인공영양에 대한 한국 어머니들의 반응에 관한 기초조사 (A Basic Treatise of Korean Mother′s Concern for the Artificial Feeding)

  • In this article, the writer attempted to study the followings: (1) mother's knowledge of milk feeding (including method of sterilization, formula and feeding) (2) reasons of artificial feeding (3) how much mothers are concerned about their artificial feeding infants (4) condition of growth and development of artificial feeding infants. As the object of study, 96 mothers with artificial feeding infants, who had consulted dep't of pediatrics of two Hospitals in Seoul (Korea Hospital and Med. College Woo Suk Hospital, Korea Uni.) and well baby clinic of the two Health Centers (Sung Dong and Dongdaemoon), were randomly sampled. The data were treated by the statistic method of chi-square and percentage, and come to the following conclusion. 1. Knowledge of milk feeding Sterilization: 70 percentages of mothers know about the milk sterilization (including bottle, nipple and instrument), but 55 percentages of them do not know the nipple sterilization correctly. Formula: 69 percentages of mothers follow the indicator or in accordance with doctor's directions, but 31 percentages do at their option by reasons that the baby often coughs up the milk, the baby is too small, the baby often has digestive troubles, or the baby grow fleshy heavily etc, except family economic problems. Feeding: only half of mothers know the correct feeding method, especially they do not know how to determine the heat degree of milk and how to bubble up the baby correctly. They just do feeding according to the accepted usages. 2. Reasons of artificial feeding Of the reasons of artificial feeding, 18 percentages were caused by infants and 82 percentages by mothers. Most of the reasons are mainly due to the lack of breast milk and sufficient supply of nourishments rather than mother's deficiency or mother's abnormality. 3. Mother's concern for artificial feeding infants Mothers who are sharply concerned for their artificial feeding baby's growth and development: 63%, mothers who made the baby (artificial feeding infant) routine vaccinated: 81%, mothers who ear anxious about the baby's future personality forming : 68%, mothers who care about the baby's condition of nourishment; 60%, mothers who are anxious about tile selection of baby's food; 54%. 4. The growth and development of artificial feeding infants compared with Korean average infants. The artificial feeding infants are above the Korean average infants in stature by 1.21 centimeters and in weight by 0.3 kilograms. Conclusion: It has been said that there is no better food for infant than the breast milk. However, the artificial feeding has been used for the supplement of nourishments and as substitute food for the breast milk. And this artificial feeding could give the married women the chance to act in society and more opportunity to develop themselves and to work for others at home and other fields. Considering these advantages, artificial feeding should not be exclusive, but preferably should be more improved and inquired positively. And even in artificial feeding, what is most important is that mothers should recognize the requirement and need of artificial feeding clearly and correctly, and they should be accustomed to the correct knowledge and skills of artificial feeding in order to practice it appropriately. In some degree, they should be properly trained in school education process.

한국 조제분유의 초분 관리 실태 및 현황 (The Current Quality Control and State of Scorched Particles in Infant formula in Korea)

  • 국내 조제분유에서 초분 함량의 법적 기준 설정에 관한 과학적 근거를 살펴보고자 하였다. 국내 조제분유는 국제규격 CODEX와 미국 USDA등에서 다루고 있는 초분 또는 이물질에 대한 규격을 엄격히 따르고 있다. 그러나 국내 법규에서는 초분에 대한 용어 설명 및 성분 규격이 마련되어 있지 않아 초분에 관한 영유아식품의 안전성에 대해 사회적 관심이 점차 증가하고 있는 실정이다. 국내 조제분유 유래의 초분 조성과 함량을 SEM-EDS 분석법과 ICP-MS 분석법을 통해 확인하였다. 그 결과 초분의 미네랄은 조제분유에서 사용하는 원료 유래의 C, O, Ca, Na, Fe, Mg을 비롯한 미량 원소로 구성되어짐을 확인하였다. 조제분유는 원유, 탈지유, 유청분말, 혼합식물성 유지, 미네랄류 및 비타민류로 구성되어 있다. 국내 조제분유의 초분 함량은 국외 조제분유에서 보고되는 수준 또는 그 이하이며, 초분 유래 미량 성분으로서 중금속은 조제분유의 미네랄 공급원으로 사용되는 종류이다. 그러므로 국내 조제분유 내 초분의 조성과 함량은 FAO/WHO에서 설정한 잠정주당섭취량(PTWI)에 대하여 거의 영향을 주지 않는 것으로 사료된다.

  • Human breast milk contains numerous biomolecules. Human milk oligosaccharides (HMOs) are the third most abundant component of breast milk, after lactose and lipids. Amongst the synthetized HMOs, 2'-fucosyllactose (2'-FL) and lacto-N-neotetraose (LNnT) are widely studied and are considered safe for infant nutrition. Several studies have reported the health benefits of HMOs, which include modulation of the intestinal microbiota, anti-adhesive effect against pathogens, modulation of the intestinal epithelial cell response, and development of the immune system. The amount and diversity of HMOs are determined by the genetic background of the mothers (HMO secretors or non-secretors). The non-secretor mothers secrete lower HMOs than secretor mothers. The breastfed infants of secretor mothers gain more health benefit than those of non-secretor mothers. In conclusion, supplementation of infant formula with 2'-FL and LNnT is a promising innovation for infant nutrition.

