Korean Aspect of the Medical Problems of the Aged (노인의료문제의 한국적 측면)

  • As the result of a significant improvement in the economic situation and development of scientific techniques in Korea during the last 20 years, the life expectancy of the Korean people has lengthened considerably and as a result the number of old aged people has markedly increased. Such an increase of the number of aged people brought about many problems. Authors would like to take a medical look at the "Problem of old aged people" presently facing us in Korea. Currently the number of people over 65 has increased rapidly and is 1,620,000, 4% of total population. But it is still much lower than 8.9% in Japan, 10.7% in U.S.A., and 14.9% in the United Kingdom. Over 25% of these aged people were found to have at least more than one disease which requires medical care. Diseases occur in the circulatory system, 30.9%, respiratory system, 17.1%, digestive system 8.6%, mental disorders, 8.4%, malignant neoplasms, 7.0%. About 51% of the aged over 65 are under medical security benefit, mostly with partial coverage plan. Their clinic visit rate was very low (2.0% in 1981), which might be due to financial reasons. Since diseases affecting the aged progress chronically, early detection and long term care are utter most important. However there is almost no special facility, long term care center or geriatric specialist. For proper management of medical problems in the growing population of the Korean eldery expansion of medical security coverage, greater number of specialized facilities, education of geriatric special manpower and efficient operating system should be established.

Generating Motion- and Distortion-Free Local Field Map Using 3D Ultrashort TE MRI: Comparison with T2* Mapping

  • Purpose: To generate phase images with free of motion-induced artifact and susceptibility-induced distortion using 3D radial ultrashort TE (UTE) MRI. Materials and Methods: The field map was theoretically derived by solving Laplace's equation with appropriate boundary conditions, and used to simulate the image distortion in conventional spin-warp MRI. Manufacturer's 3D radial imaging sequence was modified to acquire maximum number of radial spokes in a given time, by removing the spoiler gradient and sampling during both rampup and rampdown gradient. Spoke direction randomly jumps so that a readout gradient acts as a spoiling gradient for the previous spoke. The custom raw data was reconstructed using a homemade image reconstruction software, which is programmed using Python language. The method was applied to a phantom and in-vivo human brain and abdomen. The performance of UTE was compared with 3D GRE for phase mapping. Local phase mapping was compared with T2* mapping using UTE. Results: The phase map using UTE mimics true field-map, which was theoretically calculated, while that using 3D GRE revealed both motion-induced artifact and geometric distortion. Motion-free imaging is particularly crucial for application of phase mapping for abdomen MRI, which typically requires multiple breathold acquisitions. The air pockets, which are caught within the digestive pathway, induce spatially varying and large background field. T2* map, that was calculated using UTE data, suffers from non-uniform T2* value due to this background field, while does not appear in the local phase map of UTE data. Conclusion: Phase map generated using UTE mimicked the true field map even when non-zero susceptibility objects were present. Phase map generated by 3D GRE did not accurately mimic the true field map when non-zero susceptibility objects were present due to the significant field distortion as theoretically calculated. Nonetheless, UTE allows for phase maps to be free of susceptibility-induced distortion without the use of any post-processing protocols.

Effect of tannin content in sorghum on digestive enzymes (수수의 탄닌 함량이 소화효소에 미치는 영향)

  • We evaluated the distribution pattern of tannin in 164 sorghum breeding lines and the inhibition rates of amylase, protease, and lipase in sorghum lines with different tannin concentrations. Tannin was existed in the testa of sorghum grain. The tannin content in whole grain of Nampungchal sorghum was 11.54 mg/g, and that in grain (milling rate 73%) and bran fractions was 4.57 mg/g and 28.71 mg/g, respectively. The inhibition rate of ${\alpha}$-amylase, ${\alpha}$-glucosidase, and ${\beta}$-glucosidase in sorghum lines with tannin was higher than that in sorghum lines without tannin. The inhibition rate of ${\alpha}$-glucosidase was greater than 97% in sorghum lines with tannin. The inhibition rate of protease ranged from 20% to 70% in the sorghum lines, showing no discernable trends in tannin content. Lipase inhibition was either very low or not observed and did not seem to correlate with tannin concentration.

Study on the Acupoints Use in Acupuncture & Moxibustion Textbook (침구학 교재에서 활용된 경혈의 분석 연구)

