Medical Exposure of Korean by Diagnostic Radiology and Nuclear Medicine Examinations (진단방사선 및 핵의학 검사에 의한 한국인의 의료상 피폭)

  • Although medical exposure from diagnostic radiology procedures such as conventional x-rays, CT and PET scans is necessary for healthcare purposes, understanding its characteristics and size of the resulting radiation dose to patients is much of worth because medical radiation constitutes the largest artificial source of exposure and the medical exposure is in a trend of fast increasing particularly in the developed society. Annual collective doses and per-caput effective doses from different radiology procedures in Korea were estimated by combining the effective dose estimates per single medical procedure and the health insurance statistics in 2002. Values of the effective dose per single procedure were compiled from different sources including NRPB reports, ICRP 80, MIRDOSE3.1 code and independent computations of the authors. The annual collective dose reaches 27440 man-Sv (diagnostic radiology: 22880 man-Sv, nuclear medicine: 4560 man-Sv) which is reduced to the annual per-caput effective dose of 0.58 mSv by dividing by the national population of 47.7 millions. The collective dose is far larger than that of occupational exposures, in the country operated 16 nuclear power plants in 2002, which is no more than 70 man-Sv in the same year. It is particularly noted that the collective dose due to CT scans amounts 9960 man-Sv. These results implies that the national policy for radiation protection should pay much more attention to optimization of patient doses in medicine.

Epidural Administration of Morphine for Cancer Pain via Portal System (경막외 Port 및 주입기를 이용한 지속적 모르핀 투여에 의한 암성 통증 조절)

  • Background: Efficacy of spinal opioids for the treatment of intractable cancer pain has been reported by several authors. The epidural route seems to be a more reliable and effective method of pain control as compared to the intrathecal route which can lead to opioids by portal system. Methods: Medical records were reviewed of 18 patients who had been treated with epidural morphine via an implanted port-A-Cath from Mar. 1991 to Sep. 1994. Results: Patients were treated for a mean of 92 days. There were wide variation of dose requirements. The minimum daily dose ranged from 2 to 10mg, and maximum daily dose from 3 to 30 mg. Verbal rating scale were below moderate until 100th days after posrtal implantation. When 3 patients suffered from aggravated pain associated with vertebral metastasis. Five of 11 patients who were administered medication longer than 50 days reguired increased doses ranging from 3 mg to 25 mg which were higher as compared to initial doses. These patients also experienced pain due to vertebral metastasis. There were no report of epidural scarring, respiratory depression, epidural infections, meningitis, or catheter blockade. Conclusion: Continuous epidural morphine administration via Port-A-Cath is an effective method with minimal complication.

Studies on the Factors Influencing In Vitro Embryo Production in Korean Native Cattle (한우의 체내 수정란 생산에 영향을 미치는 요인에 관한 연구)

  • These studies were carried out to establish an effective and practical system for commercialization of embryo production techniques by analyzing several factors influencing in vivo embryo production in Korean native cattle. In vivo embryos were flushed 226 times from 128 donors. The results obtained in studies on the factors influencing in vivo embryo production were as follows : 1. There were no significant differences in the number of total recovered, fertilized, transferable and freezable embryos among the hormone doses(FSH-P, 28∼34mg; SUPER-OV, 75IU) used for superovulation. However, over 30mg doses of FSH-P showed a slightly higher effect than others. 2. There were slight decrease in the number of fertilized, transferable and freezable embryos in 3 times repeated superovulation. But there were no significant differences among 1, 2 and 3 times repeated superovulations. 3. Age of donors did not affect the number of transferable and freezable embryos, but the number of fertilzed embryos were highest in 2∼3 years old donors and were lowest in 8∼9 years old donors(P<0.05). 4. Season had a significant effect on the production of embryos(P<0.05). the embryo production, and the number of fertilized, transferable and freezable embryos were most effective in summer and follow by spring, autumn and winter. 5. The number of transferable and freezable embryos was highest in donors flushed at 7∼8 days after estrus(P<0.05).

