집단 운동치료가 노인의 인지기능과 일상생활동작 및 균형 수행능력에 미치는 영향 (The Effect of the Group Therapeutic Exercise Program on the Cognitive Function, ADL, and Balance-Performing Ability in Elderly Persons)

  • The aim of this study was to assess the effectiveness of group therapeutic exercise programs on the cognitive function, Activities of Daily Living (ADL), and balance-performing ability in older adults. Fifteen community-dwelling subjects (mean age 73.7 yrs, standard deviation 2.4) participated in this study. An 8-week group therapeutic exercise program, including strengthening and balance training, breathing and gait exercise, and recreation, consisted of activities related to daily living. The Mini-Mental State Examination-Korean (MMSE-K), Modified Barthel Index (MBI), Berg Balance Scale (BBS), Functional Reaching Test (FRT), and Timed Up & Go (TUG) test were measured during pre-exercise and post-exercise points. The results of this study were as follows: 1. After eight weeks, the MMSE-K and MBI total score was more significantly increased for post-exercise tests than pre-exercise tests (respectively, p<.05, p<.01). 2. Of the MBI contents, personal hygiene, dressing, ambulation, and chair/bed transfers scores were significant increased for post-exercise tests. 3. Post exercise BBS, FRT, and TUG scores were higher than the pre-exercise scores. The difference was statistically significant (respectively, p<.05, p<.05, p<.01). These findings suggest that group therapeutic exercise can be used to improve the cognitive function, ADL, and balance-performing ability in elderly persons.

중심 안정성 운동을 적용한 교각운동 시 지지면 불안정성이 체간 및 하지의 근 활성도에 미치는 영향 (The Influence of Unstability of Supporting Surface on Trunk and Lower Extremity Muscle Activities During Bridging Exercise Combined With Core-Stabilization Exercise)

  • The aim of this study was to investigate the effect of supporting surface instability to trunk and lower extremity muscle activities during bridging exercise combined with core-stabilization exercise. Thirty young healthy adults (15 males and 15 females) voluntarily participated in this study. Each subject was asked to perform bridging exercise combined with core-stabilization exercise on three different supporting surfaces (stable ground surface, the wooden balancing board, and the air cushion). The muscle activities were measured using surface electromyography (EMG) during performing exercise. To test statistical significance, one-way ANOVA with repeated measures was used with the significance level of .05. The findings of this study are summarized as follows. (1) There were significant differences in muscle activities on internal oblique, external oblique, gluteus medius, semitendinosus, biceps femoris, medial gastrocnemius and lateral gastrocnemius during exercise (p<.05). (2) The biceps femoris and lateral gastrocnemius showed significantly higher muscle activity on the wooden balancing board rather than on the ground, and semitendinosus, biceps femoris, medial gastrocnemius and lateral gastrocnemius showed significantly higher muscle activity on the air cushion rather than on the ground (p<.05). Therefore, it is concluded that muscle activities in the trunk and the lower limbs during bridging exercise combined with core-stabilization exercise was affected with instability of supporting surface. Further researches are needed to investigate the long term effect of bridging exercise on muscle activity with patient group.

치료적 운동이 노인의 균형과 보행에 미치는 효과 (The Effects of Therapeutic Exercise on the Balance and Gait in Older Adults)

  • Purpose: This study was conducted to investigate whether therapeutic exercise has effectiveness on the balance and gait in older adults. Methods: 30 patients were participated in this study. To evaluate the effects of therapeutic exercise, patients were evaluated by using One leg stand test which was static balance test, Berge balance test, Functional reach test and Get up and go test which was dynamic balance test. Finally Gait pattern were assessed by using 3-dimensional gait analysis system. The assessment parameters were evaluated before and after treatment. And we received a consent form from all patients. The data were analyzed by paired t-test for compare pre and after therapeutic exercise. Results: One leg stand test was significantly increased after than before therapeutic exercise. Get Up and Go test was significantly decreased after than before therapeutic exercise. Functional Reach test was significantly increased after than before therapeutic exercise. Burge Balance test was significantly increased after than before therapeutic exercise. Stride length was significantly increased after than before therapeutic exercise. Cadence was significantly increased after than before therapeutic exercise. Velocity was significantly increased after than before therapeutic exercise. Conclusion: We thought that therapeutic exercise that is mat exercise has effectiveness on the balance and gait in older adults.

Effects of the Instrument Pilates Exercise Based on the Schroth Exercise on the Cobb's Angle, Angle of Trunk Rotation and Low Back Pain in Patients with Idiopathic Scoliosis: A Single Subject Study

  • Purpose: This study examined the effects of the application of Schroth exercise-based instrument Pilates exercise on the Cobb's angle, angle of trunk rotation, and low back pain of female patients with idiopathic scoliosis. Methods: Three patients with idiopathic scoliosis at a Cobb's angle of 20° or more participated in this study. Among the single-subject experimental research designs, a reversal (ABA) design was performed. In particular, Schroth exercise-based instrument, Pilates exercise, was performed for 10 weeks, consisting of five weeks between the baseline and intervention one period and five weeks between the intervention 1 and intervention 2 periods, and then followed up for five weeks. Results: After the Schroth exercise-based instrument, Pilates exercise, was applied, the Cobb's angle and the angle of trunk rotation decreased compared to the baseline in all subjects, and the follow-up period also showed a continuous decline. After Pilates exercise was performed, low back pain in subjects 1 and 2 was decreased in the intervention 1 period compared to the baseline. The level of low back pain in the intervention 2 period increased compared to the intervention 1 period, but a reduction was noted in the follow-up period. The low back pain in the subject was decreased in all intervention periods and the follow-up period. Conclusion: Schroth exercise-based Pilates exercise improves the Cobb's angle and the angle of trunk rotation for female patients with idiopathic scoliosis in their teens and 20s, and an effective intervention method is proposed for low back pain.

