황기약침이 전층피부손상 마우스의 치유에 미치는 영향 (Effect of Astragali Radix Pharmacocupuncture for Wound Healing of Full-Thickness Skin Wound in Mouse)

  • Objectives : In this study, we investigated whether Astragali Radix Pharmacocupuncture (ARP) has an effective on the full thickness defect wound healing process of mouse. Methods : A total of 50 mice (ICR mouse, 7 week-old male) were divided into control group and ARP group. A single full thickness skin defect was made on the dorsal side of the each mouse using an 8mm diameter biopsy punch. Control group were treated with 0.2㎖ saline and ARP group were treat with 0.2ml ARP at 8 points around the wound every three days total 4 times during the experimental period. The change in wound size, contraction rate, healing rate, and epithelization rate was measured by digital images taken on days 3, 6, 9, and 13, and evaluated using a digital image analysis program. Tissues were collected for histological analysis, RT-PCR, and Western blot on days 3, 6, 9, and 15. Results : The results are as follows. ARP group accelerated the rate of wound contraction, wound healing and epithelization compared to the control group. ARP group showed the decrease of inflammatory cells in early inflammatory phase compared to the control group. ARP group upregulated PECAM-1 mRNA and protein expression compared to the control group. ARP group inhibited the scar width and area compared to the control group. Conclusions: ARP showed positive effects on wound healing through the inhibition of inflammatory reaction and increase of PECAM-1 expression related to the wound healing process.

  • Purpose: Many topical agents had been used for contaminated wound treatment. Although antimicrobial ointments were widely used as topical agents, their comparative antibacterial and wound healing effects were largely unreported. The purpose of this study was to compare antibacterial effects and wound healing effects of common topical ointments on infected full thickness skin defect in the mouse. Methods: One full thickness skin defects in the mice (n=60) were developed on the back and left open for twenty-four hours. Sixty mice were divided into four groups: group B (dressing with Bactroban$^{(R)}$, n=15), group I (dressing with Iodosorb$^{(R)}$, n=15), group T (dressing with Terramycin$^{(R)}$, n=15), group G (control group, dressing with dry gauze, n=15). The size of wound defects and the grades of wound healing were evaluated at 4, 7, 10 days, and antibacterial effect was evaluated with restricted zone in Mueller Hinton agar by disk diffusion method. After the wound was left open for twenty-four hours, many Staphylococcus aureus was cultured. The wound defect size was decreased in order of Bactroban$^{(R)}$ (B), Iodosorb$^{(R)}$ (I), Terramycin$^{(R)}$ and gauze dressing group in all days, but difference among experimental groups was not statistically significant. The grade score of wound healing was increased in order of Bactroban$^{(R)}$, Iodosorb$^{(R)}$, Terramycin$^{(R)}$ and gauze dressing group, and the difference was statistically significant. Antibacterial effect for was increased in order of Bactroban$^{(R)}$, Iodosorb$^{(R)}$, Terramycin$^{(R)}$ and gauze dressing group, and the difference was statistically significant. Conclusion: Topical antimicrobial ointments application was effective against wound infections by S. aureus. Bactroban$^{(R)}$ may be an optimal topical treatment for infected wounds according to this study. However, further study is necessary to evaluate the clinical efficacy of antimicrobial ointments and to search for the mechanisms that explain their effects.

  • This study was carried out to investigate the effects of the wound contraction of chitosan, aloe vera, fucidin natrium, and premycin ointment on defected wound from full-thickness skin defects(1 cm $\times$ 1 cm). Twelve dogs were designed in 5 different positions on the dorsal thoracolumbar. We examined the effects of wound contraction every four other day 16 for days. Percentage of wound contraction based on epitherial overall area of defected wound which calculated by image analyzer and computer. Four to 8 days after wound contraction in wound defected dogs, the epithelial overall rate were the most high presented by 48.1 % in the chitosan group and were the lowest by 26.8% in the premycin group. In 16 days, chitosan group were the most heigh presented by 94.4% and premycin group were the lowest by 76.5% compare to saline group of 85.9%. Thus, we conclude that the chitosan is a possible role for improvement of wound contraction by wound defected dogs.

