Chloramphenicol의 경구투여에 따른 양식 넙치, Paralichthys olivaceus의조직내 잔류량 및 혈액학적 변화 (Tissue level of chloramphenicol and haematological changes in olive flounder, Paralichthys olivaceus orally administered chloramphenicol)

  • This study was performed to investigate the pathphysiological side effects of chloramphenicol (CAP) on olive flounder, Paralichthys olivaceus. 150 mg/kg/bw/day of CAP was orally administered forcedly to flounder for 4 days and then the flounder was monitored for 11 days. The level of hematocrit was significantly decreased from 2 day to 3 day of CAP treatment in CAP-treated group. The levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP) and gamma glutamyl transferase (GGT) were slightly increased during the treatment of CAP and gradually decreased after the treatment of CAP. The levels of blood urea nitrogen (BUN) and creatinine kinase (CK), although not significant, were slightly increased during the treatment of CAP. The level of lactate dehydrogenase (LDH) was not changed during the treatment of CAP but slightly increased after the treatment of CAP. However, no histopathological changes were observed in various organ after the treatment. From these results, it is suggest that CAP can evoke several side effects on haematopoietic organs in olive flounder.

사상방과 양약의 병용투여가 간장, 신장에 미치는 영향 (The Effects of Use of Sasang-Bang with Western Medicine on Liver and Kidney Function)

  • 1. Objectives The Purpose of this study was to evaluate whether use of Sasang -Bang with western medication may injury the liver and the kidney function. We clinically studied the change Liver function test and BUN/Cr in patients who were hospitalized in Hana oriental Medical center for more than 1 month. 2. Methods The subjects were 33 patients admitted in Hana Oriental Medical center. All patients had been checked Liver Function Test and BUN/Cr three times every 2 weeks. 3. Results Aspartate Aminotransferase(AST), Alanine Aminotransferase(ALT), Gamma-Glutamyl transferase(GGT), Total billirubin, BUN, Creatine didn't change, compared with previous 2 weeks and 4 weeks. 4. Conclusions This study suggests that even though there are few toxic Sasang-Bang, in general, herbal medications with western medications which are prescribed by oriental medicine doctors should be carefully administered not to hurt liver and kidney function.

산겨릅나무 추출물을 이용한 비알콜성 지방간 개선 효과 (Effects of Non-Alcoholic Fatty Liver in Rats by Acer tagmentosum Maxim. Extract)

  • This study was performed to investigate the effects of Acer tegmentosum Maxim. extract (ATE) on non-alcoholic fatty liver in Sprague Dawley (SD) rats. During oral administration of ATE, non-alcoholic fatty liver was induced by treatment with DL-ethionine. The lipid, total cholesterol (T-CHO) and malondialdehyde (MDA) in the liver tissue of ATE-fed rats showed lower levels, as compared to ATE-unfed rats. In ATE-fed rats, the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and ${\gamma}$-glutamyl transferase (GGT) were lower than the case of ATE-unfed rats. Oil red staining of the liver showed that the lipid deposits were decreased by feeding ATE. These results strongly indicated that ATE has positive effects of protection against non-alcoholic fatty liver formation.

원발성 답증성 간경변에 대한 장기간 한방 치험례 (Long-term Clinical Case of Primary Biliary Cirrhosis Treated with Traditional Korean Medicine)

  • Objective: This study was designed to assess the clinical effect of traditional Korean medicine on primary biliary cirrhosis.Method: We provided herbal medicine and acupuncture therapy to a 38-year-old male patient who complained about jaundice, itching, and fatigue. During a six-day hospital stay, we checked any changes in symptoms and conducted liver function tests (such as aspartate aminotransferase [AST], alanine aminotransferase [ALT], alkaline phosphatase [ALP], and gamma-glutamyl transferase [GGT], among others) to evaluate any improvement.Results: We observed that herbal medicine and acupuncture therapy appeared to decrease the symptoms of primary biliary cirrhosis. During each hospital stay, the symptoms and liver function test both showed improvement, especially in ALP.Conclusion: Based on the therapeutic results of the six-day hospital stay, we report improved primary biliary cirrhosis symptoms and a decreasing tendency in ALP and GGP levels following treatment with traditional Korean medicine.

