CT Observation of Alloplastic Materials Used in Blow Out Fracture (안와골절 정복술에 사용된 인공삽입물의 전산화단층촬영 추적관찰)

  • Purpose: Distinguishing different types of implants and assessing the position and size of implants by radiologic exam after orbital wall reconstruction is important in determining the surgery outcome and forecasting prognosis. We observed time-dependent density changes in three types of implants (porous polyethylene, resorbing plate and titanium mesh plate) by performing facial bone CT after orbital wall reconstructions. Methods: A total of 32 patients, who had underwent orbital wall fracture surgery from October 2006 to March 2009 and received facial bone CT as outpatients at 1 postoperative year were included in the study. Follow-up facial bone CT was performed on the patients pre- operatively, 1 month post-operatively, and 1 year post-operatively to observe the status of the orbital implants. Medpor $^{(R)}$ (Porex Surgical, Inc., Newnan, Ga.) was used as porous polyethylene and followed-up in 14 cases; for resorbing plate, Synthes mesh plate (Synthes, Oberdorf, Switzerland) was used in the reconstruction, and followed-up in 11 cases; and titanium mesh plate usage was followed-up in 7 cases. Computed tomographic scan (CT) and water's view were done for radiography, and hounsfield unit (HU) was used to compare density of those facial bone CT. Wilcoxon signed rank test was applied to statistically verify measurement difference in each group of hounsfield units. Results: Facial bone CT examination performed in 1 month post-operative showed that the density of porous polyethylene, resorbing plate and titanium mesh plate were -42.07, 105.67 and 539.48 on average, respectively. Among the three types of implants, titanium mesh plate showed the highest density due to its radiopaque feature. Following up the density of three types of implants in CT during 1 year after the orbital wall fracture surgery, the density of porous polyethylene increased in 10.52 House Field Units and the resorbing plate was decreased in 26.87 HouseField Units. There were no significant differences between densities in 1 month post-operatively and 1 year post-operatively in each group ($p{\geq}0.05$). Conclusion: We performed facial bone CT on patients with orbital fractures during follow-up period, distinguishing the types of implants by the different concentration of implant density, and the densities showed little change even at 1 year post-operative. To observe how implant densities change in facial bone CT, further studies with longer follow-up periods should be carried out.

Emulsion Stability of Cosmetic Facial Cream O/W Emulsions Prepared by Brij Type Non-ionic Emulsifie (Brij계 비이온성 혼합유화제를 이용하여 제조된 화장용크림 O/W 유화액의 유화안정성)

  • In this study, the stability of O/W cosmetic facial cream emulsions according to HLB values was evaluated by mixing nonionic surfactants, such as Brij 78&72 and Brij 98&92. Brij 78&72 (steareth-20&steareth-2, EMS-01), saturated fatty acid, and Brij 98&92 (oleth-20&oleth-2, EMS-02), unsaturated fatty acid, were used as mixed surfactants. The stability of the O/W emulsion was evaluated by using the emulsion viscosity, particle size, particle size distribution, and zeta-potential. The viscosity of the emulsion increased with the increase of time for EMS-01 while that of EMS-02 decreased with the increase of HLB value. The particle size of both EMS-01 and EMS-02 increased with time. The emulsifier with a HLB value of 10.8, which is the most similar to the required HLB value of mineral oil, 10.5, had the smallest particle size and highest density and also showed the highest emulsion stability. The zeta-potential of both emulsions tended to increase with the HLB value. No significant changes were observed in emulsions of the HLB value of 10.8 or more. The saturated fatty acid system, EMS-01, exhibited a higher zeta-potential value than that of the unsaturated fatty acid EMS-02 and also was superior in the stability.

Error Concealment Based on Semantic Prioritization with Hardware-Based Face Tracking

  • With video compression standards such as MPEG-4, a transmission error happens in a video-packet basis, rather than in a macroblock basis. In this context, we propose a semantic error prioritization method that determines the size of a video packet based on the importance of its contents. A video packet length is made to be short for an important area such as a facial area in order to reduce the possibility of error accumulation. To facilitate the semantic error prioritization, an efficient hardware algorithm for face tracking is proposed. The increase of hardware complexity is minimal because a motion estimation engine is efficiently re-used for face tracking. Experimental results demonstrate that the facial area is well protected with the proposed scheme.

