Influences of Histamine on Permeability across Blood-brain Barrier (혈액-뇌장벽 투과성에 대한 히스타민의 영향)

  • Histamine, 0.5 mg as histamine base in 4 ml of normal saline solution, was injected into rabbits anesthetized with nembutal and the mean blood pressure was kept in the range of $52{\sim}80\;mmHg$ for over one hour by supplemental additions. Following the injection of the test substances, 300 mg of urea and 200 mg of antipyrine intravenously, serial blood samples were obtained from the femoral artery and the internal jugular vein at $0.5{\sim}3$ minutes interval. The decreasing patterns in the concentrations of arterial and venous blood plasma samples were compared with each other. The ratio of the concentration of brain tissue to that of the final arterial plasma was also studied. By these measures the degrees of penetration of the test substances in the brain in the control and in the histamine treated rabbits were observed. The concentrations of antipyrine and urea in the arterial blood plasma were decreasing exponentially with respect to the time elapsed. The venous concentrations were anticipated to increase initially and to cross the arterial concentration curve in the point of equlibrium between the plasma and the tissue. On the contrary to the expectation venous concentration also revealed the decreasing tendency similar to that of arterial plasma. The similarity between these two curves, arterial and venous, would be atributable to the fact that the cerebral blood flow rate was large enough and the rising phase in the venous concentration curve was instantly over before serial blood samples were taken. Inspite of some similarity in the decreasing tedency in both concentration curves there were appreciable discrepancies between the arterial and venous plasma which would reflect the situation far from the equlibria among several compartments in the brain. Changes in plasma potassium levels caused by the injection of histamine or bleeding were observed, too. Using 8 rabbits as the control and 12 rabbits for the histamine treated group following results were obtained: 1. Both of the concentration curves, arterial and venous, declined rapidly at_first and slowly later on and approached same equilibrium concentration with the passage of time after a single injection. The time at which attained the same concentration was $2.0{\pm}0.54\;min.$ in the control and $4.3{\pm}1.92\;min.$ in the histamine treated group with respect to antipyrine. On the other hand in the case of urea they were $2.4{\pm}0.59\;min.$ in the control and $4.4{\pm}1.31\;min.$ in the histamine group, respectively. In the histamine treated group enlarged spaces for distribution of test substances were postulated. 2. The concentration of antipyrine in the brain tissue water revealed no significant differences between the control and experimental groups, showing $212{\pm}40.2\;mg/l$ in the control and $206{\pm}64.1\;mg/l$ in the histamine treated group. On the other hand urea revealed higher value in the histamine treated group than in the control, showing an enhanced penetration of urea into the tissue after injection of histamine. Urea concentration in the brain water was $32.3{\pm}3.36\;mg%$ in the control and $39.2{\pm}4.25\;mg%$ in the histamine treated group. 3. The distribution ratio of antipyrine in the brain tissue was very close to unity in the histamine treated animals as well as in the control. 4. The average of the distribution ratio of urea in the control animals was 0.77 and it showed the presence of blood-brain barrier with regard to urea. However in the histamine treated animals the distribution ratios climbed up to 0.86 and they were closer to unity than in the control animals. Out of 12 cases 5 were greater than 0.9 and 8 exceeded 0.85. It appeared that histamine enhanced the penetration of urea through the barrier. 5. Histamine injection and or hemorrhage caused an elevation of the concentration of potassium in plasma. In the event that histamine and hemorrhage were applied together the elevation of potassium exceed the elevation seen at the histamine alone. There was no evidence that the leakage of potassium from the brain tissue was dominant in comparison with the general leakage from the whole body.

