Comparison of Dietary Habit, Lifestyle, Health Attitude and Self-efficacy by Upper Gastrointestinal Disease among Group Mental Health Care Facilities with Chronic Schizophrenia (정신요양시설 만성조현병환자의 상부위장관질환 유무에 따른 식습관, 생활습관, 건강태도 및 자기효능감 비교연구)

  • The purpose of this study was to compare lifestyle, dietary habit, health attitude and self-efficacy among chronic schizophrenia with upper gastrointestinal disease and chronic schizophrenia to improve lifestyle, dietary habit and health attitude, in each mental health care facilities field, it was attempted to provide basic data to search for alternatives to increase self-efficacy. This study surveyed them from February 20th to April 30th in 2019 for data collection. Data were analyzed using Chi-square test, Independent t-test and ANCOVA with SPSS/WIN 22.0. The sociodemographic factor of age and education, the lifestyle factors of carbonated drink and physical activity intensity, the dietary habit factors of overeat, snack, late night meals and fatty food, the other factor of self-efficacy show statistically significant differences among groups. The study provides meaningful data in finding distinctive features of Upper Gastrointestinal Disease among Group with Chronic Schizophrenia. The results indicate that need to develope a variety of intervention methods to promote lifestyle and dietary habit and to enhance self-efficacy of chronic schizophrenia.

Effects of different cardiorespiratory fitness response to exercise training on cardiovascular disease and adipocytokine in abdominal obesity women (운동 트레이닝을 통한 심폐체력 반응의 차이가 복부비만 여성의 심혈관계 위험요인과 아디포싸이토카인에 미치는 영향)

  • The purpose of the study was to assess the effects of different cardiorespiratory fitness response to exercise training on cardiovascular disease and adipocytokine release and gene expression in abdominal obesity women. forty eight middle-age women were divided to a exercise-responder (n=34) group (ERG) and a exercise-nonresponder (n=14) group (ENRG) based on cardiorespiratory fitness after exercise training (12weeks, 1200 kcal/week, moderate-vigorous intensity, walking and jogging). Cardiorespiratory fitness was measured using maximal oxygen uptake with metabolic gas analysis and body composition was determined by bioelectrical impedance. We measured lipid and glucose profiles, blood adipocytokines and adipocytokine genes expression in adipose tissue. Waist girth (p=0.040), and %body fat (p=0.031) were significantly decreased in ERG than ENRG. Triglyceride (p=0.023) and systolic blood pressure (0.046) were significantly decreased in ERG than ENRG. Blood leptin (p=0.022) was significantly decrease in ERG than ENRG but leptin gene expression was decreased both groups (p<0.001). These results show that exercise-responder group improved cardiovascular disease risk factors and adipocytokine more than exercise-nonresponder group after exercise training. Moreover, exercise-nonresponders group show that obese indices and blood leptin and leptin gene expression in adipose tissue were decreased despite the failure to improve fitness. Therefore, regular exercise training seems to give health benefits although the failure to improve fitness.

Organized Hematoma in the Maxillary Sinus (상악동에 발생한 organized hematoma)

  • Organized hematoma of the maxillary sinus is a rare, nonneoplastic benign lesion with locally destructive behavior that may mimic a malignancy. Clinically, symptoms do not usually occur while the lesion remains localized to the maxillary sinus. Because there is gradual enlargement of the lesion causing erosion and displacement of the adjacent bony structures, symptoms such as epistaxis, cheek swelling, nasal obstruc-tion, headache, and exophthalmos become manifest. Radiologically, unilateral cases are much more fre-quent than bilateral, and Waters' view shows complete opacity of the expanded maxillary sinus and some masses. CT scan shows a large heterogeneous enhancing mass causing considerable expansion of the max-illary sinus with bony erosion. On MR imaging, the mass usually has a variable signal intensity on T1- and T2- weighted images, ranging from low to high. After contrast administration, discrete areas of enhance-ment are present within the mass. Although the disease is essentially benign and nonneoplastic, differen-tial diagnosis from neoplastic disease including malignancy both clinically and radiologically has been always problematic. Accurate preoperative diagnosis of organized hematoma of the maxillary sinus is important to avoid unnecessary extensive surgery, because this condition is curative with a simple, conservative surgical approach and rarely recur. Organized hematoma of the maxillary sinus should be included in the differential diagnosis when patients have recurrent epistaxis, slow-growing mass of the cheek, nasal obstruction, and expansile mass in the maxillary sinus. A 33-year-old man was referred to the department of oral and maxillofacial surgery with a three-month history of slowly growing painful swelling of the left cheek. The mass of the maxillary sinus was resected by a Caldwell-Luc approach. Histopahtoly showed only a fibous encapsulated organized hematoma. To our knowledge, organized hematoma of the maxillary sinus has not been previously described in the Korean literature of the oral and maxillofacial surgery. We report a case of organized hematoma of the maxillary sinus presenting with an enlarging maxillary sinus mass.

