Cochlear Implant Failure in the Pediatric Population

  • Background and Objectives: In cochlear implant (CI) surgery, the results and causes of revision and reimplantation may guide surgeons in establishing surgical protocols for revision surgery with safe audiological outcomes. The aim of this study was to review our experience in terms of etiology, surgical strategy, and hearing outcomes in pediatric patients who underwent CI removal and reimplantation. Subjects and Methods: All patients received implants of the same brand. Pre and postoperative Categories of Auditory Performance score and aided free-field pure tone audiometry thresholds were noted. In vivo integrity tests were performed for each patient and the results of ex vivo tests of each implant were obtained from manufacturer. Results: A total of 149 CIs were placed in 121 patients aged <18 years. The revision rate in children was 6.7% (10/121 children). Six patients had a history of head injury leading to a hard failure. The causes of reimplantation in others were soft failure (n=1), electrode migration (n=1), infection (n=1), and other (n=1). All patients showed better or similar postreimplantation audiological performance compared with pre-reimplantation results. Conclusions: It is very important to provide a safe school and home environment and educate the family for reducing reimplantation due to trauma. Especially for active children, psychiatric consultation should be continued postoperatively.

창업자의 자기효능감 및 실패 경험이 창업지원서비스에 대한 니즈에 미치는 영향 (The Effect of Self-Efficacy and Failure Experience on the Needs of Start-up Support Services)

  • 본 연구에서는 자기효능감이나 불확실성에 대한 태도와 같은 창업자의 심리적 특성에 따라 창업지원서비스에 대한 니즈(needs)가 달라질 수 있다는 가설을 세우고 실증분석을 통해 이를 검증하였다. 서울과 대전 지역 창업보육센터에 입주한 86개 기업의 창업자를 대상으로 설문조사를 실시하고 그 데이터를 기반으로 실증분석을 수행하였다. 그 결과, 창업자의 자기효능감이 높을수록 대학 외부 번화가에 창업존을 제공하려는 창업지원서비스에 대해 적극적인 수용의사를 밝힌 반면, 과거 창업에서 실패경험을 가진 창업자는 대학 외부에 위치한 고객 유치에 대해 긍정적인 반응을 보였다. 또한, 창업기업의 성과가 좋지 않을수록, 그리고 여성 보다는 남성 창업자가 창업지원에 대한 니즈(needs)가 더 큰 것으로 나타났다. 연구결과는 창업지원에 대한 정책을 결정할 때에는 업종이나 성장단계와 같은 창업기업 차원의 특성 뿐 아니라 창업자의 심리적 상태 역시 중요한 고려대상에 포함시켜 차별화된 창업지원서비스를 설계해야 함을 시사한다.

Learning Curve of C-Arm Cone-beam Computed Tomography Virtual Navigation-Guided Percutaneous Transthoracic Needle Biopsy

  • Objective: To evaluate the learning curve for C-arm cone-beam computed tomography (CBCT) virtual navigation-guided percutaneous transthoracic needle biopsy (PTNB) and to determine the amount of experience needed to develop appropriate skills for this procedure using cumulative summation (CUSUM). Materials and Methods: We retrospectively reviewed 2042 CBCT virtual navigation-guided PTNBs performed by 7 novice operators between March 2011 and December 2014. Learning curves for CBCT virtual navigation-guided PTNB with respect to its diagnostic performance and the occurrence of biopsy-related pneumothorax were analyzed using standard and risk-adjusted CUSUM (RA-CUSUM). Acceptable failure rates were determined as 0.06 for diagnostic failure and 0.25 for PTNB-related pneumothorax. Results: Standard CUSUM indicated that 6 of the 7 operators achieved an acceptable diagnostic failure rate after a median of 105 PTNB procedures (95% confidence interval [CI], 14-240), and 6 of the operators achieved acceptable pneumothorax occurrence rate after a median of 79 PTNB procedures (95% CI, 27-155). RA-CUSUM showed that 93 (95% CI, 39-142) and 80 (95% CI, 38-127) PTNB procedures were required to achieve acceptable diagnostic performance and pneumothorax occurrence, respectively. Conclusion: The novice operators' skills in performing CBCT virtual navigation-guided PTNBs improved with increasing experience over a wide range of learning periods.