Calculation of Dose Conversion Coefficients in the Anthropomorphic MIRD Phantom in Broad Unidirectional Beams of Monoenergetic Photons (MIRD 인형팬텀의 넓고 평행한 감마선빔에 대한 선량 환산계수 계산)

  • The conversion coefficients of effective dose per unit air kerma and equivalent dose per unit fluence were calculated by MCNP4A code for antero-posterior(AP) and postero- anterior(PA) incidence of broad, unidirectional beams of photons into anthropomorphic MIRD phantom. Calculations have been performed for 20 monoenergetic photons of energy ranging from 0.03 to 10 MeV. The conversion coefficients showed a good agreement with the corresponding values given in the draft publication of joint task group of ICRP and ICRU within 10%. The deviations may arise from the differences of geometry in the MIRD phantom and the ADAM/EVE phantoms, and the differences in the codes and cross-section data used. Inclusion of a specific oesophagus model results in effective dose slightly different(5% at most) from the effective doses obtained by adopting the equivalent doses for the thymus or pancreas. Deletion of the ULI from the remainder organ appeared not to be significant for the cases of photon dosimetry covered in this study.

  • Amount of radiation exposure by seeing through fluoroscopy examination while is many patient exposure administration and unprepared misgovernment be. In this study, abdominal fluoroscopy during the scan, the dose and image quality change according to the use of grid and added filter optimized by measuring the test condition is proposed. Uses seeing through fluoroscopy examination equipment of Image Intensifier of Easy Diagnost Eleva (Philips), under tube type and uses Human phantom and measures average area dose according to grid insertion existence and nonexistence and added filter kind change. Measure sum of 29 organ dose and effective dose through PCXMC imagination simulation program and image J program through noise, SNR, image distortion was measured. Resolution, sharpness, and analyzed using the MTF curves. Fluorography the grid to insert the filter thickness and thickening and increased the average area dose and organ doses and effective dose. In the case of spot examination, when inserted grid, average area dose and organ dose and effective dose increased. Filter thickens the average area dose decreased, but the organ doses and effective dose were increased when use 0.2mmCu+1mmAl filter, decreased slightly. Noise and SNR measurements without inserting the gird, if you do not use the added filter was the lowest and when measure the distortion, 0.1mmCu+1mmAl filter was no difference of image quality in case insert grid was judged that when did not use occasion added filter that do not use grid, difference of image quality does not exist. Did not show a big difference, according to the grid and uses of the added filter sharpness, and resolution. Patient dose increases with factors that reduce the quality of the image so reckless grid and the use of the added filter when abdominal fluoroscopy examination should be cautious in using.

Sterilization Effects of $\gamma-ray$ and Ozone on Microoganisms Contaminated in Angelica keiskei Powde (신선초 분말에 오염시킨 미생물에 대한 감마선과 오존의 살균효과)

  • For the purpose of improving hygienic quality of health-foods, sterilization effects of ${\gamma}$-ray and ozone on microoganisms associated with food cultured in the media and contaminated in Angelica keiskei powder were investigated. Ozone was immersed in water and sprayed in air, on the concentration of 3 mg liter-1 at an air flow rate of 5 liter min-3. Test strains cultured in the media completely inhibited by ${\gamma}$-ray at irradiation doses of 0.25~2 kGy. In the case of ozone, test bacteria inactivated after treatment of 10~20 minutes, but test mold, Aspergillus flavus was not effective. Strains contaminated in Angelica keiskei powder completely inhibited by ${\gamma}$-ray at irradiation doses of 2.5~7.5 kGy. However, when the powder was sprayed with ozonized air for 10hours, Bacillus subtilis and Staphylococcus aureus among five strains were eliminated.

