심정지 초기 5분간 일반간호사의 심폐소생술 수행 분석: 현장 시뮬레이션을 이용하여 (Nurses' Cardiopulmonary Resuscitation Performance during the First 5 minutes in In-Situ Simulated Cardiac Arrest)

  • Purpose: The purpose of this study was to analyze the cardiopulmonary resuscitation skills and teamwork of nurses in simulated cardiac arrests in the hospital. Methods: A descriptive study was conducted with 35 teams of 3 to 4 registered nurses each in a university hospital located in Seoul. A mannequin simulator was used to enact simulated cardiac arrest. Assessment included critical actions, time elapsed to initiation of critical actions, quality of cardiac compression, and teamwork which comprised leadership behavior and communication among team members. Results: Among the 35 teams, 54% recognized apnea, 43% determined pulselessness. Eighty percent of the teams compressed at an average elapsed time of $108{\pm}75$ seconds with 35%, 36%, and 67% mean rates of correct compression depth, rate, and placement, respectively. Thirty-seven percent of the teams defibrillated at $224{\pm}67$ seconds. Leadership behavior and communication among team members were absent in 63% and 69% of the teams, respectively. Conclusion: The skills of the nurses in this study cannot be considered adequate in terms of appropriate and timely actions required for resuscitation. Future resuscitation education should focus on improving the quality of cardiopulmonary resuscitation including team performance targeting the first responders of cardiac arrest.

3D Terrain Model Application for Explosion Assessment

  • An increase in oil and gas plants caused by development of process industry have brought into the increase in use of flammable and toxic materials in the complex process under high temperature and pressure. There is always possibility of fire and explosion of dangerous chemicals, which exist as raw materials, intermediates, and finished goods whether used or stored in the industrial plants. Since there is the need of efforts on disaster damage reduction or mitigation process, we have been conducting a research to relate explosion model on the background of real 3D terrain model. By predicting the extent of damage caused by recent disasters, we will be able to improve efficiency of recovery and, sure, to take preventive measure and emergency counterplan in response to unprepared disaster. For disaster damage prediction, it is general to conduct quantitative risk assessment, using engineering model for environmentaldescription of the target area. There are different engineering models, according to type of disaster, to be used for industry disaster such as UVCE (Unconfined Vapor Cloud Explosion), BLEVE (Boiling Liquid Evaporation Vapor Explosion), Fireball and so on, among them.we estimate explosion damage through UVCE model which is used in the event of explosion of high frequency and severe damage. When flammable gas in a tank is released to the air, firing it brings about explosion, then we can assess the effect of explosion. As 3D terrain information data is utilized to predict and estimate the extent of damage for each human and material. 3D terrain data with synthetic environment (SEDRIS) gives us more accurate damage prediction for industrial disaster and this research will show appropriate prediction results.

역률개선 콘덴서를 이용한 유도전동기 기동특성 분석 (Induction Motor Starting Characterization with Power Factor Correction Capacitors)

  • 본 논문은 대용량 유도전동기의 기동특성을 분석하였다. 유도전동기 토크는 전류와 자속의 곱으로서 이 자속을 발생시키기 위해서는 여자전류에 해당되는 무효전력이 필요하다. 유도전동기에 필요한 무효전력을 공급하는 방법으로 역률 보상장치를 사용하여 전원측을 대신하여 공급하는 방법을 많이 사용하는데, 역률 보상장치를 사용할 경우 피상전력을 줄일 수 있어 역률을 향상시킬 수 있다. 역률을 개선하기 위한 보상장치를 설치하는 방법으로 부하마다 설치하는 경우와 전체를 일괄 보상하는 방법으로 고압 대용량의 부하로서 일정한 운전패턴을 가지고 있는 경우에는 주로 개별보상이 주로 사용된다. 일반적으로 대용량 유도전동기는 기동 보호장치를 이용하지만 이 논문은 전전압 기동시의 특성을 분석하는 가상의 상황을 가정하여 기동특성에 대한 이론과 실제의 운전특성을 측정 분석하고 역률개선 콘덴서를 이용하여 기동특성을 개선해 보았다.

