Principles for Evaluating Healthy Cities (건강도시 평가의 원칙)

  • Objectives: This article examines the diversity of healthy cities evaluation in practice and discusses the major conflicting issues in evaluating healthy cities to offer implications to academics and cities for application in their field. Methods: The author discusses issues on major topics that arose from a review of literature on selected articles from peer-reviewed journals, books and gray literature. The recently developed Korean Healthy Cities evaluation framework is used as a main source of reference. Results: Evaluating healthy cities is in itself a political process and requires multiple methodologies and diverse sources of data. Details of the evaluation process depend on the purposes and goals predetermined by the stakeholders. The Korean Healthy Cities evaluation framework applies these principles and suggests a participatory approach to evaluation, selection of indicators that provide evidence on the process of change and to use mixed evaluation methods. The involvement of stakeholders in the evaluation process can also be a useful tool to further strengthen partnerships and strategies for healthy cities. Conclusion: Cities need to engage more in evaluation activities and develop necessary skills and capacity to produce utility-driven evidence.

Clinical Dental Hygiene Education and Practice based on Dental Hygiene Process (치위생 과정 기반의 임상 치위생 교육과 실무)

  • Dental hygiene was originated from dentistry and dental hygiene knowledge was a component of dental knowledge body. Since the late 1980s dental hygiene theory was began to develop. Nursing theories such as metaparadigm, nursing process and human need theory affected theory development as dental hygiene process. Dental hygiene process provides a framework for high quality dental hygiene care. Dental hygiene process include five phases; assessment, dental hygiene diagnosis, dental hygiene planning, implementation, evaluation. Dental hygiene process of care is recognized as standard for dental hygiene education and clinical dental hygiene practice. Dental hygiene practice has moved from auxiliary model to professional model. Critical thinking skill and disposition are necessary to provide evidence-based dental hygiene care using dental hygiene process as clinical process and critical thinking process. Critical thinking, problem solving and evidence-based practice must be integrated into dental hygiene process for quality dental hygiene care.

Implementation of Evidence Based Nursing Education into Nursing Management Clinical Practicum: Outcome Evaluation and Diffusion Strategies (간호관리학 실습에서의 근거기반간호 교육 적용: 결과평가와 확산 전략)

  • Purpose: This practice study was done to describe the process and outcomes of application of Evidence Based Nursing (EBN) Education to nursing management clinical practicum and suggest strategies to diffuse EBN into nursing management education. Methods: Education on the evidence based nursing management process was provided to 65 senior nursing students from a university. Nursing management clinical practicum integrated with EBN. The setting was a five full day-clinical practicum. Group and individual training on EBN process with lectures, small group discussions, conferences, and educational prescription were provided. Outcomes were analyzed using paired t test for 65 participants. Results: Evidence based nursing competency increased significantly showing improvement in understanding EBN, formulating clinical question, searching & classifying the evidence, recognizing level of evidence, considering patient preference, and evaluating outcomes. Nursing students' access and use of evidence based information resource also improved. Conclusion: The results of this study indicate that the application of EBN to nursing management clinical practicum is effective to improve EBN competency in undergraduate students and should be further applied in nursing education using the systematic strategies.

Evidence-based Clinical Practice Protocol of Physical Restraints by Adaptation Process for Patients in a Geriatric Hospital (요양병원 입원 노인을 위한 신체 억제대 프로토콜의 수용개작)

  • Purpose: This study was to develop an evidence-based clinical practice protocol of physical restraints by adaptation process for patients with a geriatric hospital. Methods: Protocol adaptation process was conducted in accordance with manual for guideline adaptation version 1.0 by ADAPTE collaboration. Results: The adapted physical restraint protocol was consisted of 3 domains and 37 recommendations. The number of recommendations in each domain were: 7 nursing assessment, 19 nursing intervention, and 11 nursing evaluation. More than half (56.8%) of the recommendations were rated as grade B, 37.8% as grade C, and 5.4% were rated as grade D. Conclusion: The adapted physical restraint protocol is expected to contribute as an evidence-based clinical practice protocol for healthcare workers in geriatric hospitals for reducing and improving efficiency of appropriate physical restraints use.

  • Economic evaluations in the healthcare are used to assess economic efficiency of pharmaceuticals and medical interventions such as diagnoses and medical procedures. This study introduces the main concepts of economic evaluation across its key steps: planning, outcome and cost calculation, modeling, cost-effectiveness results, uncertainty analysis, and decision-making. When planning an economic evaluation, we determine the study population, intervention, comparators, perspectives, time horizon, discount rates, and type of economic evaluation. In healthcare economic evaluations, outcomes include changes in mortality, the survival rate, life years, and quality-adjusted life years, while costs include medical, non-medical, and productivity costs. Model-based economic evaluations, including decision tree and Markov models, are mainly used to calculate the total costs and total effects. In cost-effectiveness or costutility analyses, cost-effectiveness is evaluated using the incremental cost-effectiveness ratio, which is the additional cost per one additional unit of effectiveness gained by an intervention compared with a comparator. All outcomes have uncertainties owing to limited evidence, diverse methodologies, and unexplained variation. Thus, researchers should review these uncertainties and confirm their robustness. We hope to contribute to the establishment and dissemination of economic evaluation methodologies that reflect Korean clinical and research environment and ultimately improve the rationality of healthcare policies.

