CAC를 이용한 긴급도로 보수재의 현장 적용성 (Field Applicability Of Emergency Road Repair Material Using the CAC)

  • This study was to review the basic characteristics in order to evaluate field application of the emergency road repair materials for development of CAC(Calcium Aluminate Composite) usage. The experiment was conducted with two phases of field and laboratory conditions and the laboratory experiment consisted indoor and outdoor tests for compressive and flexural strength. In the result of an experiment, for the compressive strength test, the specimens that cured in the laboratory conditions were not satisfied the requirement of standards, while the specimens that cured in the field conditions were well satisfied with those. For flexural strength test, the result value was satisfied with the requirement on the standards only in outdoor curing condition of laboratory experiment. Based on these results, it is expected that the CAC can be used as an emergency road repair material for field conditions.

도로 및 비탈면 유실 항구적 긴급복구를 위한 골재망 콘크리트 활용기술 개발 (Emergency and Permanent Repair Technology for Damaged Road Bases and Slopes using Gravel-Netting Concrete)

  • PURPOSES : The frequency and severity of natural disasters such as torrential rain or typhoons have become increasingly significant worldwide. Events such as summer typhoons and localized torrential downpour can cause severe damages to a residential area and road networks, resulting in serious harm to the daily lives of people, especially in rural areas by isolating residents from road networks. An immediate and emergency repair technology for the collapsed road networks is urgently needed. This study introduces a new technology to repair road bases or slopes. METHODS : The development of new technology for emergency and permanent repair consists of first, packing of cement paste-coated gravel, second, combining appropriate equipment, and third, conducting a field applicability test. In this research, the compressive strength of cement pastecoated gravel, gravel-netting concrete properties, and packing efficiency were determined, and a full scale field mock-up test was carried out. RESULTS : The compressive strength of the cement paste-coated gravel concrete satisfied the required limit for road base of 5 MPa after 7 days. With appropriate netting materials and packing size, gravel-netting concrete was successful up to a slope of 1:1.5. The full scale field mock-up test showed efficiency in the field and penetration resistance performance. CONCLUSIONS : The new technology of emergency and permanent repair for damaged road bases and slopes, introduced in this study, showed satisfactory performance. The technology is expected to be applied in the field when construction procedures and quality specifications are made.

소아 안면 열상 봉합을 위한 케타민 진정시 국소 마취가 필요한가: 전향적 무작위대조, 이중맹검 연구 (Is Local Anesthesia Necessary in Ketamine Sedation for Pediatric Facial Laceration Repair?: A Double-Blind, Randomized, Controlled Study)

  • Purpose: The aim of this study was to assess the clinical efficacy of combined treatment with local anesthesia and ketamine procedural sedation for pediatric facial laceration repair in the Emergency Department (ED). Methods: Patients aged 1 to 5 years receiving ketamine for facial laceration repair were prospectively enrolled in a double-blind, randomized, and controlled study at an ED. All patients were to receive intravenous ketamine (2 mg/kg). The local anesthesia group (LA group) received a local anesthetic along with ketamine, whereas the no local anesthesia group (NLA group) received only ketamine. The total time of sedation, the patients' movements and groans, adverse events, and the satisfaction ratings of physicians, nurses, and parents were recorded. Results: A total of 186 patients were randomized (NLA group: 90, LA group: 96). The total time of sedation (30.5 minutes for the NLA group, 32.6 minutes for the LA group; p=0.660), patients' groans (26 (28.9%) versus 23 (24.0%); 0.446) and movements (27 (30%) versus 35 (36.5%); p=0.350) was not affected by the addition of local anesthesia. Other adverse events were similar between the two groups. Also, the satisfaction ratings of physicians (median 4 for the NLA group versus 4 for the LA group (p=0.796)), nurses (2 versus 2.5 (p=0.400)), and parents (4 versus 4 (p=0.199)) were equivalent between the two groups. Conclusion: In this study, we found that local anesthesia was not required along with ketamine sedation for pediatric facial laceration repair.

