Environmental Affordance of a Well Recognized Senior Center of Japan (일본의 노인종합복지관 성공사례의 환경지원성 분석 연구)

  • According to the 2005 Korean census, the 65 and over population now exceeds 9.5% of the total population and is growing rapidly. Meeting elderly environment and care needs of this rapidly growing segment of the population becomes a major challenge for public policies and planners. Since great deal amount of elderly will reside in their houses, aging in place concept becomes important. For the success of aging in place, the quality of individual house unit, community support systems, and quality of senior center of the community are crucial. Since elderly environments and facilities serve not only medical and/or care programs but also social activity program in aging society, senior centers need to promote social activities and other care programs. The purpose of this study is to analyze characteristics of environmental affordance of a well received senior center in Tokyo, Japan. In order to analyze the characteristics, Murtha & Lee user benefit criteria and Lawton's environmental affordance approaches were used. As results, design characteristics and interior elements which provide environmental affordance were enumerated by type of space. Based on needs and user benefit criteria, those features were analyzed. This study shows design characteristics, elements, and attributes which are well received and utilized by elderly users.

Comparison of the Purchase Criteria and Fashion Information Sources for the Middle-aged and Elderly Women's Fashion Markets Segmented Based on Benefits Sought (의복추구혜택에 따른 중.노년기 여성 세분시장의 구매기준 및 패션정보원 비교분석)

  • The purpose of this study was 1) to segment the middle-aged and elderly women's fashion market based on the clothing benefits sought by the buyer and 2) to compare the purchase criteria and fashion information sources among the segmented markets. The data were collected using a self-administered questionnaire in Seoul and its surrounding suburban areas. Factor analysis, ANOVA, Duncan test, and Dunnett's T3 tests were used to conduct the data analysis from 285 out of 300 questionnaires. The middle-aged and elderly women's fashion market was segmented into four groups; value-oriented, social status/trend-oriented, uniqueness-oriented, and protection/ convenience-oriented. All four groups were significantly different in terms of purchase criteria and fashion information sources. The social status/trend-oriented group used external purchase criteria, such as country of origin or brand and obtained fashion information from personal experience, advice from the salesperson, and celebrities. The protection/convenience-oriented group sourced fashion information from newspapers, the internet, and the radio. The uniqueness-oriented group put less importance on practical use/convenience criteria. Marketing strategies for these segmented markets were discussed.

Dorsal Short-Segment Fixation for Unstable Thoracolumbar Junction Fractures

  • Objective : This study is to evaluate the efficacy of dorsal short-segment fixation in unstable thoracolumbar junction fractures. Methods : The cases of 20 patients who underwent dorsal short-segment fixation were reviewed retrospectively. Clinical outcomes were analysed using Sonntag's pain level, work status, and neurological scale according to the modified Frankel classification. Radiological outcomes were analysed using Mumford's anterior body compression[%], canal compromise ratio, and Cobb's kyphotic angle. Results : At the latest clinical follow-up [average=14.6 months]. there were 19 [95.0%] in group I and 1 patient [5.0%] in II in pain level. The postoperative work status were 17 [85.0%] in group I, 2 patients [10.0%] in II, and 1 patient [5.0%] in V. Surgery brought to improve the neurologic status. In success group [19 cases, 95%], the average canal compromise ratio was reduced from 0.57 [${\pm}0.07$] to 0.05 [${\pm}0.08$] [P<005], the average anterior body compression [%] was reduced from 41% [${\pm}17$] to 18% [${\pm}14$] [P<0.05], and the average preoperative kyphotic angle was $20.0^{\circ}$ [${\pm}9.0$], and corrected to $5.7^{\circ}$ [${\pm}7.1$] postoperatively, and progressed to $7.8^{\circ}$ [${\pm}6.2$] at the latest follow-up. There was a case of implantation failure in an elderly osteoporotic patient. Conclusion : Although there are limitations in the patient number and follow-up period, the present study favors dorsal short-segment fixation for selective cases in unstable thoracolumbar junction fractures.