Closure of a full-thickness scalp burn that occurred during hair coloring using a simple skin-stretching method: A case report and review of the literature

  • Full-thickness scalp burns secondary to hair coloring are rare; however, such defects can be large and complex reconstruction of hair-bearing tissue may be necessary. Many skin-stretching devices that use gradual traction have been applied to take advantage of the viscoelastic properties of the skin. A 21-year-old female patient was seen with a burn defect on her occipital scalp leading to exposed subcutaneous tissue after chemical application of hair coloring in a salon. The dimensions of the wound were $10cm{\times}5cm$, and a skin graft or flap would have been necessary to close the defect. Two long transfixing K-wires (1.4 mm) and paired 3-wire threads (23 gauge), which are readily available in most hospitals, were applied over a period of 12 days for trichophytic closure of the defect. The remaining scalp scars after primary trichophytic closure with this skin-stretching method were refined with hair follicle transplantation. This skin-stretching method is simple to apply and valuable for helping to close problematic areas of skin shortage that would otherwise require more complicated procedures. This case shows a relatively unknown complication of hair coloring and its treatment.

  • Purpose: Iodine has been used for the prevention or management of wound infection as a topical agent. Although iodine was widely used mainly by Betadine$^{(R)}$ and cadexomer iodine, there was no comparative study on the efficacies of dressing methods of iodine. And also it's wound healing effect was not yet clear. The purpose of this study is to compare antibacterial effects and wound healing effects associated with various dressing methods of iodine on infected full thickness skin defect in the mouse. However, further study is necessary to evaluate the clinical efficacy of dressing methods of iodine and to search for the mechanisms that explain their effects.

전층피부창상에서 실크피브로인과 하이알론산 혼합 스폰지의 창상치유효과 (Silk fibroin/hyaluronic acid blend sponge accelerates the wound healing in full-thickness skin injury model of rat)

  • The primary goal of the wound healing is rapid wound closure. Recent advances in cellular and molecular biology have greatly expanded our understanding of the biologic processes involved in wound repair and tissue regeneration. This study was conducted to develop a new sponge type of biomaterial to be used for either wound dressing or scaffold for tissue engineering. We designed to make a comparative study of the wound healing effect of silk fibroin/hyaluronic acid (SF/HA) blend sponge in full-thickness dermal injury model of rat. Two full-thickness excisions were made on the back of the experimental animals. The excised wound was covered with either the silk fibroin (SF), hyaluronic acid (HA) or SF/HA (7 : 3 or 5 : 5 ratio) blend sponge. On the postoperative days of 3, 7, 10 and 14, the wound area was calculated by image analysis software. Simultaneously, the tissues were stained with Hematoxylin-Eosin and Masson's trichrome methods to measure the area of regenerated epithelium and collagen deposition. In addition, we evaluated the degree of the epithelial cell proliferation using immunohistochemistry for proliferating cell nuclear antigen (PCNA). We found that the half healing time ($HT_{50}$) of SF/HA blend sponge treated groups were significantly decreased as compared with either those of SF or HA treatment group. Furthermore, SF/HA blend sponges significantly increased the size of epithelialization and collagen deposition as well as the number of PCNA positive cells on epidermal basement membrane as compared with those of control treatment. Especially, the 5 : 5 ratio group of SF/HA among all treatment groups was most effective on wound healing rate and histological studies. These results suggest that SF/HA blend sponges could accelerate the wound healing process through the increase of epithelialization, collagen deposition and basal cell proliferation in full thickness skin injury.

Closure of radial forearm free flap donor-site defect with proportional local full-thickness skin graft: case series study of a new design

  • Objectives: The aim of this study was to describe a simple, convenient, and reliable new technique using local full-thickness skin graft (FTSG) for skin coverage of a donor-site defect of the radial forearm free flap (RFFF). Patients and Methods: Between April 2016 and April 2021, five patients with oral squamous cell carcinoma underwent mass resection combined with RFFF reconstruction. After RFFF harvesting, donor-site defects were restored by proportional local FTSG. Results: The donor-site defects ranged in size from 24 to 41.25 cm2, with a mean of 33.05 cm2. Good or acceptable esthetic outcomes were obtained in all five patients. There was no dehiscence, skin necrosis, wound infection, or severe scarring at the graft site through the end of the postoperative follow-up period, and no patient had any specific functional complaint. Conclusion: The proportional local FTSG showed promising results for skin coverage of the donor-site defect of the RFFF. This technique could decrease the need for skin grafts from other sites.