Comparison of Clinical Biochemicals in Sera of Senior Patients with Hyperglycemia

  • We analyzed the concentration of Aspartate Aminotransferase (AST), Alanine Aminotransferase (ALT), Alkaline Phosphatase (ALP), Gamma Glutamyl Transferase (GGT), total protein (TP), albumin, total cholesterol (TC) and triglyceride in sera of patients (n=455) aged 60 years and older with hyperglycemia (${\geq}120mg/dL$ in serum). The average concentration of biochemicals in patient group was compared with those in reference group. Our data showed that the average concentrations of AST and TC in sera of senior patients with hyperglycemia were significantly increased by 27.5% (P<0.05) and 82% (P<0.05) in total group, 38.5% (P<0.05) and 75.0% (P<0.05) in sixty years' group, 16.0% (P<0.05) and 89.7% (P<0.05) in seventy years' group and 27.0% (P<0.05) and 79.5% (P<0.05) in eighty years' group, respectively. Patients with hyperglycemia showed a significant decrease in albumin level by 6.7% (total group, P<0.05), 4.5% (sixty years' group, P<0.05), 8.9% (seventy years' group, P<0.05) and 4.5% (eighty years' group, P<0.05), respectively. In conclusion, the high concentration of glucose in the senior patients with hyperglycemia may be a cause of severe liver function and lipid metabolism disorder.

Novel ATP8B1 Gene Mutations in a Child with Progressive Familial Intrahepatic Cholestasis Type 1

  • Progressive familial intrahepatic cholestasis (PFIC) is a group of severe genetic disorders, inherited in an autosomal recessive manner, causing cholestasis of hepatocellular origin, later progressing to biliary cirrhosis and liver failure. This is the first report of PFIC type 1 with novel compound heterozygous mutations in Korea. The patient was presented with intrahepatic cholestasis, a normal level of serum ${\gamma}-glutamyl$ transferase, steatorrhea, and growth failure. Genetic testing of this patient revealed novel compound heterozygous mutations (p.Glu585Ter and p.Leu749Pro) in the ATP8B1 gene. After a liver transplantation at age 19 months, the patient developed severe post-transplant steatohepatitis.

약인성 간손상 환자에 대한 생간탕(生肝湯) 치험 1례 (A Case Report of Efficacy of Saenggan-tang on a Patient with Drug Induced Liver Injury)

  • Objectives: The purpose of this case study is to report the effect of Korean Medicine treatment on a patient diagnosed with drug-induced liver injury by oral administration of acetaminophen. Methods: A 21-year-old male patient with drug-induced liver injury visited the clinic five times from January 28th, 2019 to March 16th, 2019. The patient took Saenggan-tang extract during the treatment period. Results: The levels of serum alanine aminotransferase (ALT) and gamma-glutamyl transferase (GGT) were significantly decreased. The chief complaints of the patient, such as yellow-red urine and fatigue, were also relieved. Conclusion: Traditional Korean medicine has a beneficial effect on drug-induced liver injury.

Biliary ascariasis misidentified as a biliary stent in a patient undergoing liver resection

