Diagnosis of Laryngeal Cyst using Respiratory Endoscopy in Hanwoo Cattle with Chronic Bronchopneumonia

  • A seven-month-old Hanwoo steer was presented immediately after transport with respiratory symptoms including a nasal discharge, depression, and anorexia. Though repeated treatments, bronchopneumonia had not been improved and had persisted for 10 months. Then, obstructive breath sound was heard. A cyst adjacent to the epiglottis could be observed with respiratory endoscopy. Consequently, chronic bronchopneumonia induced laryngeal cyst formation, resulting in obstructive dyspnea. And respiratory endoscopy may be useful for differentiating the causes of dyspnea in bovine clinical practice.

  • 본 논문에서는 효율적인 제어를 위하여 캡슐 앤도스코피에 대한 모델링을 실시하였다. 방법론적으로 루그레 모델에 대한 시스템 특성파악을 위한 수학적 모델링을 이용하였다. 비선형 마찰 모델인 루그레 모델에 바탕을 둔 stick-slip 모션 시스템이 인체내에서의 캡슐 앤도스코피의 활동을 묘사하는 시뮬레이션 모델로 이용하였다. 다양한 상황을 고려하여 루그레 마찰모델에 대한 시뮬레이션을 Matlab Simulink 를 작성하여 수행하였다. 전체적인 모션과 파라미터의 영향이 엔도스코피의 속도에 미치는 영향에 대한 것에 주안을 두어 실시하였다.

Image-Enhanced Endoscopy in Lower Gastrointestinal Diseases: Present and Future

  • From dye-assisted conventional chromoendoscopy to novel virtual chromoendoscopy, image-enhanced endoscopy (IEE) is continuously evolving to meet clinical needs and improve the quality of colonoscopy. Dye-assisted chromoendoscopy using indigo carmine or crystal violet, although slightly old-fashioned, is still useful to emphasize the pit patterns of the colonic mucosa and predict the histological structures of relevant lesions. Equipment-based virtual chromoendoscopy has the advantage of being relatively easy to use. There are several types of virtual chromoendoscopy that vary depending on the manufacturer and operating principle. IEE plays distinctive roles with respect to histologic characterization of colorectal polyps and prediction of the invasion depth of colorectal cancers. In addition, the newest models of IEE have the potential to increase adenoma and polyp detection rates in screening colonoscopy.

건강검진센터에서 위장 및 대장 수면 내시경 검사 증가에 따른 필요 회복실 침상 수 및 대기 시간 예측 시뮬레이션 (Using Simulation to Predict the Number of Recovery Bed and Waiting Time as Increasing Client for Sleep Endoscopy Check in Health Service Center)

  • 질병의 조기진단을 위한 건강검진이 늘어나면서 상대적으로 불편감이 적은 위장 및 대장 수면 내시경에 대한 요구도가 증가하고 있다. 본 연구는 한 서울 종합병원 건강검진 센터에서의 수면 내시경 검사, 회복 및 회복실 대기 시간을 이용하여 현재 수면 내시경 대상자 및 대상자가 10%, 20% 증가할 경우 필요한 회복실 침상 수 및 회복 대기 시간을 예측하기 위해 실시되었으며 ARENA 10.0 시뮬레이션 프로그램을 이용하여 분석하였다. 현재의 수면 내시경 대상자 수인 경우 회복실 대기시간 7분, 대기 명수 2명, 침상가동률 약 50%이며, 수면 내시경 대상자 수가 10%, 20% 증가함에 따라 각각 대기 시간, 명수 및 침상가동률이 증가하였다. 그러므로 현재 상태 및 대상자 증가 시에도 대상자의 안전관리를 위해 수면 내시경 후 회복실의 침상을 3개 증가시켜야 대기시간이 2분 이내로 줄어드는 것으로 나타났다.

  • Purpose: The purpose of this study is to compare the clinical results between two different methods of hindfoot endoscopy to treat posterior ankle impingement syndrome. Materials and Methods: Between January 2008 and January 2014, 52 patients who underwent hindfoot endoscopy were retrospectively reviewed. Two methods of hindfoot endoscopy were used; Group A was treated according to van Dijk and colleagues' standard twoportal method, and group B was treated via the modified version of the above, using a protection cannula. For clinical comparison, the American Orthopaedic Foot and Ankle Society (AOFAS) hindfoot score, time required to return to activity, and the presence of complications were used. Results: There was no statistically significant difference in the AOFAS scores at the final follow-up, and there was also no statistically significant difference in the times for the scores to return to the preoperative level. There were no permanent neurovascular injuries and wound problems in either group. Conclusion: Use of protection cannula may provide additional safety during hindfoot endoscopy. We could not prove whether protection cannula can provide superior safety for possible neurovascular injury. Considering the possible safety and risk of using additional instrument, the use of this method may be optional.

VRML을 이용한 3차원 Brain-endoscopy와 2차원 단면 영상 (3D Brain-Endoscopy Using VRML and 2D CT images)

  • Virtual Brain-endoscopy is an effective method to detect lesion in brain. Brain is the most part of the human and is not easy part to operate so that reconstructing in 3D may be very helpful to doctors. In this paper, it is suggested that to increase the reliability, method of matching 3D object with the 2D CT slice. 3D Brain-endoscopy is reconstructed with 35 slices of 2D CT images. There is a plate in 3D brain-endoscopy so as to drag upward or downward to match the relevant 2D CT image. Relevant CT image guides the user to recognize the exact part he or she is investigating. VRML Script is used to make the change in images and PlaneSensor node is used to transmit the y coordinate value with the CT image. The result is test on the PC which has the following spec. 400MHz Clock-speed, 512MB ram, and FireGL 3000 3D accelerator is set up. The VRML file size is 3.83MB. There was no delay in controlling the 3D world and no collision in changing the CT images. This brain-endoscopy can be also put to practical use on medical education through internet.

