In vitro evaluation of fracture strength of zirconia restoration veneered with various ceramic materials

  • PURPOSE. Fracture of the veneering material of zirconia restorations frequently occurs in clinical situations. The purpose of this in vitro study was to compare the fracture strengths of zirconia crowns veneered with various ceramic materials by various techniques. MATERIALS AND METHODS. A 1.2 mm, $360^{\circ}$ chamfer preparation and occlusal reduction of 2 mm were performed on a first mandibular molar, and 45 model dies were fabricated in a titanium alloy by CAD/CAM system. Forty-five zirconia copings were fabricated and divided into three groups. In the first group (LT) zirconia copings were veneered with feldspathic porcelain by the layering technique. In the second group (HT) the glass ceramic was heat-pressed on the zirconia coping, and for the third group (ST) a CAD/CAM-fabricated high-strength anatomically shaped veneering cap was sintered onto the zirconia coping. All crowns were cemented onto their titanium dies with Rely $X^{TM}$ Unicem (3M ESPE) and loaded with a universal testing machine (Instron 5583) until failure. The mean fracture values were compared by an one-way ANOVA and a multiple comparison post-hoc test (${\alpha}$= 0.05). Scanning electron microscope was used to investigate the fractured interface. RESULTS. Mean fracture load and standard deviation was $4263.8{\pm}1110.8$ N for Group LT, $5070.8{\pm}1016.4$ for Group HT and $6242.0{\pm}1759.5$ N for Group ST. The values of Group ST were significantly higher than those of the other groups. CONCLUSION. Zirconia crowns veneered with CAD/CAM generated glass ceramics by the sintering technique are superior to those veneered with feldspathic porcelain by the layering technique or veneered with glass ceramics by the heat-pressing technique in terms of fracture strength.

치과용 도재의 재소성 과정중 수분 함량이 강도에 미치는 영향 (EFFECT OF WATER CONTENT ON THE FLEXURAL STRENGTH DURING REFIRING IN DENIAL PORCELAIN)

  • Statement of problem : Long-term exposure of dental porcelain to saliva during temporary cementation of a porcelain-fused to metal (PFM) restoration could affect mechanical strength of dental porcelain if the restoration is refired. Purpose : This work was performed to verify the effect of water on the mechanical strength in aged dental porcelain. Material and method : 63 specimens(Vintage Metalbond opaque and opal powder) were distributed to three experimental groups ; non-water immersed control, immersed and pedried, and immersed and non-predired groups. The changes in flexural strength and fracture toughness after specimen refiring related to Fourier Transform Infrared (FT-IR) spectroscopy. Results : 1. The FT-IR reflectances assigned to molecular bonds of $H_2O$ were noted as significantly different between the first-fired group and three refired groups and between two water-immersed groups and control group after refiring(p<0.05). They were also significantly different between predried group and non-predried group after refiring(p<0.05) 2. For opal specimens, FT-IR absorbances for hydrogen bond of $H_2O$ and silanols were significantly higher in non-predried group than in predreid group(p<0.05). 3 Predried opal group showed the highest mean flexural strength(p<0.05). Non-predried group indicated higher mean flexural strength than control group(p<0.05). 4. The mean fracture toughness for predired group was higher than non-predried group(p<0.05). 5. The difference of leucite crystal size is noted between control group and water-immersed, predried group in scanning electron microscopic study(${\times}10000$).

Comparison of traditional and simplified methods for repairing CAD/CAM feldspathic ceramics

  • PURPOSE. To evaluate the adhesion to CAD/CAM feldspathic blocks by failure analysis and shear bond strength test (SBSt) of different restorative systems and different surface treatments, for purpose of moderate chipping repair. MATERIALS AND METHODS. A self-adhering flowable composite (Vertise Flow, Kerr) containing bi-functional phosphate monomers and a conventional flowable resin composite (Premise Flow, Kerr) applied with and without adhesive system (Optibond Solo Plus, Kerr) were combined with three different surface treatments (Hydrofluoric Acid Etching, Sandblasting, combination of both) for repairing feldspathic ceramics. Two commercial systems for ceramic repairing were tested as controls (Porcelain Repair Kit, Ultradent, and CoJet System, 3M). SBSt was performed and failure mode was evaluated using a digital microscope. A One-Way ANOVA (Tukey test for post hoc) was applied to the SBSt data and the Fisher's Exact Test was applied to the failure analysis data. RESULTS. The use of resin systems containing bi-functional phosphate monomers combined with hydrofluoric acid etching of the ceramic surface gave the highest values in terms of bond strength and of more favorable failure modalities. CONCLUSION. The simplified repairing method based on self-adhering flowable resin combined with the use of hydrofluoric acid etching showed high bond strength values and a favorable failure mode. Repairing of ceramic chipping with a self-adhering flowable resin associated with hydrofluoric acid etching showed high bond strength with a less time consuming and technique-sensitive procedure compared to standard procedure.

