Mini-Mental State Examination (MMSE) and naming tests have been used clinically as screening and diagnostic tools for mild cognitive impairment and dementia. Researches in other countries have reported that residential areas would affect one's cognitive abilities. In Korean, however, there was no systematic studies on geographical influences to cognitive ability among a sample of urban/rural residents. The aim of this study was to investigate the effect of urban/rural locality of residence on cognitive tests in normal elderly Korean. Residents aged 65 years and older (N=108) were administered the K-MMSE, confrontation/generative naming tests, and medical/socio-demographic interview. When comparing the total number of correct responses between urban residents and rural residents, the performances of rural group were significantly worse than those of the urban group in K-MMSE. Based on the results, our finding suggests that a factor of residential locality should be considered to examine the function and explain the cognitive decline in rural community-dwelling elders.
Purpose: To investigate the relationship between dementia and complete tooth loss on both sides or one side using large demographic data. Materials and methods: A retrospective cohort study was designed using the National Health Insurance Service-Elderly Cohort Database (NHIS-ECD) which was established for people over the age of 60. The experimental group was the complete edentulous cohort, which had a history of treatment for national health insurance covered complete denture on both sides or one side from July 1, 2012 to December 31, 2013. And the control group was the dentulous cohort, which had a history of conservative dental treatment for the same period. All subjects had no history of diagnosis or treatment of dementia during this time. These cohorts were matched 1:1 by age, gender, and place of residence according to the propensity score matching method. Then the incidence of dementia was compared between these cohorts. Results: Compared with those groups, the incidence of dementia was significantly higher in the experimental group (12.13%) than in the control group (9.74%) (P<.05). No clear association between other factors has been identified. Conclusion: The analysis of large-scale demographic data shows that the incidence of dementia is high in complete edentulous patients on both sides or one side.
Recently, the Ministry of Health and Welfare has emphasized the necessity to build community care, and the interest of health care- long-term care-social welfare system continues. In order to examine the future vision of long-term care for elderly people, which can be a core system in establishing community models for elderly people in the future, we will implement aging in place as a national policy project. It is meaningful to analyze in depth the case of Japan's policy implementation through the amendment process of long-term care insurance system. The purpose of this study is to examine the concept definition, operating system, and major promotion process of the Integrated Care System in Japan and to suggest policy implications for the future Korea long-term care insurance system. As a result of reviewing the operation status and revision process of the long-term care insurance system for establishing the Integrated care system in Japan, it is necessary to clarify the basic principles and construction of community care system, diversification of residence type and upgrading work, establishment of linkage network of health care-long term care-social welfare, expansion of support for family and self-mind.

Among studied facilities, nine were the welfare facilities for children, five were the welfare facilities for elderly, two were the welfare facilities for a mentally and physically disabled person and the remaining two were vagabond care facilities. Methods used were mainly questionaires. These questionaires were answered by manager and interviews were also done for a same person. Equipments were evaluated by investigators using the evaluation form. The results of the study were summarized as follows. Number of cooks and assistant cooks among employee in foodservice establishment were about 2-6 persons. One dietitian is stationed in 2 places among 18 places. Food purchasing and menu plannings were mainly practiced by manager, secretary and other personnels, in more than 50% of social welfare facilities. The type of the menu in most facilities was the set menu. The period of turn over for cycle menu was a week in 50% of facilities. Seventy seven point eight percentage of the welfare facilities were used the weekly or monthly cycle. For the food preparation, there was almost no place using standard recipes. Foodstuffs were purchased in local market. Moreover, the preference test of served foods were done for residence of each facilities. But it was not applicated effectively. Conditions of most equipments in the kitchen were defective specially in dishwashing and sterilization step.
The current global economy is changing from a proprietary commercial economy to a shared economy of value sharing. In modern society, single-person households have become common type of households. The reason for the increase in single-person households is due to the rising age of first marriage, the increasing number of elderly people, which is a result of the extension of the average life expectancy, fewer marriages, more divorces, goose fathers, the separation between dwellings for work, etc. Especially a significant amount of single-person households is occupied by young people, which is common in large cities. Along with the increase in the number of single-person households, the youth has created a new demand for housing demand for single-person households. The purpose of this study is to identify which factors influence the preference of a share house as an alternative to a single-person household by making an empirical analysis. As a result of the analysis, the accessibility of a share house, the characteristics of the building and the facilities all had a significant effect on the preference.
Background: Little is known about the cancer screening prevalence and correlates in older adults from different racial backgrounds. In the context of heightened efforts for prevention and early diagnosis, we collected information on screening for two major types of cancers: cervical and breast cancer in order to establish their prevalence estimates and correlates among older South African women who participated in the Study of Global Ageing and Adults Health (SAGE) in 2008. Materials and Methods: We conducted a national population-based cross-sectional study with a multi-stage stratified cluster sample of 3,840 individuals aged 50 years or older in South Africa in 2008. In this analysis, we only considered the female subsample of (n=2202). The measures used included socio-demographic characteristics, health variables, anthropometric and blood pressure measurements. Multivariable regression analysis was performed to assess the association of socio-demographic factors, health variables and cancer screening. Results: Overall, regarding cervical cancer screening, 24.3% ever had a Papanicolaou (PAP) smear test, and regarding breast cancer screening, 15.5% ever had a mammography. In multivariate logistic regression analysis, younger age, higher education, being from the White or Coloured population group, urban residence, greater wealth, and suffering from two or more chronic conditions were associated with cervical cancer screening, and higher education, being from the White or Indian/Asian population group, greater wealth, having a health insurance, and suffering from two or more chronic condtions were associated with breast cancer screening. Conclusions: Cancer screening coverage remains low among elderly women in South Africa in spite of the national guideline recommendations for regular screening in order to reduce the risk of dying from these cancers if not detected early. There is a need to improve accessibility and affordability of early cervical and breast cancer screening for all women to ensure effective prevention and management of cervical and breast cancer.