Analysis on the Reduction of Phosphorus Release in River and Lake Sediments through Application of Capping Technology (Capping 기술을 이용한 하천 및 호소 퇴적토의 인 용출 저감 효과 분석)

  • Contaminants such as organic matters, nutrients and toxic chemicals in rivers and lakes with a weak flow rate are first removed from the water and accumulated in the sediments. Subsequently, they are released into the water column again, posing direct/indirect adverse effects on the water quality and aquatic ecosystems. In particular, phosphorus is known to accelerate the eutrophication phenomenon when it is released into the water column via physical disturbance and biological/chemical actions as one of important materials that determine the primary production of aquatic ecosystems and an element that is stored mainly in the sediments in the process of material circulation in the body of water. In this study, the effect on reducing phosphorus release in sediments was analyzed by applying different capping materials to lake water, where the effect of aquatic microorganisms is taken into account, and to distilled water, where the effect of microorganisms is excluded. The experimental results showed that capping with chemical materials such as Fe-gypsum and $SiO_2$-gypsum further reduced the phosphorus release by at least 40% compared to the control case. Composite materials like granule gypsum+Sand showed over 50% phosphorus release reduction effect. Therefore, it is determined that capping with chemical materials such as granule-gypsum and eco-friendly materials such as sand is effective in reducing phosphorus release. The changes in phosphorus properties in the sediments before and after capping treatment showed that gypsum input helped to change the phosphorus that is present in lake sediments into apatite-P, a stable form that makes phosphorus release difficult. Based on the above results, it is expected that the application of capping technology will contribute to improving the efficiency of reducing phosphorus release that occurs in river and lake sediments.

Influence of Intracerebroventricular Yohimbine on the Renal Function of the Rabbit (가토 신장기능에 미치는 측뇌실내 Yohimbine의 영향)

  • The renal function is under regulatory influence of the central nervous system, mainly through activation of sympathetic nerve to the kidney, and it was recently reported that clonidine, an agonist to ${\alpha}_2$-adrenoceptors, induces diuresis and natriuresis when injected directly into a lateral ventricle of the rabbit brain (i.c.v.). This study was undertaken, therefore, to obtain further information as to the role of the central ${\alpha}_2$-adrenoceptors in regulating renal function, by observing the effects of i.c.v. yohimbine, a specific antagonist of adrenoceptors of ${\alpha}_2$-type, on the rabbit renal function, and to elucidate the mechanism involved in it. With 10 ${\mu}g/kg$ i.c.v. of yohimbine sodium excretion transiently increased along with increasing tendency of urine flow, renal plasma flow and glomerular filtration rate. These responses decreased with increasing doses. With 100 and 300 ${\mu}g/kg$ i.c.v. marked antidiuresis and antinatriuresis as well as profound decreases of renal perfusion and glomerular filtration were noted. Systemic blood pressure transiently increased. In reserpinized rabbits, 100 ${\mu}g/kg$ yohimbine i.c.v. did not produce any significant changes in urine flow, sodium excretion as well as in renal hemodynamics. The pressor response was also abolished. In preparations in which one kidney was denervated and the other left intact as control, i.c.v. yohimbine elicited typical antidiuretic antinatriuretic response in the innervated control kidney, whereas the denervated experimental kidney responded with marked diuresis and increases in excretory rates of sodium and potassium and in osmolar clearance in spite of absence of increased filtration and perfusion . Systemic blood pressure responded as in the normal rabbits. These observations indicate that i.c.v. yohimbine affects renal function in dual ways in opposite directions, the first being the antidiuretic antinatriuretic effects which results from decreased renal perfusion and glomerular filtration due to sympathetic activation and which is predominantly expressed in the normal rabbits, and the second less apparent effect being the diuretic and natriuretic action which is not mediated by nerve pathway but brought about by some humoral mechanism and which is effected by decreased sodium reabsorption in the tubules, possibly of the proximal portion.

