A Study on the Effect of Field Shaping on Dose Distribution of Electron Beams (전자선의 선량분포에 있어서 Field Shaping의 효과에 관한 연구)

  • In electron therapy, lead cutout or low-melting alloy block is used for shaping the field. Material for shaping electron field affects the output factor as wet 1 as the collimation system. The authors measured the output factors of electron beams for shaped fields from Clinac-18 using ionization chamber of Farmer type in polystyrene phantom. They analyzed the parameters that affect the output factors. The output factors of electron beams depend on the incident energy, collimation system and size of shaped field. For shaped field the variation of output factor for the field size (A/P) has appearence of a smooth curve for all energy and all applicator collimator combination. The output factors for open field deviate from the curves for shaped fields. An output factor for a given field can be calculated by equivalent field method such as A/P method, if a combination of applicator and collimator is fixed.

Angelica keiskei Improved Beta-amyloid-induced Memory Deficiency of Alzheimer's Disease (아밀로이드 베타로 유발한 알츠하이머병 모델에서 신선초의 기억력 개선 효과)

  • Objectives : Amyloid ${\beta}(A{\beta})$ could induce cognitive deficits through oxidative stress, inflammation, and neuron death in Alzheimer's disease (AD). This study was investigated the effect of Angelica keiskei KOIDZUMI (AK) on memory in $A{\beta}$-induced an AD model. Methods : AK was extracted uses 70% ethanol solvent. Total polyphenol and flavonoids content were obtained by the Folin-Ciocalteu and the Ethylene glycol colorimetric methods, respectively. The antioxidant activities were assessed through free radical scavenging assays using 2,2-diphenyl-1-picrylhydrazyl (DPPH) and 2,2'-azino-bis (3-ethylbenzothiazolin-6-sulfonic acid) (ABTS) methods. Intracerebroventrical (i.c.v) injection of $A{\beta}$ 1-42 was used to induce AD in male ICR mice, followed by administrations of 5, 10 or 20 mg/kg AK on a daily. Animals were subjected to short and long term memory behavior in Y-maze and passive avoidance test. Results : The total polyphenol and flavonoids contents of the AK extract were $88.73{\pm}6.36mg$ gallic acid equivalent/g, $84.21{\pm}5.04mg$ rutin equivalent/g, respectively. The assays of DPPH and ABTS revealed that AK extract in treated concentrations (31.25, 62.5, 125, 250, 500, $1000{\mu}g/m{\ell}$) increased antioxidant activity in a dose-dependent manner. Oral administration of AK extract significantly reversed the $A{\beta}$ 1-42-induced decreasing of the spontaneous alternation in the Y-maze test and $A{\beta}$ 1-42-induced shorting of the step-through latency in the passive avoidance test. Conclusions : The findings suggest that AK indicated the antioxidant protective effects against $A{\beta}$-induced memory deficits, and therefore a potential lead natural therapeutic drug or agent for AD.

Antioxidant activities of ethanol extract of Shinsun-yukza-hwan, a Korean medicinal recipe (신선육자환 에탄올 추출물의 항산화 활성)

  • Objective : The Sinseon-yukza-hwan (SSY), a Korean medicinal formula which includes Radix Rehmanniae Preparata and other medicinal herbs, has long been used for treatment of alopecia and gray hair through oral administration. This study is designed to enhance the utilization of natural materials in hair and scalp-related cosmetics. Possibility of SSY as an antioxidant was examined from its 50% ethanol extract. Methods : The antioxidative capacities were evaluated by determining total phenolic and flavonoid contents, 1,1-diphenyl-2-picryl hydrazyl (DPPH), 2'-azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) (ABTS), reducing power and hydroxyl radical scavenging activity. Results : Total polyphenol and flavonoid contents of SSY were 25.53 mg TE, tannic acid equivalent/g and 18.90 mg RE, rutin equivalent/g, respectively, which correlated strongly its antioxidative activity. The DPPH and ABTS free radical scavenging activities of SSY at $0.1mg/m{\ell}{\sim}5mg/ml$ were ranged from 20% to 85% and 10% to 58%, respectively. Also the hydroxyl radical scavenger activity and reducing power increased in SSY-treated group, which were significantly lower in SSY-compared to BHA-treated group. But the highest reducing power was shown as 79% from SSY-treated group, which was higher value than 65% from BHA-treatment. These results showed that SSY extract effectively inhibited the generation of free radicals in the all assay system with dose-dependent manners. Conclusions : Thus, the present study provide preclinical data to support the expanded application of SSY, which could be potential candidates for natural antioxidants.

