성인병의 예방과 치료를 위한 영양과 운동처방 I. 비만과 당뇨병에 미치는 영향 (Exercise Prescription and Dietary Modification for Prevention and Treatment of Chronic Degenerative Disease I. On Obesity and Diabetes)

  • 비만증이란 체내에 지방이 과도하게 축적된 상태이고, 당뇨병이란 혈당을 조절하는 insulin의 부족 혹은 작용 결함으로 인한 질환이다. 비만과 당뇨병의 일반적인 원인으로는 유전, 과식, 운동부족, 스트레스 등이 있다. 비만 및 당뇨병 환자는 섭취 열량을 줄이고 신체에 영양공급이 균형있게 이루어 지도록 균형잡힌 식생활을 해야 하며 고열량식품의 섭취를 줄여야 한다. 일반적인 운동 지침은 운동전 의학적 진단과 병력 검사가 우선되어야 하며, 점진적으로 운동강도를 높여야 한다. 비만과 당뇨병의 예방 및 치료를 위해서는 일반적으로 유산소 운동(조깅, 수영, 테니스, 에어로빅 댄스 등)을 60~80$\%$ HRmax(50~70$\%$ $Vo_2$ max)강도로, 15~60분, 주 3~6회 하는 것이 적당하다. 특히 당뇨병 환자는 유형에 따른 처방을 다소 달리해야 하며, 운동중 탄수화물이 든 간식을 하고 인슐린 활동이 최고에 달했을 때 운동을 중지하며 운동 후 저혈당 발생의 가능성에 대비하여 혈당을 높이기 위한 탄수화물 15~20g 분에 해당하는 식품을 섭취한다. 비만과 당뇨병 환자의 치료에 있어서 운동요법은 식이요법과 병행해서 이루어지는 것이 더욱 효과적이다

서울지역 고등학생의 식습관과 스트레스에 관한 연구 (The Food Habit and Stress Scores of High School Students in Seoul Area)

  • 서울시내 강북의 인문계 고등학생 354명을 대상으로 식생활 태도, 식습관 변화, 식습관 평가, 스트레스정도를 조사한 결과는 다음과 같다. 1. 고등학생의 평균체중은 남녀 각각 62kg, 52kg, 평균신장은 173 cm, 162 cm로 조사되었다. 고등학생의 비만도를 판정한 결과‘정상체중’은 남학생 56.6%, 여학생 51.5%로 나타났고‘과체중’은 남학생 5.3%, 여학생 1.8%였으나 ‘저체중’은 남학생 37.0%, 여학생 46.7%로 높은 분포를 나타내고 있다. 2. 고등학생의 식사횟수, 편식여부는 성별에 따른 유의성(p<0.05)이 나타났다. 식사횟수는 65.0%가‘하루 3회’식사한다고 응답하였고 그중 남학생은 68.8%로 여학생의 60.0%보다 높았으며 여학생의 경우‘하루 2회’식사한다고 응답한 학생이 26.7%로 남학생 16.4%에 비해 월등히 높았다. 편식여부는‘편식한다’라고 응답한 여학생이 56.4%로 남학생 41.8%보다 높게 나타났다. 아침식사를 거르는 이유는 성별에 따른 유의성을 보였는데‘등교시간이 늦어서’는 여학생이 62.1%로 남학생 47.6%보다 높았고‘식욕이 없어서’는 남학생이 31.7%로 여학생 13.8%보다 높았다. 3. 최근 몇 년 사이 식습관 변화에 관한 조사에서 고등학생의 채소, 과일, 유제품의 섭취가 증가한 것으로 나타났고 그중 과일의 섭취증가가 가장 높게 나타났다. 4. 어머니가 직업이 없는 경우 고등학생의 식습관점수는 1.93으로. 직업이 있는 경우의 1.85보다 식습관이 좋았다. 고등학생의 영양지식점수는 여학생이 6.99로 남학생 6.50보다 높았으나 어머니의 직업유무에 따른 차이는 나타나지 않았다. 5. 고등학생의 성별에 따른 스트레스점수는 여학생(3.35)이 남학생(3.15)보다 높게 나타났다. 식사를‘천천히 한다’(3.03)고 응답한 고등학생의 스트레스점수가‘빠르게 한다’(3.41)고 응답한 고등학생보다 낮았고 ‘식욕이 좋을 때’의 스트레스점수(3.13)가‘식욕이 보통일때’(3.42)와‘식욕이 나쁠때’(3.43)보다 낮았다. 운동을‘주5회 이상’하는 학생의 스트레스점수가 가장 낮게 나타났다. 이상의 결과를 살펴보면 고등학생의 바람직한 식생활 태도와 적절한 운동은 스트레스를 낮추는데 도움을 주는 것으로 사료된다.

