The Effectiveness of Early Tracheostomy (within at least 10 Days) in Cervical Spinal Cord Injury Patients

  • Objective : This study aimed to determine the optimal time for tracheostomy by evaluating the benefits and safety of early versus late tracheostomy in spinal cord injury (SCI) patients. Methods : We retrospectively reviewed a total of 254 patients with spinal cord injury. Of them, we selected 21 spinal cord injury patients who required tracheostomy due to long-term mechanical ventilation and analyzed their medical records. The patients were categorized into two groups. Early tracheostomy was performed day 1-10 from intubation in 10 patients and the late tracheostomy was performed after day 10 in 11 cases. We also evaluated the duration of mechanical ventilation, stay in the ICU and complications related to tracheostomy, the injury level of and clinical severity. All data was analyzed using SPSS 18.0/WIN. Results : The early tracheostomy offered clear advantages for shortening the total ICU stay (20.8 day vs. 38.0 day, p=0.010). There was also statistically significant reduction in the total length of time on mechanical ventilation (5.2 day vs. 29.2 day, p=0.009). However, the reductions in the incidence of pneumonia (40% vs. 82%) and the length of ICU stay post to tracheostomy (6 day vs. 15 day) were found to be statistically not significant. There were also no statistically significant differences in the injury level and clinical severity between the groups. Conclusion : We concluded that the early tracheostomy (at least 10 days) is beneficial for SCI patients who are likely to require prolonged mechanical ventilation.

LVRT Control Strategy of Grid-connected Wind Power System (계통 연계형 풍력 발전 시스템의 LVRT 제어 전략)

  • This paper proposes a LVRT (Low Voltage Ride Through) control strategy which should be satisfied by grid-connected wind power system when grid faults occur. The LVRT regulation indicates rules or actions which have to be executed according to the voltage dip ratio and the fault duration. Especially the wind power system has to support the grid with specified reactive current to secure the grid stability when voltage reduction ratio is over 10%. The LVRT regulation in this paper is based on the German Grid Code and full-scale variable speed wind power conversion system is considered for LVRT control strategy. The proposed LVRT control strategy satisfies not only LVRT regulation but also makes power balance between wind turbine and power system through additional DC link voltage regulation algorithms. Because it is impossible to control grid side power when the 3-phase to ground fault occurs, the DC link voltage is controlled by a generator side inverter using the DC link voltage control strategy. Through the simulation and experiment result, the proposed LVRT control strategy is evaluated and its effectiveness is verified.

  • The reduction of risk within the workplace has long been focus of attention both through industry initiatives and legislation. Exposure matrices according to industries and processes treated DMF (N,N-Dimethylformamide) were constructed based on KOSHA (Korean Occupational Safety and Health Agency)'s 2005 exposure database which were gathered from Korean agencies of workplace hazards evaluation for business place. These exposure matrices were assessed by danger value (DV) that was calculated from combination of hazard rating, duration of use rating, and risk probability rating of exposure to chemical hazardous agents in accordance with Hallmark Risk Assessment Tool. The results of risk assessment is divided four kinds of control bands which were related with control measures. The applicability of risk assessment using exposure matrices was performed by field study and survey for high matrices group. This study found that more attentions should be paid to two industries, manufacture of sewn wearing apparel and manufacture of textiles, among 19 industries, and to 3 processes, coating, processing & mixing, and lab, among 80 processes because those were regarded as having the highest risk.

Effects Water Stress on Physiological Traits at Various Growth Stages of Rice

  • The object of this study was to determine the difference of the time course changes of transpiration, diffusion resistance and photosynthetic rate of rice at several different growth stages subjected to soil moisture stress (SMS) and recovery by irrigation. A japonica rice cultivar 'Dongjinbyeo', was grown under flooded condition in a plastic container filled with silty loam soil. At 5 main growth stages, the container was treated by SMS until initial wilting point (IWP) and then reirrigated. The duration of SMS until IWP were the longest, 13 days for tillering stage, and the shortest, 7 days for panicle initiation and meiosis stage. The transpiration rate rapidly decreased during SMS and the transpiration rate at IWP of the stressed plant showed 10∼20% compared with control, and the transpiration rate of stressed plant at most growth stages also recovered rapidly after irrigation and then reached 100% of control within a week. The shoot photosynthetic rate in all growth stages rapidly decreased by SMS, and the rates at IWP of stressed plants were de-creased nearly to 0%, beside the treatment at tillering stage. The recovery degree of photosynthetic rate by irrigation ranged from 20 to 90%, showed higher at early growth stages of SMS treatment than that of later stages. At all growth stages the leaf diffusion resistance of stressed plants was over 3 times that of the control resulting from a rapid increase at 3 to 5 days after draining for SMS, and showed quick recovery by irrigation within 3 days after drainage. The above physiological parameters changed in close relation with the decrease of the soil matric potential after SMS. These results indicate that at all main growth stages of rice plants the transpiration and photosynthesis reduction by stomatal closure reponded sensitively to the first stage of SMS closely related with decrease of soil water potential, while those recovery pattern and recovered degree by irrigation are little different by growth stage of rice.

