경주 불국사 삼층석탑에서 발생한 무기오염물과 보수물질의 생성 원인 및 재질 특성을 규명하기 위해 분석을 실시하였다. 그 결과, 황색 오염물은 외부 기원 토양 또는 풍화토의 침착으로 인한 변색, 적갈색 오염물은 산화철 광물의 한 종류인 침철석(Goethite, FeOOH)에 의한 오염, 흑색 오염물은 망간(Mn) 산화물이 부재 표면에 고착되면서 암석의 색상 변화를 유발하는 것으로 확인되었다. 과거에 사용된 보수 재료 중 시멘트 모르타르는 외부 환경과 반응하여 백화현상을 야기한 것으로 판단된다. 이를 통해 석탑 오염물질과 관련된 기초 자료를 확보하고, 효율적인 보존처리 방안마련 자료로 활용하였다. 한편, 외부환경에 노출되어 있는 석탑은 오염물 재형성 가능성이 높으므로 지속적인 모니터링을 통한 체계적인 관리가 요구된다.
Objectives Through the clinical records of Seungjeongwon Ilgi, we reviewed the usage of Bi-Jeung to know the concept of the word and studied therapeutic strategies for managing Bi-Jeung. Methods We investigated the clinical records of the mid-Chosun dynasty containing the key word "Bi" from electronic database (Seungjeongwon Ilgi). Results Of 4,039 records, 249 articles thought to have medicinal value were lastly selected. We subdivided the cases into 13 categories according to time, the connection of contents and the change of associated symptoms. "Bi" was not used alone but used in combination with body parts or other symptoms. Etiological causes of "Bi" involved dampness, phlegm, fire, heat and qi disorders. We suggested that "Bi" of the mid-Chosun dynasty meant a symptom group mainly of sensory impairment and additionally pain or motor disturbance. Among the 22 herbal medicine formulas used, 15 were based on internal medical pattern identifications and 7 were symptomatic treatments. Acupuncture and moxibustion therapy were primarily applied to adjacent acupoints. In addition, External therapies were used together, such as washing therapy, plaster therapy, cupping therapy and thermotherapy. Conclusions "Bi" principally indicated sensory impairment on limbs and the main etiological cause was considered to be dampness. Herbal medicine, acupuncture, moxibustion and external therapies were used to cure Bi-Jeung.
Background: Stretch-oriented home exercise programs are often used as treatments for patients with adhesive capsulitis; however, there is lack of research on home exercise programs to strengthen rotator cuffs. Objects: The aim of this study is to investigate the effect of home exercise programs for rotator cuff strengthening on pain, range of motion (ROM), disability level, and quality of life in patients with adhesive capsulitis. Methods: Twenty-two patients with adhesive capsulitis volunteered to participate in this study. The subjects were randomly divided into an experimental group (n = 11) and control group (n = 11). For the experimental group, manual therapy and home exercise programs for rotator cuff strengthening were applied for 6 weeks; for the control group, only manual therapy was applied for 6 weeks. Shoulder pain (quadruple visual analogue scale, QVAS), ROM, disability (shoulder pain and disability index-Korean version, SPADI), and quality of life (world health organization quality of life scale-Korean version, WHOQOL-BREF) were evaluated at baseline, after 3 weeks, and after 6 weeks of intervention. The changes in the measurement variables were analyzed by using repeated measure analysis of variance. Results: Significant differences were observed between the experimental group and control group in the QVAS; SPADI-pain scores; SPADI-disability scores; SPADI-total scores; flexion, abduction, internal and external rotation ROM of the glenohumeral joint; and WHOQOL-BREF total, overall, physical health, and psychological scores. All groups displayed statistically significant improvements as observed in the QVAS, SPADI, flexion, abduction, external and internal rotation ROM of the glenohumeral joint, and WHOQOL-BREF. Conclusion: Home exercise programs for rotator cuff strengthening had a positive impact on shoulder pain, shoulder ROM, disability level, and quality of life in patients with adhesive capsulitis. Therefore, we propose the use of home exercise programs for rotator cuff strengthening in the exercise rehabilitation of patients with adhesive capsulitis.
