The correlations between acupuncture sensation and analgesic effects (침감과 침 진통 효과의 상관성 연구)

  • Objectives : The elicit of DeQi, a composite of sensation including numbness and fullness that develops at the site of acupuncture stimulation, is considered to be clinically essential to establish treatment efficacy. However, there was little studies investigating the relationship between the subjective feeling of acupuncture sensation and acupuncture analgesia, The current study was therefore performed to determine the correlation between the acupuncture sensation scale (ASS) and the degree of acupuncture analgesia. Methods : Ninety-two female subjects were stimulated at LI4 acupoint on non-dominant hand with real (RA) or sham acupuncture (SA). Finger withdrawal latency (FWL) of each group was also measured to evaluate analgesic effect of acupuncture on the noxious heat stimuli on the dominant hand. All subjects were asked to complete seven point-Likert scale ASS developed by Vincent et al. Results : The increase rate of FWL of the RA group was significantly higher than that of the SA group(36.03 ${\pm}$ 4.45% vs 24.50 ${\pm}$ 3.73%). RA stimulation produced significantly higher rating of all the ASS except for dull sensation, compared to SA stimulation. Significant correlations between analgesic effect of RA, but not of SA, and the degree of the ASS, including burning sensation(r=0.349), Intense sensation(r=0.299), pulsating sensation (r=0.335), and stinging sensation (r=0.306) were found. Conclusions : The results of our study indicate that the DeQi sensation are associated with acupuncture analgesia. Our findings suggest that the evocation of DeQi might be useful clinical indicator of acupuncture treatment.

Analysis of Existing Guidelines and Randomized, Controlled, Clinical Trials for Development of [Guideline of Clinical Trial with Herbal Medicinal Product for gastric cancer] (위암 한약제제 임상시험 가이드라인 개발을 위한 한약제제 무작위배정 대조군 임상시험 고찰)

  • Objectives: This study aimed to learn what should be considered in [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer)] by analyzing the existing guidelines and clinical trials. Methods: The development committee searched guidelines for herbal medicinal product or gastric cancer developed already. Then, clinical trials for gastric cancer using herbal medicine were searched. The searched trials were analyzed in terms of inclusion and exclusion of participants, intervention, comparator, outcomes and trial design. Then, we compared the results of analysis with the regulations and guidelines of Ministry of Food and Drug Safety to suggest the issue that we will have to consider when making the [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer]. Results: As a result, few guidelines for anti-tumor agent and clinical trial with herbal medicinal product were searched in the national institution homepage. In addition, 10 articles were searched by using the combination following search term; 'stomach neoplasm', 'herbal medicine', 'Medicine, Korean traditional', 'Medicine, Chinese Traditional', 'TCM', 'TKM', 'trial'. Most trials included gastric cancer participants with medical history of operation. The type of intervention was various such as decoction, granules, and fluid of intravenous injection. Comparators were diverse such as placebo, conventional treatment including chemotherapy and nutritional supplement. The most frequently used outcome for efficacy was quality of life. Besides, the symptom score, tumor response, and survival rate were used. Safety was investigated by recording adverse events. Conclusion: We found out some issue by reviewing the existing guidelines and comparing it with clinical trials for gastric cancer and herbal medicinal products. These results will be utilized for developing [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer].

  • Various types of implantable drug delivery devices have attracted significant attention for several decades to improve drug bioavailability and reduce side effects, thus enhancing therapeutic efficacy and patients' compliance. However, when implanted into the body, the devices may be influenced by the changes in physiological condition, such as temperature, pH or ionic concentration. Thus, the drug release rates could be also altered concurrently. Therefore, in this work, we employed an implantable ALZET$^{(R)}$ Osmotic Pump, which has been widely used to locally deliver various therapeutic agents and examined the effect of pH, temperature and ionic concentration on its drug release rate. For this, we performed in vitro cell tests to simulate the condition of local tissues influenced by the altered drug release rates, where we used diclofenac sodium as a model drug.

