본태성 고혈압 환자의 자기실현 및 욕구구조에 관한 연구 (Manifest Weeds and Self-Actualization of Patients with Essential Hypertension)

  • Much of a person's energy is spent in the effort of becoming a productive member of to-day's complex society. This activity may cause tension, and chronic unrelieved tension is an influential factor in blood pressure elevation. The problem of this study was to identify manifest needs and self-actualization of patients with essential hypertension, and to analyse and compare their manifest needs and selt-actualization with the selected general characteristics of We, sex, religion, occupation and level of education with a control group of patients with normal blood pressure readings. The purpose was to contribute to the planning of nursing interventions toward reducing the impact of complex psycho-somatic factors on the anxiety of patients with essential hypertension. The instruments used included selected items from the Edwards (1959) Personal Preference Schedule (EPPS) as adapted by Hwang (1965) and from the Personal Orientation Inventory (POI) (Shostrom 1964, 1974) adapted by Kim and Lee (1977) to measure manifest needs and self-actualization. The convenience sample was chosen from 149 persons who presented themselves for general physical examinations at Ewha University Medical Centre and 41 patients diagnosed with essential hypertension at three general hospitals in Seoul during June 1 and August 31, 1977. Forty-nine persons from the Ewha group with blood-pressure readings exceeding 150/90 were added to the experimental group. Data were analysed by the S.P.S.S. computer programme using t-test and tests for statistical significance. Statistically significant findings were as follows: A. Blood Pressure and Manifest Needs. 1. with the exception of Autonomy, patients with hypertension had significantly high scores on all variables Abasement, Achievement, Affiliation, Aggression, Dominance, Emotionality, Exhibitionism and Sex. 2. When mean scores of normal persons were compared by age groups, normal persons had higher scores in the following order on Abasement (50's, 40's, 20's, 30's), Achievement (50's, 30's, 40's, 20's), Affiliation (50's, 40's, 30's, 20's), Dominance (50's, 40's, 40's, 20's) and Exhibitionism (30's, 50's, 40's, 20's). In each case, there was a significant difference between the first and last age group scores. 3. When the mean scores of normal persons were compared by sex, normal men had higher scores than women on Achievement, Affiliation, Aggression, Dominance, Exhibitionism and Sex. Male patients had higher scores than female patients on Achievement, Dominance, Exhibitionism and Sex, but female patients scored higher in Emotionality. 4. Normal persons had higher scores related to religion in the following order on Achievement (Buddhism, no religion, Christianity). Hyper tensive patients had higher scores on. Exhibitionism (no religion, Christianity, Buddhism). 5. Normal persons had higher scores related to occupation in the following order on Achievement and Exhibitionism (unemployed, office workers, teachless, businessmen), Emotionality (office workers, unemployed, businessmen, teacher) and Sex (office workers, unemployed, teachers, businessmen). Hypertensive patients had higher scores on Achievement and Aggression (teachers, businessmen, office worker, unemployed), Dominance and Exhibitionism (businessmen, teacher, of ace workers, unemployed) and Sex (teachers, office worker, businessmen, unemployed). 6. Normal persons had higher scores related to level of edification in the following order on Abasement, Emotionality and Autonomy (secondary school graduation, university). Hypertensive patients had higher scores on Abasement (no education, primary, university, secondary), Achievement (no education, secondary, university, primary) , Dominance (university, no education, secondary, primary), Exhibitionism (university, secondary, no education, primary), and Sex (university, secondary, primary, no education). B. Blood Pressure and Self_Actualization 1, Patients with hypertension had significantly lower scores on all variables. 2. Normal persons had higher scores related to age groups in the following order on Existentiality (20's, 30's, 40's, 50's). Hypertensive patients showed no significantly different scores. 3. Normal women had higher scores than men on Time Competence. Normal men had higher scores on Feeling Reactivity. Male patients had higher scores than women on Self-Actualizing Value and Self-Regard. 4. Normal persons ha 1 higher scores related to religion on spontaneity (Buddhism, no religion, Christianity). Hypertensive patients had higher scores on Time Competence and Nature of Man (Buddhism, Christianity, no religion). 5. Normal persons had higher scores related to occupation in the following order on Existentiality (teachers, office workers, businessmen, unemployed) and Self-Regard (unemployed, office workers, teachers, businessmen). Hypertensive patients showed no significantly different scores. 6. Normal persons had higher scores related to level of education in the following order on Existentiality and Self-Acceptance (university, secondary). Hypertensive patients had higher scores on inner-Director (university, secondary, no education, primary) and Existentiality (university, secondary, primary, no education). Recommendations for nursing interventions with hypertensive patients with emotional problems or low self-actualization were made. 1. The nurse should encourage the patient through her interactions with other members of the medical team to accept counselling and health education. 2. Through her therapeutic interpersonal relationships with the patient, the nurse should help him discover the causes of his emotional tension. 3. Through her health teaching with the family, the nurse should encourage them to participate with the medical team in the patient's therapeutic plan and in providing him with the minimum possible emotional support. 4. Through frequent counselling with the obsessive-thinking and inflexible patient, the nurse should reevaluate the patient's behaviour and her interventions. 5. Seriously ill patients should be given needed reeducation by members of the professional medical team.