양생법의 연구 현황 - 운동을 중심으로 - (The Situation of Research of Yangsaeng Method - Mainly on Exercises -)

  • Background and purpose: Well being which emphasizes peace of mind and richness of spirit, has been already used from several thousands years ago, as the name of Yangsaeng(養生) when we regard it in the view point of oriental medicine. Yangsaeng means a person's daily life according to natural rules in order to compensate his essence qi spirit(精氣神) and to prevent and cure his disease for his long life, and there are environment, spirit, qigong, food, dwelling labor, sleeping and sexual intercourse as its methods. But in Korea, we are mainly using the health and concept of the health management and use of it, with the one developed in Western. Thus, in this study, literature study was made for the situation of research and feature of Yangsaeng method. Among them, I tried to study mainly on adapting method of Exercise Yangsaeng and its possibility which can derive harmonious development of Spirit and body for human body, in the view point of identity(整體觀). Methods: Diploma thesis and words of 'Yangsaeng', 'exercise cure', 'exercise therapy', and 'exercise prescription', in 'KERIS(Korea Education & Research Iformation Service).' Results and conclusion: In this study, in order to adapt oriental method of Yangsaeng to the reality, I selected and researched only Exercise Yangsaeng among various Yangsaeng methods including environment, spirit, qigong, food, dwelling labor, sleeping and sexual intercourse. The Exercise Yangsaeng has its advantage that it ran train both mind and body as well as it has various programs, but it is regarded at it is in need of more study as lows; First, it requires assessment on the effect of construction itself as well as degree management of Excercise Yangsaeng as a method of exercise which is being done in daily life or in the literature. Second, above all, standardization of Yangsaeng Exercise and specialized programs are required in order to introduce the Exercise Yangsaeng which was verified for its effect, to the cure of disease. Third, researches are required for the system of assessment management for measurement of relative effect of standardized programs. fourth, continuous compensation is required in the future by adapting researched and developed programs into the field.

한국인의 일부 도시인에서 비만, 이상혈당, 이상지질혈증의 집락과 고혈압의 관련성 (Association of Hypertension with Cluster of Obesity, Abnormal glucose and Dyslipidemia in Korean Urban Population)

  • To examine the association of hypertension with cluster of obesity, abnormal glucose and dyslipidemia in Korean urban population, we conducted this cross-sectional study among 3027 men and 2127 women age 20-85 years who visited a prevention center between May 1991 and June 1995 for a multiphasic health check at St. Mary's Hospital, Seoul. By the self-administered questionnaire, the informations of educational attainments, monthly income, alcohol consumption, cigarette smoking, and physical excercise level were obtained. Height, weight, and blood pressure were measured by a trained nurse. The fasting blood sugar (FBS), total cholesterol, high density lipoprotein (HDL) cholesterol and triglyceride were tested by enzyme method. Low density lipoprotein (LDL) cholesterol was calculated by 'total cholesterol - HDL cholesterol - triglyceride/5'. For testing the differences of cardiovascular risk factors between hypertension and normotension group, 1-test and $\chi^2$-test were performed. For the age adjusted odds ratios of hypertension in persons with obesity, abnormal glucose, and dyslipidemia compared with normal, logistic regression was performed by using SAS pakageprograme. The results obtained were as follows: 1. Age, weight, body mass index(BMI), blood glucose, total cholesterol, LDL cholesterol, and triglyceride of hypertension group in men and women were significantly higher than normotension group, but height and HDL cholesterol of hypertension group only in women significantly lower than normotension group. The frequency of obesity $(BMI\geq25kg/m^2)$, abnormal glucose $(\geq\;120mg/dl)$, hypercholesterolemia $(\geq\;240mg/dl)$, lower HDL cholesterol (<45 mg/dl in women only), higher LDL cholesterol $(\geq\;160mg/dl)$, and hyper hypertriglyceridemia $(\geq\;250mg/dl)$ in hypertension group of men and women were significantly higher than normotension group. 2. Systolic and diastolic blood pressure were negatively correlated with hight, but positively with age, weight, BMI, total cholesterol, LDL cholesterol, and triglyceride in men and women. BMI was positively correlated with fasting blood sugar, total cholesterol, LDL cholesterol and triglyceride but negatively with HDL cholesterol. 3. The age adjusted odds ratios of hypertension were as follows in men and women : among persons who were obese compared with those nonobese, 2.53 (95% Confidence Intervals [C.I.] 2.08-3.07) and 2.22 (95%C.I. 1.71-2.87): among persons who were abnormal glucose compared with those normoglycemic, 1.43 (95%C.I 1.13-1.82) and 2.01 (95%C.I 1.36-2.94): and among persons who were dyslipidemia (hypercholesterolemia or lower HDL cholesterol or higher LDL cholesterol or hypertriglyceridemia) compared with those normal lipid, 1.59 (95%C.I 1.30-1.95) and 1.51 (95%C.I 1.16-1.96). After combined more than one risk factor, the odds ratios were increased. Among persons with cluster of obesity, abnormal glucose, and dyslipidemia, the odds ratio of hypertension was 2.25 (95%C.I 1.47-3.37) in men and 3.02 (95%C.I 1.71-5.30) in women. In conclusion, it was suggested that hypertension was associated with cluster of obesity, abnormal glucose, dyslipidemia in this Korean urban population.

