A Study on Application of Internet-based Personal Health Record(PHR) System: Using Google Health (인터넷기반의 개인전자건강기록 시스템 적용사례 연구: 구글헬스를 중심으로)

  • With the help of fast growing popularization of internet, all areas of e-Health have expanded rapidly; such that people have become interested in digital personal health record and its management. This paper examined the characteristics of personal health record and made the analysis of the structure of Google Health, the internet-based personal health record system. Google Health allows you to store and manage all of your health information, import medical records from hospitals and pharmacies, share your health records, and explore online health services. This examples represents not only a significant change of current medical systems but also enables to estimate the future stream of it. As a result, this paper, in the areas of e-Health which will be expanded in various service areas, may give you a greater sense of importance of personal health record and will eventually provide more complemental structure of future personal health record through comparative studies on the strength and weakness of it.

Changes in Health Care Utilization during the COVID-19 Pandemic (코로나19 유행 시기 의료이용의 변화)

  • Background: After the first case of coronavirus disease 2019 (COVID-19) in January 2020, Korea has experienced three waves in 2020. This study aimed to analyze changes in health care utilization according to the period of the 1st to 3rd waves of the COVID-19 pandemic. Methods: We analyzed 3,354,469,401 national health insurance claims from 59,104 medical facilities between 2017 and 2020. Observed-to-expected ratios (O:E ratio) with data from 2017 to 2019 as expected values and data from 2020 as observed values were obtained to analyze changes in medical utilization. T-test was used to test whether the difference of observed and expected values was statistically significant. Results: In 2020, the O:E ratio was 0.894, indicating a decrease in health care utilization overall during the pandemic. The O:E ratio of the 1st wave was 0.832, which was lower than those of the second (0.886) and third (0.873) waves. Health care utilization decreased relatively more among outpatient, women, children and adolescents, and health insurance patients. And health care utilization decreased more in small medical facilities and in Daegu and Gyeongbuk during the first wave. During the pandemic, the O:E ratios of respiratory diseases were 0.486-0.694, while chronic diseases and mental diseases were more than 1.0. Conclusion: Health care utilization decreased during the COVID-19 pandemic overall, and there were differences by COVID-19 waves, and by the characteristics of patients and medical facilities. It is necessary to understand the cause of changes in health care utilization in order to cope with the prolonged COVID-19 pandemic.

  • E-Health allows you to supersede the central patient wireless healthcare system. Wireless Body Sensor Network (WBSN) is the first phase of the e-Health system. In this paper, we aim to understand e-Health architecture and configuration, and attempt to minimize energy consumption and latency in transmission routing protocols during restrictive latency in data delivery of WBSN phase. The goal is to concentrate on polling protocol to improve and optimize the routing time interval and schedule communication to reduce energy utilization. In this research, two types of network models routing protocols are proposed - elemental and clustering. The elemental model improves efficiency by using a polling protocol, and the clustering model is the extension of the elemental model that Destruct Supervised Decision Tree (DSDT) algorithm has been proposed to solve the time interval conflict transmission. The simulation study verifies that the proposed models deliver better performance than the existing BSN protocol for WBSN.

Outcome of Extracorporeal Ventricular Assist Device for Cardiogenic Shock as a Bridge to Transplantation

  • Background: The extracorporeal ventricular assist device (e-VAD) system is designed for left ventricular support using a permanent life support console. This study aimed to determine the impact of temporary e-VAD implantation bridging on posttransplant outcomes. Methods: We reviewed the clinical records of 6 patients with the Interagency Registry for Mechanically Assisted Circulatory Support (INTERMACS) profile 1, awaiting heart transplantation, who were provided with temporary e-VAD from 2018 to 2019. The circuit comprised a single centrifugal pump without an oxygenator. The e-VAD inflow cannula was inserted into the apex of the left ventricle, and the outflow cannula was positioned in the ascending aorta. The median follow-up duration was 8.4±6.9 months. Results: After e-VAD implantation, lactate dehydrogenase levels significantly decreased, and Sequential Organ Failure Assessment scores significantly improved. Bedside rehabilitation was possible in 5 patients. After a mean e-VAD support duration of 14.5±17.3 days, all patients were successfully bridged to transplantation. After transplantation, 5 patients survived for at least 6 months. Conclusion: e-VAD may reverse end-organ dysfunction and improve outcomes in INTERMACS I heart transplant patients.