Occurrence of Fusarium wilt on Cyclamen Casued by Fusarium oxysporum f. sp. cyclaminis and Selection of Resistant Cultivars (Fusarium oxysporum f. sp. cyclaminis에 의한 시클라멘 시들음병 발생 및 저항성 품종 선발)

  • A wilt disease of commercial cyclamen (Cyclamen persicum) which grown in greenhouse was found in Gyeo-nggi province of Korea during the period from August, 2001 to July, 2002. The disease incidence was up to 42.7% in Kimpo, Gyeonggi province. The disease was more severe in ebb and flow irrigation system than con-ventional overhead flooding\'s. The wilted cyclamen plants showed the chlorosis of leaves and followed by the death. The vascular tissues of the infected basal stem and bulb were discolored with black streaks. The casual fungus was identified to be Fusarium oxysporum f. sp. cyclaminis on the basis of mycological characteristics. Effect of infected soil showed 100% infection rate when the cyclamen plants were grown in potting with infested soil. Examine resistant cultivar showed ‘Matis red’, ‘Schubert’, ‘Victoria’ and ‘Chopin’ were the resistant cultivars but most of cultivars were susceptible to Fusarium wilt. The control effect of chemicals for cyclamen wilt was not much efficient to curative effects even though. Benomyl WP and Fludioxonil SC on the Fusarium wilt had only preventive effects by soil drenching in the pot before inoculation of F. oxysporum. This is the first report on the fusarium wilt of cyclamen caused by F. oxysporum f. sp. cyclaminis in Korea.

The Study of Modified Sequencing Batch Reactor Process for Small Advanced Wastewater Treatment (소규모 고도하수처리를 위한 변형 연속회분식공정에 관한 연구)

  • This study was carried out to estimate the performance of modified sequencing batch reactor (SBR) process by the application of SBR process for small advanced wastewater treatment plant. Organic, nitrogen and phosphorus were able to remove in the unit reactor by SBR process and it would be able to select the suitable operation method. The plant was operated to achieve high performance with influent control, optimum anoxic/oxic condition using intermediate aeration method, and solid (sludge) /liquid (effluent) separation by modified decanter. The optimum operating mode was 3Cycles a day and intermediate input and aeration. Under these conditions, the treatment efficiencies were good with 60% of designed flow rate and low influent quality. When the influent concentrations of BOD and CODMn were 120.4 mg/L and 95.7 mg/L, respectively. The effluent concentrations of BOD and CODMn were 6.8 mg/L and 11.0 mg/L, respectively. The average removal efficiencies of BOD and CODMn were 94.4% and 88.5%, respectively. The removal efficiencies of T-N and T-P were 69.6% and 73.6%, respectively when the average T-N and T-P concentrations were 32.2mg/L and 4.65mg/L, respectively. The T-N and T-P removal efficiencies were slightly decreased to 58.8% and 68.5%, respectively in the winter season but its were also stable efficiencies. BOD, T-N and T-P were removed by 90%. 67% and 46% respectively in the first anoxic/oxic condition, in addition to T-P was removed by 70% in the second anoxic/oxic condition. From the results, modified sequencing batch reactor (SBR) process is suitable for small advanced wastewater treatment.

A STUDY ON THE JUJEON OF AUTOMATIC CLEPSYDRA IN EARLY JOSEON DYNASTY (조선 전기 자동물시계의 주전(籌箭) 연구)

