  • Purpose: This study was conducted to compare the level of depression, self-esteem and mother-adolescent (M-A) communication perceived by both mothers and adolescents between the early adolescent (E-A) group and the late adolescent (L-A) group; and to examine the actor effect and the partner effect of self-esteem and M-A communication on depression in mothers and adolescents. Methods: Participants were 107 Kirogi families who resided in the Midwest region of the U. S. Data were collected from September, 2008 to March, 2009 using the scales of Center for Epidemiologic Studies Depression (CES-D), Self-esteem and Parent-Adolescent Communication Inventory. Results: Mothers in E-A group reported higher scores on depression than mothers in L-A group. Adolescents in L-A group reported higher scores on depression and lower scores on self-esteem than adolescents in E-A group. In the E-A group, mothers' selfesteem had big actor effect on mothers' depression and partner effect on adolescents' depression. In the L-A group, selfesteem of mothers and adolescents had actor effect on their depression respectively without partner effect. M-A communication of mothers influences mothers' depression negatively and adolescents' depression positively. In both group, M-A communication influences their depression with mediating effect of self-esteem. Conclusion: To promote Kirogi families' mental health, programs for mothers and adolescents should be developed differently according to adolescents' development stage.

  • Studies on menstruation have focused only on menstruation itself and menstrual disorders. The menstruating girls or women have been neglected. So, the purpose of this study was to understand menstrual experience of adolescent girls in their perspective and build a theory on it, The specific purpose of this study were to find initial reaction of the girls, their strategies to adapt to menstruation. consequences of their efforts, influencing factor, and patterns of experience. The subjects of this study were eleven adolescent girls who experienced menarche three months to twenty-six months before the interview time. They were selected purposively. Their ages were in range of twelve and sixteen. One of them was a elementary school girl, three high school girls, and seven middle school girls. Two girls were handicapped because of cerebral palsy. All of them had some knowledge about menstrual physiology and hygiene during menstruation. Data were collected from September, 1994 to July, 1995. Data collection & analysis were done according to the grounded theory methodology by Strauss & Corbin(1990). Data collecting method was the long interviews and observation. Each interview took from 1 hour to 2 hours. Interview were tape-recorded and transcribed later by author. Data were analyzed immediately after interviews. Based on the results of previous interview, next interview were planned until gathered data reached the saturation point. Results were as follows. One hundred and six concepts were found. Those concepts were grouped into twenty eight categories and then fourteen higher categories. Twenty eight categories were as follows. “want to hide”, “bewildered”, “sense of burden”, “sense of heterogeneity”. “gladness”. “sense of superiority”, “negative empathy”, “positive empathy”, “limited hygenic control”, “sense of timing”, “lack of knowledge”, “lack of support”, “advance knowledge”, “informational support”, “emotional support”, “endurance”, “prayer”, “disclosing”, “avoidance”, “diversion”, “sense of powerlessness”, “discovery of sex identity”, “sense of maturation”, “sense of stability”, “acceptance of menstruation ”. fourteen higher categories were as follows. “negative feeling”, “posive feeling”, “exchange of feeling”, “limited hygenic control”, “sense of timing”, “accumulated experience”, “dysmenorrhea”, “level of knowledge”, “need for support”, “perceived support”, “sharing of feeling”, “self-control”, “passive acceptance”, “active acceptance”. The core category was “emotional shaking”, which consisted of “positive feeling” and “negative feeling”. “Emotional shaking”comes up to every adolescent girls experiencing menarche, independently of any contextual conditions, and its dimension has two directions : positive one and negative one. Its influencing factors were time of menarche, advance knowledge, support from the significant persons, expression and self-regulation. Even if they showed different process of adaptation to menstruation, general process of adaptation were as follows : 1. stage of emotional shaking 2. stage of acceptance 3. stage of internalization of the menstrual experience. Seven patterns existed on the process of adaptation to menstruation after menarche. Those are as follows. 1. If girls thought their menarche came too early and they had not much knowledge on menstruation, they had a kind of negative feeling. If they did not get enough support and dysmenorrhea superimposed, they came to accept menstruation passively. 2. If girls had menarche too early. they had negative feeling, even though they had enough advance knowledge. But support helped them accept menstruation easily. 3. If girls had menarche too early, they had negative feeling, even though they had enough advance knowledge on menstruation. But by experiencing subsequent menstruations and disclosing feeling, they began to accept menstruation. 4. If girls had menarche too lately and they had enough advance knowledge on menstruation. they had positive feeling. If dysmenorrhea superimposed later, their feeling turned in to negative one. But they came to accept menstruation positively by disclosing feeling and getting support. 5. If girls had menarche too early, they had negative feeling, even though they had enough advance knowledge on menstruation. In addition to this. if dysmenorrhes superimposed while they did not get enough support, they felt powerless and came to accept menstruation passively. 6. If girls had menarche too early and did not get enough advance knowledge, they had negative feeling. But disclosing feeling and support made them get sense of homogeneity and began to accept menstruation. 7. If girls had handicap, they had negative feeling, even though they had enough advance knowledge and menarche was late. But Menarche made them get feel sexual identity. Their limited hygenic control and negative empathy from their mothers made them accept menstruation passively. To let adolescent girls take their menstrual experience as a part of their lives forming a positive sense of feminine identity, it needs qualified teaching and, support and deep concern of the significant others. Nurses including school nurses should try to develop an educational program, which include menstrual physiology. hygiene during menstrual period, meaning of menstruation and impact of menstruation on the development of female sexual identity.

