Single Cell Transcriptomic Re-analysis of Immune Cells in Bronchoalveolar Lavage Fluids Reveals the Correlation of B Cell Characteristics and Disease Severity of Patients with SARS-CoV-2 Infection

  • The coronavirus disease 2019 (COVID-19) pandemic (severe acute respiratory syndrome coronavirus 2) is a global infectious disease with rapid spread. Some patients have severe symptoms and clinical signs caused by an excessive inflammatory response, which increases the risk of mortality. In this study, we reanalyzed scRNA-seq data of cells from bronchoalveolar lavage fluids of patients with COVID-19 with mild and severe symptoms, focusing on Ab-producing cells. In patients with severe disease, B cells seemed to be more activated and expressed more immunoglobulin genes compared with cells from patients with mild disease, and macrophages expressed higher levels of the TNF superfamily member B-cell activating factor but not of APRIL (a proliferation-inducing ligand). In addition, macrophages from patients with severe disease had increased pro-inflammatory features and pathways associated with Fc receptor-mediated signaling, compared with patients with mild disease. CCR2-positive plasma cells accumulated in patients with severe disease, probably because of increased CCL2 expression on macrophages from patients with severe disease. Together, these results support the hypothesis that different characteristics of B cells might be associated with the severity of COVID-19 infection.

  • The main current in the pathology of oriental medicine is composed of etiologic factor, pathogenesis and clinical manifestation. The access of a disease is consist of etiologic factor, location of the lesion, nature of the disease and patient's condition. The nature of disease and the property of a drug are inseparably related to each other. The nature of disease is composed of six exogenous factors, cold and heat, deficiendy and excess, Yinyang and pain. Cold nature is divided into cold symptom due to excess and asthenia cold, fever nature is divided into sthenic fever and asthenic fever. According to the location, cold and heat can be subdivided into heat in the upper and cold in the lower, cold in the upper and heat in the lower, exterior cold and interior heat, exterior heat and interior cold. Yin syndrome characterized by hypofunction of the viscera is generated from insufficiency of yang-qi, excess of yin-coldness, deficiency of both qi and the blood.

The Utilization of Western and Oriental Medical Services by Outpatients with Musculoskeletal System Disorders and Its Related Factors (근골격계질환자의 양.한방 외래 의료이용과 관련요인)

  • This study is to find out the correlation among the predisposing, enabling, and need factors in Anderson Behavioral Model using the data from Korea Medical Panel Survey conducted in the early part (April 1 - October 31) of 2008. The findings are as follows. It was found that the utilization rate of western medical service was far higher. the influential factor to choose western or oriental medical service taking western medical institutions as the reference group, the influential factor to choose oriental medical institution has significantly increased when the patient who have covered by medical insurance has one accompanied disease and their age was between 45 - 74, compared to the people less than 45 years old. It also increased when the age of the patient was between 45-54 years old, and in the event those who are not covered by medical insurance have accompanied disease and that the disease mobility period is 2-4 years. reviewing the several characteristics of the utilization of western and oriental medical services by the patient with musculoskeletal system disorders, the number of accompanied disease is an influential factor for the utilization of oriental medical services. And, disease mobility period is a significant factor for the utilization of both western and oriental medical services together, though it is not identified in this study. Therefore, it is expected that mutual cooperation between western and oriental medical services is more required for the patient with musculoskeletal system disorders as the aging society rapidly develops. In order to foster oriental medicine, it is required to specialize in competitive disease such as musculoskeletal system disorders.

Quotations from the ${\ulcorner}$Linzheng zhinan yian${\lrcorner}$ in the ${\ulcorner}$Wenbing tiaobian${\lrcorner}$: How were they transformed and corrected? ("온병조변"에서 "임증지남의안" 의 온병관련 의안을 변형하고 첨삭한 방법)

  • The Wenbing tiaobian(온병조변) is the first systematic disease monograph on warm factor disease(온병), written by Wu Jutong(오국통). It was most influenced by the Linzheng zhinan yian(임증지남의안), which was a book of gathering medical case records of Ye tianshi(엽천사). Therefore, there are plenty of quotations from this book in the Wenbing tiaobian. This study investigates the quotations from Linzheng zhinan yian, especially focusing on the way of transformation and correction of the original medical case records on the warm factor disease. The results are as follows: About 104 of 265 provisions in the Wenbing tiaobian were directly quoted from the Linzheng zhinan yian. The provisions quoted from Ye's case records were rearranged according to the categories of the triple burners pattern differentiation(삼초변증) and the causes of warm factor disease. And eve case record was transformed into more general descriptive form in order to put it into the book. For example, the specific figures, and some patients' peculiar symptoms, causes, sex and disease names were omitted. On the other hand, the tongue moss, pulse shape and some symptoms, which were necessary for differentiating patterns, were added. In the case of the formula, some formulas originated from Ye's case records were named newly. And the dose of each herb consisting a formula was determined, and therapeutic principle, taking method and detailed explanation was added to every formula.