  • Jagyeokru, an automatic striking water clock described in the Sejong Sillok (Veritable Records of King Sejong) is essentially composed of a water quantity control device and a time-signal device, with the former controlling the amount or the flow rate of water and the latter automatically informing the time based on the former. What connects these two parts is a signal generating device or a power transmission device called the 'Jujeon' system, which includes a copper rod on the float and ball-racked scheduled plates. The copper products excavated under Gongpyeong-dong in Seoul include a lot of broken plate pieces and cylinder-like devices. If some plate pieces are put together, a large square plate with circular holes located in a zigzag can be completed, and at the upper right of it is carved 'the first scheduled plate (一箭).' Cylinder-like devices generally 3.8 cm in diameter are able to release a ball, and have a ginkgo leaf-like screen fixed on the inner axis and a bird-shaped hook of which the leg fixes another axis and the beak attaches to the leaf side. The lateral view of this cylinder-like device appears like a trapezoid and mounts an iron ball. The function of releasing a ball agrees with the description of Borugak Pavilion, where Jagyeokru was installed, written by Kim Don (1385 ~ 1440). The other accounts of Borugak Pavilion's and Heumgyeonggak Pavilion's water clocks describe these copper plates and ball releasing devices as the 'Jujeon' system. According to the description of Borugak Pavilion, a square wooden column has copper plates on the left and right sides the same height as the column, and the left copper plate has 12 drilled holes to keep the time of a 12 double-hours. Meanwhile, the right plate has 25 holes which represent seasonal night 5-hours (Kyeong) and their 5-subhours (Jeom), not 12 hours. There are 11 scheduled plates for seasonal night 5-hours made with copper, which are made to be attached or detached as the season. In accordance with Nujutongui (manual for the operation of the yardstick for the clepsydra), the first scheduled plate for the night is used from the winter solstice (冬至) to 2 days after Daehan (大寒), and from 4 days before Soseol (小雪) to a day before the winter solstice. Besides the first scheduled plate, we confirm discovering a third scheduled plate and a sixth scheduled plate among the excavated copper materials based on the spacing between holes. On the other hand, the width of the scheduled plate is different for these artifacts, measured as 144 mm compared to the description of the Borugak Pavilion, which is recorded as 51 mm. From this perspective, they may be the scheduled plates for the Heumgyeonggak Ongru made in 1438 (or 1554) or for the new Fortress Pavilion installed in Changdeokgung palace completed in 1536 (the 31st year of the reign of King Jungjong) in the early Joseon dynasty. This study presents the concept of the scheduled plates described in the literature, including their new operating mechanism. In addition, a detailed model of 11 scheduled plates is designed from the records and on the excavated relics. It is expected that this study will aid in efforts to restore and reconstruct the automatic water clocks of the early Joseon dynasty.

Effects of Dietary Supplementation of Fermented Garlic Powder on Immune Responses, Blood Components, and Disease Resistance against Principal Fish Disease of Juvenile Olive Flounder, Paralichthys olivaceus in Low Temperature Season (저수온기 넙치 치어에 있어서 발효마늘분말의 사료 내 첨가가 면역반응, 혈액성분 및 주요 어병세균에 대한 질병저항성에 미치는 영향)

  • We report non-specific immune responses and disease resistance against Vibrio anguillarum, Streptococcus iniae and Edwardsiella tarda by dietary supplementation of fermented garlic powder (FGP) in olive flounder for the first time. Four isonitrogenous (45% crude protein) and isocaloric (17.1 MJ/kg) diets were formulated to have 0%, 0.5%, 1% and 2% of the FGP (G-0, G-0.5, G-1 and G-2). The experimental diets were fed to juvenile olive flounder averaging 23.4 g in triplicate groups (90 fish/group) in a flow-through system. After a five-week feeding trial, healthy fish with similar sizes from each tank were selected and injected with 1 ml of three bacteria each to evaluate disease resistance of the fish. During the 5-week feeding trial, the weight gain, specific growth rate, feed conversion ratio, protein efficiency ratio, and survival of the fish were not significantly affected by the experimental diets. However, feed intake was significantly lower (P<0.05) in the fish fed the G-2 diet compared with the control group. Hemoglobin, myeloperoxidase activity, cholesterol and HDL-cholesterol were not different between the dietary groups. However, hematocrit, nitroblue tetrazolium (NBT) activity, and lysozyme activity were increased (P<0.05) with an increment of dietary FGP. Plasma triglyceride of the fish fed the G-0.5 diet was significantly lower than that of fish fed the control diet. The cumulative mortality was lower in the fish fed diets containing FGP compared with the control group in the challenge test except for the bacteria Edwardsiella tarda. The results in this study indicate that dietary supplementation of FGP can enhance the non-specific immune responses and disease resistance of olive flounder against V. anguillarum and S. iniae.