  • A series of N-[4-cyano-2-fluoro-5-(2-pyrimidinyloxy, 2-benzyloxy or 2-pyridinyloxy)phenyl]-3,4,5,6-tetrahydrophthalimides was synthesized, and the herbicidal activities of those deivatives were evaluated through pre- and post-emergence application under upland conditions in a greenhouse. The results showed that most compounds resulted in stronger herbicidal activity on broadleaf weeds than on grass weeds and higher through post-emergence than pre-emergence application. The N-[(4-cyano-2-fluoro-5-(2-pyrimidinyloxy) phenyl]-3,4,5,6-tetrahydrophthalimide showed the best weed control efficacy and marginal corn safety at a rate of 60 g/ha through pre-emergence application.

Ethanol Production by Repeated Batch and Continuous Fermentations by Saccharomyces cerevisiae Immobilized in a Fibrous Bed Bioreactor

  • In this work, a fibrous bed bioreactor with high specific surface area and good adsorption efficacy for S. cerevisiae cells was used as the immobilization matrix in the production of ethanol. In batch fermentation, an optimal ethanol concentration of 91.36 g/l and productivity of 4.57 g $l^{-1}\;h^{-1}$ were obtained at an initial sugar concentration of 200 g/l. The ethanol productivity achieved by the immobilized cells was 41.93% higher than that obtained from free cells. Ethanol production in a 22-cycle repeated batch fermentation demonstrated the enhanced stability of the immobilized yeast cells. Under continuous fermentation in packed-bed reactors, a maximum ethanol concentration of 108.14 g/l and a productivity of 14.71 g $l^{-1}\;h^{-1}$ were attained at $35^{\circ}C$, and a dilution rate of 0.136 $h^{-1}$ with 250 g/l glucose.

A Systematic Review Focused on Health Behavior and Physiological Indicators of Diabetic Patients in Interventional Studies Based on Health Belief Model (건강신념모델 기반 중재연구가 당뇨환자의 건강행위와 생리적지표에 미치는 효과를 중심으로 한 체계적 문헌고찰)

  • Purpose: Diabetes Mellitus (DM) is a leading cause of death with a prevalence rate of 12.4% in South Korea. Self-management is crucial for patients with DM, because many studies have reported that self-management intervention based on the Health Belief Model (HBM) is effective. The purpose of this study was to investigate the current state of HBM based intervention studies and the components and effects of the theories used in the study for diabetes patients. Methods: A systematic review was conducted using the Pubmed, Cochrane Library and Embase databases from January 2009 to May 2019. We reviewed characteristics of intervention based on the HBM in randomized controlled clinical trials (RCTs), quasi-experimental study intervention. Results: Eight studies published in English between 2009 and 2019 were included in this review. The key components of the health behavior promotion program applied to the DM patients were perceived susceptibility, perceived severity, perceived benefits, perceived barriers and self-efficacy. The intervention based on these components has reported to significantly increase the health behavior change, likelihood of taking health action and improve physiological indicators (HbA1c, and fasting blood sugar etc.). Conclusion: This study highlighted the importance of intervention programs based on the HBM for DM patients.

Evaluation of the Efficacy of Zoledronic Acid and Amifostine on Radiation-induced Bone Loss in Mice

  • Background: This study investigated the effects of zoledronic acid (ZA) on radiation-induced bone loss in C3H/HeN mice. Materials and Methods: C3H/HeN mice were divided into sham control and three irradiated groups (3 Gy, gamma ray). The irradiated mice were treated for 12 weeks with vehicle, amifostine (intraperitoneal injection), or ZA (subcutaneous injection). Grip strength, uterus weight, and serum alkaline phosphatase (ALP), and tartrate-resistant acid phosphatase (TRAP) levels were measured. Tibiae were analyzed using micro-computed tomography. Results and Discussion: Treatment of ZA ($100{\mu}g{\cdot}kg^{-1}{\cdot}week^{-1}$) significantly preserved trabecular bone volume, trabecular thickness, trabecular number, trabecular separation, bone mineral density of proximal tibia metaphysic, and cortical bone volume, but did not alter the uterus weight of the mice. The administration of ZA for 12 weeks lowered serum ALP and TRAP levels in irradiated mice, suggesting that ZA can reduce the bone turnover rate in mice. No differences were apparent between the amifostine-treated group and the irradiation control group. Conclusion: The results indicate that ZA can prevent radiation-induced bone loss in mice.