Effects of a Water Exercise on the Lower Extremities Coordination during Obstacle Gait in the Female Elderly - Focusing on Training and Detraining Effects - (수중운동이 여성노인 장애물보행 시 하지 협응에 미치는 영향 - 훈련 및 훈련잔여효과 중심으로 -)

  • The purpose of this study was to investigate the training and detraining effects of a 8-week water exercise on lower extremities coordination during obstacle gait in the female elderly. Eight elderly participants (age: $76.58{\pm}4.97$ yrs, height: $148.88{\pm}7.19$ cm, body mass: $56.62{\pm}6.82$ kg, and leg length: $82.36{\pm}2.98$ cm), who stayed at the Seoul K welfare center, were recruited for this study. All participants had no history of orthopedic abnormality within the past 1 year and completed the aquatic exercise program which lasted for 8 weeks. To identify the training and detraining effect of 8 weeks of water exercise, a 3-D motion analysis with 7 infrared cameras and one force plate sampling frequency set at 100 Hz and 1,000 Hz, respectively, was performed. A two-way ANOVA was performed to find training and detraining effects among diferent obstacle heights. In this study significant level was set at .05. Significant training effects of LTS (lead foot thigh and shank) coordination in all obstacle height were found (p<.05). It is also found that the training effect of LTS remained 37%, 58%, and 25% in obstacle height of 30%, 40%, and 50%, respectively. Lead foot showed the greater detraining effect of coordination compared with trail foot, and SF (shank and foot) coordination revealed better detraining effects of coordination compare with TS (thigh and shank) in both feet. Based on the findings, a 8 week water exercise give an positive effects to the elderly in terms of segment cooperation which potentially helps reducing their accident falls. The magnitude of detraining may also help the elderly to find the retraining moment.

Effect of the Elderly Consumers' Education Level on Eating-Out Decision Making Process (노인소비자의 학력수준이 외식구매의사결정 과정에 미치는 영향에 관한 연구)

  • As Korea has approached the aging society, older Koreans have become an important force in restaurant sales today. To succeed with this silver market, it is important for restaurant managers to know who they are and what factors influence older Koreans' eating-out decision making process. The purpose of this study was to investigate the effect of the silver consumers' education level on eating-out decision making process. Data were collected from 178 older consumers above 55 years old and analyzed using the descriptive statistic analysis, MANOVA, and one-way ANOVA. The results showed that the elderly consumers' education level significantly influenced the decision making process in determining where to eat out. Significant differences were found in the Problem Recognition Step(Wilks' Lambda=0.817, F=2.991), Information Search Step(Wilks' Lambda=0828, F=2.218), Alternative Evaluation Step II(Wilks' Lambda=0.741, F=3.596), Purchase Decision Step(Wilks' Lambda=0.859, F=2.223), and the Post-Purchase Behavior(Wilks' Lambda=0.885, F=1.780). The higher education level was, the more directly involved in the eating out decision process. The elderly consumers with university education were likely to 'propose to eat out by themselves'(F=9.346), to obtain restaurant information from the 'printed materials'(F=7.452), to go to 'family restaurant'(F=9.057), 'Japanese restaurant'(F=8.7891) and 'fine dining restaurants'(F=3.936), and to directly express their emotion when they had complaints about restaurant service(F=3.206). In conclusion, older Koreans will become more healthy and wealthy which means the dining out activity will be an important part of their life to socialize with people. Therefore, food service operations should consider the elderly consumers' needs and expectation of restaurant services and actively position themselves for this new market segment.