탄력밴드를 이용한 슬관절 굴곡근에 대한 저항운동이 정적.동적 고유수용성감각에 미치는 영향 (The Effect of Static and Dynamic Proprioception on Resistance Exercise with Elastic Band)

  • Background: The purpose of this study is to examine the effect of resistance exercise on static and dynamic proprioception by applying elastic bands to the knee flexor, which can be performed earlier than proprioception, a weight bearing pattern. Proprioceptive training may be applied to the early period of recuperation training along with resistance exercise to prevent damage to the knees and the joints in the lower body, as well as the reoccurrence of injury, which is expected to improve the effect of static and dynamic proprioceptive training. Methods: The subjects of this study were 32 men and women in their 20s who volunteered to participate in the experiment which was carried out over the course of 6 weeks from March 14 to April 24 at K Hospital located in Yongin City. They were tested in two groups: the elastic band resistance exercise group (RE: Resistance Excercise Group, n=17) and the joint motion range exercise group(ROM: Range of Motion Group, n=15). In this experiment, static and dynamic proprioceptive tests were given to both groups before and after the test. In this study, SPSS Win ver.12.0 was utilized to perform a T-test on the independent samples in order to validate the identical features of subjects in the RE and ROM Group and substantiate the significance as to the difference between both groups before and after the test. Also, a paired T-test was conducted to compare the static and dynamic proprioception of both groups before and after the test. The statistical significance levels were ${\alpha}$=.05. Results: 1.The static proprioception in both groups, RE and ROM, increased in a statistically significant way after the test(p<.05). 2.The static propriocetion of the RE group increased in a statistically significant way after the test(p<.05). However, the increase of the static proprioception in the ROM group was not statistically significant. 3.Static and dynamic proprioception of the RE group increased significantly more than that of the ROM group after the test(p<.05). Conclusion: The resistance exercise using elastic bands improved the static and dynamic proprioception more than the joint motion range exercise did, reducing the risk of injury by enhancing the location sense of the joints and the sense of motion and shortening the period of recovery from injury. The outcome of the experiment may provide basic data for developing an effective way to reduce the risk of injury among ordinary people and athletes.

메타 분석을 통한 만성 관절염 환자의 통합 프로그램과 운동 프로그램의 효과 비교 (The Comparison Between the Effects of Integrated Arthritis Self-helf Programs and the Effects of Arthritis Exercise Programs Through Meta-analysis)