  • Objectives : There has been a need for developing and establishing operational curriculum for the education of acupuncture, but defining the level and step of the acupuncture education in clinical perspectives was not thoroughly recognized so far. Methods : We analysed the usage of acupuncture points in the official textbook to recognize the most frequently used acupoints in clinical medicine. It was found that $ST_{36}$ $LI_4$ $SP_6$ $HT_7$ $LR_3$ $CV_{12}$ $BL_{23}$ $CV_6$ $BL_{20}$ $CV_4$ $LI_{11}$ $PC_6$ $KI_3$ $GB_{20}$ $GV_{20}$ $GB_{34}$ $BL_{18}$ $GV_{14}$ $BL_{17}$ $BL_{40}$ are the most frequently used 20 acupuncture points and GB(足少陽擔經), CV(任脈), ST(足陽明胃經) are the most frequently used meridians. Results : The $ST_{36}$, $GB_{34}$, $LI_{11}$ and GB(足太陽膀胱經) meridian are most frequently used for muskuloskeletal disease, $ST_{36}$, $LI_4$, $LI_{11}$ and GB(足太陽膀胱經) meridian are most frequently used for neuromuskular disease, and $ST_{36}$, $CV_{12}$, $BL_{20}$ and CV(任脈) meridian are most frequently used for the digestive system disorders. Conclusions : This study was the first systematic approach to get essential acupuncture points for the education of clinical perspectives of TKM especially for the acupuncture and moxibustion. We found that the $ST_{36}$, $GB_{34}$, $LI_{11}$ and GB(足太陽膀胱經) are the most frequently used acupuncture point and meridian. This study will be used for the development of TKM clinical curriculum.

Preparation of Fragrant Microcapsule for Reducing Stress (긴장완화를 위한 향기나는 마이크로캡슐의 제조)

  • A fragrant microcapsule was prepared for use by students to reduce the stress of taking examinations. Rosmarinic acid was used as a fragrant oil which had the effect of relaxing stress, polycaprolactone (PCL) was used as a capsule wall material, and poly(vinyl alcohol) (PVA) as a stabilizer. The solvent evaporation method was used to form the microcapsule. The microcapsules were prepared by changing the stirring rate, the concentration of the stabilizer, and the molecular weight of PCL. The shape of the microcapsule was characterized by scanning electron microscopy (SEM). The size of the microcapsule was reduced by increasing the stirring speed. The release rate of rosmarinic acid was decreased when the higher molecular weight PCL was used. When the prepared microcapsule was tested in an aromatherapy class, the microencapsulated fragrant oil had a longer release time than the original fragrant oil. The study data showed that this fragrant oil was effective for increasing concentration ability, reducing stress, increasing digestive power, and increasing memory for the students.

Acute Oral Toxicity of Atractylodes macrocepala KOIDZ. (Atractylodes macrocepala KOIDZ.(백출) 추출물의 급성 경구투여 독성 연구)

  • Atractylodes macrocepala KOIDZ. (AmK) is a herbal medicine and resources of functional food which has been used for the treatment of indigestion, anorexia, diarrhea and digestive dysfunction. Recently AmK is frequently used as resources of functional food and whitening cosmetics. In this study was carried out to evaluate the acute oral toxicity of Amk in Sprague-Dawley(SD) rats. male and female rats were administered orally with Amk extract of 1,000 mg/kg (low dosage group), 2,000 mg/kg (middle dosage group) and 4,000 mg/kg (high dosage group). We daily observed number of deaths, clinical signs and gross findings for 7 days. No dead SD rats and no clinical signs were found during the experiment period. Also other specific changes were not found between control and treated groups in hematology and serum biochemistry. But we found out feeble histopathological changes in liver fat tissues. In addition no significant changes of gross bady and individual organs weight. These results suggest that water soluble extract of AmK has not acute oral toxicity and oral $LD_{50}$ value was over 4,000 mg/kg in SD rats.

Enhanced Extraction of Bioactive Compounds from Bee Pollen by Wet-grinding Technology (벌 화분에서 습식 나노화 공정에 의한 유효성분의 추출)

  • Bee pollen is produced by honeybees and is considered one of the most balanced and nourishing nutritional supplements available. Historically, bee pollen has been prescribed for its healing properties and consumed for its high-energy supply. Recent research has provided evidence that bee pollen has diverse biological activities, such as anti-oxidant, anti-inflammatory, anti-bacterial, and even anti-cancer effects. However, the outer membrane of the pollen grain, exine, is highly resistant to most acidic solutions, high pressure, and even digestive enzymes, and the resulting low bioavailability limits its nutritional and clinical applications. This study applied a wet-grinding method to destroy the exine effectively, and it then examined the pollen's enhanced biological activity. First, microscopic observations provided strong evidence that wet grinding destroyed the exine time-dependently. In addition, the content of polyphenols, well-known ingredients of bee pollen and used as internal standards for the quality control of commercial pollen preparations, increased up to 11-fold with wet grinding. Further, the anti-oxidant activity demonstrated on the ABTS anti-oxidant assay, as well as the DPPH radical scavenging assay, was also dramatically increased. Together, the results presented here support a new technology by which bee pollen can be used as a resource for medical, nutritional, and cosmetic applications.