병원 내 방사선 작업종사자의 만성적 방사선 피폭에 의한 혈액학적 성분 변화에 관한 연구 (Occupational Radiation Exposure to Hospital Workers: On the Effect of Hematological Change)

  • 본 연구는 의료기관에 종사하는 의료방사선 작업종사자에게서 직업적 방사선 피폭으로 인한 인체 내 생리적인 변화를 통해 만성적 저선량 방사선피폭의 위험도를 예측하며 현 수준의 변화 상태를 파악하기 위한 목적으로 서울시내 일개 대형병원에서 최근 1~9년간 방사선 작업종사자로 근무한 370명을 대상으로 남녀 간의 체내 혈액성분의 수치변화를 대조군과의 비교 분석을 실시하여 다음과 같은 결론을 얻었다. 1. 종사자의 1~9년간 평균 방사선 누적선량은 남자가 $9.65{\pm}15.2\;mSv$, 여자가 $4.89{\pm}5.55\;mSv$로 남자에서 높았으며 이는 통계적으로도 유의하였다(p<0.01). 2. 남녀 종사자 모두에서 누적선량과 종사기간의 차이에 따른 검사항목들과의 상관성은 매우 미약하였다(r<${\pm}0.25$) 3. 종사기간이 증가할수록 남자에서는 백혈구의 감소자와 증가자, 혈색소의 감소자, 당질의 증가자 비율 유의하게 높았으며(p<0.05), 여자에서도 백혈구의 감소 및 증가자, 호산구의 감소자의 비율이 유의하게 높았다(p<0.01). 4. 누적선량의 증가할수록 남녀모두에서 오히려 이상분포를 보이는 항목은 줄어들었으며, 남자는 림프구와 혈소판의 감소자가 유의하게 높았고(p<0.05) 여자는 림프구와 적혈구의 감소뿐만 아니라 종사기간에서 이상을 보였던 백혈구의 감소 및 증가자 역시 유의한 증가를 나타냈다(p<0.05). 5. 전체적인 변화를 보면 남자 종사자에서는 혈액성분 중 적혈구, 혈소판, 림프구가 주로 감소되며 백혈구는 감소와 증가양상을 같이 보였다. 화학성분 중 당질의 증가가 유의했지만 큰 의미는 없었다. 여자 종사군에서는 남자와 똑같게 백혈구가 감소, 증가 양상을 동시에 보였으며 그밖에 적혈구와 림프구, 호산구의 감소가 유의했다. 이상의 결과를 종합해보면 비록 낮은 선량에 피폭이 되고 있는 의료방사선 종사자일지라도 방사선 피폭에 있어서는 자유롭지 못하여 그 영향으로 인한 체내 생물학적 변화를 겪을 수 있으며 그 변화는 선량에 의존하기 보다는 다분히 확률적일 수 있다는 사실이다. 그러므로 항상 미연에 피폭을 방지하기 위한 노력을 종사자 스스로 기울임을 물론 관련 당국 또한 종사자들의 건강을 체계적으로 관리할 수 있는 시스템을 하루빨리 구축하여 방사선 피폭으로 인한 직업병의 발생의 우려를 없애야 할 것으로 사료된다.