체간 안정화 운동과 고관절 운동이 만성 요통 환자의 허리가동범위, 통증, 장애정도에 미치는 영향 (Effects of Trunk Stability Exercise and Hip Exercise on Lumbar Range of Motion, VAS, Disability Chronic Low Back Pain Patients)

  • Background: This study compared the effects of trunk stabilization exercise and hip joint exercises on the range of motion of the lumbar spine, pain severity, and severity of disability in patients with chronic lower back pain. Methods: A total of 30 participants were enrolled and divided into group 1 (n=10), group 2 (n=10), and group 3 (n=10) were performed by each group thrice a week for a total of 8 weeks. Group 1 performed warm-up exercise (15 min), trunk stabilization exercise (25 min), finish-up exercise (15mins). Group 2 performed warm-up exercise (15 min), hip exercise (25 mins, finish-up exercise (15mins). Group 3 warm-up exercise (15 min), trunk stabilization and hip exercise (25 min), Finish-up exercise (15 min). Participants were assessed for the range of motion of the lumbar spine, pain severity (visual analog scale score; VAS), and severity of disability (Oswestry disability index score; ODI) before and after the interventions. Results: All three groups showed a significant increase in the range of motion of the lumbar spine, but there was no significant difference among the groups. Moreover, the severity of pain and ODI were significantly decreased in all groups; however, the intergroup differences were non-significant. Conclusion: The results from this study confirmed the effectiveness of trunk stabilization and hip joint exercise in improving the lumbar range of motion, pain severity, and chronic lower back pain in patients. Thus, trunk and pelvic stabilization exercises and hip joint exercise can be used as clinical practices to treat and prevent chronic lower back pain.

  • Neurophysiological therapeutic exercise had started 1940-1960 that used for CNS patient's rehabilitation is the world. In America, PNF, Bobath, Brunnstrom technique are reviewed through the NUSTEP and II STEP on 1967. The NUSTEP provide common interest of neurophysiological therapeutic exercise. The II STEP provide new concept of neurophysiological therapeutic exercise from scholar, professor and clinician. New concept include not only using the reflex but also motor learning, motor control and motor behavior. It is including neurobiologic interaction, biomechanics and motor science. Therefore, neurophysiological therapeutic exercise have to change with the purpose of the therapeutic exercise.

  • The neuromuscular reaction can be delayed and velocity of nerve conduction can be slowed with aging. General activities ate changed to be slowly due to the decreased proprioceptor. The exercise for preventing aging in sites of the muscle strengthening exercise for maintaining the ability of activity. balance exercise for preventing from falling down and breathing exercise for the ability of respiration. The exercise have to be done without caring about the place and time. also have to be done enjoyably than competitively.

EST와 등척성 운동이 상완이두근의 근력증진에 미치는 효과에 관한 비교연구 (The Comparison Effects between Electrical Stimulation Therapy and Isometric Exercise on Improvement of Biceps brachii Muscle Strength)

  • Purpose : To compare the effect between isometric exercise group(n=10) and electrical stimulation therapy group(n=10) on improvement of biceps brachii muscle strength. Methods : The experiments were performed for 4 weeks and three times a week. The participants of this study were 20 students(10 male, 10 female) who had not any medical problems. We divided into the two groups. One group received isometric exercise and the other group received EST for 4 weeks. The biceps brachii muscle strength was evaluated by making use of the KIN-COM. Results : The results were as follows 1. One group which received isometric exercise increased 36% for biceps brachii muscle strength. 2.The other group which received EST increased 54% for biceps brachii muscle strength. Conclusion : The results of this study showed that EST group was more effective than isometric exercise group.

손동작 운동프로그램이 치매 노인의 인지기능 향상에 미치는 효과 (The Effects of Hand Movement Exercise Program on the Improvement of Cognitive Function in the Dementia Old Adults)

  • Background: The purpose of this study was to find out the effects of the hand movement exercise program on the cognitive function of the older with dementia. Methods: A total of twenty-four subjects were divided into two research groups and one control group, respectively. Each exercise was conducted in twelve weeks: three times a week, twice a day for fifteen minutes. Enhancement of cognitive function in each exercise program was evaluated four times using MMSE-KC, the Korean version of the Mini-Mental State. Results: As for enhancement of cognitive function, the hand movement exercise program represented an increase of $1.63{\pm}.92$ over those before study, showing significant differences (p<.05); the large muscle exercise program, $.75{\pm}.89$; and the activities of daily living program, $.00{\pm}2.14$, showing no significant differences. Conclusion: Hand Movement Exercise Program can be inferred from the results that consistent exercise program is more effective than any other ways for enhancement of cognitive function of the older with dementia.

The Effect of Aquatic Task Training on Gait and Balance Ability in Stroke Patients

  • Purpose: The purpose of study was to measure stroke patients' ability to balance and their degrees of clinical function and to examine the effect of the aquatic exercise method using tasks related to these features. Methods: Twenty stroke patients were randomly assigned to an aquatic task exercise group and a land task exercise group. Both groups used the same exercise method for 60 minutes each session, three times a week for 12 weeks at the same time point and with the same amount of exercise. Results: Before and after the exercise, static balance was measured using balance measuring instruments locomotive faculties, muscular strength, and dynamic balance were assessed through the Berg balance and 10 m gait tests. Finally, gait abilities were measured, and the data obtained were analyzed to generate the results. Conclusion: Both groups showed significant improvement, but the aquatic exercise group showed slightly more significant results in static balance, Berg balance, and upright walking tests. It is thought that the improvement of stroke patients' balance and gait ability can be triggered through the application of aquatic exercise programs in the future.