The Value of I-Scan Image-Enhanced Endoscopy in the Diagnosis of Vocal Cord Leukoplakia

  • Background and Objectives : Detection of vascular abnormalities in vocal cord (VC) leukoplakia is important for the diagnosis of neoplastic change of the mucosa. The aim of this study was to investigate the value of i-scan in the differential diagnosis of VC leukoplakia based on visualization of abnormal vascular features. Material and Methods : Fifty-two patients with leukoplakia were enrolled in the study. Images of their larynx obtained using conventional white light endoscopy and an i-scan-enhanced endoscopy (Pentax DEFINA EPK-3000 Video Processors, with Pentax VNLJ10) were reviewed. The microvascular features of the lesions and vascular changes were analyzed and the results were compared with the histopathologic diagnosis. Results : Among the 52 leukoplakia patients, 7 (13.5%) patients had squamous hyperplasia, 10 (19.3%) mild dysplasia, 2 (3.8%) moderate dysplasia, 14 (26.9%) severe dysplasia, 4 (7.7%) carcinoma in situ, and 15 (28.8%) invasive squamous cell carcinoma on histopathologic examination. Using i-scan-enhanced endoscopy, abnormal vascular change with neoplastic neoangiogenesis was detected in most cases of malignant VC lesion [severe dysplasia : 9/14 (64.3%), carcinoma in situ: 2/4 (50.0%), and invasive squamous cell carcinoma : 11/15 (73.4%)]. Conclusion : i-scan-enhanced endoscopy is a useful optical technique for the diagnosis of VC leukoplakia. Our results suggest that i-scan may be a promising diagnostic tool in the early detection of laryngeal cancer.

Upper Endoscopy up to 3 Years Prior to a Diagnosis of Gastric Cancer Is Associated With Lower Stage of Disease in a USA Multiethnic Urban Population, a Retrospective Study

  • Objectives: In the USA, certain races and ethnicities have a disproportionately higher gastric cancer burden. Selective screening might allow for earlier detection and curative resection. Among a USA-based multiracial and ethnic cohort diagnosed with non-cardia gastric cancer (NCGC), we aimed to identify factors associated with curable stage disease at diagnosis. Methods: We retrospectively identified endoscopically diagnosed and histologically confirmed cases of NCGC at Mount Sinai Hospital in New York City. Demographic, clinical, endoscopic and histologic factors, as well as grade/stage of NCGC at diagnosis were documented. The primary outcome was the frequency of curable-stage NCGC (stage 0-1a) at diagnosis in patients with versus without an endoscopy negative for malignancy prior to their index exam diagnosing NCGC. Additional factors associated with curable-stage disease at diagnosis were determined. Results: A total of 103 racially and ethnically diverse patients were included. Nearly 38% of NCGC were stage 0-Ia, 34% stage Ib-III, and 20.3% stage IV at diagnosis. A significantly higher frequency of NCGC was diagnosed in curable stages among patients who had undergone an endoscopy that was negative for malignancy prior to their index endoscopy that diagnosed NCGC, compared to patients without a negative endoscopy prior to their index exam (69.6% vs. 28.6%, p=0.003). A prior negative endoscopy was associated with 94.0% higher likelihood of diagnosing curable-stage NCGC (p=0.003). No other factors analyzed were associated with curablestage NCGC at diagnosis. Conclusions: Endoscopic screening and surveillance in select high-risk populations might increase diagnoses of curable-stage NCGC. These findings warrant confirmation in larger, prospective studies.

A Nationwide Survey on Gastrointestinal Endoscopy Practice Patterns among Pediatric Endoscopists in South Korea

  • Purpose: Gastrointestinal (GI) endoscopy is an important tool for diagnosing and treating GI diseases in children. This study aimed to analyze the current GI endoscopy practice patterns among South Korean pediatric endoscopists. Methods: Twelve members of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition developed a questionnaire. The questionnaire was emailed to pediatric gastroenterologists attending general and tertiary hospitals in South Korea. Results: The response rate was 86.7% (52/60), and 49 of the respondents (94.2%) were currently performing endoscopy. All respondents were performing esophagogastroduodenoscopy, and 43 (87.8%) were performing colonoscopy. Relatively rare procedures for children, such as double-balloon enteroscopy (DBE) (4.1%), endoscopic retrograde cholangiopancreatography (ERCP) (2.0%), and endoscopic ultrasound (EUS) (2.0%), were only performed by pediatric gastroenterologists at very few centers, but were performed by adult endoscopists in most of the centers; of all the respondents, 83.7% (41/49) performed emergency endoscopy. In most centers, the majority of the endoscopies were performed under sedation, with midazolam (100.0%) and ketamine (67.3%) as the most frequently used sedatives. Conclusion: While most pediatric GI endoscopists perform common GI endoscopic procedures, rare procedures, such as DBE, ERCP, and EUS, are only performed by pediatric gastroenterologists at very few centers, and by adult GI endoscopists at most of the centers. For such rare procedures, close communication and cooperation with adult GI endoscopists are required.