Effects of fractional fourier transform of facial images in face recognition using eigenfeatures (고유특징을 이용한 얼굴인식에 있어서 얼굴영상에 대한 분수차 Fourier 변환의 효과)

  • We studied the effects of fractional fourier transform in face recognition, in which only the amplitude spectra of transformed facial images were used.We used two recently developed face recognition methods, the most effective feature (MEF) method (i.e., eigenface method) and most discriminating feature (MDF) method, and the effects of th etransform for th etwo methods were consistent. We confirmed that the recognition rate by the use of MDF method is better than that consistent. We confirmed that the recognition rate by the use of MDF method is better than that by MEF regardless of the order to transform, these methods provided slightly better results when the order was 1 than for any other order values. Only when the order was close to 1, the recognition rates were robust to the shift of the input images, and the trend that the recognition rates decreased as the input size varied was independent of the order. From these results, we fond that it is most advantageous to use the amplitude spectra of the conventional fourier transform whose order is 1.

  • The purpose of this study was to find out the craniofacial skeletal characteristics and to establish standards in facial patterns of Korean adolescences with normal occlusion. The subjects consisted of 54 males and 71 females ranging in age from 12 to 16 years. To classify facial patrons, number the clinical deviations from the normal five key measurements: the facial axis, facial angle, mandibular plane angle, lower face height, mandibular arc, are added and averaged with the proper sign to divide subjects into three groups ; brachyfacial, mesofacial dolichofacial groups. For the comparison of each group, a total of 43 morphologic variables were employed and the data were analyzed by statistical methods. The findings of this study can be summerized as follows; 1. The mesofacial group was $54.4\%$, the brachyfacial group was $29.6\%$ and the dolichofacial group was $16.0\%$ in this subjects. 2. There were no significant differences in size and shape of cranial base among each groups. 3. The brachyfacial group manifested the forward positioned maxilla and mandible to anterior cranial base, smaller cant of the mandibular plane, square shaped mandible, and prominent symphysis as compared with dolichofacial group. 4. There were no significant differences in the cant of the maxilla to the anterior cranial base among each groups. 5. Ramal height and madibular body length of brachyfacial group were larger than those of dolichofacial group. 6. Brachyfacial group indicated the smallest degree in divergency of maxilla and mandible while dolichofacial group showed the largest degree.

Characteristics of Distance, Angle and Ratio among the Face Point on Photo in Sasang Constitutional Medicine (사상체질별 안면의 거리 각도 비율 특성)

  • 1. Objectives: We aimed to derive constitutional facial characteristics with front and side picture of the people in this study 2. Methods: Through November 2007 to July 2009, we obtained front and side face photograph data of 715 male/female constitution confirmed subjects within the age range of 10-80 from 19 oriental medical facilities in the country. According to sex, we divided the subjects into two groups, real constitution group and non-constitution group. We analyzed significant variables of distance, angle and ratio of facial point through unpaired T-test. 3. Results: There are different significant variables according to Sasang Constitution and even though they are much different between sex, the interpretation of the meaning of those variables are similar with the written characteristics of ancient Writing. The face size of the Taeeumin is bigger than that of the non-Taeeumin, and especially they have long length from mouth to mandible angle. Soeumin have small jaw area, long philtrum and narrow facial feature like egg. Soyangin has developed forehead and glabella, short jaw, long eye side. Taeyangin has developed forehead, weak nose especially side end of nose, long eye side. 4. Conclusions: Sasang Constitutional facial characteristics are different between man and woman but there are much similarity with the facts as known on the interpretation of significant variables characteristics.

A SOFT TISSUE ANALYSIS ON FACIAL ESTHETICS OF KOREAN YOUNG ADULTS (한국인 성인의 악안면 연조직의 심미적 안모형태 분석에 관한 연구)

  • This study was designed to analyze morphological characteristics of Korean young adults, norms and standard deviation of variables, sexual differences, correlationship between each area of face and correlationship between hard tissue and soft tissue. The primary sample consisted of 45 males and 57 females who were early and middle twenties and had acceptable profile, no history of previous orthodontic treatment, absence of remarkably large overjet and overbite, full complement of permanent teeth, Class I skeletal and dental relationships and good vertical facial proportions. Their cephalograms were analyzed morphologically with a computer morphometrics. Then the final sample - 25 males and 38 females - were selected within 1 S.D. of E-line, ANB, P/A facial height ratio, Interincisal angle, L1 to A-Pog, ODI and APDI. The results of the study were as follows: 1 In the form and proportion of facial skeleton there were no significant differences between males and females, but in the size males were larger than females. 2. The dental protrusion patterns had no significant sexual difference and no significant correlationship between protrusion of upper lip and inclination of upper incisor. But mentolabial angle had positively correlated with interincisal angle and negatively with inclination of upper and lower incisor. 3. In the relationship between nose and soft-tissue profile, males were larger than females in nasal length, height and angular measurements. 4. In analysis of soft-tissue profile, males were larger than females in the length and thickness. In the angular measurements and proportion of soft-tissue profile, there were no significant differences between males and females.

