  • Medical institutions face the challenge of securing competitiveness among medical institutions due to the rapid spread of ICT convergence, and managing data that is growing at an enormous rate due to the emergence of big data and the emergence of the Internet of Things. The big data paradigm of the medical community is not just about large data or tools and processes for processing and analyzing it, but also means a computerized shift in the way people live, think and study. As the medical data is recently released, the demand for the use of medical data is increasing. Therefore, the research on disease detection system based on public data using open source that can help rational and efficient decision making was conducted. As a result of the experiment, unlike a simple disease inquiry or a symptom inquiry about a single disease provided by a public institution, related diseases are searched by a symptom or a cause.

A Study on the Operation Regional Emergency Medical Center and the Using Behavior by Visiting Patients (한 지역 권역 응급의료센터 내원환자의 이용 행태 분석)

  • Currently, we have seen sudden increase of demand for emergency medical services by reason for high-speed economic development increase of traffics, etc. in this society. Consequently the government enforced & operated emergency medical system in 1991 as a link of more positive countermeasure against it, but many problem; still remain in reality. In particular, the regional emergency medical center designated by the Ministry of Health and Welfare falls short of our expectations for its essential role by the reason of such as insufficiency in professional manpower and institutions concerned in small-scale hospital, matters with transportation system, preference of large hospital, etc. Therefore, this study was conducted grasp for the actual conditions of emergency medical system based upon literature research & the preceding studies and interview research the motive of coming hospital, satisfaction & understand to the subject of 150 persons of patients and their guardians who used regional emergency center of Pusan National University Hospital, thereby examining & analysing the cause of emergency room overcrowding by non-emergency cases, one of the problem; enumerated from preceding studies. The main result of this study is as follows. First the actual condition of non-emergency patients coming hospital for examining overcrowding of emergency medical center showed that, of the patients who used the emergency medical center, non-emergency patients accounted for 49.3%, which acted as the main cause of delaying medical care for emergency cases, cases of which medical person or first-aid man decided to come hospital accounted for 36.1 %, thereby suggesting essential need for re-education & wide public information to even the professional manpower besides patients & guardians for their using emergency medical system. Second, as the result of researching patient acknowledgement with reference to their using emergency medical center, the rate of their giving right answer is no more than 60%, which means that non-emergency cases' using as such is due to the shortage of their knowledge of the said emergency medical center, which suggests us that wider P.R for emergency medical system to common people who may be one of the patients of it at any time is still in need. Third, the result of researching for finding out a future remedy of emergency medical system showed that the users who know well of the way of using emergency medical center had relatively high satisfaction of it, ones who have lesser knowledge of it lower satisfaction and users who feel in need of emergency specialized manpower feel the necessity of public information of emergency medical information center(1339) at the same time. The finding of examining the subject of study, in conclusion, showed that the degree of the patients & their guardians' understanding of emergency medical system is lower and the medical persons concerned also had no distinct difference in their understanding of it from the common people's, which suggests us for extensive enforcement of systematic education and public information in aspect of the government via various media for the purpose of effective operation of emergency medical center.

A Brief History of Korea's Study of 『Internal Classic』 (한국(韓國) 『황제내경(黃帝內經)』 연구(硏究) 약사(略史))

  • The practice of studying "黃帝內經" is one that is deeply rooted throughout Korean history. The main textbook used in educational institutions as early as the Era of the Three States, it was also actively examined. A doctor examination system was introduced in the Koryo dynasty, which reinforced "Internal Classic"'s status as an important subject of study. Following the nation-wide acceptance of Confucism, scholar-physicians came to be and this book was studied as an important scripture. By Chosun dynasty, the entire medical systems becomes centered around "Internal Classic. Internal Classic is not only included in medical examination manual written in the period of Sejong's rein but was cited in many medical texts published during this time span including Euibangyoochi, Hyngyakjipseongbang, and thus can bee seen as the conceptual basis of medicine. In late Chosun dynasty with the emergence of the Korean Traditional Medicine's scripture, The Treasured Mirror of Eastern Medicine, "Internal Classic" is spotlighted as a scripture of importance among medical families. Lee Kyu-Jun, a doctor of the Japanese occupation era, examined "Internal Classic" based on a theory called fuyang theory and produced a creative medical theory.

Combining Multi-Criteria Analysis with CBR for Medical Decision Support

  • One of the most visible developments in Decision Support Systems (DSS) was the emergence of rule-based expert systems. Hence, despite their success in many sectors, developers of Medical Rule-Based Systems have met several critical problems. Firstly, the rules are related to a clearly stated subject. Secondly, a rule-based system can only learn by updating of its rule-base, since it requires explicit knowledge of the used domain. Solutions to these problems have been sought through improved techniques and tools, improved development paradigms, knowledge modeling languages and ontology, as well as advanced reasoning techniques such as case-based reasoning (CBR) which is well suited to provide decision support in the healthcare setting. However, using CBR reveals some drawbacks, mainly in its interrelated tasks: the retrieval and the adaptation. For the retrieval task, a major drawback raises when several similar cases are found and consequently several solutions. Hence, a choice for the best solution must be done. To overcome these limitations, numerous useful works related to the retrieval task were conducted with simple and convenient procedures or by combining CBR with other techniques. Through this paper, we provide a combining approach using the multi-criteria analysis (MCA) to help, the traditional retrieval task of CBR, in choosing the best solution. Afterwards, we integrate this approach in a decision model to support medical decision. We present, also, some preliminary results and suggestions to extend our approach.