  • Digital forensics is a vital part of almost every criminal investigation given the amount of information available and the opportunities offered by electronic data to investigate and evidence a crime. However, in criminal justice proceedings, these electronic pieces of evidence are often considered with the utmost suspicion and uncertainty, although, on occasions are justifiable. Presently, the use of scientifically unproven forensic techniques are highly criticized in legal proceedings. Nevertheless, the exceedingly distinct and dynamic characteristics of electronic data, in addition to the current legislation and privacy laws remain as challenging aspects for systematically attesting evidence in a court of law. This article presents a comprehensive study to examine the issues that are considered essential to discuss and resolve, for the proper acceptance of evidence based on scientific grounds. Moreover, the article explains the state of forensics in emerging sub-fields of digital technology such as, cloud computing, social media, and the Internet of Things (IoT), and reviewing the challenges which may complicate the process of systematic validation of electronic evidence. The study further explores various solutions previously proposed, by researchers and academics, regarding their appropriateness based on their experimental evaluation. Additionally, this article suggests open research areas, highlighting many of the issues and problems associated with the empirical evaluation of these solutions for immediate attention by researchers and practitioners. Notably, academics must react to these challenges with appropriate emphasis on methodical verification. Therefore, for this purpose, the issues in the experiential validation of practices currently available are reviewed in this study. The review also discusses the struggle involved in demonstrating the reliability and validity of these approaches with contemporary evaluation methods. Furthermore, the development of best practices, reliable tools and the formulation of formal testing methods for digital forensic techniques are highlighted which could be extremely useful and of immense value to improve the trustworthiness of electronic evidence in legal proceedings.

The Tendency of Children's Response on Scientific Evidence Materials (과학적 증거 자료에 대한 아동들의 반응유형 경향성)

  • This study was designed to investigate children's response types, which are always consistent when they meet with different evidence materials. The participants in this study were 119 students of 6th grade in elementary school, who were acquainted with evidence materials presented and able to state logically. Evidence materials were constructed on the main subjects of a cause of the dinosaur extermination and a purpose of the astronomical observatory establishment. The criteria, 'whether or not believe evidence materials presented', 'whether or not believe evidence materials newly presented', 'where or not decrease their belief in the preinstructional evidence materials', and 'whether or not change their belief' were used to classify the type of their responses. So the students' responses to the evidence materials were classified into low types: rejection, reinterpretation, belief decrease, and valuation by evidences. Only about a third of them was also consistent with response types, but the others showed individually different responses to new evidence materials. The types of belief decrease and valuation by evidences were relatively below, while those of the rejection and reinterpretation were likely to represent them again, and these results did not reflect a meaningful difference between male and female. These results show that children's response type are not always coherent, and that they respond variously according to the evidence materials except for an individual cognitive view and a background in estimating evidence materials, and most of them are contents dependent in evaluating evidences. Therefore we are expected to apply it to studies on a thought process of the student as data on the basis of them and develop more various evidence materials responded by basis on evidence.

Application of Evidence Theory for the Evaluation of Mechanical Rock Mass Properties (암반설계정수 산정을 위한 증거이론의 적용)

  • The evaluation process of rock mass properties intrinsically contains some uncertainty due to the inhomogeneity of rock mass and the measurement error. Although various empirical methods for the determination of rock mass properties were suggested, there is no way of integrating various information on rock mass properties except averaging. For these reasons, this research introduces evidence theory which can model epistemic uncertainty and yield reasonable rock mass properties through combining various information such as empirical equations, in-situ test results, and so on. Through the application of evidence theory to the real site investigation and in situ experiment results, an interval of deformation modulus, cohesion and friction angle of rock mass were obtained. The ratios between lower and upper bound of those properties ranges from 1.6 to 3.6. Numerical analyses of circular hole using the properties for TYPE-2 rock mass were carried out. The magnitude or size of plastic region and radial displacement in case of lower bound properties is about 4 times larger than that of upper bound properties.

A study on the usefulness of standard evaluation tools for ambulance ride practices of paramedic students (응급구조(학)과 학생들의 구급현장실습 표준 평가도구의 유용성에 관한 연구)

  • Purpose: This study tests the validity of a standard evaluation tool of ambulance ride practices with new evaluation items and supports its application by 119 preceptors. Methods: We tested the validity of standard evaluation items collected from 19 on-site professionals assessing ambulance ride practices. New evaluation items, 'understanding of emergency rescue equipment' and 'ability to communicate', were added as additional criteria. The modified Delphi technique was used to test the three evaluation areas and ten evaluation items. We used the analytic hierarchy process to analyze the weighting value of the reconstructed evaluation tool model. Results: All three evaluation areas and 10 evaluation items within the standard evaluation tools used for ambulance ride practices corresponded with the consistency index, degree of convergence, and agreement in the modified Delphi panel. Conclusion: These results provide evidence of the consistency and usefulness behind preceptors' use of this standard evaluation tool in ambulance ride practice.

Development, Application and Evaluation of the Evidence-based Nursing Practice Guideline for Hemodialysis in the Patients with Risk of Bleeding (출혈위험이 있는 신부전환자의 혈액투석을 위한 근거중심 간호실무 가이드라인 개발과 적용 및 평가)

  • Purpose: The purpose of this study is to develop evidence-based nursing practice guidelines for hemodialysis in the renal failure patients with risk of bleeding and to evaluate the guidelines by applying it to the practice. Method: Referring to the SIGN, the first draft for guidelines was developed based on the advices and recommendations obtained from the procedure of critical literature analysis. Then, the draft was modified by the procedures of the expert group evaluation and pilot application to the practice. The final draft was evaluated by the expert group using AGREE instrument. Result: The first draft of guidelines was developed through 8 stages of process and was evaluated by seven experts in terms of the appropriateness, applicability, and effectiveness using a 9 point scale. The mean score of 11 items was 7.90 or above. The quality of the final draft was evaluated by 5 experts using the AGREE instrument. The mean standard score was 73.0% or above in the 19 items. Conclusion: The clinical guidelines developed by this research can be utilized as systematic and scientific guidelines for hemodialysis in the renal failure patients with risk of bleeding. In addition, the research can contribute to improving care services.