  • The purpose of this study was to use meta-analysis to analyze result of 17 studies which investigated the effects of integrated programs, and 11 studies which examined the effects of exercise programs on pain, depression, and disability. The 28 studies analyzed in this work were selected from the following sources. MEDLINE Search, bibliographies of related studies, main academic journals of nursing in Korea, and journals on arthritis issues. For the analysis of the data, homogeniety of effect sizes which were calculated based on data in the 28 studies was tested and its average effect size was computed by using meta analysis software package which was developed by Song(1992, 1998). The results can be summarized as follows : 1) Homogeneity tests were conducted on integrated programs on pain. In the prelimiary homogeneity tests on effect size of all 17 studies, no homogeneity was found. When homogeneity tests on the effect size of the remaining 15 studies were performed, excluding two studies which had extremely larger effect size compared to other studies, the 15 studies were found to be homogeneous(Q=16.38, p=.23). The obtained average effect size, D(Mean Standardized Difference Between Means), was .25. When homogeneity tests on effect size on pain was conducted for the excercise programs, effect size for all nine studies were found to be homogeneous (Q=7.42, p=.49) and the average effect size D=.30. Therefore, Hypothesis 1 was rejected from the results, that an average effect size of the integrated programs on pain was not significantely different from that of the exercise programs on pain. 2) Since only two studies investigated the effect of exercise programs on depression, comparison between the average effect size of integrated programs and that of exercise programs on depression could not be conducted, and hypothesis 2 could not be tested. Thereby, only the average effect size of integrated programs on depression was obtained. Eight studies were tested to be positive on the homogeneity of effect sizes(Q=18.31, p=.02) at $\alpha$<.01 and its average effect size was D=.11. 3) For the analysis of integrated program on disability, 13 studies, except for four which had an extremely large effect size compared to the others were found to be homogeneous at $\alpha$<.01 (Q=22.30, p=.04) and the average effect size on disability was D=.16. For analysis of the exercize programs on disability, eight studies, except for one which had an extremely large effect size compared to others, were found to be homo geneous(Q=7.87, p=.34) and the average effect size on disability was D=.60. Therefore, Hypothesis 3 was accepted from the results that an average effect size of exercise programs on disability was significantly larger than that of integrated programs on disability.

비만 어린이와 청소년들의 체지방 평가를 위한 electronic pedometer 의 정확성 분석 (Accuracy of Electronic Pedometers to Assess Body Fatness in Obese Children and Youth)

  • 본 연구는 미국 유타주에 거주하는 10-15세 어린이와 청소년들의 허리둘레가 electronic pedometer (전자 보수계)의 타당성과 신뢰성에 미치는 영향을 분석하기 위하여 비만집단 10명(HWC, BMI $28.91\pm3.07$)과 일반집단 10명 (LWC, BMI $18.05\pm1.55$)으로 총 20명(14 Hispanic and 6 Caucasian)의 어린이와 청소년들을 대상으로 실시하였다. Electronic pedometer 의 신뢰성을 측정하기 위하여 각 어린이는 Yamax SW-701 electronic pedometer 를 허리에 차고 15계단 오르기와 15계단 내리기를 3 sets 실시한 결과, HWC 집단이 LWC 집단보다 더 많은 보수를 기록했으나 오르기와 내르기 시행의 주 효과는 유의한 차이가 없었고, 또 두 집단간의 상호작용에도 유의한 차이가 없었다. 보수를 측정하여 pedometer의 타당성에 허리둘레가 미치는 영향을 분석하기 위하여 각자의 보행 속도에 따라 400 m를 걷게 한 결과 주 효과는 두 집단 간에 유의한 차이가 있었으나 counting technique (hand-counted vs. pedometer counted)의 주 효과는 유의한 차이가 없었으며, 또 400 m 걷기의 시기와 group별 시기의 상호작용도 유의한 차이가 없었다. 이러한 결과는 pedometer가 walking steps 측정에 HWC 집단과 LWC 집단에 동일하게 타당도가 있음을 나타내고 있다. 추후의 연구에서는 어린이들의 walking speed, pedometer tilt angle, leg length, 및 stride length 등을 고려한 연구가 필요하다고 사료된다.

혈압 건강군과 비건강군 근로자의 건강실천 관련 요인 조사연구 (A Study on the Relationship between Health Behavior Factors and Blood Pressure of Workers)