Experimental investigation of the photoneutron production out of the high-energy photon fields at linear accelerator (고에너지 방사선치료 시 치료변수에 따른 광중성자 선량 변화 연구)

  • Purpose : Photoneutron dose in high-energy photon radiotherapy at linear accelerator increase the risk for secondary cancer. The purpose of this investigation is to evaluate the dose variation of photoneutron with different treatment method, flattening filter, dose rate and gantry angle in radiation therapy with high-energy photon beam ($E{\geq}8MeV$). Materials and Methods : TrueBeam $ST{\time}TM$(Ver1.5, Varian, USA) and Korea Tissue Equivalent Proportional Counter (KTEPC) were used to detect the photoneutron dose out of the high-energy photon field. Complex Patient plans using Eclipse planning system (Version 10.0, Varian, USA) was used to experiment with different treatment technique(IMRT, VMAT), condition of flattening filter and three different dose rate. Scattered photoneutron dose was measured at eight different gantry angles with open field (Field size : $5{\time}5cm$). Results : The mean values of the detected photoneutron dose from IMRT and VMAT were $449.7{\mu}Sv$, $2940.7{\mu}Sv$. The mean values of the detected photoneutron dose with Flattening Filter(FF) and Flattening Filter Free(FFF) were measured as $2940.7{\mu}Sv$, $232.0{\mu}Sv$. The mean values of the photoneutron dose for each test plan (case 1, case 2 and case 3) with FFF at the three different dose rate (400, 1200, 2400 MU/min) were $3242.5{\mu}Sv$, $3189.4{\mu}Sv$, $3191.2{\mu}Sv$ with case 1, $3493.2{\mu}Sv$, $3482.6{\mu}Sv$, $3477.2{\mu}Sv$ with case 2 and $4592.2{\mu}Sv$, $4580.0{\mu}Sv$, $4542.3{\mu}Sv$ with case 3, respectively. The mean values of the photoneutron dose at eight different gantry angles ($0^{\circ}$, $45^{\circ}$, $90^{\circ}$, $135^{\circ}$, $180^{\circ}$, $225^{\circ}$, $270^{\circ}$, $315^{\circ}$) were measured as $3.2{\mu}Sv$, $4.3{\mu}Sv$, $5.3{\mu}Sv$, $11.3{\mu}Sv$, $14.7{\mu}Sv$, $11.2{\mu}Sv$, $3.7{\mu}Sv$, $3.0{\mu}Sv$ at 10MV and as $373.7{\mu}Sv$, $369.6{\mu}Sv$, $384.4{\mu}Sv$, $423.6{\mu}Sv$, $447.1{\mu}Sv$, $448.0{\mu}Sv$, $384.5{\mu}Sv$, $377.3{\mu}Sv$ at 15MV. Conclusion : As a result, it is possible to reduce photoneutron dose using FFF mode and VMAT method with TrueBeam $ST{\time}TM$. The risk for secondary cancer of the patients will be decreased with continuous evaluation of the photoneutron dose.

Internal Dose Assessment of Worker by Radioactive Aerosol Generated During Mechanical Cutting of Radioactive Concrete (원전 방사성 콘크리트 기계적 절단의 방사성 에어로졸에 대한 작업자 내부피폭선량 평가)

  • Removing radioactive concrete is crucial in the decommissioning of nuclear power plants. However, this process generates radioactive aerosols, exposing workers to radiation. Although large amounts of radioactive concrete are generated during decommissioning, studies on the internal exposure of workers to radioactive aerosols generated from the cutting of radioactive concrete are very limited. In this study, therefore, we calculate the internal radiation doses of workers exposed to radioactive aerosols during activities such as drilling and cutting of radioactive concrete, using previous research data. The electrical-mobility-equivalent diameter measured in a previous study was converted to aerodynamic diameter using the Newton-Raphson method. Furthermore, the specific activity of each nuclide in radioactive concrete 10 years after nuclear power plants are shut down was calculated using the ORIGEN code. Eventually, we calculated the committed effective dose for each nuclide using the IMBA software. The maximum effective dose of 152Eu constituted 83.09% of the total dose; moreover, the five highest-ranked elements (152Eu, 154Eu, 60Co, 239Pu, 55Fe) constituted 99.63%. Therefore, we postulate that these major elements could be measured first for rapid radiation exposure management of workers involved in decommissioning of nuclear power plants, even if all radioactive elements in concrete are not considered.

The Effects of the CT Voltages on the Dose Calculated by a Commercial RTP System (CT 관전압이 상용 전산화치료계획장치의 선량계산에 미치는 영향)

  • The relationship between the dose calculated with a radiotherapy treatment planning system (RTPS) and CT number verses the relative electron density curve was investigated for various CT voltages and beam qualifies. We obtained the relationship between the CT numbers and electron densities of the tissue equivalent materials for various CT voltages and beam qualifies. At lower CT voltages, the higher density materials, like cortical bone, showed larger CT numbers and the soft tissues showed no variations. We peformed a phantom study in a RTPS, where a phantom consisted of lung and bone legions in water. We calculated the dose received behind the lung and bone regions for 6 MV photon beams, in which the regions below the lung, water and bone received higher doses in this listed order. The result was the same for 10 MV photon beams. For the clinical application, the doses were calculated for the lung and pelvis. No difference was observed when using different electron density conversion tables with various CT voltages from a same CT. A relative dose difference of 1.5% was obtained when the CT machine for the density conversion table was different from that for the CT image for planning.

