  • This paper describes the symptoms with the failure causes of pole transformers analyzed at KEPCO branch offices' request in 2001, and ultimately aims for devising proper countermeasures against the failures. The failure causes are generally divided into problems in manufacturing, mounting and operating. The pole transformers encounter the failure with unique mechanisms. To verify the mechanisms to failures and help devise proper countermeasures, a flowchart is proposed based on the authors' experience and foreign case studies. The failure causes of pole transformers can be analyzed more exactly by using the flowchart.

  • The purpose of this paper is to analyze piping failure trend of safety pipings In domestic nuclear power plants. First, database for the piping failure was constructed with 105 data fields. The database includes plant population data, event data, and service history data. 7 kinds of piping failures in domestic NPPs were investigated. Among the 7 cases, detailed root causes were investigated for 3 cases. The first one is pipe wall thinning in main feedwater pipings of Westinghouse 3 loop type plants. The root cause of the wall thinning was flow accelerated corrosion near welding area. The next one is leak event in chemical and volume control system(CVCS) due to vibration. Some cracks occurred in socket welding area. The events showed that the integrity or socket weld is very vulnerable to vibration. The last one is also a leak event in primary sampling line in Korean standard reactor due to thermal fatigue. Although the structural integrity was not maintained by the events, there was no effect on nuclear safety in the above 3 piping failure eases.

  • In the semiconductor manufacturing, yield enhancement is an urgent issue. It is ideal to prevent all the failures. However, when a failure occurs, it is important to quickly specify the cause stage and take countermeasure. Reviewing wafer level and composite lot level yield patterns has always been an effective way of identifying yield inhibitors and driving process improvement. This process is very time consuming and as such generally occurs only when the overall yield of a device has dropped significantly enough to warrant investigation. The automatic method of failure pattern extraction from fail bit map provides reduced time to analysis and facilitates yield enhancement.   • Systems with journal bearings generally operate in large scale and under severe loading conditions such as steam generator turbines and internal combustion engines, in contrast to the machineries using rolling element bearings. Failure of the bearings in these machineries can result in the system breakdown. To avoid the time consuming repair and considerable economic loss, the detection of incipient failure in journal bearings becomes very important. In this experimental approach, acoustic emission monitoring is employed to the detection of incipient failure caused by intervention of foreign particles most probable in the journal bearing systems. It has been known that the intervention of foreign materials, insufficient lubrication and misassembly etc. are principal factors to cause bearing failure and distress. The experiment was conducted under such designed conditions as inserting alumina particles to the lubrication layer in the simulated journal bearing system. The results showed that acoustic emission could be an effective tool to detect the incipient failure in journal bearings.

  • This paper deal with the stability evaluation and suggestion of progressive failure slope in biotite granite area of Andong. Based on geological site investigation and field test, stability analysis of slope was performed in conjunction with limit equilibrium methods and stereographic projection. Additionally, initial design and construction procedure was critically evaluated. Series of the slope stability analysis reveals the detection of local wedge and plane failure under the current slope condition. It is additionally appeared that a certain synthetic behavior of circle and plane failure exists on the right spot where the overall failure's going in progress. In order to construct more stable slope based on the suitability for the real state of the slope circumstances, this study issues a solution to eliminate the primary factors which cause the instability, by means of the grade of weathering and RMR classification of rock mass.

  • This paper describes an investigation on bearing failure due to spragging that has been continuously occurred in turbine hearings. The spragging is defined as the damage found on the leading edge of unloaded pads in the tilting pad journal bearing, In general, the damage mechanism by spragging is classified into fatifgue failure, The principle cause of spragging could be thought as the self-excited vibration by the absence of a stable static equilibrium position of upper pads with no preload. Because of serious consequences of system breakdowns due to bearing failures, determination ar the causes of failure and effective method for countermeasures are very important. This paper describes both the causes of spragging and countermeasures for prevention of such failure, which are taken place in the electric power plants.

  • In this paper, we consider a standby redundant structure with a function of switchover processing which may not be not perfect. The switchover processing is governed by a control module whose failure may cause the failure of the whole system. The parameters measuring such an effect of failure of the control module is included in our reliability model. We compute several reliability measures such as reliability function, failure rate, MTBF, mean residual life function, and the steady state availability. We also compare a single unit structure and the redundant structure with regard to those reliability measures. An example is given to illustrate our results.

  • An aerospace vehicle in flight can be exposed to random gust which may cause critical structural failure. In this paper, the failure analysis is conducted for composite wing subjected to random gust. For this, the profile of random gust is idealized as a stationary Gaussian random process and the power spectral density (PSD) of wing bending moment induced by gust is obtained. The PSD function is converted to probabilistic distributions and the failure probability during total flight time is calculated by Monte Carlo simulation.

  • Mission Computer performs important role both managing a whole system and dealing with a specific mission in avionics system. In general, the fault of SPOF(Single Point Of Failure) in unity system can lead to failure of whole system. It can cause a failure of a mission and also can threaten to the life of the pilot. So, in this paper, we design the HA(High-availability) system so that dealing with the failure. And we use HA software like Heartbeat, Fake, DRBD and Bonding to managing HA system.

  • Copper-based leadframe sheets were immersed in two kinds of hot alkaline solutions to form brown-oxide or black-oxide layer on the surface. The oxide-coated leadframe sheets were molded with epoxy molding compound (EMC). After post mold curing, the oxide-coated EMC-leadframe joints were machined to form sandwiched double-cantilever beam (SDCB) specimens. The SDCB specimens were used to measure the fracture toughness of the EMC/leadframe interfaces under quasi-Mode I loading conditions. Fracture surfaces were analyzed by various equipment to investigate failure path. The present paper deals with the failure path, and the cause of the failure path formation with an adhesion model will be treated in the succeeding paper.