Differences in Clinical Laboratory Data between the Elderly and the Young Adults (노인군과 청장년군 간의 정상 검사치의 비교 분석)

  • Due to the lowering of biological functions resulted from old age, the elderly is known to have many different clinical laboratory data compared with the young adults. But, in korea, such study is lacking. This research is to find the differences between the elderly and the young adults, and also to know the sexual differences, by comparing the outcomes of the clinical. laboratory data. Along with that, it is to help clinical usage of the data in the future. The age of the elderly was between 60 and 83(average age 63.8), and that of the young. controls was between 20 and 35. In both sexes, MCV, MCH, ESR, CRP, AST, ALT, ${\gamma}$GTP, ALP, BUN, total cholesterol were significantly higher in the elderly than in the controls. And lymphocyte count(%), total bilirubin, direct bilirubin, total protein, albumin, $T_3$ were significantly lower in the elderly than in the controls(P<0.05). Hemoglobin, Hct, platelet count, $T_4$ were significantly lower only in the male elderly, and eosinophil count(%), creatinine were significantly higher only in the female elderly(P<0.05). HDL cholesterol was significantly higher only in the male elderly(P<0.01). There were no significant difference between two groups regarding WBC count, segment neutrophil count(%), monocyte count(%), TSH. Many clinical laboratory data are different between the elderly and the young adults, and some clinical laboratory data also have sexual differences.

Detachable Coil Embolization for Saccular Posterior Inferior Cerebellar Artery Aneurysms

  • Objective : Surgical treatment of posterior inferior cerebellar artery (PICA) aneurysms is challenging due to limited surgical accessibility. Endovascular approach has a benefit of avoiding direct injury to the brainstem or lower cranial nerves. Therefore, it has recently been considered an alternative or primary modality for PICA aneurysms. We retrospectively assessed outcomes following detachable coil embolization of saccular PICA aneurysms. Methods : From February 1997 to December 2007, we performed endovascular procedures to treat 15 patients with 15 PICA aneurysms. Fourteen patients with 14 PICA aneurysms morphology of which was saccular were reviewed retrospectively. Twelve patients had ruptured aneurysms. The aneurysms arose from the PICA origin site (n=12), the PICA lateral medullary segment (n=1), or the PICA tonsilomedullary segment (n=1). Results : Complete aneurysm occlusion was achieved in 10 patients, residual neck in 3, and residual sac in one. Radiological follow-up was performed in 7 patients with mean duration of 34.7 months (range, 1-97 months) and showed stable or complete occlusion in 6 patients. There were no rebleeding or retreatment after endovascular treatment. Thromboembolism was the only procedure-related complication (n=4 ; 28.6%). Asymptomatic PICA infarction occurred in two patients and symptomatic PICA infarction in two elderly patients with poor clinical grade. Of these procedural PICA infarction cases, 1 symptomatic PICA infarction patient developed ventriculitis and septic shock leading to death. The clinical outcome was good in 10 patients (71.4%). Conclusions : In the present study, detachable coil embolization has shown as an efficient modality for PICA saccular aneurysms challenging indications of microsurgery. However, thromboembolic complications should be considered, especially in poor clinical elderly patients with ruptured aneurysms.

Physical Health Status and Depression of a Community-Dwelling Elderly Group (일부 지역사회 노인의 신체건강 상태와 우울)

  • The purpose of this study was to describe physical health and depression status, as well as to assessing factors that influence the physical health status. Method: The data was collected from July to August 2000. Study participants were 252 community-dwelling elderly who were recruited from 10 senior centers located in Seoul, Korea. Their physical health status was measured using the Physical Health Status Measurement Scale developed by Choi and Jung (1991), and depression was measured using BDI-II developed by Beck et al. (1996). Results: 1) The physical health status score was 4.00 $\pm$0.68 (range :1- 5). The sub-dimension that showed the highest score was personal hygiene ability at 4.62$\pm$0.95, and the lowest score was sexual function at 2.20$\pm$1.38. 2. The depression score was 17.99+9.79 (range : 0-63). Regarding the sub-dimensions, the depression scores were higher in the domain of interest with sexuality, general weakness, difficulty in concentration, and fatigue. 3. Deeper levels of depression were correlated with a declining physical health status. 4. The most influential factor on physical health was depression, and the explaining variance was 31.68%. Conclusion: It is concluded that elder subjects in senior centers had fairly good physical health and self-care ability. Also, they did not have significantly high levels of depression. Therefor, health promotion of elderly, it is recommended that elder individuals should be regarded as a respectful and useful segment of our society. Along with this basic concept, there should be a social milieu that does not snow prejudice. Moreover, health care professionals should give more attention to helping the elderly achieve a minimal level of ALD, and, particularly, to raise sexuality and help energize the lives of elder individuals.