Objective : ${\ulcorner}$Wai-Tai-Mi-Yao${\lrcorner}$ had been made by Wang-Dao(王燾) in Tang Dynasty(唐朝). It included not only in those days medical knowledge, but also before medical knowledge. So it is regarded as important classic in the oriental medicine. And there are various bathing therapy methods. Therefore we would like to bring out use sphere and detailed method of bathing therapy in Tang Dynasty and before period. Methodologies :We have researched bathing therapy of ${\ulcorner}$Wai-Tai-Mi-Yao${\lrcorner}$ according to below the procedure. (1) Making a related words list: We have used existing external treatments technical books to make a list. The list is consist of 23 words and includes not only technical terms, but also general terms. (2) Searching sentence: We have searched sentence that contain terms that related with bathing therapies. (3) Analysis of related sentence: We have searched and classified sentence by disease. Conclusions :(1) ${\ulcorner}$Wai-Tai-Mi-Yao${\lrcorner}$ has described 15,180 records. Bathing therapies of ${\ulcorner}$Wai-Tai-Mi-Yao${\lrcorner}$ had been used to cure 726 records from the whole volume. The contents account for 4.8% of the whole volume. (2) ${\ulcorner}$Wai-Tai-Mi-Yao${\lrcorner}$ has described 1,104 diseases. Bathing therapies of ${\ulcorner}$Wai-Tai-Mi-Yao${\lrcorner}$ had been used to cure 293 diseases from the whole diseases. The contents account for 26.5% of the whole volume. (3) These diseases belong to dermatologic, internal, ophthalmic, otolaryngologic, obstetrics, gynecologic, pediatric, surgical and veterinary diseases.
In "The region of the body in which abdomen pain manifests(腹痛有部分)" of "Dongeuibogam(東醫寶鑑)", the abdomen is divided into the upper abdomen(大腹), umbilicus area(臍腹) and lower abdomen(小腹), which are in turn assigned to 3 Yins of greater yin(太陰), lesser yin(少陰) and reverting yin(厥陰) respectively to explain the causes and treatments of abdomen pain according to their locations. Meanwhile, the present Korean medicine tends to issue herbal formula without considering the cause of regional abdomen pain and the aspect of cold and heat(寒熱), deficiency and excess(虛實) in the symptom. This fragmentary treatment may bring out a serious failure. For the correction of this problem, the literature study on "The region of the body in which abdomen pain manifests(腹痛有部 分)" of "Dongeuibogam(東醫寶鑑)" was carried out to obtain the following conclusions. In "The region of the body in which abdomen pain manifest (腹痛有部分)" of "Dongeuibogam(東醫寶鑑)", it turned out that the use of herbal formula for the abdomen pain was the integrated one of herbal formulas for the abdomen pain in the external contraction such as cold damage yin condition(傷寒陰證) and cold in the middle condition(中寒證) and that the cause of abdomen pain was the explanation of diverse internal causes for the abdomen pain. And it was also found that the assignment of the upper abdomen(大腹), umbilicus area(臍腹) and lower abdomen(小腹) to 3 Yins had the locational and meridian meanings of 3 Yins according to internal causes and external contraction to get the basis of abdomen pain treatment in dividing it into 3 Yins.
Purpose: Rigid external distraction(RED) is a highly effective technique for correction of maxillary hypoplasia in patients with cleft or syndromic craniosynostosis. Despite many advantages of RED, it also has the problem of relapse as the conventional advancement surgery. Bicoronal approach, that is the common approach to gain access to the craniofacial skeleton, had some morbidity, such as hair loss, sensory loss, wide scar and temporal hollowing. We present our clinical experience of RED distraction with minimal invasive approach and early rigid fixation to overcome these disadvantages. Methods: A 27-year-old female patient with Crouzon's disease underwent Le Fort III osteotomy and RED device application through the minimal invasive direct skin incisions. After the latent period of 5 days, distraction was undertaken until proper convexity and advancement were obtained. During the rigid retention period, inflammation occurred on the right cheek, and proper conservative managements were done including continuous irrigation. To maintain the stability of distraction, early rigid fixation was undertaken on the osteotomy sites through another skin incisions. Preoperative and postoperative orthodontic treatments were performed. Serial photographs and cephalometric radiographs were obtained preoperatively, after distraction and 6 months after distraction. Results: The cephalometric analysis demonstrated postoperatively significant advancement of the maxilla and improvement of facial convexity. After 6-month follow-up period, the maxilla was stable in the sagittal plane and no relapse was found. Facial scars were not noticeable and other deformity and morbidity did not occur. Conclusion: This effective and stable technique will be a good alternative for the patients who need large amount of distraction and for adult patients with severe maxillary hypoplasia or syndromic craniosynostosis.