사업위험에 대한 인지가 창업의욕 및 사업유지의욕에 미치는 영향 분석 (Analysis of Recognition on Business Risks and Entrepreneurial Intention)

  • 본 연구에서는 현재 사업을 운영하고 있는 사업자와 사업을 운영하지 않고 있는 잠재창업자인 20세 이상 남녀를 조사대상으로 사업에 대한 태도 및 위험인지가 여러 특성에 따라 어떤 차이가 있는지 그리고 어떤 변수들에 의해 영향 받는지를 조사하였다. 또한, 태도 행동 모델과 소비자의사결정 모델을 응용하여 사업태도와 위험인지 변수가 창업 및 사업유지의사에 미치는 영향력을 조사하였다. 본 연구 결과를 정리 요약하면 다음과 같다. 첫째, 남성, 비전문직 종사자, 부모가 창업경험을 가진 경우 사업에 대한 태도가 긍정적인 것으로 나타났다. 한편, 취업하지 않은 경우, 연령이 40대 이상인 경우, 과거 사업경험이 있는 경우 사업에 대한 위험인지 수준이 높은 것으로 드러났다. 둘째, 남성, 부모가 창업한 경험이 있는 경우, 자아실현의 창업동기를 갖는 경우 창업의사가 강한 것으로 드러났다. 한편, 사업실패경험이 없는 경우, 101억부터 1천억 사이의 매출액을 갖고 있는 사업체 운영자, 긍정적인 사업태도를 가진 경우 사업유지의사가 높은 것으로 나타났다.

스트레스 사건의 경험이 주의변화에 미치는 영향 (Influence of Stress Experience on Change of Attention)

  • For a man to maintain attention, he needs to keep a certain level of arousal. An inordinate increase or decrease in the level of arousal eventually has a negative influence on attention. Precedent research has shown that the degree of attention changes when an experience of stress is related to anxiety resulting in a rise in arousal. This research was done to examine this hypothesis by looking at the 27 female students, 14 of whom had failed in the annual examination. The results of the investigation are as follows : The stress of failure in the examination was seen to raise the level of physiological arousal. Although pulse and electromyography showed no significant change, further inquiries should be made based on other types of methodology. In spite of the rise of arousal, the performance of selective task was degraded. This suggests those students failed to give moderate attention to given information for that kind of task. But the exact reason of that failure was not identified : that is it was difficult decide whether they gave too much attention to the anxiety brought about by stress. Performance of integral tasks, however, did not show any degradation. Judging from these results, stress seems to exert significant influence on attention in the selection of the appropriate information among the various options given. This offers an important hint in relation to the health care situation where nursing information is offered. Clients who receive nursing information in stressful situations may have difficulty in separating and selecting this helpful information from other options which they have acquired through their life experience. The content and terminology of nursing information may be strange and unintelligible to clients, although they are quite familiar and distinct to nurses. So, it is desirable for nurses to give, in addition and at the same time when nursing information is given, some certain related information as devices for selection, instead of merely giving nursing informations as such. So far it is not clear whether the concepts of information processing theory can be suitably applied to nursing. However, it is obvious, according to this research, that the quality of attention is disturbed in the stress situation. This is why further inquiries should be made into attention in practical nursing situation.