A Study on Facial Wrinkle Detection using Active Appearance Models (AAM을 이용한 얼굴 주름 검출에 관한 연구)

  • In this paper, a weighted value wrinkle detection method is suggested based on the analysis on the entire facial features such as face contour, face size, eyes and ears. Firstly, the main facial elements are detected with AAM method entirely from the input screen images. Such elements are mainly composed of shape-based and appearance methods. These are used for learning the facial model and for matching the face from new screen images based on the learned models. Secondly, the face and background are separated in the screen image. Four points with the biggest possibilities for wrinkling are selected from the face and high wrinkle weighted values are assigned to them. Finally, the wrinkles are detected by applying Canny edge algorithm for the interested points of weighted value. The suggested algorithm adopts various screen images for experiment. The experiments display the excellent results of face and wrinkle detection in the most of the screen images.

Experience of Single Stage Treatment of Caniosynostosis, Hypertelorism, Exophthalmos Patient (두개조기유합증, 안와격리증, 안구돌출증 환자의 동시 교정 치험례)

  • The simultaneous correction of the hypertelorism and exophthalmos combined with craniosynostosis is very rarely performed operative procedures in the world. The craniosynostosis is the congenital anomaly that designates premature fusion of one or more sutures in either cranial vault or cranial base. Hypertelorism is not a distinct clinical syndrome in itself, but is a physical finding secondary to facial and cranial maldevelopment and it is defined as a increase in the distance between the medial orbital walls. Exophthalmos can occur following the decrease in the size of the orbit in patients with developmental skeletal disorders such as craniofacial synostosis. The authors experienced 9-year-old male patient, who has complex cranio-facial abnormality. The craniosynostosis was oxycephaly type and primary fronto-orbital advancement surgery had been performed in other hospital. The abnormal cranial vault combined with hypertelorism and exophthalmos due to maldeveloped both orbital walls. Surgical correction was obtained by various cranio-fronto-orbital remodeling technique such as calvarial bone craniotomy, fronto-orbital advancement, paramedian resection, medial canthopexy, Tessier-Wolfe three wall orbital expansions. We achieved a quite satisfactory result both functionally and aesthetically in a complex cranio-facial deformity patient by combination and modification of previously developed various cranio-facial plasty technique and hereby report the case with brief discussion and review of literature.

The treatment of post-traumatic facial skin defect with artificial dermis

  • Background: When a skin defect occurs, clinicians must work to restore the original skin quality as soon as possible. Accordingly, an artificial dermis can be used to supplement the wound and prevent severe scar contracture formation. The Terudermis is an artificial dermis that is simple and easy to use. We investigated the effectiveness of the Terudermis in the treatment of facial skin defects by analyzing previous relevant cases treated in our institution. Methods: We retrospectively examined 143 patients who were treated with the Terudermis graft in facial skin defect at Dong Kang General Hospital in 2015 and 2016. The patients' age, sex and location, wound size, complications were analyzed. In addition, the patients were asked to complete a self-satisfaction questionnaire after 18 months from the completion of treatment. The results were compared with that of autologous full-thickness skin graft (FTSG) and split-thickness skin graft (STSG) patients in same period. Results: The mean self-satisfaction scores evaluated by patients were $4.1{\pm}1.0$, $4.0{\pm}1.3$ and $3.5{\pm}1.8$ for the Terudermis graft, FTSG and STSG patients, respectively. With respect to complications, there were fewer incidences of hematoma, partial skin loss and complete skin loss in the Terudermis graft patients. Conclusion: In the present study, the Terudermis, when used to treat post-traumatic facial skin defects, is a good alternative option to obtain satisfactory aesthetic outcomes. Also, the Terudermis grafting is a simple and easy treatment method to perform.