The Effects of Office Workers' Attitudes to Internet Health Information, e-health Literacy on Health Information Seeking Behavior and Health-related Behavioral Intention (직장인의 인터넷 건강정보에 대한 태도, e-헬스리터러시가 건강정보추구행동과 건강관련행위의도에 미치는 영향)

  • This study was to investigate the effects of workers' attitudes to internet health information(AIHI), e-health literacy(eHL) on internet health information seeking behavior(IHISB) and health-related behavioral intention(HBI). 152 workers conducted the structured questionnaires from May to July 2017. Data analyzed by t-test, ANOVA, and multiple regression analysis. Factors affecting IHISB were identified to be eHL (${\beta}=.33$, p<.011), and perceived usefulness for internet health information(PU)(${\beta}=.28$, p=.002), and factors affecting HBI were identified to be PU(${\beta}=.33$, p=.001), information utilization(IU)(${\beta}=.24$, p=.003), and eHL(${\beta}=.18$, p=.002). As above results, considering the eHL of workers, it is necessary to build an internet health information system and develop education program that increase IU.

Service Analysis of Community Health Nurse Practitioner using Information System (보건진료소 정보시스템을 이용한 보건진료원의 업무 분석)

  • The purpose of this study was to analyze the activities of Community Health Nurse Practitionses using the Community Health Post Information System(CHPIS). The information system that have been introduced in 1994 and used by 400 Community Health Posts(CHPs) since 1997, which is about $20\%$ of the total CHPs nationwide. Twenty-five CHPs from two provinces participated in the analysis. Seventy-two percent of the CHPs among the participating CHPs started using the system since 1996. The degree of utilization of the information system was classified into three groups (i. e., high. medium, and low). The results revealed that only $48\%$ utilized the system with high level. The areas of analysis of the information system included characteristics of community residents, environmental attributes, and job analysis of Community Health Nurse Practitioners(CHNPs). The study results indicated that primary health care and drug demand and supply system showed the highest level of satisfaction in utilizing the information system by CHNPs.

The Ubiquitin-Proteasome System and F-box Proteins in Pathogenic Fungi

  • The ubiquitin-proteasome system is one of the major protein turnover mechanisms that plays important roles in the regulation of a variety of cellular functions. It is composed of E1 (ubiquitin-activating enzyme), E2 (ubiquitin-conjugating enzyme), and E3 ubiquitin ligases that transfer ubiquitin to the substrates that are subjected to degradation in the 26S proteasome. The Skp1, Cullin, F-box protein (SCF) E3 ligases are the largest E3 gene family, in which the F-box protein is the key component to determine substrate specificity. Although the SCF E3 ligase and its F-box proteins have been extensively studied in the model yeast Saccharomyces cerevisiae, only limited studies have been reported on the role of F-box proteins in other fungi. Recently, a number of studies revealed that F-box proteins are required for fungal pathogenicity. In this communication, we review the current understanding of F-box proteins in pathogenic fungi.