Porcelain repair - Influence of different systems and surface treatments on resin bond strength

  • PURPOSE. The purpose of this study was to evaluate the bond strength of composite resin on the fracture surface of metal-ceramic depending on the repair systems and surface roughening methods. MATERIALS AND METHODS. A total of 30 disk specimens were fabricated, 15 of each were made from feldspathic porcelain and nickel-chromium base metal alloy. Each substrate was divided into three groups according to the repair method: a) application of repair system I (Intraoral Repair Kit) with diamond bur roughening (Group DP and DM), b) application of repair system I with airborne-particle abrasion (Group SP and SM), and c) application of repair system II (CoJet Intraoral Repair System, Group CP and CM). All specimens were thermocycled, and the shear bond strength was measured. The data were analyzed using the Kruskal-Wallis analysis and the Mann-Whitney test with a significance level of 0.05. RESULTS. For the porcelain specimens, group SP showed the highest shear bond strength ($25.85{\pm}3.51MPa$) and group DP and CP were not significantly different. In metal specimens, group CM showed superior values of bond strength ($13.81{\pm}3.45MPa$) compared to groups DM or SM. CONCLUSION. Airborne-particle abrasion and application of repair system I can be recommended in the case of a fracture localized to the porcelain. If the fracture extends to metal surface, the repair system II is worthy of consideration.

  • ;Dentistry has benefited from tremendous advances in technology with the introduction of new techniques and materials, and patients are aware that esthetic approaches in dentistry can change one's appearance. Increasingly. tooth-colored restorative materials have been used for restoration of posterior teeth. Tooth-colored restoration for posterior teeth can be divided into three categories: 1) the direct techniques that can be made in a single appointment and are an intraoral procedure utilizing composites: 2) the semidirect techniques that require both an intraoral and an extraoral procedure and are luted chairside utilizing composites: and 3) the indirect techniques that require several appointments and the expertise of a dental technician working with either composites or ceramics. But, resin restoration has inherent drawbacks of microleakage. polymerization shrinkage, thermal cycling problems. and wear in stress-bearing areas. On the other hand, Ceramic restorations have many advantages over resin restorations. Ceramic inlays are reported to have less leakage than resin restoration and to fit better. although marginal fidelity depends on technique and is laboratory dependent. Adhesion of luting resin is more reliable and durable to etched ceramic material than to treated resin composite. In view of color matching, periodontal health. resistance to abrasion, ceramic restoration is superior to resin restorationl. Materials which have been used for the fabrication of ceramic restorations are various. Conventional powder slurry ceramics are also available. Castable ceramics are produced by centrifugal casting of heat-treated glass ceramics. and machinable ceramics are feldspathic porcelains or cast glass ceramics which are milled using a CAD/CAM apparatus to produce inlays (for example, Cered. They may also be copy milled using the Celay apparatus. Pressable ceramics are produced from feldspathic porcelain which is supplied in ingot form and heated and moulded under pressure to produce a restoration. Infiltrated ceramics are another class of material which are available for use as ceramic inlays. An example is $In-Ceram^{\circledR}$(Vident. California, USA) which consists of a porous aluminum oxide or spinell core infiltrated with glass and subsequently veneered with feldspathic porcelain. In the 1980s. the development of compatible refractory materials made fabrication easier. and the development of adhesive resin cements greatly improved clinical success rates. This case report presents esthetic ceramic inlays for posterior teeth.teeth.