  • Transoral microsurgery has come a long way as a go-to surgical intervention technique for head and neck cancers. This minimally invasive procedure had gained acceptance through comparative clinical studies against radical neck surgical procedures, radiotherapy, and chemotherapy. Laser technology has vastly improved the oncological outcomes of this procedure and brought about an appreciation of transoral laser surgery (TLM) as a mainstay for re-sectioning malignant tumors along the throat. As an established procedure, TLM has undergone several upgrades regarding the different energy devices used for cutting, ablation, and hemostasis. Continued advances in automation have eventually led to surgical robotics, resulting in the emergence of transoral robotic surgery (TORS) as a viable advanced alternative for TLM. Similarly, expansions and enhancements (image-based guidance, fluorescence spectroscopy, and advanced robotic system) have also been investigated as potential upgrades for TORS. This paper reviews a selection of publications on the significant technological advancements to TLM and TORS over the past five years.

  • The emergence of diverse medical Internet of Things (IoT) devices has facilitated the collection and analysis of medical contexts to assess health conditions. However, the complexity of IoT-based systems for health assessment is quiet high and it requires high development cost, since the designs of the systems highly depend on the heath aspects to be evaluated. In this paper, we propose a design of the platform that provides generic functionalities to various health evaluation applications. We first define a metric for computing a health index, which is a generic health assessment model. And, based on the proposed generic health index, we propose a design of the platform to evaluate diverse aspects of the health including its hardware architecture, software architecture and database design. We describe the result of developing Rainbow Toilet System based on the proposed platform, and assess the practical applicability.

  • Since the beginning of the 21st century, the emergence of innovative technologies made further advances in minimal access surgery possible. Robotic surgery and telepresence surgery effectively addressed the limitations of laparoscopic procedures, thus revolutionizing minimal access surgery. Surgical robots provide surgeons with to technologically advanced vision and hand skills. As a result, such systems are expected to revolutionize the field of surgery. In that time, much progress has been made in integrating robotic technologies with surgical instrumentation. However, robotic surgery will not only require special training, but it will also change the existing surgical training pattern and reshape the learning curve by offering new solutions, such as robotic surgical simulators and robotic telementoring. This article provides an introduction to medical robotic technologies, develops a possible classification, reviews the evolution of a surgical robot, and discusses future prospects for innovation. In the future, surgical robots should be smaller, less expensive, easier to operate, and should seamlessly integrate emerging technologies from a number of different fields. We believe that, in the near future as robotic technology continues to develop, almost all kinds of endoscopic surgery will be performed by this technology.

Results of Extracorporeal Cardiopulmonary Resuscitation in Children

  • Background: Survival of children experiencing cardiac arrest refractory to conventional cardiopulmonary resuscitation (CPR) is very poor. We sought to examine current era outcomes of extracorporeal CPR (ECPR) support for refractory arrest. Methods: Patients who were <18 years and underwent ECPR between November 2013 and January 2016 were including in this study. We retrospectively investigated patient medical records. Results: Twelve children, median age 6.6 months (range, 1 day to 11.7 years), required ECPR. patients' diseases spanned several categories: congenital heart disease (n=5), myocarditis (n=2), respiratory failure (n=2), septic shock (n=1), trauma (n=1), and post-cardiotomy arrest (n=1). Cannulation sites included the neck (n=8), chest (n=3), and neck to chest conversion (n=1). Median duration of extracorporeal membrane oxygenation was five days (range, 0 to 14 days). Extracorporeal membrane oxygenation was successfully discontinued in 10 (83.3%) patients. Nine patients (75%) survived more than seven days after support discontinuation and four patients (33.3%) survived and were discharged. Causes of death included ischemic brain injury (n=4), sepsis (n=3), and gastrointestinal bleeding (n=1). Conclusion: ECPR plays a valuable role in children experiencing refractory cardiac arrest. The weaning rate is acceptable; however, survival is related to other organ dysfunction and the severity of ischemic brain injury. ECPR prior to the emergence of end-organ injury and prevention of neurologic injury might enhance survival.

Design and Development of Digital Contents Authoring System for Cyber University Using Programing Skills (프로그래밍 기법을 활용한 가상대학 컨텐츠 제작 시스템 설계 및 개발)

  • The authoring systems for digital contents using multimedia technologies ate requested in many fields such as education, medical science, entertainment market e-commerce and etc. Especially, the emergence of cyber universities and the rapid expansion of on-line education market require the effective contents authoring systems, which have various functions to generate the qualified contents. Therefore, many systems arc developed and currently used. However, since the developed systems considered only the developer's convenience, the generated digital contents by using these systems are failed to draw tile users'(or learners') active interaction with contents. That is, since the users just watch the contents like watching a drama or a film, it causes many problems in delivering the contents effectively or in evaluating the users. This paper presents, develops, and implements the new contents authoring system by using programing languages and/or software tools. The presented, developed, and implemented system mimics the face-to-face education in off-line system, induces the users' active interaction with contents, and continuous evaluation to the users.