Application of the New Ballard Scale with Extended Scoring System in Full-term Newborns (New Ballard Scale(NBS)의 확장 적용)

  • Purpose: An exploratory study was done to examine the validity of the new Ballard scale with extended scoring system(eNBS) in estimating gestational age(AG) in full-term newborns. Method: The eNBS scoring system was extended to include all numbers of total score of NBS and GA to allow a 3-days variation in GA estimatio compared to the original scale which has a 2-week variation due to the application of a 5-score interval for the total NBS score and only even numbers for GA. GA by eNBS(GA-eNBS) was compared with GA by LMP(GA-LMP) and GA by standard NBS(GA-sNBS) in 133 full-term newborns. Difference between GA-LMP and GA-eNBS was analyzed for each GA. Results: Positive correlations were observed in GA-sNBS and GA-eNBS with GA-LMP. There was no difference between GA-LMP and GA-eNBS at 39GA and 40GA. At 37GA and 38GA, GA-eNBS overestimated GA-LMP up to 1 week, while underestimating up to 1 week at 41GA. Conclusions: The accuracy of eNBS was validated within 3 days of variation in GA estimation at 39-40GA. Overestimation by eNBS suggests the possible acceleration of fetal maturity in premature newborns, while underestimation, of the deceleration of fetal maturity in postterm newborns.

  • Toll-like receptors (TLRs) are the mammalian ortholog of Drosophila melanogaster protein Toll, originally identified for its involvement in embryonic development. In mammals, TLRs are mainly known for their ability to recognize pathogen- or damage-associated molecular patterns and, consequently, to initiate the immune response. However, it is becoming clear that TLRs can play a role also in mammal embryo development. We have previously described TLR4 and TLR7 expression in developing mouse peripheral nervous system and gastrointestinal tract. In the present study, we extended the investigation of TLR4 and TLR7 to the respiratory system and to the two main accessory organs of the digestive system, the liver and pancreas. TLR4 and TLR7 immunostaining was performed on mouse conceptuses collected at different stages, from E12 to E18. TLR4 and TLR7 immunoreactivity was evident in the embryo pancreas and liver at E12, while, in the respiratory apparatus, appeared at E14 and E17, respectively. Although further studies are required to elucidate the specific role of these TLRs in embryo development, the differential spatiotemporal TLR4 and TLR7 appearance may suggest that TLR expression in developing embryos is highly regulated for a possible their direct involvement in the formation of the organs and in the acquisition of immune-related features in preparation for the birth.

Effective Detective Quantum Efficiency (eDQE) Evaluation for the Influence of Focal Spot Size and Magnification on the Digital Radiography System (X-선관 초점 크기와 확대도에 따른 디지털 일반촬영 시스템의 유효검출양자효율 평가)

  • The magnification technique has recently become popular in bone radiography, mammography and other diagnostic examination. However, because of the finite size of X-ray focal spot, the magnification influences various imaging properties with resolution, noise and contrast. The purpose of study is to investigate the influence of magnification and focal spot size on digital imaging system using eDQE (effective detective quantum efficiency). Effective DQE is a metric reflecting overall system response including focal spot blur, magnification, scatter and grid response. The adult chest phantom employed in the Food and Drug Administration (FDA) was used to derive eDQE from eMTF (effective modulation transfer function), eNPS (effective noise power spectrum), scatter fraction and transmission fraction. According to results, spatial frequencies that eMTF is 10% with the magnification factor of 1.2, 1.4, 1.6, 1.8 and 2.0 are 2.76, 2.21, 1.78, 1.49 and 1.26 lp/mm respectively using small focal spot. The spatial frequencies that eMTF is 10% with the magnification factor of 1.2, 1.4, 1.6, 1.8 and 2.0 are 2.21, 1.66, 1.25, 0.93 and 0.73 lp/mm respectively using large focal spot. The eMTFs and eDQEs decreases with increasing magnification factor. Although there are no significant differences with focal spot size on eDQE (0), the eDQEs drops more sharply with large focal spot than small focal spot. The magnification imaging can enlarge the small size lesion and improve the contrast due to decrease of effective noise and scatter with air-gap effect. The enlargement of the image size can be helpful for visual detection of small image. However, focal spot blurring caused by finite size of focal spot shows more significant impact on spatial resolution than the improvement of other metrics resulted by magnification effect. Based on these results, appropriate magnification factor and focal spot size should be established to perform magnification imaging with digital radiography system.
