Clincal Efficacy of Korean Red Ginseng for Erectile Dysfunction -Multi-national Approach

  • Ginseng has been used in maintaining physical vitality all over the East Asian countries and recently its metabolism and actions on neurologic, cardiovascular and endocrinologic systems are being elucidated. Korean red ginseng (KRG) has been used in various aliments, and to prove its efficacy icy erectile dysfunction an international study on Asians other than Korean was performed. Patients with borderline organic and psychogenic erectile dysfunction were included. KRG were given daily, and placebos were given as controls. Treatment lasted a total of 3 months. Surveys including libido, erectile potency, the sexual satisfactions were given. Serum testosterone and erectile function study were taken. Among the 23 patients with KRG, 18 patients were followed. Four had diabetes, 2 hypertension, 3 hyperchole, iterolemia, 1 low testosterone,4 psychogenic, and 4 idiopathic. In 10 patients with placebo, 7 were followed for more than three months. The clinical efficacy of KRG was 66.7% on objective Questionnaire and 72.2% on subjective analysis. When KRG were given, all parameters surveyed have shown improvements compared to the controls. Serum testosterone Bevels were normalized in 2 patients with KRG, whose serum testosterone levels were reduced from prestudy. When the erectile functions after audiovisual stimuli evaluated using Rigiscan in 6 patients with KRG,4 showed rigidity move than 70oyc. One patient reported constipation, and 2 gastric up-sets in the KRG group. In conclusion, KRG has beneficiary action on male erectile capabilities with minimal side effects. Thus KRG has been proven effective in Koreans, and a result on other Asians is pending. The exact action mechanism and the active ingredients in KRC need to be itudied.

The Effect of Herbal Formula KH-204 on Erectile Dysfunction in Hyperlipidemic Rat (고지혈증 흰쥐에서 생약제제 KH-204의 발기부전 치료연구)

  • This study was designed to investigate the effects KH-204 on erectile dysfunction in hyperlipidemic rat. KH-204 has been evaluated antihyperlipidemic and antierectile dysfunction effects on experimental hyperlipidemic rats induced by high fat diet. After oral administration of the water extract KH-204 (50, 100, 200, 300 mg/kg) to hyperlipidemic rats for 8 weeks, the variables including body weight, total cholesterol, HDL and LDL levels in serum, the expression of eNOS and nNOS in penis were measured. And erectile function was determined by measurement of intracavernosal pressure (ICP) and maximal arterial pressure (MAP) after electrical stimulation of the cavernosal nerve. Oral administration of KH-204 significantly inhibited the increases of serum total cholesterol and LDL-cholesterol levels and the decreased of serum HDL-cholesterol levels in hyperlipidemic rats induced by high fat diet. The penile expression level of the two enzyme (eNOS, nNOS) were increased significantly after oral administration of the KH-204 50 mg/kg. Erectile function after 10 volts stimulation was significantly decreased in the hyperlipidemic rat compared with the normal rat, but increased in KH-204 group compared with hyperlipidemic group. These results suggest that KH-204 is effective for erectile dysfunction in hyperlipidemia.

Effects of Albizzia Julibrissin on Chronic Ethanol-treated Erectile Dysfunction in Rats (Ethanol 에 의해 발기부전을 유도한 흰쥐의 성기능 개선에 미치는 합환피(合歡皮)의 영향)

  • Objectives : Albizzia Julibrissin was formulated to contain various natural products known to cure erectile dysfunction. This study was aimed to investigate the effects of Albizzia Julibrissin on the nitric oxide synthase (NOS) activity, nitrite level, antioxidation and erectile responses induced by ethanol in corpus cavernosum penis of rats. Methods : The crushed Albizzia Julibrissin was extracted 3 times, each time with 3 volumes of methyl alcohol at $60^{\circ}C$ for 24 h. The extract was filtered and evaporated under a reduced pressure using a rotary evaporator to yield 45.3 g. Albizzia Julibrissin extract was oral-administered 100 mg per 1 kg of body weight for 20 days, while the normal group was administered only with a saline. The efficacy of Albizzia Julibrissin against erectile function was examined as described in the text. Results : The level of urethral NOS activity and nitrite were increased by Albizzia Julibrissin. The level of lipid peroxide was decreased by Albizzia Julibrissin. The level of urethral lipid peroxide in the ethanol-Albizzia Julibrissin double administered rats was decreased as low as in the norma! group, while the one in the ethanol-treated group was increased. The level of urethral nitrite, NOS activity, glutathione and serum testosterone in the ethanol-Albizzia Julibrissin double administered rats were as high as in the normal group, while the one in the ethanol-treated group was decreased. The erectile response to cavernous nerve stimulation in the ethanol-Albizzia Julibrissin double administered rats increased as high as in the normal group while the one in the ethanol-treated group decreased. Conclusions : Albizzia Julibrissin was shown to be effective for the treatment of erectile dysfunction induced by ethanol in rats.