전층식피술에 적용한 폴리우레탄 반밀봉드레싱 (Polyurethane Semi-occlusive Dressing for Full Thickness Skin Graft Application)

  • A traditional tie-over dressing may be applied to support the take of a skin graft. Although there are many advantage of this method, it has significant disadvantages, including time-consuming application. Furthermore, when the dressing is changed, the gauze becomes hard and can be stuck to the graft, causing damage and pain upon removal. The purpose of our study is to evaluate the effect of semi-occlusive dressing using polyurethane foam and film dressing($Allevyn^{(R)}$, $Opsite^{(R)}$) after full thickness skin graft. The authors treated 45 cases including burn scar contracture(n=38), syndactyly (n=1), absence of nipple-areolar complex(n=4), traumatic skin defect(n=1) and contact burn(n=1) with authors' method and 39 patients including burn scar contracture (n=39) with the tie-over dressing between 2000 and 2004. The patients in polyurethane foam and film dressing group ranged from 1 to 62 years of age (mean age, 15.1 years) and the patients in tie-over dressing group ranged from 2 to 60 years of age(mean age, 21.3 years). The postoperative results were analyzed according to the following measures: (1) the duration of graft-taking, (2) the admission period, (3) complications. Compared with the traditional tie-over dressing, polyurethane foam and film dressing was shown to be more successful in a reduced duration of graft-taking, in which was similar to the former in the rate of graft-taking, a reduced admission period and patient's discomfort. We concluded that semi-occlusive dressing using $Allevyn^{(R)}$ and $Opsite^{(R)}$ was an effective method after full thickness skin graft, which was easy to shape to difficult body locations, such as web spaces, fingers and maintains a moist environment for wound healing and does not stick to the wound.

손상된 피부 재건을 위한 바이오인공피부의 개발 동향 (Development of Bioartificial Skin for Skin Regeneration)

  • There are many different approaches to healing of acute and chronic ulcer and large skin defect, such as burn. Currently available wound covers fall into two categories. Permanent covering, such as autografts, and temporary ones, such as allograft including de-epidermized cadaver skin, bioartificial skin, xenografts, and synthetic dressings. Autologous skin grafting in the form of split- or full-thickness skin is still the good standard. Following on from developments in the 1980s involving the use of cultured keratinocyte grafts in wound healing, the last decade has been great progress in the fabrication of composite bioartificial skin grafts. However, two bottleneck on producing cultured bioartificial skin, whether of the simple epithelial cell sheet type, or the more complex composite type, continue to be the generation of sufficient keratinocytes cheaply and quickly and develop biocompatible dermal scaffolds. This article covers the development, clinical application, and current research directions associated with bioartificial skin.

개에서 손바닥 선인장의 피부 결손창 치료 효과 (The Wound Healing Effects of Opuntia Ficus-indica in Dogs)

  • The aim of this study is to investigate the wound healing effects of the Opuntia ficus-indica. Four full-thickness skin wounds(2.5 cm × 2.5 cm) were made in different areas on the back in each of five dogs. Two wounds were used for wound size measurement and two for skin biopsy. The ten full-thickness wounds were treated with the extract of Opuntia ficus-indica, and the ten full-thickness wounds were treated with normal saline. The wound areas were measured every other day for three weeks after wounding. Statistical analysis was performed with the paired t-test. The wound healing rates were 32.9±4.0%, 28.2±7.1%, 42.2±4.1%, 55.0±4.3%, 76.6±2.8%, 86.8±2.3%, 94.0±1.l%, 97.3±0.8%, 99.3±0.3%, 99.8±0.2%, 100.0±0.0% in the experimental group, and 18.0± 8.2%, 13.S± 11.6%, 15.2±6.1 %, 36.9±8.8%, 62.4±4.1%, 80.0±2.3%, 86.0±1.4%, 94.0±0.9%, 96.9±0.8%, 97.8±0.3%, 99.7±0.1% in the control group on the 1/sup st/, 3/sup rd/, 5/sup th/, 7/sup th/, 9/sup th/, 11/sup th/, 13/sup th/, 15/sup th/, 17/sup th/, 19/sup th/, 21/sup st/ day. On the 17/sup th/ day, the wound closed above 99.9% in 1 dog of the experimental group. On the 19/sup th/ day, the wound closed above 99.9% in 3 dogs of the experimental group. On the 21/sup st/ day, the wound closed above 99.9% in 5 dogs of the experimental group, and 1 dog of the control group. The wound healing period was shortened by 2 to 4 days in the experimental group as compared with the control group. The tensile strength was 4,508±513 g/㎠ in the experimental group and 2,772±400 g/㎠ in the control group on the 21/sup st/ day after wounding. The tensile strength was significantly higher in the experimental group than in the control group(p< 0.05). Inflammation rapidly spread and disappeared in the experimental group but not in the control group. Fibroblast and vascularization were found on the 5/sup th/ day in both group and increased only in the experimental group on the 9/sup th/ day. The firs finding of epithelialization was on the 13/sup th/ day in the experimental group and the 21/sup st/ day in the control group. A significantly higher wound healing rate(p< 0.05), tensile strength(p< 0.05) and better histopathological findings were observed in the experimental group than in the control group throughout the experimental period. These results suggest that topical application of Opuntia ficus-indica extract can promote wound healing in dogs.