The effect of thermocycling on the bonding of different restorative materials to access opening through porcelain fused to metal restorations

  • PURPOSE. Porcelain fused to metal (PFM) crowns provide the best treatment option for teeth that have a large or defective restoration. More than 20% of teeth with PFM crowns or bridges require non-surgical root canal treatment (NSRCT). This may be due to the effect of restorative procedures and the possible leakage of bacteria and or their by-products, which leads to the demise of the tooth pulp. Thus, this study was planned to compare the ability of the restorative materials to seal perforated PFM specimens. MATERIALS AND METHODS. The study evaluates the ability of amalgam, composite or compomer restorative materials to close perforated PFM specimen's in-vitro. Ninety PFM specimens were constructed using Ni-Cr alloys and feldspathic porcelain, and then they were divided into 3 groups: amalgam (A), composite + Exite adhesive bond (B) and compomer + Syntac adhesive bond (C). All the PFM samples were embedded in an acrylic block to provide complete sealing of the hole from the bottom side. After the aging period, each group was further divided into 3 equal subgroups according to the thermocycling period (one week for 70 cycles, one month for 300 cycles and three months for 900 cycles). Each subgroup was put into containers containing dye (Pelikan INK), one maintained at $5^{\circ}C$ and the other at $55^{\circ}C$, each cycle for 30 sec time. The data obtained was analyzed by SPSS, 2006 using one way ANOVA test and student t-test and significant difference level at (P<.01). RESULTS. The depth of dye penetration was measured at the interfaces of PFM and filling materials using Co-ordinate Vernier Microscope. The lowest levels of the dye penetration for the three groups, as well as subgroups were during the first week. The values of dye leakage had significantly increased by time intervals in subgroups A and C. CONCLUSION. It was seen that amalgam showed higher leakage than composite while compomer showed the lowest level of leakage.

유기조성물을 이용한 페이스트형 일반도재 시스템 (Development of Organic Paste Porcelain for Fixed Prostheses)

  • 덴틴, 에나멜, 투명 일반도재를 페이스트화 하여 치아의 색상, 투명도 및 구조를 고려한 복합층구성물을 미리 제조하여 심미수복물을 용이하게 제작하는 공정을 개발하였다. 페이스트 상태에서 성형성과 유동특성이 확보되고, 소성 후 색도, 및 강도 등의 제특성들을 만족시킬 수 있는 결합제 시스템으로서 UV (creation universal liquid)를 선정하였고 조성 및 최적화가 이루어졌다. 페이스트는 24시간까지도 점성 및 작업성이 유지되었고, 이를 이용하여 복합층 구성물의 용이한 성형이 가능하였다. 소성수축률은 15.2%로 기존의 방법으로 제조한 시편에 비해 0.8% 낮았으나 유의차는 없었다. spectrophotometer에 의한 색도특성 관찰결과 덴틴재를 기존 제조방식으로 제조한 시편과 페이스트형 도재 시스템으로 제조한 시편에서 $L^*$, $a^*$, $b^*$가 각각 60~80, -1.5~+1.5, 5~20 사이의 값을 나타냄을 확인하였다. 측정값으로부터 구해진 제조방식에 따른 시편 간의 색차(${\Delta}E^*$)는 0.156으로 극히 근소한 차이를 나타냄을 확인하였다. 유기물 첨가에 따른 소성 후 잔류물 함량은 검출오차범위 내에 있었으며, 기존의 제조방식으로 제조한 소성체와 페이스트형 도재 시스템을 이용한 소성체의 강도는 각각 $70{\pm}4MPa$, $74{\pm}3MPa$으로 유의차가 없이 근사한 물성을 나타내었다. 페이스트형 도재를 이용한 도재축성법을 기존방법과 비교분석한 결과, 기계적 또는 광학적 특성의 변화없이 적용이 가능하였다.

2종의 열가압 도재와 법랑질 간의 마모에 관한 연구 (An in-vitro wear study of human enamel opposing heat-pressed ceramics)