Attitudes of male and female older adults concerning death (성별에 따른 죽음에 대한 태도 비교 연구 - 남.녀노인들의 임종과 죽음에 대한 불안도 측정을 중심으로 -)

  • The research is a comparative study of death attitudes between male and female elderly people. There is no doubt as to the inevitability of death. And yet, there is a vast conspiracy involved in the word of dead or death. The aged are considered to be nearer death than are people in other age groups. Kalish(1976) emphasized that for the aged two meanings of death have significance for evaluating their life ; first, that older people are known to have a limited life time and face death ; second that older people are known to have suffered many death-imposed losses that are often associated with the dying process. In considering these implications, the level of anxiety regarding death and dying is a crucial factor in determining mental health. In the study, 152 male elderly and 145 female elderly residing in Seoul, Korea was compared on the four dimensions of death anxiety and assigned personal variables. Therefore, the purpose the research was (1) to examine the characteristics of subjects on the independent variables(age, marital status, family relationship, social activities, religiousity etc.) ; (2) to examine the relationship between the independent variables and each dimension of death axniety ; (3) to determine the proportion of variance in the respective of death anxiety which is accounted for by the respective independent variables ; (4) to examine whether a significant difference between the respective independent variables and each dimension of death anxiety has ; (5) to determine the combination of variables which is the most successful in explaining the variance in death anxiety. Finding from this study support the following conclusions; 1. There was a significant differences between the male and female subjects in the level of death anxiety. In turn, the male older adults had lower death anxiety than did the female elderly. This implies that male tend to look forward to death rather than deny it. 2. As there was evidences from several studies, this research found that fear of death decreases as age increases. 3. The following two variables that correlate best with dying anxiety of others in both male and female older adults : 'marriage life', 'social interaction'. 4. The variables 'age' and 'children' for both female and male elderly accounted for the most variance in death anxiety of self. The findings of the study lend this investigator several suggestions, implications and recommendations for future research. There can be no death without life, and conversely, no life without death. Psychologists and health-related professionals may be learn as much about death as they can in order to develop more healthy attitudes and in order to be able to better aid and comfort dying people and their familities. Perhaps most importantly, professionals may be help those who are not faced with death at present to develop an understanding of it and healthy attitudes toward it. The programs of death education are needed for dedication to the evitability of death and the preparation of life for the older adults. More seminars, symposiums and research on death attitudes are needed. Finally, study for female older adults has been negelected topic in the areas of women's study and health education. Future study, for female elderly, have to deeply investigate where those problems come from and how to cope with in order to the female elderly segment can live the rest of their lives in satisfaction with well-being.

Kinematic Difference between the Lower Limb Joints and the Lower Extremities Given Elderly Women's Walking through the Lower-limb Resistance Exercises (하지 저항운동을 통한 여성고령자 보행 시 하지관절 및 분절의 운동학적 차이)

  • The purpose of this study is to offer basic data for the fall prevention by analyzing the kinematic difference between the lower limb joints and the lower extremities in elderly people's walking given the lower-limb resistance exercises. For this, three-dimensional image analysis was carried out by selecting 7 elderly women from over 70s to under 80 years old. To obtain the three-dimensional location coordinates in the lower limb joints and the lower extremities, it shot with 100Hz/s by using MCU(Qualisys, Sweden) camera. The shot image gained raw data on the location coordinates by using QTM(Qualisys, Sweden). As a result of calculating three-dimensional angle by using program of Matlab 6.5, the following conclusions were obtained. Flexion and extension in the thigh and the lower-leg extremities were indicated to be big in motion of flexion after exercising at E5. Foot segment indicated statistical difference while showing eversion at E4. Knee joints showed flexion at E4 after exercising. Ankle joints showed statistical difference while indicating motion in inversion at E3 and in eversion at E4(p<.05).