Evaluation of Clinical Efficacy of Korea Red Ginseng for Erectile Dysfunction by International Index of Erectile Function(IIEF) (국제 발기능측정설문지(IIEF)를 이용한 발기부전 환자에서의 홍삼 효능 평가)

  • International index of erectile function (IIEF) has a high sensitivity and specificity for the evaluating treatment of erectile dysfunction. IIEF was used to evaluate on an international level the effectiveness of red ginseng on patients with erectile dysfunction and determine its possible development as a natural drug. 50 patients with erectile failure who were 20 years or older and without organic cause were randomly divided into two groups. Group 1(25 patients) received red ginseng 600mg three times a day and group 2 (25 patients) received placebo in the same fashion for 8 weeks. Each group was evaluated with IIEF at the start and end of therapy. Of the 50 patients 47 (group 1 24, group 2 23) who completed the 8 week therapy were evaluated. The mean age and symptom duration of the patients was 45.7$\pm$8.7 (27-68) and 6.2$\pm$5.6(1-29) years, respectively. Patient age, symptom duration. marriage status, marriage duration, partner age were not statistic significance between the two groups (p>0.05). Blood chemistry, urinalysis, and hormonal assay did not reveal difference between the two groups. primary efficacy (erectile rigidity and its maintenance) and all domains in secondary efficacy were significantly better after therapy in group 1 compared to those of group 2 (p<0.05) except for frequency of sexual desire, degree of sexual desire, and frequency of intercourse trial (p>0.05). Group 1 patients were significantly more satisfied after treatment compared to group 2 patients with 58.3% (14/24)and 26.1%(6/23) satisfaction rates, respectively. There were no adverse reactions or complications with ginseng adiminstration. Further study into the effects of saponin and ginsenoside that are the main substances in ginseng for erectile dysfunction is needed. In addition, its possible additive effect with ginseng and sildenafil should be elucidated in the future.

Effects of Ojawhan on the ethanol-induced erectile dysfunction in rats (오자환(五子丸)이 Ethanol로 발기부전을 유도한 흰쥐의 성기능 개선에 미치는 영향)

The extract was filtered and evaporated under a reduced pressure using a rotary evaporator to yield 62g. Ojawhan extract oral-administered 100 mg per 1 kg of body weight for 30 days. First, samples were treated with Ojawhan, then ethanol-treated rats and L-N-Nitroarginine methyl ester(L-NAME) treated rats were put with the samples. Result : The level of urethral lipid peroxide in the ethanol-Ojawhan double administered rats was decreased as low as in the normal group, while the one in the ethanol-treated group was increased. The urethral NOS activity, the level of urethral nitrite, the level of testosterone and the erectile response to cavernous nerve stimulation in the ethanol-Ojawhan double administered rats were increased as high as in the normal group while the one in the ethanol-treated group was decreased. The electile response to cavernous nerve stimulation and the level of nitrite in L-NAME ($10^{-4}$)-treated rats was restored by the administration of Ojawhan as high as in the normal group. Conclusions : Ojawhan was effective in restoring the ethanol-induced or L-NAME-induced erectile dysfunction in rats.