  • The effect of atractylis on the renal function of the dog was investigated in this study. Alcohol extract of atractylis, when given intravenously in dose 15mg/kg, elicited antidiuresis rather than diuresis. Also, free water clearance $(C_{H_2O})$ decreased in proportion. No alterations of the renal clearances of creatinine and PAH were detected in the range of doses which are effective in inhibiting diuresis. The effects were obtained with smaller doses when the atractylis was given through the intracarotid artery route rather than intravenously. When infused directly into a renal artery, atratylis exhibited identical action on both kidneys, indicating that the renotropic action is mediated by some endogenous humoral agents. It is therefore suggested that atractylis is capable of releasing ADH from the neurohypophysis.

  • The radiation degradation of five insulating materials such as silicone rubber, ethylene-propylene rubber polybutylene terephthalte, poly carbonate, nylon 66 were evaluated by using thermogravimetry analysis (TGA) and tensile properties as a function of radiation doses. The results of temperature at 5% weight loss and activation energy from TGA showed linearly decreasing and increasing tendency as radiation doses comparing with tensile properties. Consequently, the assessment of TGA for the radiation degradation of materials was effective.

  • For bioassay of the various extracts of ginseng, the growth determination method using Saccharomyces cerevisiae which was cultured with various doses of the extracts, was studied The water extract, Powder. and ethanol extract were more effective (about 45∼ 110% increase) than saponins or its fractions (about 20∼35% increase). The cold methanol residue showed a increase effect but it was not significant. The bioassay curves for the water extract, ethanol extract, the butanol extracted saponins and the cold methanol- residue were made from the experimental data. From these curves it is possible to find the relation between dose and effectiveness and the optimal doses of various ginseng extracts, and the amount of extract in a sample can be estimated The .angers of sample amount were 0.01% (100ppm) ∼0.32% (3200ppm) fo. the water extract, 0.025% (150ppm)∼0.1% (1000ppm) for the ethanol extract, and 0.008% (80ppm)∼0.016% (160ppm) for the saponins. It was impossible to determine the range for the cold methanol- residue, The acceleration effects on the cell proliferation by a only 0.0008% (8ppm) of the diol- and triol-saponin were measurable in earlier Period (24 hour treatment).

Intent to Use a Smartphone Application for Radiation Monitoring in Correlation with Anxiety about Exposure to Radiation, Recognition of Risks, and Attitudes toward the Use of Radiation

  • Background: Radiation is used in a variety of areas, but it also poses potential risks. Although radiation is often used with great effectiveness in many applications, people perceive potential risks associated with radiation and feel anxious about the possibility of radiation exposure. Various methods of measuring radiation doses have been developed, but there is no way for the general public to measure their doses with ease. Currently, many people use smartphones, which provide information about the location of an individual phone through network connections. If a smartphone application could be developed for measuring radiation dosage, it would be a very effective way to measure individuals' radiation doses. Thus, we conducted a survey study to assess the social acceptance of such a technology by the general public and their intent to use that technology to measure radiation doses, as well as to investigate whether such an intention is correlated with anxiety and attitudes toward the use of radiation. Materials and Methods: A nationwide online survey was conducted among 355 Koreans who were 20 years old or older. Results and Discussion: Significant differences were found between the genders in attitudes, perceptions of radiation risk, and fears of exposure to radiation. However, a significant difference according to age was observed only in the intent to use a smartphone dose measurement application. Attitudes towards the use of radiation exerted a negative effect on radiation risk perception and exposure anxiety, whereas attitudes towards the use of radiation, risk perception, and anxiety about exposure were found to have a positive impact on the intent to use a smartphone application for dose measurements. Conclusion: A survey-based study was conducted to investigate how the general public perceives radiation and to examine the acceptability of a smartphone application as a personal dose monitoring device. If such an application is developed, it could be used not only to monitor an individual's dose, but also to contribute to radiation safety information infrastructure by mapping radiation in different areas, which could be utilized as a useful basis for radiation research.