Most overseas Korean seniors live in China and Japan due to Korea's historical background. Although Korean seniors living in Korea China and Japan share some similarities, the overseas seniors have become culturally different as they adapted to their countries of residence. Therefore, the aim of this study was to provide basic reference data for future senior health enhancement models by comparing and analyzing the health of Korean and overseas Korea seniors. The number of subjects in this study was 603 people, which was comprised of 231 Korean seniors, 246 overseas seniors in China, and 126 overseas seniors in Japan. The average age of the subjects was 74.4 years old. The Korean seniors displayed higher average health score than the overseas seniors. Therefore, the study believes that cultural elements should be considered in a future senior health enhancement model.

In recent years, the population composition of rural area is changing due to the decreasing population growth of the aged with disability and people returning to farming. Since 1970's the rural community facilities have been spreaded out across most rural villages. Although the rural community facilities are the most widely used facilities, the existing facilities are need to be new built or remodeled according to demographic changes, usage of facilities, and environmental condition. Given this reality, the rural community facilities such as village assembly hall and nursing home for senior citizen are not only used for daily life place but also used like co-residence having meals and sleeping together. Therefore the rural community facilities have the potential to become welfare service space for rural elderly people. This research focuses on the current situation of universal design applied community facilities. According to the research, most of the village community facilities were built in the 1990s and 82% of the buildings were more than 15 years old. Furthermore, 45% of the village community facilities more than 15 years old most have a ground floor in masonry structure. The area of the building is 65% less than $100m^2$. The width of the access entrance, the height of the sink, gas safety valve and so forth were relatively well designed. However, the handrail of the entrance, space in front of the toilet bowl, the height difference between the entrance and floor, the installation of the width of the ramp and stair handrail was relatively incomplete. Village community facilities to be built in the future should be universal design fundamentally.
This study purposes to examine direct effects of the physical environments and indirect effects of the satisfaction with life on the settlement consciousness of the aged, using the raw data of 2015 Busan Social Survey. The 5,324 cases were selected from the raw data of those more than 65 years old. The results were as following: (1) When age, spouse with/without, diseases with/without, length of residence, and number of communicating persons were controlled, the satisfaction with housing, the number of cultural facilities, and public transportation availability affected directly on the settlement consciousness of the aged, but the levels of the satisfaction with the number of parks were not directly correlated with it. (2) The satisfaction with housing and public transportation affected indirectly on the settlement consciousness through the satisfaction with life. That is, the satisfaction with life had mediating effects on the two variables. (3) The satisfaction with parks did not affect directly on the settlement consciousness, but it affected on the settlement consciousness through the satisfaction with life. That is, the satisfaction with life shows the complete mediating effects on the settlement consciousness. These results imply that the physical environments such as housing, cultural facilities, public transportation affect on the aged's settlement consciousness so that they should be age-friendly.
