The alpha and beta diversities were analyzed, and the differences in microbiome composition and function profiles were analyzed among the groups, as well as the correlation between changes in the oral and gastric microbiomes during carcinogenesis. Results: We observed significant differences in microbial diversity and composition between the disease and control groups, primarily in the gastric juice. Specific bacterial strains, including Schaalia odontolytica, Streptococcus cristatus, and Peptostreptococcus stomatis, showed a significant increase in abundance in the gastric juice in the low-grade dysplasia and gastric cancer groups. Notably, the correlation between the oral and gastric microbiota compositions, increased as the disease progressed. Predictive analysis of the metagenomic functional profiles revealed changes in functional pathways that may be associated with carcinogenesis (ABC transport and two-component systems). Conclusions: During gastric carcinogenesis, the abundance of oral commensals associated with cancer increased in the stomach. The similarity in microbial composition between the stomach and oral cavity also increased, implying a potential role of oral-gastric bacterial interactions in gastric cancer development.

  • Bifidobacteria comprises up to 25% of the cultivable fecal bacteria in adults and 80% in infants. Many in vivo and clinical research results supporting its efficacy in the prevention and improvement of gastrointestinal health have been accumulated. As a consequence, expert committee WHO/FAO expert committee recommended Bifidobacterium as representative probiotics together with Lactobacillus acidophilus. In this review, research trends in bifidobacteria concerning the classification and identification of the genus Bifidobacterium, modulation of intestinal microflora, improvement of constipation, prevention of diarrhea, alleviation of atopy and allergy, barrier function through antimicrobial activity andimmune enhancement of the host will be introduced. Several gene expression systems based on bifidobacterial plasmids have been developed and successfully used to express several heterologous genes including anticancer proteins in Bifidogacterium. In animal test, bifidobacteria was proven to be a promising candidate for safe gene delivery system which can specifically colonize in the solid tumor.