The Effect of Walking Exercise Program on Cardiorespiratory Function and Flexibility in Elderly Women (걷기운동프로그램이 노인여성의 심폐기능, 유연성에 미치는 효과)

  • Recently, the ratio of elderly in the population are fast growing due to socio-economical development and the better medical service. Proportionally, the health problems in elderly are increasing, too. Medical professionals must try so that the elderly have the better life through health promotion and disease prevention as well as disease treatment. This study evaluated the effect of walking exercise program on the cardiorespiratory function and the flexibility in the elderly women. The design of research was one group pretest-posttest design. The subjects were eleven elderly women over sixty years old to live in K-city, Kyonggi-do. The type of exercise was walking, which was the most popular exercise in questionnaire. The exercise intensity was 40%∼60% of the target heart-rate by Karvonen's method and maintained by the heart-rate monitor. The exercise period was five weeks and the exercise frequency was three times per week. The exercise duration was forty minutes at first and gradually increased up to an hour. In order to evaluate the effect of walking exercise, we measured VO/sub₂ max, resting heart -rate, systolic/diastolic blood pressure, FVC, FEV/sub₁, the flexibility before and after the five week's exercise program. The data are analyzed by the paired t-test and Wilcoxon signed rank test using SAS package. The results are as follows : 1) The hypothesis that cardiorespiratory function will be improved was partly supported. In VO/sub₂ max(p=0.0001), resting heart-rate(p=0.0030), systolic/diastolic blood-pressure(p=0.0387/ p=0.0024), there was significant difference. FVC and FEV/sub₁ were increased after the exercise, but there were no significant difference. 2) The hypothesis that the flexibility will be improved was supported. There was significant difference in the flexibility(p=0.0140). As the further study, it is necessary to reevaluate the effect with more refined design. We also need to try meta-analysis about the results of previous studies obtained in the experimental setting and compare our result obtained in the field setting with them.

Effect of Temperature and Shading on the Growth and Major Disease Incidence of Wasabi(Eutrema wasabi Matsum.) (온도 및 차광이 고추냉이의 생장과 주요 병해발생에 미치는 영향)

  • In order to find out the effect of temperature and shading on the growth and major disease infection of Eutrema wasabi Matsum., experiments were at growth chamber condition and field. Plant height, petiole and rhizome of wasabi were obvious decreased with increase the temperature, from which the optimum air temperature for the plant growth were estimated to be 17$^{\circ}C$. The light intensity of the treatment with 75% shading net was favorable to the length of main rhizome, rhizome weight and whole plant weight. Infection ratio of black leg and soft rot were increased at high temperature condition, however their infection ratio were decreased with increase the shading condition.

Risk factors for short term thyroid dysfunction after hematopoietic stem cell transplantation in children

  • Purpose: The purpose of this study was to evaluate short-term thyroid dysfunction and related risk factors in pediatric patients who underwent hematopoietic stem cell transplantation (HSCT) during childhood. Methods: We studied 166 patients (100 boys and 66 girls) who underwent HSCT at the Catholic HSCT Center from January 2004 through December 2009. The mean age at HSCT was $10.0{\pm}4.8$ years. Thyroid function of the patients was tested before and during 3 months of HSCT. Results: Out of 166 patients, 165 (99.4%) underwent allotransplantation. Acute graft-versus-host disease (GVHD, grades II to IV) developed in 76 patients. Conditioning regimens before HSCT include total body irradiation (n=57), busulfan (n=80), and reduced intensity (n=29). Forty-five (27.1%) had thyroid dysfunction during 3 months after HSCT (29 euthyroid sick syndrome [ESS], 6 subclinical hyperthyroidism, 4 subclinical hypothyroidism, 3 hypothyroxinemia, 2 overt hyperthyroidism, and 1 high $T_4$ syndrome). In a univariate logistic regression analysis, age at HSCT (P=0.002) and acute GVHD (P=0.009) had statistically significant relationships with thyroid dysfunction during 3 months after HSCT. Also, in a univariate logistic regression analysis, ESS (P=0.014) showed a strong statistically significant association with mortality. Conclusion: In our study 27.1% patients experienced thyroid dysfunction during 3 months after HSCT. Increase in age and acute GVHD may be risk factors for thyroid dysfunction during 3 months after HSCT. There was a significant association between ESS and mortality.

High Level Physical Activity and Prevalence of Cardiovascular Disease Using the Korea National Health and Nutrition Examination Survey Data, 2007-2013

  • Objectives: The purpose of our study was to evaluate the association between the intensity of physical activity (PA) and prevalence of cardiovascular disease (CVD) using Korean representative data. Methods: We analyzed 39 804 participant data from the Korea National Health and Nutrition Examination Survey, 2007-2013. Exposure variable was three levels of PA (low, medium, and high) in a week, and outcome variable was prevalence of CVD based on patient self-recognition and doctor's diagnosis. Complex logistic regression analysis was performed to evaluate the relationship between level of PA and CVD adjusted by body mass index, hypertension, hypercholesterolemia, diabetes mellitus, stress recognition, household income, smoking, and current drinking. The indices of association w ere estimated as crude prevalence odds ratio (POR), adjusted POR, and their 95% confidence interval (CI). All statistical analyzes were performed using complex sample analysis procedure of the SPSS version 23.0. Results: When all variables were adjusted, only high level PA in women showed a significant association with stroke (adjusted POR by patient's self-recognition, 0.57; 95% CI, 0.32 to 0.99, adjusted POR by doctor's diagnosis, 0.55; 95% CI, 0.34 to 0.87) and CVD (adjusted POR by doctor's diagnosis, 0.68; 95% CI, 0.48 to 0.96). Conclusions: High level PA in women has a significant reverse association with prevalence of stroke and CVD in Korea. Further study for elucidating the mechanism will be needed.