Trainees Can Safely Learn Video-Assisted Thoracic Surgery Lobectomy despite Limited Experience in Open Lobectomy

  • Background: The aim of this study was to establish whether pulmonary lobectomy using video-assisted thoracic surgery (VATS) can be safely performed by trainees with limited experience with open lobectomy. Methods: Data were retrospectively collected from 251 patients who underwent VATS lobectomy at a single institution between October 2007 and April 2011. The surgical outcomes of the procedures that were performed by three trainee surgeons were compared to the outcomes of procedures performed by a surgeon who had performed more than 150 VATS lobectomies. The cumulative failure graph of each trainee was used for quality assessment and learning curve analysis. Results: The surgery time, estimated blood loss, final pathologic stage, thoracotomy conversion rate, chest tube duration, duration of hospital stay, complication rate, and mortality rate were comparable between the expert surgeon and each trainee. Cumulative failure graphs showed that the performance of each trainee was acceptable and that all trainees reached proficiency in performing VATS lobectomy after 40 cases. Conclusion: This study shows that trainees with limited experience with open lobectomy can safely learn to perform VATS lobectomy for the treatment of lung cancer under expert supervision without compromising outcomes.

대학생의 자살시도 경험 (Experience of College Students on Suicide Attempts)

  • Purpose: The purpose of the study was to explore and describe the experience of Korean college students on suicide attempts. Methods: Eight students participated and data were collected through in-depth individual interviews between September, 2011 and April, 2012. Data were analyzed using Strauss and Corbin's grounded theory method. Results: Analysis showed that the central phenomenon of suicide attempt experience of college students was 'inextricable despair'. Causal conditions were 'sense of shame by failure to achieve perfect independence' and 'a big gap between reality and ideals'. Contextual conditions were 'extreme situation of being cornered' and 'excessive changes in emotions'. Intervening conditions were 'important others' and 'perspectives on the world'. Action/interaction strategies were 'temporary efforts' and 'gathering up one's mind'. Consequences were 'trauma as one's own hurt', 'conflict between life and death' and 'becoming mature'. Conclusion: The results of this study provide an in-depth understanding of the experience of college students with attempted suicide. It is necessary to develop programs to prevent suicide attempts by college students and these results can be used as a basis for program development.

  • 기업은 실시간으로 쏟아져 나오는 제조품과 서비스에 대한 체험마케팅을 위해 테마파크와 제휴를 맺는다. 테마파크는 기업과의 스폰서십으로 인해 재정적 부담을 덜고, 기업은 마케팅 실패의 위험을 감소시킬 수 있는 상호 이익적관계이다. 이러한 이해 관계의 만남은 새로운 패러다임의 테마파크인 '키자니아'를 탄생시켰다. 본 연구에서는 새로운 패러다임의 테마파크인 '키자니아'의 스폰서십 운영을 살펴보고, 콘텐츠의 체험요소를 Pine II, B. Joseph 와 Gillmore, James H의 체험경제 이론을 대입하여 분석한 후, 그것의 한계점과 새로운 패러다임으로써 발전할 수 있는 효율성 극대화 방안을 제언하고자 한다.

중소기업의 해외마케팅에서 고객경험이 수출성과에 미치는 영향: 인적접촉과 민첩성의 조절효과 (The Effect of Customer Experience on Export Performance in Overseas Marketing of SMEs: Moderating Effects of the Personal Contacts and Agility)

  • As the digital era accelerates, traditional perspectives have limitations in explaining the success or failure of export performance. The purpose of this study is to analyze new factors affecting export performance from the perspective of customer experience, which has emerged as an important factor in securing a competitive advantage and generating organizational performance. After deriving hypotheses based on literature review and discussion, a research model is designed in which three factors of customer experience such as understanding customer's objectives, customer value creation capability, and customer journey management are the antecedents, and export performance is the dependent variable. This model also includes organizational agility and personal contact as the moderating variables. To verify the hypotheses, multiple regression analysis was conducted on the collected data drawn from 198 SME exporters. According to the analysis results, it was found that all three antecedents positively affected export performance. In particular, the organizational agility and personal contact were confirmed to have a moderating effect that creates better export performance by interacting with customer value creation capability. The theoretical significance of this study is to find that effective customer experience management can be a key factor in creating export performance. The results suggest that checking the overall customer journey, exporters should select and intervene to intensively manage key touch points that can have a decisive impact on the quality of customer experience. At the end of the paper, practical implications to be considered in creating export performance through effective customer experience management are presented.