Regulation of Matrix Metalloproteinase-1 Expression by the Homeodomain Transcription Factor Caudal in Drosophila Intestine (초파리 장조직에서 Caudal 전사조절인자에 의한 matrix metalloproteinase-1 발현 조절)

  • The matrix metalloproteinase (MMP) family plays essential roles in physiological processes such as embryonic development, angiogenesis, wound healing, and tissue homeostasis as a consequence of MMPr capacity for breaking down many types of extracellular matrix proteins. Imbalanced regulation of MMP expression can also lead to pathological conditions such as tumor progression. We recently reported that the Drosophila Mmp1 gene is highly expressed in the digestive tract and is required for the maintenance of intestinal homeostasis such as by restriction of uncontrolled intestinal stem cell proliferation. However, the regulatory mechanisms of MMP gene expression in the intestine remain unclear. In this study, we determined that the expression of Mmp1 is regulated by the homeodomain transcription factor Caudal. Experiments using the targeted expression of Caudal under the regulation of Gal4-UAS system indicated that endogenous Caudal is required for the Mmp1 gene expression in the adult Drosophila intestine and that exogenous Caudal induces Mmp1 expression. Transient transfection experiments indicated that Caudal can activate the promoter activity of Mmp1 and that several putative Caudal binding sites in the 5'-flanking region of the Mmp1 gene may be critical to the upregulation by Caudal. Our data suggest that Mmp1 is one of the target genes of Caudal in physiological normal condition and in tumorigenesis.

A Comparison of Lovastatin and Simvastatin in Treatment of Hyperlipidemia (Lovastatin과 Simvastatin의 고지혈증 치료 비교)

  • Hypercholesterolemia is one of main causes of coronary heart disease(CHD). Clinical trials demonstrated that lowering serum cholesterol levels would reduce incidence of new cardiovascular events and mortality by primary or secondary preventions. The objective of this retrospective study was to compare efficacy and side effects of lovartatin and simvastatin in treatement of hypercholesterolemia. In Boramae Hospital, patients were included when they have taken lovastatin 20 mg or simvastatin 10 mg for 52 weeks with laboratory monitoring for cholesterol at baseline, 3, 6 and 12 month period. As results, total 128 outpatients were included with their total cholesterol level <240 mg/dl and triglyceride level <400 mg/dl at baseline. Total cholesterol and LDL cholesterol of lovastatin group (n=60) and simvastatin group (n=68) were significantly reduced from baseline (p=0.001). Lovastatin maximally reduced total cholesterol by $23.9\%,\;triglyceride\;by\;12.3\%$, LDL cholesterol by $36.1\;\%$ and increased HDL cholerterol by $7.8\%$ and simvastatin reduced by $24.1\%,\;20.5\%,\;34.3\%\;respectively$ and HDL increased by $11.2\%$. There were no significant differences between lovastatin and simvastatin in mean percent change of lipid levels at 12, 24 and 52 weeks from baseline. Cumulative percentage of patients reaching the target LDL cholesterol concentration by 24 weeks was $61.7\%$ in lovastatin and $64.7\%$ in simvastatin. Average time to reach the target LDL goal was 100.1 days in lovastatin and 99.8 days in simvastatin. Both lovastatin and simvastatin also significantly reduced total cholesterol and LDL cholesterol in all subgroups (diabetes mellitus, hypertension, and coronary heart disease). In this study, treatment efficacy in patients with coronary heart disease was lower than other patients. Considering clinical importance of secondary prevention, more intensive treatment is necessary to decrease LDL cholesterol level of 100 mg/dl or lower in patients with coronary heart disease or other clinical atherosclerotic disease. There were no serious side effects during the study period. Digestive side effects were most frequently reported (lovastatin $8.3\%\;vs\;simvastatin\;8.8\%$). In conclusion, both lovastatin and simvastatin were similar in lipid lowering effects and there was no difference in incidence of side effects.

Effects of Phyto-Extract Mixture on Adiposity and Serum Lipid Levels in Obese Mice Induced by High Fat Diet (고지방식이로 유도된 비만쥐에서 식물혼합 추출물의 체지방 형성 및 혈청 지질에 미치는 영향)

  • In this study, we investigated the inhibition of phyto-extract mixture (PEM) in several digestive enzymes ($\alpha$-amylase, $\alpha$-glucosidase and lipase) for anti-obesity. The current study also examined the effects of PEM on adiposity and serum lipid levels in obese mice fed with high fat diet. ICR male mice weighing $33{\pm}1.1\;g$ were randomly divided into three groups, one normal diet group (control, ND group) and two high fat diet groups with or without PEM supplement (HFD group and PEM group). The mice were fed the PEM experimental for 6 weeks and then they were sacrificed. The results showed that the final weight, weight gain, food efficiency ratio and body fat were decreased by the addition of PEM compared to those of HFD group. White adipose tissue weights of epididymal, mesenteric and retroperitoneal areas in the PEM group were reduced to 31.2%, 8.8%, and 37.8%, respectively, compared to the HFD group. The levels of serum triglyceride, total cholesterol, LDL-cholesterol in the PEM group were significantly lower than those of HFD group. The body weight gain and food efficiency ratio of PEM group were significantly lower compared with those of HFD group. From the above results, the PEM may be effective material for anti-obesity through reducing serum triglyceride and body fats as well as decreasing body weight.
