Since permanent magnet coupling transfers power by magnetic force without contact, it has little shock, vibration, noise. In case of overload, it protects a pump or a motor which is relatively important by slipping internally. In this study, failure analysis and test evaluation on the permanent magnet coupling have been proposed and the process that reliability of the product improves through design improvement has been presented. And failure cause of typical failure case has been investigated and improvement plan has been presented. Finally, reliability improvement is established by analysis of the test results of before and after acceleration test.

Paraquat is a non-selective contact herbicide. When it is consumed, it may cause fatal disorders such as acute renal failure, hepatic dysfunction, and progressive respiratory failure. In spite of many efforts to cure patients poisoned with paraquat, the mortality rate still remain high. In this case, after using Gamdutanghaphwangryunhaedoktang-gamibang and Cheongsangboha-tang we got positive result in hepato-renal function, but progressive respiratory failure was unstoppable.

Congestive heart failure is a cardiac dysfunction that can't supply sufficient amount of blood. The disorder usually causes hypertension, ischemic cardiac disease and give rise to pulmonary congestion and pulmonary epileptic edema. The symptoms of the disease are dyspnea, palpitation, edema, etc. We treated a 82 year-old female patient who had severe dyspnea, nausea, and dizziness. At the point of differentiation of syndromes(辨證), the case was diagnosed as Su-Cheon(水喘) induced by Yang-deficiency of Both Spleen and Kidney(脾腎陽虛) and was administered Yerunpang(胃?湯). After two weeks of the treatment, we witnessed improvement in the symptoms of CHF and general depressed condition. This report describes the process and contents about the way the patient was treated.

We develop in this paper the likelihood ratio test (LRT) for testing $H_1 : F_1 \preceq F_2$ against $H_2 - H_1$ where $H_2$ imposes no restriction on $F_1$ and $F_2$ and '$\preceq$' means failure rate ordering. Both one and two-sample problems will be considered. In the one-sample case, one of the two distributions is known, while we assume in the other case both are unknown. We derive the asymptotic null distribution of the LRT statistic which will be of chi-bar-square type. The main issue here is to determine the least favorable distribution which is stochastically largest within the class of null distributions.

Prosthetic reconstruction of musculoskeletal defects about the knee for tumor has many advantages, particularly the maintenance of motion and immediate functional restoration. But, prosthetic reconstruction has inherent limitations in terms of long-term durability. The authors have reported here a patient who had mechanical failure at 61 months later following use of a modular resection system to reconstruct the segmental defect of proximal tibia in osteosarcoma. In this case, another technique of extensor mechanism reconstruction using Achilles tendon allograft was attempted. Because of the concerns involving durability of tumor prosthesis, increased emphasis has to be placed on innovation in prosthetic design.

As the number, complexity and interaction of electrical, electronic and programmable electronic (E/E/PE) systems increase, a growing emphasis has been placed on the concept of functional safety during product development. IEC 61508 provides guidelines and standardized procedures in the development of reliable and dependable E/E/PE systems to assure functional safety. Determining risk classes (i.e., safety integrity levels, SILs) associated to a specific E/E/PE item may be recognized as one of the most crucial activities in the product development per IEC 61508 since SILs are used to specify necessary safety requirements for achieving an acceptable residual risk. This article presents a case study on the assessment of SILs applying failure modes, effects and diagnostic analysis (FMEDA) from which failure rates may be derived for each important failure category by combining a standard FMEA with online diagnostic techniques.

IgA nephropathy usually presents as asymptomatic microscopic hematuria or proteinuria or episodic gross hematuria after upper respiratory infection. It is an uncommon cause of end-stage renal failure in childhood. Pulmonary hemorrhage associated with IgA nephropathy is an unusual life-threatening manifestation in pediatric patients and is usually treated with aggressive immunosuppression. Pulmonary hemorrhage and renal failure usually occur concurrently, and the pulmonary manifestation is believed to be caused by the same immune process. We present the case of a 14-year-old patient with IgA nephropathy who had already progressed to end-stage renal failure in spite of immunosuppression and presented with pulmonary hemorrhage during oral prednisone treatment. His lung disease was comparable to diffuse alveolar hemorrhage and was successfully treated with plasmapheresis followed by oral prednisone. This case suggests that pulmonary hemorrhage may develop independently of renal manifestation, and that plasmapheresis should be considered as adjunctive therapy to immunosuppressive medication for treating IgA nephropathy with pulmonary hemorrhage.

The adhesively bonded tubular single lap joint shows a nonlinear relationship between the applied torque and the resulting displacement under the static-torsional loading, which is induced from the nonlinear properties of the adhesive. However the torque transmission capability in the case of the dynamic-torsional loading is much less than that in the case of the static-torsional loading, the stress level of the adhesive is usually in the region of the linear stress and strain relation and the stress distributions of the joint can be obtained by the linear analysis. In this paper, a failure model for the adhesively bonded tubular single lap joint under the torsional fatigue loading was developed with respect to the adhesive thickness that was a critical factor in predicting the static torque transional-cyclic loadings was proposed.

Calciphylaxis is a rare disease that appear in patients with secondary hyper-parathyroidism or chronic renal failure or that show defect in calcium phosphate metabolism which is characterized by fibrin deposit or calcification of medial wall of vessels causing gradual ischemic skin necrosis. Calciphylaxis is a disease with poor prognosis as skin necrosis can progress rapidly. If left untreated, calciphylaxis will progress to sepsis with high mortality. The treatment is controversial but kidney transplantation or parathyroidectomy is suggested to recover calcium-phosphate metabolism. The authors have experienced calciphylaxis in a patient with chronic renal failure caused by DM nephropathy with characteristic skin lesion and rapid skin necrosis. We describe this case with documentary reviews.

Traumatic tricuspid regurgitation is a rare complication of blunt chest trauma caused by chordal rupture, anterior papillary muscle rupture and anterior leaflet tear. Since clinical symptoms are vague, early diagnosis is difficult and some patient exhibit symptoms of right heart failure. Right heart failure has been the traditional indication for surgical treatment, such as tricuspid valve replacement. Recently, early detection using transthoracic echocardiography and surgical treatment, like valve repair, prior to overt right heart failure have been shown to better prognosis. We report a case of traumatic tricuspid regurgitation with chordal rupture in patient due to traffic accident.