  • Early onset scoliosis (EOS) describes the onset of scoliosis before the age of 10 years and is associated with significant health risks. Compared to adolescent idiopathic scoliosis, studies on the etiology of EOS in children are more common. EOS includes inhomogeneous groups of patients, and the etiology of EOS may be congenital, neuromuscular, syndromic, or idiopathic. The identification of the molecular etiology underlying patients with EOS has been increasing. This could provide valuable information for optimizing the management and care of these children. In this review, EOS was reviewed from a medical genomic perspective.

  • Objectives:The primary purpose of this study is to understand the psychopathology of the victims of school violence in terms of early psychosis. By doing this, the early detection of psychosis among the victims is possible, and early detection may lead to early intervention. Methods:Two-thousand and nine-hundred seventy two students from 16 middle schools in Seoul were asked to fill out questionnaire comprised of popularity and intellectual and school status of Piers-Harris Children's Self Concept Inventory, Symptom Check List-90-Revised, and Ostracism Scale. The subjects whose scores upon Ostracism Scale were higher than average by two standard deviation were labeled as ‘Repelled and Isolated group', and subjects whose scores on popularity were significantly lower than average and whose scores on psychoticism of SCL-90-R were higher than average were defined as 'tentative early psychosis group'. Odds ratios were calculated from the numbers of subjects with and without high psychoticism scores and high ostracism scores. On the subjects of 'tentative early psychotic group', we examined every clinical characteristic and conducted correlation analysis and regression analysis in order to find out the risk factors and to construct theoretical model that explains the psychoticism scores. Results:The results were as follows:1) Total 157(5.3%) subjects were rated significantly higher on ostracism scale, and among them, 47 subjects(29.9%) were rated significantly higher than average on psychoticism scale, while only 50 subjects among 2,135 students who were rated within normal range showed significantly higher score on psychoticism scale. Odds ratio for psychotic group of isolated group were 17.82 and it was statistically significant. 2) Forty-seven subjects(31 boys, 16 girls) who were rated as they were unpopular and rejected from peers had significantly higher psychoticism scores. They were not significantly different from simply high psychoticism subjects in anxiety, social anhedonia scale, magical thinking, obsessivecompulsive symptoms, phobic anxiety, psychoticism, somatization, but showed higher ostracism scores and paranoid tendencies. Among school violence victims, who rated themselves unpopular and showed higher psychoticism scores, the psychoticism scores were mainly explained by anxiety, depression, hostility, interpersonal sensitivity, obsessive-compulsive symptoms, paranoid tendency, somatization scales($r^2=0.93$). Conclusion:Thus, it can be concluded that the subjects with higher ostracism score have the substantially high risk for psychosis development. By these results, we propose that school violence victims with anxiety, depression, hostility, interpersonal sensitivity, obsessive-compulsive symptoms, paranoid tendency, somatization should be tested individually considering school adjustment, attentional deficit, concept formation problems.

  • Objectives : Among adolescents, the proportion of daily smoking tends to increase with age, and does not return to the previous level. Therefore, we examined the factors associated with smoking frequency in a nationwide sample of Korean adolescents to help prevent the progression of smoking. Methods : Among 127,804 adolescents who participated in the 2016 and 2017 Korean Youth Risk Behavior Web-Based Survey, data from 7,781 adolescents who replied that they currently smoked were analyzed. According to the frequency of cigarette smoking, participants were classified into daily and non-daily smokers. Participants' socio-demographic characteristics, smoking behavior, and psychological problems including perceived stress, depressive mood, and suicidality were assessed. Results : There were 3,627(47.4%) daily smokers and 4,154(52.6%) non-daily smokers. Low parental educational level, and early smoking experience were positively associated with daily smoking. High academic achievement, and high socio-economic status were negatively associated with daily smoking. Daily smoking was positively associated with lower happiness, lower sleep satisfaction, and severe stress during the past 12 months. Factors affecting suicide attempts of smokers were sex, residential type, socioeconomic status, and early smoking experience. Conclusion : The results indicated that socially disadvantaged families, and smoking experience with young age influenced daily smoking among Korean adolescents. Daily smoking was associated with psychological problems including lower happiness, lower sleep satisfaction, and severe stress. Early smoking experience influenced suicidal attempts among Korean adolescent smokers. Early intervention for smoking cessation in this high-risk group may contribute to prevention of these psychiatric complications of smoking.