일부지역 근로자의 요통으로 인한 의료보험 이용 조사 (A Study on the Medical Insurance Utilization of Workers Suffering from Low Back Pain in an Area)

  • 근로자의 요통으로 인한 의료보험 이용 실태를 파악하기 위해 1993년부터 1995년까지 광주의 한 직장 의료보험조합에 소속된 남자 8,783명, 여자 1,400명계 10,153명의 의료보험 급여자료로부터 한국표준질병사인 분류코드상 요통과 관련된 변형성 배병증, 기타 배병증, 요추염좌에 해당되는 질환군의 치료시 이용한 의료보험 급여자료와 연구대상자의 일반적 특성을 결합시켜 분석한 결과는 다음과 같다. 1. 1993년부터 1995년까지 3개년간 연구대상자의 요통으로 인한 수진율은 남자 17.1%, 여자가 19.4%였으며, 요통의 원인중 남녀 모두 '기타 배병증'이 가장 많은 비율 차지하였다. 2. 남녀 모두에서 연령 및 입사시 연령이 높을수록 요통으로 인한 수진율이 증가하는 경향이 있었다(p<0.001). 남녀 모두 근무기간이 길수록 수진율이 증가하는 경향을 보였으나, 통계적 유의성은 없었다. 남자에서는 시멘트 콘크리트제품 제조업 종사자가 유의하게 수진율이 높았으며(p<0.01), 여자에서는 생산직이 사무직보다 유의하게 높았다(p<0.01). 3. 1993년부터 1995년까지 3개년간 요통의 수진율과 요통으로 인한 새로운 수진자의 발생율은 비슷하였지만, 세부적으로 남녀 모두에서 기타 배병증이 증가하는 경향이 있었다. 4. 수진량에 있어서는 연령별, 근무기간별, 산업별, 소득수준에 따라 수진자 1인당 평균외래수진건수에 있어서 유의한 차이가 있었고(p<0.05), 근무기간별로 수진자 1인당 평균외래방문회수가 유의한 차이를 보였다.(p<0.05). 이상을 종합해보면 매년 요통으로 인한 의료보험 이용자수가 증가하고 있으며, 특히 만성적인 장애를 유발하는 추간판탈출증을 포함하고 있는 '기타 배병증'이 현저하게 증가하고 있는 경향을 볼 때 산업장에서 효과적인 요통예방 및 관리 사업이 요구된다 하겠다.

일부지역 산업재해환자 실태 연구 -대구, 경북지역 일부 종합병원 중심으로- (A Study of Industrial Patients from Selected General Hospitals in the Kyung Pook and Taegu City Areas)