비탈면 긴급 복구를 위한 하이볼륨 플라이애시 및 현무암 섬유 보강 보수재료의 기초 품질 특성에 대한 실험적 연구 (Experimental Study on the Use of High-Volume Fly Ash and Basalt Fiber as Emergency Repair Materials for Slope Stabilization: An Analysis of Basic Quality Characteristics)

  • 본 논문에서는 홍수와 같은 재해시 비탈면 긴급 복구를 위한 보수재료를 제조하기 위한 일환의 연구로써, 플라이애시 혼합율에 따른 모르타르, 현무암 섬유 보강에 따른 모르타르를 각각 제조하여 보수재료의 기초 품질 특성에 미치는 특성을 평가하였다. 긴급 복구를 위한 보수재료는 각각의 특성으로부터 최적의 플라이애시 사용량을 선정하였으며, 현무암 섬유 혼합에 따른 기초 품질 향상을 위하여 화학혼화제 및 증점제의 최적 사용량을 도출하였다. 특히 하이볼륨 플라이애시는 알려진 장점으로 인하여 고성능 감수제 사용량 감소 및 작업성 향상이 가능함을 확인하였으며, 증점제 효과와 함께 섬유 뭉침 현상을 해소할 수 있는 것으로 나타났다. 본 실험결과를 통하여 얻어진 보수재료는 초기재령 강도를 중점으로 평가하여 긴급 복구를 위한 보수재료로써의 활용 가능성을 제시하고자 하였다.

Experience with the emergency vascular repair of upper limb arterial transection with concurrent acute compartment syndrome: two case reports

  • Upper extremity vascular injuries occurring with acute compartment syndrome are very challenging to manage in an emergency context in resource-poor settings. The need to always recognize the likelihood of coexisting compartment syndrome guides surgeons to perform concomitant fasciotomy to ensure a better outcome. We managed three vascular injuries in the upper extremities in two patients with concomitant imminent compartment syndrome observed intraoperatively. The first injury was complete brachial artery disruption following blunt trauma, while the second and third injuries were radial and ulnar artery transection caused by sharp glass cuts. Both patients were treated with vascular repair and fasciotomy. Secondary wound coverage was applied with split-thickness skin grafting, and the outcomes were satisfactory. Concomitant fasciotomy potentially improves the outcomes of vascular repair in emergency vascular surgery and should be considered for all injuries with the potential for acute compartment syndrome.

전위를 동반한 갑상연골 골절의 자연 치유 치험 1례 (Spontaneously Healed Thyroid Cartilage Fracture with Displacement: Report of a Case)

  • A thyroid cartilage fracture is a rare entity and can be overlooked easily. Such cases are difficult to diagnose, and assessment and treatment guidelines are difficult to determine. CT of the neck region may be useful when acute airway intervention is not required or when more information regarding the neck's anatomy is required for management decisions. We describe a case of a thyroid cartilage fracture with displacement. In the emergency department (ED), neck CT and fiberoptic nasopharyngoscopy were used to assess the status of the patient's (a male) vocal chords immediately. He remained unable to phonate continuously. After an immediate assessment, we decided to use steroid and conservative therapy. The patient had a good recovery and was without symptoms one month after injury. There is no question that early surgical repair of neck injuries affords the best results for airway and voice patency in most cases however, we suspect that surgical repair is not needed in all cases. Early recognition and an accurate therapy plan for a thyroid fracture with displacement are essential. Therefore, the emergency physician's immediate and careful decision based on endoscopy and neck CT is important for the patient's long-term recovery.

Aortoesophageal Fistula after Thoracic Endovascular Aortic Repair for Blunt Thoracic Aortic Injury

  • Although thoracic endovascular aortic repair (TEVAR) has grown to become the standard of care to treat blunt thoracic aortic injury (BTAI), the long-term effects of TEVAR are still unclear. We here present a 72-year-old man with BTAI due to a traffic accident. He successfully underwent TEVAR and was transferred to another rehabilitation hospital 2 months after the accident. However, 1 month later, he underwent gastroscopy with fever and hematemesis and was diagnosed with aorto-esophageal fistula (AEF). After being re-transferred to Niigata University Medical and Dental Hospital, we tried to convince him to undergo surgical treatment, but he strongly refused. He received palliative care and died due to rupture of the aortic pseudoaneurysm 3 days after the hospital transfer. Fatal complications like AEF may occur after TEVAR, so clinicians need to carefully follow patients who underwent TEVAR.