Objective: The influence of konjac gel level on fermentation process and product qualities were assessed to evaluate the feasibility of using it as fat analog in Northeastern Thai fermented sausage (Sai Krok E-san). Methods: Five treatments of fermented sausages were formulated by replacing pork backfat with 0%, 7.5%, 22.5%, and 30% konjac gel. The changes in lactic acid bacteria (LAB) and important physicochemical properties of samples were assessed during 3 days of fermentation. After the end of fermentation at day 3, water activity ($a_w$), instrumental texture, color, microbial counts, and sensory evaluation were compared. The best product formulation using konjac for replacing pork back fat were selected and used to compare proximate composition and energy value with control sample (30% pork backfat). Results: An increase in konjac gel resulted in higher values of LAB, total acidity, and proteolysis index with lower pH and lipid oxidation during 3 days of product fermentation (p<0.05). It was noted that larger weight loss and product shrinkage during fermentation was observed with higher levels of konjac gel (p<0.05). The resulting sausage at day 3 with 15% to 30% konjac gel exhibited higher hardness, cohesiveness, gumminess, springiness, and chewiness than control (p<0.05). The external color of samples with 22.5% to 30% konjac gel were redder than others (p<0.05). Mold, Salmonella spp., Staphylococcus aureus, and Escherichia coli in all finished products were lower than detectable levels. Product with 15% konjac gel had the highest scores of sourness linking and overall acceptability (p<0.05). Conclusion: The product with 15% of konjac gel was the optimum formulation for replacing pork backfat. It had higher sensorial scores of sourness and overall acceptability than control with less negative impact on external appearance (product shrinkage) and weight loss. Moreover, it provided 46% fat reduction and 32% energy reduction than control.

Background: The nonsurgical treatment of chest wall deformity by a vacuum bell or external brace is gradual, with correction taking place over months. Monitoring the progress of nonsurgical treatment of chest wall deformity has relied on the ancient methods of measuring the depth of the excavatum and the protrusion of the carinatum. Patients, who are often adolescent, may become discouraged and abandon treatment. Methods: Optical scanning was utilized before and after the intervention to assess the effectiveness of treatment. The device measured the change in chest shape at each visit. In this pilot study, patients were included if they were willing to undergo scanning before and after treatment. Both surgical and nonsurgical treatment results were assessed. Results: Scanning was successful in 7 patients. Optical scanning allowed a visually clear, precise assessment of treatment, whether by operation, vacuum bell (for pectus excavatum), or external compression brace (for pectus carinatum). Millimeter-scale differences were identified and presented graphically to patients and families. Conclusion: Optical scanning with the digital subtraction of images obtained months apart allows a comparison of chest shape before and after treatment. For nonsurgical, gradual methods, this allows the patient to more easily appreciate progress. We speculate that this will increase adherence to these methods in adolescent patients.
The foramen of Huschke (FH) or foramen tympanicum is a persistent bony defect connecting the external auditory canal (EAC) to the temporomandibular joint (TMJ). It arises from an incomplete ossification of the tympanic part of the temporal bone that persists after the age of 5. If a herniation exists in the TMJ, otological symptoms may occur. An 80-year-old female patient complained of noise in her left TMJ and otorrhea in her left ear. On her cone beam computed tomography images, there were only degenerative joint disease signs on her left mandibular condyle. However, her computed tomography images revealed that the soft tissue of the TMJ herniate into the EAC. Additional examination was planned for the further evaluation. But the patient no longer visited the hospital due to her systemic health status, and symptoms disappeared spontaneously without any treatments. Usually this type of herniation is very rare, but years of mechanical stress from mastication may result in weakening and widening of the foramen with age. Therefore, although FH is usually congenital, sometimes it may be acquired in the elderly. The treatment plan should be determined in consideration of the patient's symptom level and the patient's general health status. If the clinical symptoms are not severe, no treatment is required.
Purpose: In order to know therapies used in the field of Obsterics & Gynecology of Oriental medicine. Methods: This survey investigated papers about clinical study, literature investigation for therapeutic methods and treatment reports published from 1999. 2${\sim}$2008. 11. Results: In the clinical studies, acupuncture is more frequently used than herbal medicine. But in the treatment reports, herbal medicine is most frequently used except some cases. Besides acupuncture and moxibustion was used frequently. In the treatment reports various acupuncture methods were used except vaginal bleeding or abdominal pain cases. Whole body acupuncture was used mainly. Pharmacopuncture, Sa-am Acupuncture, auricular acupuncture was also used frequently. And Dong's acupuncture, Hwa acupuncture was used sometimes. Moxibustion was tend to be used for dysfunctional uterine bleeding, menopausal disorder, urinary incontinence, vaginal bleeding or abdominal pain during pregnancy period, hyperemesis, recurrent miscarriage, postpartum disease, lochiorrhea. Also other treatment methods was used such as external therapy, aromatherapy, herbal retention enema, fumigation, electric lipolysis acupuncture, Chuna manual medicine, obesity management, acupuncture at uterus cervix, magnetic innervation therapy, exercise, cupping and physical therapy. Conclusion: These results suggests that acupuncture, moxibustion and herbal medicine are most frequent therapy in the field of Obsterics & Gynecology of Oriental medicine. And other therapy are used such as external therapy, aromatherapy, herbal retention enema, fumigation and so on.