  • The purpose of this study is to research the actual conditions of industrial accident patients and to produce worker satisfaction and a rational and effective counter measure pain. Direct interviews with 179 cases (in and out patients) were carried out during a three month period from April to July 1990, at six hospitals two general hospitals Sun Lin and Sung Mo in Po Hang, and four general hospitals in Taegu Kyung Pook University Hospital, Dong San Medical Center, Young Nam Medical Center and Catholic Hospital. The results of this study are summarized as follows: 1. Among the 179 cases, 51.6 % were male and 48.4 % were female. The two largest age groups were 30~39, 31.8 % and 20~29, 27.4 %. Among the 179 cases, 51.6% were married, the largest family number was 2 to 3, 41.1% and 4 to 5, 25.6%. Educationally, graduation from high school was the largest group, 46.4% among the patients, followed by middle school and primary school. The largest group income level was from 40~69만원, 45.2%. The largest group of patients who worked over 50 hrs. a week was 52.0%. The largest group of patients who worked less than 1 year was 44.7%, of the patients in work places of less than 100 people, 60.3% were injured and in work places of 100~299 people, 20.1% were injured. In manufacturing, the lagest group injured was 55.3%, the next group was transport, stroage, communication. The largest group of production workers injured was 40.2%. 2. The cause of injury in the largest group was facility problems, 33.5%. The next group was unsafe habits, 30.2% a lack of safety knowledge, 17.9% and insufficient supervision, 12.3%. The 30~39 year age group was head the highest number of injuries, 40.4% work places with more than 10 yeras of work, 44.4% work palces with more than 1000 people, 56.3% and mining accidents, 80.0%. Among these groups the highest cause of injury was due to facility problems. 3. The accident pattern showed machinery injuries 28.5% as the largest group, followed by falls & falling objects 17.3%, fire & electric 15.1%, struke by an object 14.5%, followed by overaction and vehicular accidents. The accident pattern showed 46.4 % among workers over the 50 year age group, workers in the 5~10 year group, 50.0 % places employing more than 1000 workers, 35.3 % : construction 73.7%, and construction workers 57.1%, among these fall & falling objects caused the greatest number of injuries. 4. The largest group of injuries was fractures 54.8%, trauma 14.5%, amputation 11.7%, open wound, and burns. The largest number of fractures occurred in people in the 30~39 year age group, 63.2 % over 10 years of work, 55.6% in work places of 300~400 people, 63.6% construction 63.2% and general workers 57.2 %. 5. The largest group of injuries was upper extremity 45.3%, lower extremity 24.0%, trunk 18.5 % and head or neck 12.2%. Of these groups, upper extremity injuries were the highest in those less 20 years old 75.0%, less than 1 years of work 59.5%, in work places of 500~999 people 60.0%, manufacturing 56.6 % and production workers 55.6%. 6. Periods of injury showed 34 people injured in September, to be the largest followed by October, 32 August, 22 people July, 19 people and the lowest December, 2 people. During the week, Friday had the largest group injured, 35 people followed by Saturday, 26 people and the lowest was Wednesday, 17 people, During the day 1400 hours had the largest group injured, 38 people followed by 800 hours, 31 people. 7. On a basis of 5 as the highest mark, the average, according to worker satisfaction showed facility safety 3.55, work environment 3.47, income 3.44, job 3.21 and treatment 2.98. 8. The correlation between general characteristics and injury showed that age was directly correlated to the duration of work(r=.2591) p<0.01, age was directly correlated to industry (r=2311) p<0.01, and the duration was directly correlated to occupation(r =.4372) p<0.001.

교육용 치과 엑스선 발생장치에 대한 방사선 안전 관리 실태 조사 (Research on the Actual Condition of the radiation Safety Management(RSM) for the Educated Training of the Dental Diagnostics X-ray Generators)

  • 본 연구의 목적은 교육용 치과 엑스선 발생장치를 설치 및 가동 중인 전국 치위생(학)과를 대상으로 방사선 작업종사자의 방사선 안전 관리 실태를 설문 조사하고 이를 체계적으로 추이 분석하여 교육용 치과 엑스선 발생장치의 안전한 이용 기반을 확립하기 위한 기초 자료로 제공하고자 하였다. 이를 위하여 교육용 치과 엑스선 발생장치의 방사선 안전 관리와 관계되는 원자력 관계 법령 및 국내 방사선 안전성 평가 및 규제 기술 개발 현황에 대한 용역 보고서를 참고하여 포괄적인 문헌 조사를 하였으며 이를 토대로 응답자의 일반적 특성, 방사선안전관리자의 위상, 방사선 안전관리 현황과 지식 및 의식 수준에 대한 설문지를 작성하였다. 본 연구는 국내 교육기관 내 방사선안전관리자와 교육용 치과 엑스선 발생장치를 운전 가능한 학사 및 실습 조교, 전임 교원 224명을 대상으로 설문 조사를 시행하여 설문 응답 누락 및 불충분한 응답을 제외한 95부를 회수하여 분석 자료로 이용하였다. 분석 방법은 일반적 특성 및 방사선안전관리자의 위상은 빈도와 백분율을 구하였으며 방사선 안전 관리 현황과 지식 및 의식 수준은 빈도 분석과 설문 문항별 연관성 분석을 위한 ${\chi}^2$ 검정(chi-square test)과 수준간 연관성을 구하기 위한 피어슨 상관분석(Pearson correlation analysis)을 하였다. 결과적으로 교육용 치과 엑스선 발생장치의 운영은 대부분 대학 이상의 사회적 교육 수준이 높고 치위생학을 전공한 20대에서 40대의 여성이 담당하고 있었으며 남성과 비교하여 방사선 안전 관리에 대한 의식 수준이 높게 나타났으며 통계적으로 유의한 선형적 관계를 보였다(${\chi}^2$ >5, 0.1${\chi}^2$ >5, 0.3${\chi}^2$ >5, 0.3