Effect of Additional 1 hour T-piece Trial on Weaning Outcome to the Patients at Minimum Pressure Support (최소압력보조 수준에서 추가적 1시간 T-piece 시도가 이탈에 미치는 영향)

  • Background: Extubation is recommended to be performed at minimum pressure support (PSmin) during the pressure support ventilation (PSV). In field, physicians sometimes perform additional 1 hr T-piece trial to the patient at PSmin to reduce re-intubation risk. Although it provides confirmation of patient's breathing reserve, weaning could be delayed due to increased airway resistance by endotracheal tube. Methods: To investigate the effect of additional 1 hr T-piece trial on weaning outcome, a prospective study was done in consecutive 44 patients who had received mechanical ventilation more than 3 days. Respiratory mechanics, hemodymic, and gas exchange measurements were done and the level of PSmin was calculated using the equation (PSmin=peak inspiratory flow rate $\times$ total ventilatory system resistance) at the 15cm $H_2O$ of pressure support. At PSmin, the patients were randomized into intervention (additional 1 hr T-piece trial) and control (extubation at PSmin). The measurements were repeated at PSmm, during weaning process (in cases of intervention), and after extubation. The weaning success was defined as spontaneous breathing more than 48hr after extubation. In intervention group, failure to continue weaning process was also considered as weaning failure. Results: Thirty-six patients with 42 times weaning trial were satisfied to the protocol. Mean PSmin level was 7.6 (${\pm}1.9$)cm $H_2O$. There were no differences in total ventilation times (TVT), APACHE III score, nutritional indices, and respiratory mechanics at PSmin between 2 groups. The weaning success rate and re-intubation rate were not different between intervention group (55% and 18% in each) and control group (70% and 20% in each) at first weaning trial. Work of breathing, pressure time product, and tidal volume were aggravated during 1 hr T-piece trial compared to those of PSmin in intervention group ($10.4{\pm}1.25$ and $1.66{\pm}1.08$ J/L in work of breathing) ($191{\pm}232$ and $287{\pm}217$cm $H_2O$ s/m in pressure time product) ($0.33{\pm}0.09$ and $0.29{\pm}0.09$ L in tidal volume) (P<0.05 in each). As in whole, TVT, and tidal volume at PSmin were significantly different between the patients with weaning success ($246{\pm}195$ hr, $0.43{\pm}0.11$ L) and the those with weaning failure ($407{\pm}248$ hr, $0.35{\pm}0.10$L) (P<0.05 in each). Conclusion : There were no advantage to weaning outcome by addition of 1 hr T-piece trial compared to prompt extubation to the patient at PS min.

Development of a High Heat Load Test Facility KoHLT-1 for a Testing of Nuclear Fusion Reactor Components (핵융합로부품 시험을 위한 고열부하 시험시설 KoHLT-1 구축)

  • A high heat flux test facility using a graphite heating panel was constructed and is presently in operation at Korea Atomic Energy Research Institute, which is called KoHLT-1. Its major purpose is to carry out a thermal cycle test to verify the integrity of a HIP (hot isostatic pressing) bonded Be mockups which were fabricated for developing HIP joining technology to bond different metals, i.e., Be-to-CuCrZr and CuCrZr-to-SS316L, for the ITER (International Thermonuclear Experimental Reactor) first wall. The KoHLT-1 consists of a graphite heating panel, a box-type test chamber with water-cooling jackets, an electrical DC power supply, a water-cooling system, an evacuation system, an He gas system, and some diagnostics, which are equipped in an authorized laboratory with a special ventilation system for the Be treatment. The graphite heater is placed between two mockups, and the gap distance between the heater and the mockup is adjusted to $2{\sim}3\;mm$. We designed and fabricated several graphite heating panels to have various heating areas depending on the tested mockups, and to have the electrical resistances of $0.2{\sim}0.5$ ohms during high temperature operation. The heater is connected to an electrical DC power supply of 100 V/400 A. The heat flux is easily controlled by the pre-programmed control system which consists of a personal computer and a multi function module. The heat fluxes on the two mockups are deduced from the flow rate and the coolant inlet/out temperatures by a calorimetric method. We have carried out the thermal cycle tests of various Be mockups, and the reliability of the KoHLT-1 for long time operation at a high heat flux was verified, and its broad applicability is promising.