  • This study was examined 718 workers who had consistent blood pressure results in 2001 and 2002 general health examinations that were held at a work places managed by a health care agency in Seoul. Significant results are found as follows by analysing SPSS 11.0 on the result of self-recorded questionnaires investigated from Mar 1, 2003 to April 30, 2003. 1. A sampled healthy group and a sampled unhealthy group had significant differences in four variables out of possible nineteens that are sex, age, marriage and occupation. The unhealthy group had more males than females, more aged (over 50 years old) than youngers (under 50 years old), more married than singles, more manufacturing workers than non-manufacturing workers. In the case of systolic blood pressure, as the healthy group had 16.52mmHg while that of the other group had 149. 58mmHg, 33.06mmHg of difference between those groups were detected. In the case of diastolic blood pressure, 74.93mmHg of the healthy group and 96.53mmHg of the unhealthy group yielded 21.60mmHg of difference between them. This result implies that a guidance of health care is required to be aware of 20-30mmHg volatility in blood pressure rate or to understand and treat properly own blood pressure. as it is difficult to detect hypertension in early stage due to no initial symptom. According to the result. an establishment of management system of workers, companies and health care agencies is required for consist health care. 2. In terms of risky habits to health, the unhealthy group had more proportion of past smokers, over-twice-a-week drinkers, people with higher obesity rate. However, in terms of excercise, the proportion of regularly exercising people is higher in the unhealthy group while that of non-exercising people is higher in the healthy group. On the other hand. the average grade of health practicing behaviour in two groups are not significantly different as the health group had 3.00 out of possible 6.00 while the other had 3.10. This result means that as workers are not interested in health practicing behaviour. health promoting programmes must be developed in such a way of various method of motivations and incentives. Particularly this implies that distortional objectives of exercises should be readjusted through health guidance. 3. Systolic blood pressure in the healthy group can be explained by sex and the obesity rate while that in the unhealthy group can be explained by subjective health awareness and the obesity. Diastolic blood pressure in the healthy group can be explained by sex and the obesity rate like the former. The obesity rate was significant variable affecting the blood pressure of both groups, and particularly the effect to the unhealthy group was remarkably higher than that to the healthy group. Therefore, this research identified that the health care on the blood pressure of workers is not only limited to hypertension patients, but also extended to all workers. In order for consistent care, an establishment of management system of workers, companies and health care agencies is required.

운동형태에 따른 중년여성의 신체구성과 혈중지질의 변화 (Effects of aerobic and combined exercise on body composition and blood lipid in the middle-aged women)

  • 이 논문의 연구목적은 운동형태에 따라 중년여성의 신체구성과 혈중지질에 미치는 영향을 규명하기 위한 것이다. 연구대상은 40 50대 중년여성으로 유산소운동집단 8명, 복합트레이닝집단 8명 두 그룹으로 나누었으며, 12주간의 운동프로그램을 실시하였다. 운동강도는 주4회, 1회 70~90분, HRmax 60%~75%, 1RM 60%~80%로 점진적으로 증가시켰다. 자료처리는 Windows 용 SPSS ver. 21.0 통계 프로그램을 사용하여 평균과 표준편차 등을 산출하고, 비모수통계분석 방법인 윌콕슨 부호순위검정과 윌콕슨 순위합검정을 통해 그룹별 사전-사후 평균차와 변화량에 대한 그룹간 차이를 검정하였다. 연구결과를 살펴보면, 먼저 각 집단별 사전-사후 평균차에서는 복합운동 집단의 BMI, 체지방량, 그리고 중성지방에서만 통계적으로 유의한 차이가 나타났다. 또한 변인별 사전-사후 변화량에 대한 두 집단간의 차이 검정에서는 체지방량에서만 통계적으로 유의한 차이가 나타났다. 부가적으로 통계적 유의성은 검증되지 않았지만 각 별인별 사전-사후 평균 변화에 대한 기술통계를 살펴보면 복합운동이 유산소운동에 비해 상대적으로 더 효과적일 수 있다는 것을 알 수 있다.

산림 운동이 혈압, 심박수, 과산화지질, 항산화효소에 미치는 영향 (The Influence of Excercise in a Forest Environment on Blood Pressure, Heart Rate, MDA and SOD)

  • 산림의 건강 기능은 최근 많은 사회적 관심을 끌고 있다. 본 연구는 6명의 실내 운동집단과 6명의 산림내 운동집단의 혈압, 심박수, 과산화지질, 항산화효소의 비교를 통하여 산림 내 운동이 인체의 생리 변화에 미치는 영향을 조사하기 위하여 수행되었다. 12명의 참여자의 평균 연령은 40세 이상의 여성이었고, 운동 전 후 혈압 및 심박수의 측정과 채혈을 통하여 자료가 수집되었다. 자료의 분석 결과 산림 운동 시 이완기 혈압은 실내 운동 집단보다 현저하게 낮아졌다. 또한 산림 운동과 실내 운동 참여자의 심박수 및 과산화지질(MDA) 수준의 변화에서는 두 집단 간 유의한 차이를 보이지 않았다. 그러나 항산화효소(SOD) 변화에서는 운동 후 안정 시 30분에 산림 운동 집단의 SOD 수준이 급격히 감소되는 변화를 보였다.