병원 구성원들의 직종별 조직몰입의 영향요인에 관한 연구 (Factors affecting the Organizational Commitment of Industrial Accident Hospital Employees by Job Category)

  • The purpose of this study was to analyze the characteristics of socio-demographic, organizational culture, organizational conflict and organizational commitment, and to examine the interrelation of influential factors in the organizational commitment. The data for this study were collected through a self-administrated survey with a structured questionnaire to 1,027 subjects from several medical doctor staff members, nursing staff members, administration staff members, pharmacist, and technical expert of eleven hospitals. The data were collected by self-reporting questionnaire from July 29 to September 7, 2002. In this analysis frequency test, t-test, ANOVA, multiple regression were used. The main results of this research is as following; 1. According to socio-demographic characteristics the respondents' level of the organizational commitment was higher in a administration staff members than others, for males than females, and for employees aged more than forty, as working for longer time, and as higher level of the working position. 2. According to the organizational culture characteristics the progressive culture, affiliative culture, and task-focused culture among many types of organizational culture were moderately and positively correlated with the level of the organizational commitment while the hierarchical culture was negatively correlated. 3. According to the organizational conflict characteristics as the respondents who got less conflict experience in the organization, their level of the organizational commitment was higher. And, technical conflict experiences were expressed greater than hierarchical conflict experiences. 4. According to the job satisfaction characteristics as his or her satisfaction that is about the promotion, working itself, salary, and fellowship in the organizational was higher, the level of the organizational commitment was higher. The most important factor of the satisfactions was the fellowship among the respondents while the level of job satisfaction for the promotion and salary was average. 5. Overall, according to the result of Multiple Regression as older age and longer working hours, the level of the organizational commitment was higher and as a higher level of the satisfaction for the promotion, working itself, salary, and fellowship in the organization, it caused more effective factors for the organizational commitment. 6. According to the result of Multiple Regression for the doctor staff members in special hospitals rather than general hospitals the hierarchical culture and task focused culture was positively correlated with the satisfaction of working itself while hierarchical conflict factors in the organizational conflict characteristics was negatively correlated with the organizational commitment. For the nursing staff members the affiliative culture and the job satisfaction for the promotion, working itself, salary, and fellowship were positively correlated with the organizational commitment. For the administration staff members as the job satisfaction for the fellowship was positively correlated with the organizational commitment. For medical and pharmacy staff members as more working experience, correlation with the organizational commitment was positive. Besides, as he or she has a high perception of the affiliative culture, it caused statistically more effective factors for the organizational commitment. For the skill and technicians, male worker expressed greater organizational commitment in the organization than female worker. And also older age along with higher education also showed higher organizational commitment. Moreover, the job satisfaction for the fellowship was positively correlated with the organizational commitment. This study identified the major effective factors of the organizational commitment and analyzed the differences among the job category. In that respect, it is significant for the study to be able to provide a reference for managing hospital of industrial accident and organizational development. However, this study has a problem, which is not to identify a valuable model for examining the relationship between organizational factors such as organizational culture, conflict, satisfaction, and commitment. Therefore, further study is needed and strengthened in the field of organizational commitment for hospital for industrial accident.

  • 목적: 본 연구는 근거리 작업자의 주시선 각도 변화에 따른 조절효율과 순목횟수, 조절래그에 대한 상관관계를 조사하여 독서, 컴퓨터나 TV등을 오랜 시간동안 주시할 때 두통을 동반하거나 눈의 피로를 호소하는 원인 및 안정피로를 완화시키는 적합한 주시각도를 알아보고자 실시하였다. 방법: 컴퓨터 등의 근거리 작업을 많이 하는 20세에서 36세까지의 대학생 남자 12명과 여자 15명 총 27명을 대상으로 근거리 작업자의 주시선의 각도 변화에 따른 조절효율과 눈 깜빡임 횟수, 조절래그량의 크기를 조사하였다. 굴절이상을 완전교정한 후 포롭터를 근거리 모드로 전환하여 40 cm에 근거리 시표를 위치시키고 수평면을 기준으로 $40^{\circ}$ 아래방향, $20^{\circ}$ 아래방향, 수평방향, $20^{\circ}$ 윗 방향을 주시하였을 때 각각의 주시방향에서 1분 동안의 조절효율성, 눈의 깜빡임 횟수를 측정하였고 검영기를 이용하여 동적검영법으로 조절래그를 측정하였다. 결과: 주시선이 $40^{\circ}$ 하방보다 윗 방향쪽으로 이동할수록 조절효율은 감소하였고, 눈 깜빡임 횟수와 조절래그량은 증가하여 근거리 작업 시 눈이 건조해지고 조절반응량이 감소하는 것으로 조사되었다. 결론: 근거리 작업 시 책이나 컴퓨터 화면의 위치를 수평방향보다 $40^{\circ}$ 하방에 위치시키면 눈의 피로도를 최소화할 수 있다.