  • Ascaris lumbricoides causes one of the most common soil-transmitted helminthiases globally. The worms mostly infect the human small intestine and elicit negligible or nonspecific symptoms, but there are reports of extraintestinal ectopic ascariasis. We describe a rare case of biliary ascariasis mistaken for biliary stent in a 72-year-old female patient with a history of liver resection. She visited our outpatient clinic complaining of right upper quadrant pain and fever for the past week. She had previously undergone left lateral sectionectomy for recurrent biliary and intrahepatic duct stones 2 years ago. Besides mildly elevated gamma-glutamyl transferase levels, her liver function tests were normal. Magnetic resonance cholangiopancreatography revealed a linear filling defect closely resembling an internal stent from the common bile duct to the right intrahepatic bile duct. A live female A. lumbricoides adult worm was removed by endoscopic retrograde cholangiopancreatography (ERCP). Despite a significant decrease of the ascariasis prevalence in Korea, cases of biliary ascariasis are still occasionally reported. In this study, a additional case of biliary ascariasis, which was radiologically misdiagnosed as the biliary stent, was described in a hepatic resection patient by the worm recovery with ERCP in Korea.

Association between SMAD2 Gene and Serum Liver Enzyme Levels in the Korean Population

  • Genome-wide association studies (GWAS) have identified a number of common variants associated with serum liver enzyme homeostasis in population. In the previous study, single nucleotide polymorphisms (SNPs) in several genes have been reported to be associated with serum liver enzyme levels in European population. We aimed to confirm whether the genetic variation of SMAD2 (SMAD family member 2) gene influence the serum liver enzyme levels in Korean population. We genotyped variants in or near SMAD2 in a population-based sample including 994 unrelated Korean adult. Here, we performed association analysis to elucidate the possible relations of genetic polymorphisms in SMAD2 gene with serum liver enzyme levels. By examining genotype data of a total of 944 subjects in 5 hospital health promotion center, we discovered the SMAD2 gene polymorphisms are associated with serum liver enzyme levels. The common and highest significant polymorphism was rs17736760 (${\beta}$=3.51, P=5.31E-07) with glutamic oxaloacetic transferase (GOT), rs17736760 (${\beta}$=5.99, P=1.25E-05) with glutamic pyruvate transaminase (GPT), and rs17736760 (${\beta}$=15.68, P=9.93E-07) with gamma glutamyl transferase (GGT) in all group. Furthermore, the SNP rs17736760 was consistently associated with GOT (${\beta}$=5.25, P=1.72E-06), GPT (${\beta}$=9.97, P=1.16E-05), GGT (${\beta}$=26.13, P=3.43E-06) in men group. Consequently, we found statistically significant SNP in SMAD2 gene that are associated with serum levels of GOT, GPT, and GGT. In addition, these results suggest that the individuals with the minor alleles of the SNP in the SMAD2 gene may be more elevated serum liver enzyme levels in the Korean population.

오미자 Schizandrin C 유도체 DDB 복합물 DWP-04가 Acetaminophen 해독계에 미치는 영향 (Evaluation of a Schzandrin C Derivative DDB-mixed Preparation(DWP-04) on Acetaminophen Detoxification Enzyme System in the Animal Model)

  • The effects of the DWP-04 [DDB:selenium yeast:glutathione (31.1 : 6.8 : 62.1 (w/w%)] on acetaminophen detoxification enzyme system were studied in rats. Treatment with DWP-04 was prevented againt acetaminophen-induiced hepatotoxicity in rat as evidenced by the decreased formation of lipid peroxide. Effect of DWP-04 on the activities of free radical-generating enzymes, free radical scavenging enzymes and glutathione-related enzymes as well as detoxification mechanism of DWP-04 against acetaminophen-treated was investigated in rat. Activities of cytochrome p450, cytochrome b5, aminopyrine demethylase and aniline hydroxylase as free radical-generating enzymes activities were decreased by the treatment with DWP-04 against acetaminophen treated. Although acetaminophen-induced hepatotoxicity results in the significantly decrease in the level of hepatic glutathione and activities of glutathine S-transferase, quinone reductase, glutathione reductase and ${\gamma}-glutamyl-$cysteine synthetase, these decreasing effects were markedly lowered in the DWP-04-treated rat. Therefore, it was concluded that the mechanism for the observed preventive effect of DWP-04 against the acetaminophen-induced hepatotoxicity was associated with the decreased activities in the free radical-generating enzyme system.