  • Purpose: The aim of this descriptive study was to explore the relationship between non-technical skills (NTSs) and cardiopulmonary resuscitation (CPR) performance of nurses' teams in simulated cardiac arrest in the hospital. Methods: The sample was 28 teams of nurses in one university hospital located in Seoul. A high fidelity simulator was used to enact simulated cardiac arrest. The nurse teams were scored by raters using both the CPR performance checklist and the NTSs checklist. Specifically the CPR performance checklist included critical actions; time elapsed to initiation of critical actions, and quality of cardiac compression. The NTSs checklist was comprised of leadership, communication, mutual performance monitoring, maintenance of guideline, and task management. Data were collected directly from manikin and video recordings. Results: There was a significant difference between the medians of the NTSs and CPR performance (Mann Whitney U=43.5, p=.014). In five subcategories, communication (p=.026), mutual performance monitoring (p=.005), and maintenance of guideline (p=.003) differed significantly with CPR performance in medians. Leadership (p=.053) and task management (p=.080) were not significantly different with CPR performance. Conclusion: The findings indicate that NTSs of teams in addition to technical skills of individual rescuers affect the outcome of CPR. NTSs development and assessment should be considered an integral part of resuscitation training.

Multi-Agent Systems: Effective Approach for Cancer Care Information Management

  • Physicians, in order to study the causes of cancer, detect cancer earlier, prevent or determine the effectiveness of treatment, and specify the reasons for the treatment ineffectiveness, need to access accurate, comprehensive, and timely cancer data. The cancer care environment has become more complex because of the need for coordination and communication among health care professionals with different skills in a variety of roles and the existence of large amounts of data with various formats. The goals of health care systems in such a complex environment are correct health data management, providing appropriate information needs of users to enhance the integrity and quality of health care, timely access to accurate information and reducing medical errors. These roles in new systems with use of agents efficiently perform well. Because of the potential capability of agent systems to solve complex and dynamic health problems, health care system, in order to gain full advantage of E- health, steps must be taken to make use of this technology. Multi-agent systems have effective roles in health service quality improvement especially in telemedicine, emergency situations and management of chronic diseases such as cancer. In the design and implementation of agent based systems, planning items such as information confidentiality and privacy, architecture, communication standards, ethical and legal aspects, identification opportunities and barriers should be considered. It should be noted that usage of agent systems only with a technical view is associated with many problems such as lack of user acceptance. The aim of this commentary is to survey applications, opportunities and barriers of this new artificial intelligence tool for cancer care information as an approach to improve cancer care management.

드라마 <태양의 후예>를 통해 본 의료인의 역할 (The Role of the Medical Person in the Drama )

  • 본 연구의 목적은 드라마 '태양의 후예' 중 의료현장에서 이루어지는 의료인의 역할을 분석하는 것이다. 본 연구설계는 드라마 '태양의 후예' 중 의료현장에서 이루어지는 의료인의 역할을 탐구하기 위한 질적연구이다. 본 연구를 수행하기 위해 '태양의 후예' 중 의료현장에서 이루어지는 의사소통 장면을 본 연구의 분석자료로 선택하여 의료현장에서 의료진의 의사소통 및 역할 등에 대해 분석하였다. 연구분석을 위해 본 연구팀에서는 태양의 후예를 보며 의료현장 장면을 필사하였으며, 필사가 끝난 후 필사내용을 다시 한 번 확인하여 빠진 내용이 없는지 검토한 후 최종 분석자료를 내용분석 방법을 통해 정리하였다. 본 연구 결과 '태양의 후예' 에서 의료진의 역할은 '응급상황에서의 의사결정', '의료팀간의 협력', '환자에 대한 지지', '의료인으로써의 책임감/윤리적 딜레마'로 나타났다. 본 연구는 하나의 드라마를 분석하여 의료진의 역할을 분석하였다는 것에 있어 한계가 있으나, 드라마를 통해 제 3자의 입장에서 의료진의 역할을 탐색하였다는 것에 의의가 있다.

환자안전 위험요인에 대한 병원간호사의 인식수준과 안전간호활동 수행정도 (Perception of Patient Safety Risk Factors and Performance Level of Safety Care Activities among Hospital Nurses)

  • Purpose: The purpose of this study was to identify the level of perception of safety risk factors and the degree of performance of safety nursing activities in order to develop an education program to improve the safety of patients. Methods: The subjects were 217 nurses from 3 university hospitals in Incheon. Data were collected with structured questionnaires and analyzed using descriptive analysis, t-test, and ANOVA using SPSS 22.0. Results: The level of perception of patient's safety risk factors and safety care activities was 2.93 and 4.68, respectively. Perception of patient safety risk factors which belonging to the risk type of therapeutic devices, equipment, service and infrastructure all scored below average; also in addition, behavior, performance, and violence risk type and work system, information and communication risk type showed relatively low perception levels. Safety nursing activities showed a low level of performance in accurate communication among medical teams, management of fire and disaster, security management, use of restraints, identification of patients, and correct performance of operations and procedures. Conclusion: Based on these results, it is necessary to improve the safety of patients by taking proper management measures along with education.