요양보호사의 구강보건인식 및 지식에 따른 구강보건제공현황 (Status of oral health care provided by care workers in accordance with their oral health awareness and knowledge)

  • 본 연구는 요양시설에 근무 중인 요양보호사의 구강보건인식 및 지식 따른 구강보건제공현황에 관하여 연구함으로써 요양시설에서 노인들의 구강건강을 증진시킬수 있는 발전 방안을 모색하고자 하였다. 2016년 8월 16일부터 9월 2일까지 D시에 소재한 장기노인요양시설 14개소에 근무하는 요양보호사 200명을 대상으로 설문을 실시하였다. 수집한 자료는 SPSS 18.0을 이용하여 빈도분석 및 독립표본 t-test를 실시하였다. 연구결과, 대상자 성별은 여자가 91.5%로 남자보다 많았고, 결혼은 기혼자, 연령은 50대, 학력은 고등학교, 종교는 기독교가 가장 높았다. 하루케어인원은 6~7명이 가장 많았다. 요양보호사의 구강보건제공현황과 구강보건인식에서는 구강위생지침서유무에서 통계적으로 유의하였고, 구강보건제공현황과 구강보건지식에서는 틀니 물에 보관여부, 가글액 사용에서 유의한 차이가 있었다(p<.05).

산업장 근로자들의 BMI 변화가 혈청총콜레스테롤의 변화에 미치는 영향 (Effects of Change in Body Mass Index on Change in Serum Total Cholesterol Levels among Industrial Workers)

  • 본 연구의 목적은 근로자들의 BMI 변화에 따른 혈청총콜레스테롤의 변화 및 고콜레스테롤혈증 발생비율과의 관련성을 파악하고자 시도하였다. 연구는 2002년부터 20012년까지 11년간 각 연도별로 1회 이상 정기건강검진을 받았던 30~69세 근로자 28,249명(남자 25,548명, 여자 2,701명)을 대상으로 검진기간을 전기(2002-2005), 중기(2006-2009) 및 후기(2010-2012)로 구분하여 각 기간별 BMI 구분에 따른 혈청지질의 평균치를 분석하였다. 또한, 기준년도(2002)로부터 10년간의 BMI의 변화량 구분별로 혈청총콜레스테롤의 평균변화량을 산출하였으며, 혈청총콜레스테롤 정상자에서의 BMI 구분별 10년간의 고콜레스테롤혈증 발생의 년차별 비율을 분석하였다. 연구결과, 서로 다른 3시기(전기; 2002~2005, 중기; 2006~2009 및 후기; 2010~2012)에서의 평균혈청총콜레스테롤은 BMI가 낮은 군($18.5kg/m^2$미만군)에서 중간군($18.5-25.0kg/m^2$군), 높은 군($25.0kg/m^2$ 이상군)으로 갈수록 유의하게 증가하였으며, 고콜레스테롤혈증 발생비율도 BMI가 낮은 군에서 높은 군으로 갈수록 유의하게 증가하였다. 위와 같이 10년간의 BMI 변화량에 대한 혈청총콜레스테롤의 평균변화량의 경년적 추이를 볼 때, BMI의 증가는 혈청총콜레스테롤의 증가에 유의한 영향을 미치는 것으로 밝혀졌다. 따라서 혈청지질치의 위험 관리를 위해서는 비만관리에 대한 평가와 중재가 필요함을 시사하고 있다.