This study was aimed to investigate the effects of Scolopendra on the nitric oxide synthase activity, nitrite level, cyclic-GMP and erectile responses in rat's corpus cavernosum penis. The crushed Scolopendra was extracted 3 times, each time with 3 volumes of methyl alcohol at $60^{\circ}C$ for 24 h. The extract was filtered and evaporated under a reduced pressure using a rotary evaporator to yield 14.2 g. Scolopendra extract was oral-administered 50 mg per 1 kg of body weight for 30 days. First, samples were treated with Scolopendra, and then ethanol-treated rats and L-N-Nitroarginine methyl ester (L-NAME) treated rats were fed with the samples. The level of urethral nitrite and nitric oxide synthase activity in the ethanol-Scolopendra double administered rats were as high as in the normal group, while the one in the ethanol-treated group was decreased. The level of urethral cyclic-GMP and guanylate cyclase actiyity in the ethanol-Scolopendra double administered rats were as high as in the normal group, while the one in the ethanol-treated group was decreased. The level of urethral phosphodiesterase activity in the ethanol-Scolopendra double administered rats was as low as in the normal group, while the one in the ethanol-treated group was increased. The erectile response to a cavernous nerve stimulation in L-NAME $(10^{-4})-treated$ rats was restored by the Scolopendra to the similar level seen in the normal group. The electile response to cavernous nerve stimulation in the ethanol-Scolopendra double administered rats were increased as high as in the normal group while the one in the ethanol-treated group was decreased. Scolopendra was effective in restoring the ethanol-induced or L-NAME-induced erectile dysfunction in rats.

Effect of KH-305 on the Nitric Oxide Synthase Activity and Erectile Dysfunction in Young Rats (KH-305 투여가 흰쥐 음경조직의 Nitric Oxide Synthase활성 및 Erectile Dysfunction에 미치는 영향)

  • This study was designed to investigate the effects of KH-305 on erectile dysfunction in young rats, via nitric oxide (NO)-cGMP pathways. After oral administration of the KH-305 mixture (50, 100, 200, 300 mg/kg) to young rats for 10 days, NOS and SOD protein expressions in penile tissue and testosterone in plasma were measured. cGMP degradation was also investigated using bovine vascular smooth muscle cells pretreated with an NO donor, S-nitroso-N-Acetylpenicillamine (SNAP). The penile expression levels of nNOS and eNOS-dependent NOS activities as well as SOD preventing oxidative stress by overproduction of NO were increased significantly. Also, the concentration of testosterone in the plasma was increased. In vitro, cGMP concen-trations were decreased dose dependently in the KH-305. These results suggest that KH-305 may be useful in erectile dysfunction.

  • This study was designed to investigate effects of KH-304 in improving erectile dysfunction (ED), particularly in terms of nitric oxide (NO)-cGMP pathways. After oral administration of the KH-304 water extract, 1OOmg, 300mg, 500mg or 700mg per 1 kg of Dody weigh for 10days, We examined the expression and activity of two enzyme: neuronal NO synthase (nNOS), endothelial NO synthase (eNOS) and that act upon the major NO-cGMP signaling pathway in penile tissue. Effect of KH-304 on COMP degradation was also examined using bovine vascular smooth muscle cells pretreated with an NO donor, S-nitroso-N-Acetylpenicillamine (SNAP), Also, it examined the endothelial NO synthase (eNOS) for seaching effecting period (100mg, 300mg/kg for 10 and 30days) and peak intracavernous pressures (ICPS) in penile tissues rabbit copus cavernosum contracted by 10-6 M phenylephrine. The severely reduced peak intracavernous pressures (ICPS) in penile tissues were restored completely after KH-304 treatment, and KH-304 treatment significantly made the latency period earlier. Furthermore, the penile expression levels of nNOS, eNOS dependent NOS activities and COMP concentrations were increased significantly in the KH-304 100, 300mg treated rats. These results suggest that KH-304 with high expression of NOS may be useful in erectile dysfunction.

Analysis of erectile dysfunction drugs and their analogues in counterfeit drugs and herbal medicines by LC-ESI-MS/MS

  • Distribution of various illegal or counterfeit drugs of seven approved erectile dysfunction drugs and their analogues has been increased, causing health problems such as cardiovascular disorder, tachycardia, headache, or vision disturbance. We used liquid chromatography-electrospray ionization-tandem mass spectrometry (LC-ESI-MS/MS) to determine the erectile dysfunction drugs and their analogues in various counterfeit drugs. Eleven erectile dysfunction drugs and their analogues were detected, with sildenafil and its analogues being the most counterfeited compounds (73.8 %), followed by tadalafil and its analogues (25.4 %). The limits of detection (LOD) and the limits of quantitation (LOQ) of liquid-type and solid-type negative samples ranged from 0.1 to 3.3 ng/mL or ng/g and from 0.3 to 10.0 ng/mL or ng/g, respectively. The recoveries ranged from 84.3 to 112.3 % and 83.2 to 110.2 %, respectively. The contents of sildenafil and tadalafil in the various counterfeit drugs ranged from 21.0 to 947.5 mg/g and from 0.2 to 170.2 mg/g, respectively.