Development of a Cardiovascular Disease Resilience Scale (심혈관질환용 회복력(Cardiovascular Disease Resilience) 측정도구 개발 및 평가)

  • Purpose: The purpose of this study was to develop a Cardiovascular Disease Resilience (CDR) scale to evaluate disease specific resilience for recovery. Methods: The study was conducted as follows: items generation, and test of validity and reliability. Items were developed via literature review, review of instruments, and data acquired from the interviews. In order to test validity and reliability, seven panels of experts reviewed the preliminary questionnaire and then data were collected from 550 cardiovascular disease patients. Factor analysis, Pearson correlation, ANOVA, and Cronbach's alpha were used to analyze the data. Results: In the preliminary stage, forty-four items were generated. A reduction to 40 items was accomplished through content validity analysis. Factor analysis extracted 7 factors with a total of 25 items. The CDR items were moderately correlated with the subscales of the CD-RISC (Connor-Davidson Resilience Scale) and the mean score of CDR was associated with quality of life measured with CD-QOL (Cardiovascular Disease Quality of Life). Cronbach's ${\alpha}$=.84. Conclusion: Content validity, construct validity, criterion validity, and reliability of the CDR were established. The CDR is a reliable and valid instrument which the resilience of cardiovascular disease specific recovery state can be evaluated.

Necrotic enteritis in a captive mute swan (Cygnus olor) in the breeding season

  • A captive mute swan (Cygnus olor) in the brooding period was found dead without any specific signs. On necropsy, the entire digestive tract was severely necrotized with loosely adherent pseudomembranes, and Clostridium perfringens (CP) colonies isolated from the intestinal contents were identified as CP type A by polymerase chain reaction. Microscopically, the intestine and the liver were necrotized and intralesional bacterial clusters were observed. In this case, there were no other mortalities, so the condition of a single bird, in particular, stress from incubation effort, could act as the predisposing factor for necrotic enteritis. In this study, we report a unique case of necrotic enteritis associated with incubation in a captive mute swan.

Association between Periodontitis and Preeclampsia: a Systematic Review (임신기 치주질환과 자간전증 발생간의 연관성)

  • Purpose : Preeclampsia(PE) is a pregnancy-specific disease which is characterized by hypertension and proteinuria. This disease occurs in about 2-8 % of pregnancies in developing countries and remains among the major causes of maternal and neonatal mortality and morbidity throughout the world. Although the causes of PE are not fully understood, the infection has been considered as the main risk factor for this diseases. Periodontal disease may provide a chronic burden of endotoxin and inflammatory cytokines and the disease has been considered as risk factors of systemic illnesses including cardiovascular disease, atherosclerosis, and cerebrovascular ischemia. So, studies performed over the last 15 years have suggested that periodontal disease may be associated with adverse pregnancy outcomes such as PE. However, this association has not been found in all populations. The aim of this review article was to evaluate whether periodontal status and the presence of specific periodontal pathogens may influence the incidence of PE. Methods : Many research articles searched at the electronic databases(MEDLINE; 2000 to July 2017) including search term as periodontal disease and preeclampsia. Result : There were 10 case-control studies and 5 cohort studies meeting our inclusion criteria. The results showed that maternal periodontitis (adjusted odds ratio: 1.5 to 9.3) was associated with preeclampsia in 15 epidemiological studies. Conclusion : It is clear that maternal periodontitis is a risk factor associated with preeclampsia, emphasizing the importance of periodontal care in prenatal programs.

Effects of Tissue Factor, PAR-2 and MMP-9 Expression on Human Breast Cancer Cell Line MCF-7 Invasion

  • Objective: This study aimed to explore the expression of tissue factor (TF), protease activated receptor-2 (PAR-2), and matrix metalloproteinase-9 (MMP-9) in the MCF-7 breast cancer cell line and influence on invasiveness. Methods: Stable MCF-7 cells transfected with TF cDNA and with TF ShRNA were established. TF, PAR-2, and MMP-9 protein expression was analyzed using indirect immunofluorescence and invasiveness was evaluated using a cell invasion test. Effects of an exogenous PAR-2 agonist were also examined. Results: TF protein expression significantly differed between the TF cDNA and TF ShRNA groups. MMP-9 protein expression was significantly correlated with TF protein expression, but PAR-2 protein expression was unaffected. The PAR-2 agonist significantly enhanced MMP-9 expression and slightly increased TF and PAR-2 expression in the TF ShRNA group, but did not significantly affect protein expression in MCF-7 cells transfected with TF cDNA. TF and MMP-9 expression was positively correlated with the invasiveness of tumor cells. Conclusion: TF, PAR-2, and MMP-9 affect invasiveness of MCF-7 cells. TF may increase MMP-9 expression by activating PAR-2.

Physical and Psychological Factors Affecting Fall in Elderly Patients with Parkinson's disease (파킨슨병 노인의 낙상에 영향을 미치는 신체적, 심리적 요인)

  • Elderly people with Parkinson's disease have higher rates of physical and mental risk factors for falls than non-Parkinson's disease elderly. The purpose of this study is to investigate this by using national data that includes the entire population of the elderly in Korea. As a secondary analysis study using data survey on the elderly by the Ministry of Health and Welfare in 2017, there were a total of 103 elderly people with Parkinson's disease, and a total of 96 subjects were analyzed excluding missing values. In the elderly with Parkinson's disease, the factor most influencing the fall was IADL, and IADL is related to motor control function. Decreased motor control limits physical movements essential for daily life, and even affects self-protective behavior in emergency situations, affecting falls. Based on the research results that IADL can affect falls, various exercise therapies for fall prevention interventions in the elderly with Parkinson's disease can be suggested.