Effects of ${\alpha}_1-Adrenergic$ Receptor Stimulation on Intracellular $Na^+$ Activity and Twitch Force in Guinea-Pig Ventricular Muscles

  • The effects of ${\alpha}_1-adrenergic$ receptor stimulation on membrane potential, intracellular $Na^+$ activity, and twitch force were investigated in ventricular muscles from guinea-pig hearts. Action potentials, intracellular $Na^+$ activity, and twitch force of ventricular papillary muscles were measured simultaneously under various experimental conditions. Stimulation of the ${\alpha}_1-adrenergic$ receptor by phenylephrine produced variable changes in action potential duration, a slight hyperpolarization of the diastolic membrane potential, a decrease in intracellular $Na^+$ activity, and a biphasic inotropic response in which a transient negative inotropic response was followed by a sustained positive inotropic response. These changes were blocked by prazosin, an antagonist of the ${\alpha}_1-adrenergic$ receptor, but not by atenolol, an antagonist of the ${\beta}-adrenergic$ receptor. This indicates that the changes in membrane potential, intracellular $Na^+$ activity, and twitch force are mediated by stimulation of the ${\alpha}_1-adrenergic$ receptor, but not by stimulation of ${\beta}-adrenergic$ receptor. The decrease in intracellular $Na^+$ activity was not observed in quiescent muscles, depending on the rate of the action pontentials in beating muscles. The intracellular $Na^+$ activity decrease was substantially inhibited by tetrodotoxin. However, the decrease in intracellular $Na^+$ activity was not affected by an inhibition of the $Na^+-K^+$ pump. Therefore, the decrease in intracellular $Na^+$ activity mediated by the ${\alpha}_1-adrenergic$ receptor appears to be due to a reduction of $Na^+$ influx during the action potential, perhaps through tetrodotoxin sensitive $Na^+$ channels. Our study also revealed that the decrease in intracellular $Na^+$ activity might be related to the transient negative inotropic response. The intracellular $Na^+$ activity decrease could lower intracellular $Ca^{2+}$ through the $Na^+-Ca^{2+}$ exchanger and thereby produce a decline in twitch force.

Effects of Rehmanniae Radix on HPA Axis System and Catecholamienrgic System in FST (생지황(生地黃)이 강제수영부하시험에서 HPA Axis System과 Catecholaminergic System에 미치는 영향)

  • Object : The effects of Rehmanniae Radix(RR) on HPA Axis system and Catecholaminergic system was investigated. Methods : we performed the Forced Swimming Test(FST). Also the expression of Corticotropin -Releasing Factor(CRF), c-Fos and Tyrosine Hydorxylase(TH) was measured by immunohistochemical method at Paraventricular Nucleus(PVN), Locus Coeruleus(LC) and Ventral Tegmental Area(VTA). Results : 1. The duration of immobility in the Forced Swimming Test was significantly decreased in the Rehmanniae Radix 400 mg/kg groups in comparison with the control group(p<0.05). 2. Comparing to the control group, CRF expression was significantly decreased in the Rehmanniae Radix 400 mg/kg treated group in comparison with the control group(p<0.01). 3. c-Fos expression was significantly decreased at PVN in the Rehmanniae Radix 100 mg/kg treated group in comparison with the control group(p<0.01). 4. TH expression was significantly increased at LC in the Rehmanniae Radix 100 mg/kg and 400 mg/kg treated group in comparison with the control group(p<0.001). 5. TH expression was significantly decreased at VTA in the Rehmanniae Radix 100 mg/kg and 400 mg/kg treated group in comparison with the control group(p<0.001). Conclusion : According to the results, it can be considered that Rehmanniae Radix has antidepressant effect by showing the reduction of immobility in FST through the decreased expression of CRF, c-Fos in PVN and TH in VTA.

A double-blind placebo-controlled heart rate variability investigation to evaluate the quantitative effects of the organic nanoscale aeon patch on the autonomic nervous system

  • In this first double-blind-placebo-controlled investigation of the Aeon Patch, electrocardiographic data were acquired from 50 well-hydrated volunteers [21 females and 29 males, age: 19-79, weight: 117-334 lbs, height: 5'-6',3"], under 3 different conditions for a total duration of 15 min (5 min/condition). Condition1: Control (C) - while wearing no Patch, Condition 2: Placebo (P) or Active (A) - after wearing either the Placebo or the Aeon Patch for 20 min, and Condition 3: Active (A) or Placebo (P) - after wearing either the Aeon or the Placebo Patch for 20 min. There was a washout period of 15 min between Conditions 2 and 3. All involved in the investigation were blind to the (A) and (P) Patches as they looked exactly the same and were assigned in a random fashion. The hypothesis to be tested was: Wearing the Aeon Patch for 20 min reduces stress. Data were first quality assured and those subjects who showed a parasympathetic response after wearing the Patch within 20 min were identified as early responders. Thirty subjects (60%) achieved a relaxed state after wearing the Aeon Patch as early as 20 min. Statistical analysis (one-sample inference) was used to compare the spectral features of the responders. The normalized LF/HF decreased significantly ($24%{\pm}9%$ after 20 min) in condition (A) compared to condition (P) with a p-value < 0.047 (n = 30) in responders. Therefore, the hypothesis that wearing the Aeon Patch for 20 min reduces stress was accepted as true.