  • 연구목적: 2종의 열가압 도재 (e.max Press, Empress Esthetic)와 법랑질 간의 마모양상을 알아보기 위하여 장석계 도재 및 제 3형 금합금과 비교, 관찰하였다. 연구 재료 및 방법: 열가압 도재 e.max Press, Empress Esthetic 및 금속도재관 제작용 장석계 도재 Cermaco 3, 제 3형 금합금으로 원반형 시편을 제작하고, 상대 마모재로는 발거된 건전한 소구치의 교두를 이용하여 Pin-on-disk tribometer 상에서 마모시험 하였다. 법랑질의 마모량은 마모 전후 치아 시편의 무게 변화를 측정하여 부피로 환산하였고, 도재 및 금합금 시편의 평균 표면 조도와 마모트랙 깊이를 표면조도기로, 마모트랙 폭을 주사전자현미경으로 측정하여 마모 양상을 비교하였다. 결과: 1. 법랑질의 마모량은 Ceramco 3군에서 가장 크고 Empress Esthetic군, e.max Press군, 금합금군 순으로 작았으며, Ceramco 3군과 Empress Esthetic군 사이와, Empress Esthetic군, e.max Press군 및 금합금군 사이에는 유의한 차이가 없었다 (P>.05). 2. 마모시험 후 도재 시편의 평균 표면 조도는 Ceramco 3군, Empress Esthetic군, e.max Press군 순으로 작았으며, Empress Esthetic군과 Ceramco 3군 사이에는 유의한 차이가 없었다 (P>.05). 3. 마모트랙의 깊이는 모든 군 사이에 유의한 차이는 없었고 (P>.05), 마모트랙의 폭은 법랑질 마모량 순서와 동일하게 Ceramco 3군, Empress Esthetic군, e.max Press군, 금합금군 순으로 작았으며, e.max Press군과 Empress Esthetic군 사이 및 Empress Esthetic군과 Ceramco 3군 사이에 유의한 차이는 없었다 (P>.05). 결론: 열가압 도재는 장석계 도재보다 법랑질을 더 마모시키지는 않았다.

Evaluation of marginal fidelity of copy-milled and CAD/CAM all ceramic crowns

  • Statement of the problem. The interest in all-ceramic restorations has increased as more techniques have become available. With the introduction of machinable dental ceramics and CAD/CAM systems or Copy-milling systems there is a need for evaluating the quality levels of these new fabrication techniques. Purpose. This study was to evaluate the fitting accuracy of machined all-ceramic crowns made out of an industrially prefabricated feldspathic porcelain. Material and Methods. Three master models with different cutting depth (0.8mm/1.0mm/1.2mm)were produced using a palladium-silver alloy. A total of 36 working dies, 12 of each form, was used for the modellation of prototype resin copings and 36 additional crowns, 12 of each cutting depth, were produced by using the $CEREC^{(R)}2$ system for all crowns. The maginal fit of all 72 crowns was then evaluated on their respective master die at 54 circularly staggered points of measurement per crown under a fixation pressure of 30 N by using a computerized video image system. Results. The medians of the copy-milled $CELAY^{(R)}$ crowns ranged from 29 to $36{\mu}m$. The highest value for the marginal gap was found in group B (cutting depth 1.0mm) at $107{\mu}m$. The median for the $CEREC^{(R)}2$ crowns was found between 43.5 and $70{\mu}m$. The maximum values for all three groups ranged from $181{\mu}m$ to $286{\mu}m$. With $286{\mu}m$ the highest value for marginal gap was found in group C. the Kruskal-Wallis test and multiple comparisons analysis procedure revealed a significant influence of the production technique on the marginal fit in all three groups (p<0,02). Conclusion. 1. The $CELAY^{(R)}$ system is capable to produce all-ceramic crowns with a significantly better marginal fit than the $CEREC^{(R)}2$ system. 2. As far as premolar crowns produced with the $CEREC^{(R)}2$ system are concerned, the cutting depth has a significant influence on fitting accuracy. 3. The production of crowns with an acceptable marginal fit is possible with both systems. However, adhesive luting is recommended for milled feldspathic porcelain crowns.

이온교환법에 의한 치과용도재의 강도증진 효과에 관한 실험적 연구 (AN EXPERIMENTAL STUDY OF THE EFFECT OF ION EXCHANGE ON STRENGTHENING OF DENTAL PORCELAIN)

  • Ion exchange strengthening is a chemical process whereby large alkali ions(such as potassium) are substituted for smaller ions(sodium) within the surfaces of glasses and ceramics, thereby reducing the thermal expansion coefficient of this surface region, and creating beneficial state of compressive stress within the near surface region. The purpose of this study was to determine the effects of ion exchange and etching treatments on the strength of some dental porcelains. Two feldspathic dental porcelains(Vitadur-N, G-Cera) were used in this study. A commercial ion exchange paste and etching gel containing 8% hydrofluoric acid were used for surface conditioning. Transverse strength was measured using a universal testing machine and the technique of EPMA(electron probe micro analysis) was used to access the potassium contents. The results were as follows: 1. Improvement in strength was only obtained by treating the surface placed in tension. 2. No changes in the dimensions of the treated specimens were detected when samples were measured with a micrometer. 3. There was significant increase in transverse strength of G-Cera IV group treated with etching and ion exchange, compared with G-Cera II group only treated with ion exchange. 4. From the results of EPMA test, increase in potassium contents was observed on the surface treated with ion exchange paste.