  • Objectives: Among the many intervention programs for children with autism spectrum disorder (ASD), the Early Start Denver Model (ESDM) is one of the few approaches that has succeeded in demonstrating clinical efficacy in randomized control trials. Here, we investigate the clinical efficacy of ESDM intervention in young children with ASD in a community setting within Japan. Methods: All subjects were children with ASD who received ESDM intervention during the study period. Each ESDM session lasted 75 min and occurred once per week for at least 12 weeks. The outcome measures consisted of the Kyoto Scale of Psychological Development (K-test), Aberrant Behavior Checklist-Japanese version (ABC-J), and the Clinical Global Impression-Severity scale (CGI-S). Results: Twenty-seven subjects (29.4±6.4 months old) received ESDM intervention that lasted for 8.0±2.6 months on average. The score on Language and Social developmental quotient on the K-test increased significantly after the intervention. The total scores on the ABC-J and CGI-S significantly decreased after completion of the ESDM intervention. Conclusion: Our results suggest that ESDM intervention could reduce the severity of distinct clinical features of ASD, such as impairments in social interaction and communication assessed by the K-test, and maladaptive behavior rated by the ABC-J and CGI-S. We believe that the ESDM adapted to each institution might become one of the standard options for children with ASD in Japan.

  • We conducted a comprehensive review of behavioral and educational interventions for individuals with autism spectrum disorder (ASD). The most prominent type of intervention, Comprehensive Early Intervention, often referred to as Early Intensive Behavioral Intervention (EIBI), has been found to be particularly effective in improving intelligence and adaptive behaviors. The naturalistic developmental behavioral intervention, designed to enhance social and communication abilities, showed effectiveness in improving language, cognitive function, and social initiation. However, more studies are needed to examine its effectiveness. Intensive individualized intervention, which provides a tailored intervention for a specific target behavior, was effective in improving social skills and communication, as well as reducing sleep, eating, and toileting problems. Cognitive behavioral therapy (CBT) is the most effective method for dealing with emotional difficulties, but it has not been widely used because of the shortage of trained experts. Parent-mediated intervention (PMI) involves parents acquiring knowledge and specific skills to improve their child's functioning or reduce challenging behaviors. Speech and language therapy, sensory integration, Treatment and Education of Autistic and related Communications Handicapped Children, developmental approaches, and social stories are frequently used interventions. However, evidence of their effectiveness has yet to be well established. Based on these findings, intervention recommendations for autism include EIBI, Early Start Denver Model, intensive individualized intervention, CBT, and PMI. The choice of intervention should be tailored to the individual's needs and delivered by qualified professionals with expertise in the specific intervention.

  • A dolescence can be defined as the period between pubescence and physical maturity, and the passage through adolescence(age of 12-19) forms a critical period. Major developmental tasks of adolescence are construction of an emancipated identity, realistic ambitions, and reasonable ideals, and the further development of sexual and social identities. In early adolescence one struggles to adjust psychologically to the spurt in growth and tensions filled with sexual and aggressive impulses. There are reshuffling of peer groups due to differing rates of maturation, early adolescence crushes, sublimation of sexuality, and the resurgence of oedipal feelings. One reaches so-called period of formal operations in cognitive development. In mid-adolescence one struggles to overcome family attachments and controls, and it is a time marked by revolt and conformity. One has to overcome sexual repression while caught in reactivation of oedipal attachment. Peer groups gains in importance, and the peer group changes into a youth group that carries the youth culture. Late adolescence is a period of achieving an ego identity and capacities for intimacy. One has to prevent premature closure : Identity foreclosure limits both opportunity and the ability to guide one's own life. Gradually adolescent begins to have a loss self-centered and narcissistic orientation to one's sexual and affectional needs, and becomes involved in love relationship.

  • The purpose of this study is to examine the trajectory of early adolescent aggression and to assess the effects of perceived family factors on it. This study investigated the changes in aggression over time and the effects of three family factors, parental attachment, parental monitoring, and family conflict, on the changes using multilevel modeling. The data from Korea Youth Panel Survey(KYPS) of fourth graders who were followed over five years were analyzed. The results indicated that there was a significant average increase followed by a slight decrease in aggression over time; that is, the data followed a quadratic trend. Parental attachment, parental monitoring, and family conflict had significant time-independent effects on aggression, while only family conflict had a significant time-dependent effect on aggression. These results imply that family factors need to be emphasized in planning prevention and intervention programs to reduce aggression during adolescence.