Estimation of Design Rainfall by the Regional Frequency Analysis using Higher Probability Weighted Moments and GIS Techniques (III) - On the Method of LH-moments and GIS Techniques - (고차확률가중모멘트법에 의한 지역화빈도분석과 GIS기법에 의한 설계강우량 추정 (III) - LH-모멘트법과 GIS 기법을 중심으로 -)

  • This study was conducted to derive the regional design rainfall by the regional frequency analysis based on the regionalization of the precipitation suggested by the first report of this project. According to the regions and consecutive durations, optimal design rainfalls were derived by the regional frequency analysis for L-moment in the second report of this project. Using the LH-moment ratios and Kolmogorov-Smirnov test, the optimal regional probability distribution was identified to be the Generalized extreme value (GEV) distribution among applied distributions. regional and at-site parameters of the GEV distribution were estimated by the linear combination of the higher probability weighted moments, LH-moment. Design rainfall using LH-moments following the consecutive duration were derived by the regional and at-site analysis using the observed and simulated data resulted from Monte Carlo techniques. Relative root-mean-square error (RRMSE), relative bias (RBIAS) and relative reduction (RR) in RRMSE for the design rainfall were computed and compared in the regional and at-site frequency analysis. Consequently, it was shown that the regional analysis can substantially more reduce the RRMSE, RBIAS and RR in RRMSE than at-site analysis in the prediction of design rainfall. Relative efficiency (RE) for an optimal order of L-moments was also computed by the methods of L, L1, L2, L3 and L4-moments for GEV distribution. It was found that the method of L-moments is more effective than the others for getting optimal design rainfall according to the regions and consecutive durations in the regional frequency analysis. Diagrams for the design rainfall derived by the regional frequency analysis using L-moments were drawn according to the regions and consecutive durations by GIS techniques.

Evaluation of Lysozyme to Control Vinification Process and Histamine Production in Rioja Wines

  • Lysozyme and the reduction of metabisulfite addition to red wine were evaluated during a winemaking process and after malolactic fermentation (MLF). Treatment, with lysozyme, of the must from Tempranillo grapes and at the end of alcoholic fermentation (AF) caused the 100% implantation of the inoculated bacterial strain and shortened the duration of MLF by 7 days. At the end of the MLF, wines treated with lysozyme showed lower volatile acidity, color intensity, and biogenic amine content. The differences in color intensity disappeared during wine stabilization. The lysozyme addition after MLF led to lower histamine concentrations in wines. These phenomena occurred irrespective of the lactic acid bacteria (LAB) proliferation control and of the Oenococcus oeni dominant strain identified at this period. The results of this study show the significance of preventive use of lysozyme in vinification of red wine to maintain low histamine levels and ensure a successful implantation of inoculated O. oeni starters.

Long Term Results of Microsurgical Dorsal Root Entry Zonotomy for Intractable Pain Associated with Brachial Plexus Injury

  • Objective : Brachial plexus injury can produce a intractable chronic neuropathic pain. This study was undertaken to assess the long term outcome of microsurgical dorsal root entry zonotomy[MDT]. Methods : Between October 1997 and December 2002, 21 patients received MDT because of a intractable pain resulting from brachial plexus injury. Of these, 19 patients were followed for more than 2 years. Fourteen of 19 patients were male and patient ages ranged from 22 to 69 years. Mean pain duration was 36.8 months and all patients had severe pain of $9{\sim}10$ visual analogue scale. To achieve complete destruction of abnormal dorsal horns, thermocoagulation of the posterolateral sulcus were performed and careful gluing was done to prevent postoperative adhesion and pain recurrence. Results : Of the 19 patients, 15 patients had excellent [>75% reduction in pain] and good [$51{\sim}75%$ pain relief] results in a average postoperative period of 4.1 years. One patient had a poor [less than 25% pain relief] result. Three patients were considered to have a fair result [$26{\sim}50%$ pain relief]. Postoperative complications were 2 transient ipsilateral ataxia and 1 CSF fistula that resolved without surgical revision. Conclusion : These results indicate that MDT provides excellent long-term pain relief in medically intractable chronic neuropathic pain following brachial plexus injury without significant complications.