Dosimetric Characteristics of the KCCH Neutron Therapy Facility (원자력병원 중성자선치료기의 물리적특성)

  • For the physical characterization of neutron beam, dosimetric measurements had been performed to obtain physical data of KCCH cyclotron-produced neutrons for clinical use. The results are presented and compared with the data of other institutions from the literatures. The central axis percent depth dose, build-up curves and open and wedge isodose curve values are intermediate between that of a 4 and 6 MV X-rays. The build-up level of maximum dose was at 1.35cm and entrance dose was approximately $40\%$. Flatness of the beam was $9\%$ at Dmax and less $than{\pm}3\%$ at the depth of $80\%$ isodose line. Penumbra begond the $20\%$ line is wider than corresponding photon beam. The output factors ranged 0.894 for $6\times6cm$ field to 1.187 for $30\times30cm$ field. Gamma contamination of neutron beam was $4.9\%$ at 2 cm depth in $10\times10cm$ field.

  • This paper aimed to analyse dose sensitivity to the controllable parameters of indoor radon $(^{222}Rn)$ and its decay products (Rn-D) by applying the input~output linear system theory. Physical behaviors of $^{222}Rn\;&\;Rn-D$ were analyzed in terms of $(^{222}Rn)$ gas -generation, -migation and -infiltration to indoor environments, and the performance output-function, i. e. mean dose equivalent to Tracho-Bronchial (TB) lung region, was assessed to the following extented ranges of the controllable paramenters; a) the ventilation rate $constant({\lambda}_v)\;:\;0{\sim}50[h^{-l}].\;b)$ the attachment rate $constant({\lambda}_a)\;:\;0{\sim}500[h^{-l}].\;c)$ the unattached-deposition rate constant (${\lambda}^u_d)\;:\;0-50[h-l]$. A linear input-output model was reconstructed from the original models in literatures, as follows, which was modified into the matrices consisting of 111 nodal equations; a) indoor $^{222}Rn\;&\;Rn-D$ Behaviour; Jacobi-Porstendoerfer-Bruno model.

  • A single-dosimeter worn on the anterior surface of body of a worker was found to provide significant underestimation of dose to the worker when radiation comes from behind of the human body. Recently, several researchers suggested that this kind of underestimation can be corrected to a certain extent by using an extra dosimeter on the back. But this multiple dosimetry also has the disadvantages like overestimation lowering work efficiency or cost burden. In this study, a single dosimeter introducing asymmetric filters enabled to identify PA exposure was designed by monte-carlo simulation and experiments and its dose evaluation algorithm for AP-PA mixed radiation field was established. This algorithm was applicable to penetrating radiation which had the effective energy more than 100 keV. Besides, the dosimeter and algorithm in this study were possible to be applied to near PA exposure.

Prescription Pattern of 1 Year Clozapine Maintenance and Augmentation Agents in Schizophrenia Spectrum Disorders (조현병 스펙트럼 장애의 1년 유지 치료에서 클로자핀과 병용 치료제의 처방 양상 분석)

  • Objectives Clozapine is the most effective atypical antipsychotic agent for the treatment-resistant schizophrenia (TRS), however, only 40%-70% of TRS patients respond to clozapine. Moreover, TRS encompasses various symptom dimensions. Therefore, augmentation with other medications for clozapine is frequently applied. However, the prescription pattern of clozapine and combined medications in Korea is yet to be examined. This study aims to investigate the maintenance treatment pattern of clozapine and augmentation agents in one Korean tertiary hospital. Methods The patients with schizophrenia spectrum disorders under clozapine maintenance, defined as one-year clozapine continuation, were subjected for analysis. Medication data at one-year time-point after clozapine initiation was extracted and analyzed. Results Among total 2897 patients having clozapine prescription experience from January 2000 to December 2018, 1011 patients were on clozapine maintenance. The mean age of clozapine initiation was 30.2 ± 11.3 years, and the maintenance dose of clozapine was 217.8 ± 124.3 mg/day. Combination rate of antipsychotics, mood stabilizers, and antidepressants were 43.5%, 25.3%, 38.6%, respectively. Most frequently prescribed drugs in each category were aripiprazole, valproate, and sertraline. Olanzapine equivalent dose of combined antipsychotics was 10.4 ± 7.7 mg/day. Male patients were prescribed higher dose of combined antipsychotics and higher rate of antidepressants. Female patients had later onset of clozapine prescription. Patients with two or more combined antipsychotics were prescribed higher dose of clozapine and higher rate of antidepressants compared to patients with one combined antipsychotic. Conclusions Taken together, among the patients taking clozapine, a substantial rate of patients were under polypharmacy. The present findings based on the real-world prescription pattern could provide the valuable clinical information on the treatment of TRS-related conditions.