여성들의 수유양상 및 수유방식의 선택에 영향 주는 요인들 (Infant Feeding Practices and the Factors that Influence feeding Practices among Women in Seoul and the Chungbuk Area)

  • In order to obtain baseline data for the development of an educational program on breastfeeding, a survey was carried out to investigate infant feeding practices, the characteristics the subjects have in common, and the factors that influence the feeding methods among women in Seoul and the Chungbuk area. Subjects included 671 lactating mothers who visited public health centers or pediatric clinics between December 1999 and February 2000, and were interviewed using a questionnaire. The results were as follows : With respect to feeding methods, the incidence of breastfeeding, formula feeding, mixed feeding and formula feeding switched from breastfeeding was 20.6%, 29.8%, 11.6%, and 38.0%, respectively. With respect to the characteristics the subjects had in common, the incidence of women who planned their infants' feeding methods before pregnancy, during pregancy, and after delivery was 48.7%, 31.0%, and 20.3%, respectively. The incidence of women who started breastfeeding in the hospital after delivery was 38.8%. About seventy seven percet of the subjects had not previously attended an educational program on breastfeeding, and most of these wanted to participate in the future in an educational program to learn about breastfeeding. Some of the topics they were interested in were“Nutritional Management for Sufficient Breast Milk”(60.3%),“Breast Care”(25.0%), and“Correct Nursing Positions”(9.8%). Most (88.2%) of the women who breastfeed suffered from physical discomforts including discomfort of the waist, and legs and discomfort due to cracked or sore nipples. “Insufficient breast milk”was the main reason for breastfeeding cessation or for switching to formula feeding. With respect to formula feeding practices, the main reasons for selecting a specific brand of formula were“the same brand the hospital used after delivery”(34.3%) and“an advertised brand”(23.3%). The strongest factor for promoting breastfeeding was“the support of husband or parents”, next were “breastfeeding in the hospital after delivery”and“planning to breastfeed before pregnancy”in that order. The characteristics the subjects had in common relating to formula feeding were“mother's job”,“high economic level”,“Caesarian section”and “planning to breastfeed after delivery”. In conclusion, it is recommended that breastfeeding be pro-moted, and educational programs be developed and offered as soon as possible to each group which had unfavorable attitudes toward breastfeeding. In addition, the monitoring and supervision of formula advertisements is required to protect consumers from the adverse effects of exaggerated advertising.

Thermal Resistance and Inactivation of Enterobacter sakazakii Isolates during Rehydration of Powdered Infant Formula

  • Enterobacter sakazakii may be related to outbreaks of meningitis, septicemia, and necrotizing enterocolitis, mainly in neonates. To reduce the risk of E. sakazakii in baby foods, thermal characteristics for Korean E. sakazakii isolates were determined at 52, 56, and $60^{\circ}C$ in saline solution, rehydrated powdered infant formula, and dried baby food. In saline solution, their D-values were 12-16, 3-5, and 0.9-1 min for each temperature. D-values increased to 16-20, 4-5, and 2-4 min in rehydrated infant formula and 14-17, 5-6, and 2-3 min in dried baby food. The overall calculated z-value was 6-8 for saline, 8-10 for powdered infant formula, and 9-11 for dried baby food. Thermal inactivation of E. sakazakii during rehydration of powdered infant formula was investigated by viable counts. Inactivation of cultured E. sakazakii in infant formula milk did not occur for 20 min at room temperature after rehydration with the water at $50^{\circ}C$ and their counts were reduced by about 1-2 log CFU/g at $60^{\circ}C$ and 4-6 log CFU/ml with the water at 65 and $70^{\circ}C$. However, the thermo stability of adapted E. sakazakii to the powdered infant formula increased more than two times. Considering that the levels of E. sakzakii observed in powdered infant formula have generally been 1 CFU/100 g of dry formula or less, contamination with E. sakazakii can be reduced or eliminated by rehydrating water with at least $10^{\circ}C$ higher temperature than the manufacturer-recommended $50^{\circ}C$.

모유영양아와 인공영양아의 혈청무기질 및 미량원소 함량비교 (Concentrations of Major Minerals and Trace Elements in Sera of The Breast-fed and Formula-fed Infants)

  • The serum concentrations of the major minerals(calcium, phosphorus, magnesium, sodium, potassium) and trace elements(iron, Zinc, copper manganese)were datermined in 23 breast fed infants living in Eumsung, Choong-buk region. The results obtained are summarized as follows: 1)The mean levels of calcium, phosphorus, magnesium, sodium and potassium of the serum of total subjects were 8.15$\pm$0.33mg/dI, 11.06$\pm$0.16mg/dI, 2.00$\pm$0.14mg/dI, 3.4476$\pm$17.99mg/dI and 9.06$\pm$2.04mg/dI respectively. 2)The serum concentrations of iron, zinc, copper and manganese in total subjects averaged 95.83$\pm$0.33mg/dI, 93.79$\pm$7.06$\mu$g/dI and 98.57$\pm$7.06 $\mu$g/dI and 4.93$\pm$0.62$\mu$g/dI respecitively. 3)Breast fed infants had significantly higher serum calcim, magnesium, sodium and iron concentrations than the formula fed groups. Otherwise, serum potassium, copper and manganese levels in breast milk fed infants were significantly lower than those in formula fed infants. 4) In formula fed infants, serum potassium and copper levels increased increased significantly with months after birth.