Future Direction of National Health Insurance (국민건강보험 발전방향)

  • It has been forty years since the implementation of National Health Insurance (NHI) in South Korea. Following the 1977 legislature mandating medical insurance for employees and dependents in firms with more than 500 employees, South Korea expanded its health insurance to urban residents in 1989. Resultantly, total expenses of the National Health Insurance Service (NHIS) have greatly increased from 4.5 billion won in 1977 to 50.89 trillion won in 2016. With multiple insurers merging into the NHI system in 2000, a single-payer healthcare system emerged, along with separation policy of prescribing and dispensing. Following such reform, an emerging financial crisis required injections from the National Health Promotion Fund. Forty years following the introduction of the NHI system, both praise and criticism have been drawn. In just 12 years, the NHI achieved the fastest health population coverage in the world. Current medical expenditure is not high relative to the rest of the Organization for Economic Cooperation and Development. The quality of acute care in Korea is one of the best in the world. There is no sign of delayed diagnosis and/or treatment for most diseases. However, the NHI has been under-insured, requiring high-levels of out-of-pocket money from patients and often causing catastrophic medical expenses. Furthermore, the current environmental circumstances of the NHI are threatening its sustainability. Low birth rate decline, as well as slow economic growth, will make sustainment of the current healthcare system difficult in the near future. An aging population will increase the amount of medical expenditure required, especially with the baby-boomer generation of those born between 1955 and 1965. Meanwhile, there is always the problem of unification for the Korean Peninsula, and what role the health insurance system will have to play when it occurs. In the presidential election, health insurance is a main issue; however, there is greater focus on expansion and expenditure than revenue. Many aspects of Korea's NHI system (1977) were modeled after the German (1883) and Japanese (1922) systems. Such systems were created during an era where infections disease control was most urgent and thus, in the current non-communicable disease (NCD) era, must be redesigned. The Korean system, which is already forty years old, must be redesigned completely. Although health insurance benefit expansion is necessary, financial measures, as well as moral hazard control measures, must also be considered. Ultimately, there are three aspects that we must consider when attempting redesign of the system. First, the health security system must be reformed. NHI and Medical Aid must be amalgamated into one system for increased effectiveness and efficiency of the system. Within the single insurer system of the NHI must be an internal market for maximum efficiency. The NHIS must be separated into regions so that regional organizers have greater responsibility over their actions. Although insurance must continue to be imposed nationally, risk-adjustment must be distributed regionally and assessed by different regional systems. Second, as a solution for the decreasing flow of insurance revenue, low premium level must be increased to an appropriate level. Likewise, the national reserve fund (No. 36, National Health Insurance Act) must be enlarged for re-unification preparation. Third, there must be revolutionary reform of benefit package. The current system built a focus on communicable diseases which is inappropriate in this NCD era. Medical benefits must not be one-time events but provide chronic disease management. Chronic care models, accountable care organization, patient-centered medical homes, and other systems that introduce various benefit packages for beneficiaries must be implemented. The reimbursement system of medical costs should be introduced to various systems for different types of care, as is the case with part C (Medicare Advantage Program) of America's Medicare system that substitutes part A and part B. Pay for performance must be expanded so that there is not only improvement in quality of care but also medical costs. Moreover, beneficiaries of the NHI system must be aware of the amount of their expenditure through a deductible payment system so that spending can be profiled and monitored. The Moon Jae-in Government has announced its plans to expand the NHI system; however, it is important that a discussion forum is created so that more accurate analysis of the NHI, its environments, and current status of health care system, can take place for reforming NHI.