The Impact of Workplace Empowerment on University Hospital Administrative Staff's Job Satisfaction and Organizational Commitment (경인지역 대학병원 행정직원이 인지하는 임파워먼트 정도가 직무만족과 조직몰입에 미치는 영향)

  • Empowerment is a multifaceted concept which can be described somewhat differently. In general, it is defined as the motivational concept of autonomy and self-efficacy. Recently, this concept of empowerment is applied to improve organizational staff's job satisfaction and organizational commitment in many organizations. Empowerment in service organizations has certainly generated more publicity than any other organizations. The objective of this study is to measure the degree of hospital administrative staff's empowerment using Spreitzer(1995)'s empowerment theory, and also to analyze the relationship of empowerment and job satisfaction and organizational commitment. Spreitzer argues that the work empowerment is composed of 4 dimensions (meaning, competence, self-determination, and impact) and each dimension influences employee's job satisfaction, organizational commitment, and organizational performance in the end. In order to carry out this study, data were collected by self-administered Questionnaires from 181 hospital administrative staff at 3 university hospitals in Inchon and Kyunggi-Do. The response rate is 86%. The Collected data were analyzed with SPSS statistical package V10.0. The major findings of this study are as follows: First, the degree of hospital administrative staff's empowerment is a high level point at the 7 Likert Scale. Second, the reliability of 3 variables, empowerment, job satisfaction and organizational commitment, was statistically satisfied(Cronbach's alpha>0.80). Third, as a result of correlation analysis, four components of empowerment, job satisfaction and organizational commitment showed a positive relationship. Finally, multiple regression analysis was conducted to find the impact of empowerment on job satisfaction and organizational commitment. In the case of job satisfaction, the meaning and competence factors have a positive effect(P<0.05). And the case of organizational commitment, the meaning and impact components have a positive relation(p<0.05). These results showed that the workplace empowerment is significantly related to employee's job satisfaction and organizational commitment.

Low Power MAC Protocol Design for Wireless Sensor Networks using Recursive Estimation Methods (회귀적 추정 방식을 이용한 무선 센서 네트워크용 저전력 MAC 프로토콜)

  • In the context of wireless sensor networks, one of major issues is energy conservation. For low power communication, by utilizing our experimental results for the relation between clock drift and synchronization interval, we designed a new protocol which can support a wide range of duty cycles for applications with very low traffic rate and insensitive delay. The transmission (TX) node in the protocol synchronizes with the reception (RX) node very before transmitting a packet, and it can adaptively estimate the synchronization error size according to the synchronization interval from minutes to hours. We conducted simulations and a testbed implementation to show the efficacy of the proposed protocol. We found that our protocol substantially outperforms other state-of-the-art protocols, resulting in order-of-magnitude increase in network lifetime over a variety of duty cycles.

The Clinical Study on 193 Cases with Herniated Lumbar Disc (요추 추간판 탈출증 입원환자 193명에 대한 임상적 고찰)

  • Objective : Lower back pain is the most common pain in our life, and Heniated Intervetebral Disk(HIVD) of L-spine is one of the most common disease that cause lower back pain. This study is designed to invertigate the general distributions for HIVD patients. Subjects and Methods : The 193 patients who had taken diagnosis of HIVD by L-spine CT or L-spine MRI and admitted to Cheonan oriental medical hospital in Daejon university from January 2008 to December 2009 were reviewed. They were analyzed according to sex, age, prevalent duration, and causing factor. They were treated with acupuncture, herbal medicine and physical therapy. And treatment efficacy was evaluated according to each distribution respectively. Result 1. Among 193 patients, males were more than females in the ratio of 1.47:1. 2. According to age, 33.2% patients were thirties. 3. According to causing factor, 39.9% patients with unknown origin were most. 4. According to prevalent duration, 49.7% patients were on acute stage and 20.2% patients were on 6-10days. 5. According to HIVD types, patients with protruded disc type and with lesion of L4-5 were most common respectively 45.1%, 63.3%. 6. When effective rates were analyzed according to each distribution, patients on twenties, acute stage and with extruded disc type were showed good effective rates, respectively 92.7%, 90.6%, 89.3%. Conclusion: Total effect rate was 87.5%. Prevalence of HIVD is the highest in the age of 30 to 40, L4-5 occupy the most. And early treatment was better than late treatment.