Usefulness of Twinkling Artifacts in Color Doppler Ultrasonography (컬러 도플러 초음파에서 Twinkling artifacts의 유용성)

  • The aim of the study was to investigate the diagnostic value of the color Doppler twinkling artifact in disease of urinary system. The intensity of twinkling artifact(TA) with color Doppler was classified into 3 levels, 0(non-TA) to 3(distinct TA). In the in vitro study, acorn jelly with various materials on top was examined using color Doppler at B-mode sonography in a water bath for TA. 31 patients with diagnosis of urinary calculi(renal stones 16, urinary stones 15) based on B-mode sonography were studied in vivo for TA. The materials with rough surfaces such as salt, screw and cubics at B-mode sonography with color Doppler contributed to causing TA. At B-mode sonography without color Doppler 37% of renal stones and 60% of ureter stones were detected. but at B-mode sonography with color Doppler TA was demonstrated for all cases. Superficial roughness of materials affected occurrence of TA at B-mode sonography with color Doppler. Therefore, TA at B-mode sonography without color Doppler could play a role in confident diagnosis of the disease of urinary system.

Evaluation of the Therapeutic Effects in Pain Management Using Infrared Thermal Imaging (적외선 체열촬영을 이용한 통증 치료효과의 평가)

  • Background: Infrared Thermal Imaging (ITI) is an effective tool for the diagnosis of disease and evaluation of the therapeutic effects following pain treatment. Patients who were treated for pain in pain clinic described the intensity of pain and the degree of change of their pain using a visual analogue scale (VAS). In this study, the usefulness of ITI following multimodal methods for pain management were compared with the change of VAS. Methods: 1119 patients were evaluated. The patients were treated with stellate ganglion block, epidural block or trigger points injection. Before treatment, the temperature difference (${\Delta}T$) of the involved area and the corresponding area on the opposite side of the body was measured using ITI and VAS was assessed. After treatment, the temperature difference (${\Delta}T$) between the normal and involved areas, the change of ${\Delta}T$ (${\Delta}dT$), VAS and the change of VAS (${\Delta}VAS$) were measured. Statistic correlations between ${\Delta}dT$and ${\Delta}VAS$ were calculated in all groups. Results: Correlation of the ${\Delta}dT$ and ${\Delta}VAS$ was significant by contingency coefficient test. (SGB group, C = 0.358, Epi group, C = 0.377, TPI group, C = 0.374, P < 0.05) Conclusions: ITI is a reliable tool for the assessment of therapeutic effects following multidimensional management of painful disease.

Bleomycin, Etoposide and Cisplatinum (BEP) Chemotherapy for Metastatic Germ Cell Tumours: Treatment Outcomes at UKM Medical Centre, Malaysia

  • Introduction: Although bleomycin/etoposide/cisplatinum (BEP) chemotherapy is established as the standard treatment for germ cell tumours, it requires significant experience in administration and toxicity management to maintain optimal dose intensity. A retrospective review of 30 patients was conducted at UKMMC to study treatment outcomes. Methods & Materials: Patients with GCTs and treated with at least two cycles of BEP chemotherapy between January 2003 and Oct 2009 were eligible for this study. Patients received 4-6 cycles of bleomycin 30,000IU IV D1, D8 & D15 and either etoposide $100mg/m^2$ IV D1-D5 and cisplatin $20mg/m^2$ IV D1-D5 (5 day BEP regimen) or etoposide $165mg/m^2$ D1-D3 and cisplatin $50mg/m^2$ D1-3 (3 day BEP regimen) every three weeks per cycle. All patients received prophylactic granulocyte colony-stimulating factor (GCSF) from days 6 to 10 of each cycle. The overall response rates, 2 year progression-free survival and overall survival of the whole cohort were assessed. Results: Thirty patients fulfilled the inclusion criteria. Non-seminomatous GCTs comprised 93.3% of cases and gonadal and mediastinal primary sites were the most common. Sixty percent were classified as IGCCCG poor risk disease. Median follow-up was 26.6 months. The overall response rate (CR+PR) was 70%. The two year PFS and OS were 70% and 66%. There was a significant difference in terms of the overall response rate (85% vs 40%, p = 0.03) and in PFS (94.7% vs 50%, p = 0.003) between gonadal and extragonadal primary sites. Conclusion: It is possible to achieve outcomes similar to those in international clinical trials with close monitoring and good supportive care of patients undergoing BEP chemotherapy. There is a strong argument for patients with IGCCCG poor prognosis disease to be treated in specialist tertiary centres to optimize treatment outcomes.