IoT 기반 홈 보안 시스템 (Home Security System Based on IoT)

  • 본 논문은 휴대폰에서 사용할 수 있는 IoT 기반 홈 보안 시스템을 개발한다. 제안된 시스템은 데이터를 수집하는 센서, 카메라, 게이트웨이와 Xively 플랫폼으로 구성된다. 라즈베리파이는 3 종류의 센서로부터 데이터를 수집하고, Xively로 데이터를 전송하고, 카메라의 영상화면을 인터넷을 통하여 휴대폰의 클라이언트로 전송한다. 서버는 Xively, 라즈베파이에 존재하는 소켓 서버와 Google에 있는 이메일 서버로 구성된다. 제안된 시스템은 움직임, 화재 위험, 가스 누출이 발생하면 이메일, 텍스트 문자와 영상화면을 전송하고, 라즈베리파이를 통하여 가스 밸브를 제어할 수 있다. 실험결과로부터 가스 누출 등으로 위험이 발생하면 사용자는 이메일, 문자 메서지를 전송받고, WIFI 또는 LTE를 통하여 스마트폰으로 집내의 영상화면을 볼 수 있다는 것을 확인하였다.

화재안전트리 이론에 따른 초고층건축물의 소방력 공급방안: 수동화재진압 전략을 중심으로 (The Method of Force of Fire in High-Rise Building by Guide to the Fire Safety Concepts Tree: Focusing on Manually Fire Suppression Strategy)

  • 이 연구는 초고층건축물의 고층부에서 소방력의 공급의 효율성을 분석하고 이를 효율적으로 공급하는 방안을 제안하였다. 연구결과는 다음과 같다. 첫째, 화재감지는 외부에서 경보·진동·음성 등 화재경보의 다양화, 화재발생지점의 명확화 및 건축물 외부에도 화재층 표시를 통해 소방력 공급시간을 단축할 필요가 있다. 둘째, 신호통신은 무선통신보조설비의 설치대상을 강화하고, 중계기 설치를 보완하며, 다중통신망을 구성할 것을 제안하였다. 셋째, 행동결정은 인접건축물에서 도하의 방법으로 진압대원 및 소방장비를 공급하고, 일정 층마다 파괴가 용이한 유리창을 설치하며 식별표시도 필요하다. 넷째, 현장대응은 활강과 수직구조기법을 적용하여 소방장비의 공급 및 진압대원의 진입을 돕고 드론을 이용하여 소방장비를 공급한다. 마지막으로 충분한 소화액 제공은 헬리콥터와 인접한 건축물을 이용하여 소방용수를 공급한다.

DEVELOPMENT OF NEW TAXONOMY OF INAPPROPRIATE COMMUNICATION AND ITS APPLICATION TO OPERATING TEAMS IN NUCLEAR POWER PLANTS

  • Inappropriate communications can cause a lack of necessary information exchange between operators and lead to serious consequences in large process systems such as nuclear power plants (NPPs). In this regard, various kinds of taxonomies of inappropriate communications have been developed to prevent inappropriate communications. However, there seems to be difficult to identify inappropriate communications from verbal protocol data between operators. Because the existing taxonomies were developed for use in report analysis, there is a problem of 'uncertainty'. In consequence, this paper proposes a new taxonomy of inappropriate communications and provides some insights to prevent inappropriate communications. In order to develop the taxonomy, existing taxonomies for four industries from 1980 to 2010 were collected and a new taxonomy is developed based on the simplified one-way communication model. In addition, the ratio of inappropriate communications from 8 samples of audio-visual format verbal protocol data recorded during emergency training sessions by operating teams is compared with performance scores calculated based on the task analysis. As a result, inappropriate communications can be easily identified from the verbal protocol data using the suggested taxonomy, and teams with a higher ratio of inappropriate communications tend to have a lower performance score.