Polymorphisms in DNA Repair Genes and Risk of Glioma and Meningioma

  • Polymorphisms in DNA repair genes have been shown to influence DNA repair processes and to modify cancer susceptibility. Here we conducted a case-control study to assess the role of potential SNPs of DNA repair genes on the risk of glioma and meningioma. We included 297 cases and 458 cancer-free controls. Genotyping of XRCC1 Gln399Arg, XRCC1 Arg194Trp, XRCC2 Arg188His, XRCC3 Thr241Met, XRCC4 Ala247Ser, ERCC1 Asn118Asp, ERCC2 Lys751Gln and ERCC5 Asp1558His were performed in a 384-well plate format on the Sequenom MassARRAY platform. XRCC1 Arg194Trp (rs1799782) and ERCC2 Asp312Asn rs1799793 did not follow the HWE in control group, and genotype distributions of XRCC1 Gln399Arg rs25487, XRCC2 Arg188His rs3218536 and ERCC2 Asp312Asn rs1799793 were significantly different between cases and controls (P<0.05). We found XRCC1 399G/G, XRCC1 194 T/T and XRCC3 241T/T were associated with a higher risk when compared with the wild-type genotype. For ERCC5 Asp1558His, we found G/G genotype was associated with elevated susceptibility. In conclusion, our study has shown that XRCC1 Gln399Arg, XRCC1 Arg194Trp, XRCC3 Thr241Met and ERCC5 Asp1558His are associated with risk of gliomas and meningiomas. This finding could be useful in identifying the susceptibility genes for these cancers.

The Usefulness of Leukosan SkinLink for Simple Facial Laceration Repair in the Emergency Department

  • Background Repair of facial laceration in the emergency department can pose a number of difficulties. Children can be uncooperative, but adults can also be if they have sustained head trauma or are intoxicated. Leukosan SkinLink consists of topical adhesive and adhesive tape that can be applied easily to long or tense wounds. In this study, the authors compared conventional suturing with Leukosan SkinLink for facial laceration patients in the emergency department. Methods The prospective study was carried out from March 2013 to September 2013 with linear facial laceration patients visiting the emergency department. Exclusion criteria were open fractures, joint injuries, skin defects, hairy skin, and mucosa. The author used Leukosan SkinLink for skin closure in the experimental group and used conventional suturing in the control group. The scar evaluation using the Patient and Observer Scar Assessment Scale (POSAS) along with satisfaction scores, procedure times, and complications were compared. Results A total of 77 patients (30 in the control group and 47 in the experimental group) participated and underwent follow-up for 6 months postoperatively. The scar assessment using the POSAS and the satisfaction score in both groups were similar. The average procedure time in the experimental group was shorter. In the control group, there were four cases of wound dehiscence, two of infection, and one of skin necrosis, whereas four cases of wound dehiscence and one allergic reaction occurred in the experimental group. Conclusions With a simple application technique, Leukosan SkinLink is a new effective method for facial laceration repair especially useful for children and uncooperative adults.

Analysis of Patients with Facial Lacerations Repaired in the Emergency Room of a Provincial Hospital

  • Background Facial laceration is the most common injury encountered in the emergency room in the plastic surgery field, and optimal treatment is important. However, few authors have investigated this injury in all age groups or performed follow-up visit after repair. In the present study, the medical records of patients with lacerations in the facial area and underwent primary repair in an emergency room over a 2-year period were reviewed and analyzed. Methods Medical records of 3,234 patients with lacerations in facial area and underwent primary repair in an emergency room between March 2011 and February 2013 were reviewed and identified. Results All the 3,234 patients were evaluated, whose ratio of men to women was 2.65 to 1. The forehead was the most common region affected and a slip down was the most common mechanism of injury. In terms of monthly distribution, May had the highest percentage. 1,566 patients received follow-up managements, and 58 patients experienced complications. The average days of follow-up were 9.8. Conclusions Proportion of male adolescents was significantly higher than in the other groups. Facial lacerations exhibit a 'T-shaped' facial distribution centered about the forehead. Careful management is necessary if a laceration involves or is located in the oral cavity. We were unable to long term follow-up most patients. Thus, it is necessary to encourage patients and give them proper education for follow-up in enough period.
