The purpose of this study is to analyze the effectiveness of policies in response to low fertility in Korea. Using a recent nationwide survey on marriage and fertility, this study analyzes the effect of low fertility policies on plan of further birth among young married women according to the number of their children. Awareness of polices in response to low fertility is relatively high, showing the great differences among policy measures. In contrast, the degree of polices in response to low fertility is very low. ranging from 2.0% to 27.6%. The distribution of awareness and experience is different according to the number of children. The results of logistic regression analysis show that awareness of polices in response to low fertility such as economic support, promotion of equality of the sexes significantly affects on plan of further birth. However, this effects disappear when we apply parity specific approach. For example, the plan for first child is quite universal as no difference is found by their awareness of policies in response to low fertility. Similar results are found in married women having plan for second child and third child. With respect to the degree of experience of policies in response to low fertility, the results show that economic support, work-family coexistence, and promotion of equality of the sexes significantly affects the plan of further birth. Like awareness of policies in response to low fertility, these effects disappear as we apply parity specific approach. The results indicate that the effect of policies in response to low fertility differs by the current number of children. Thus, consideration of such difference would help us deepen our understanding of fertility behaviors and polices in response to low fertility.
Background: In the field of cancer, the ICD-10 coding convention is based on the site of a neoplasm in the body and usually ignores the morphology, thus the same code may be assigned to tumors of different morphologic types in an organ. Nowadays, all general (provincial) and center hospitals in Thailand are equipped with the hospital information system (HIS) database. Objective: This study aimed to find the characteristics and magnitude of agreement represented by the positive predictive value (PPV) of provisional cancer diagnoses in the HIS database in Pattani Hospital in Thailand in comparison with the final cancer diagnosis of the ICD-10 codes generated from a well established cancer registry in Songklanagarind Hospital, the medical school hospital of Prince of Songkla University. Materials and Methods: Data on cancer patients residing in Pattani province who visited Pattani Hospital from January 2007 to May 2011 were obtained from the HIS database. The ICD-10 codes of the HIS computer database of Pattani Hospital were compared against the ICD-10 codes of the same person recorded in the hospital-based cancer registry of Songklanagarind Hospital. The degree of agreement or positive predictive value (PPV) was calculated for each sex and for both sexes combined. Results: A total of 313 cases (15.9%) could be matched in the two databases. Some 222 cases, 109 males and 113 females, fulfilled the criteria of referral from Pattani to Songklanagarind Hospitals. Of 109 male cancer cases, 76 had the same ICD-10 codes in both hospitals, thus, the PPV was 69.7% (95%CI: 60.2-78.2%). Agreement in 76 out of 113 females gave a PPV of 67.3% (95%CI: 57.8-75.8%). The two percentages were found non-significant with Fisher's exact p-value of 0.773. The PPV for combined cases of both sexes was 68.5% (95%CI: 61.9-74.5%). Conclusions: Changes in final diagnosis in the referral system are common, thus the summary statistics of a hospital without full investigation facilities must be used with care, as the statistics are biased towards simple diseases able to be investigated by available facilities. A systematic feedback of patient information from a tertiary to a referring hospital should be considered to increase the accuracy of statistics and to improve the comprehensive care of cancer patients.
This study was performed to identify the difference of the area-based deprivation and the educational level on the cerebrovascular mortality in Korea. Data used in this study was obtained from the Death Certificate Data 2000 and the 2000 Census produced by Korean National Statistics(NSO). We classified the whole country into 246 areas based on the administrative districts. Then, the Standardized Mortality Ratio (SMR) in cerebrovascular disease was calculated according to the sex, education level and 246 areas. Its Predicted SMR was calculated by the Empirical Bayes Methods to reduce the variation of the SMR values. The area-based deprivation of 246 areas were measured using the modified Carstairs index in which the 5 indicators consisted of overcrowding, the unemployment ratio of men, the percentage of households classified low social class, the percentage of non home owners, and finally those houses lacking basic amenities. The correlation between the area-based deprivation and the SMR of the whole country and the correlation between the area-based deprivation and the SMR of each metropolitan cities or provinces was analyzed by the Pearson correlation analysis method. After classifying the deprivation of 246 areas into 5 levels, we performed the random intercept Poisson regression analysis after adjusting education level and age using Empirical Bayes Method to investigate the relationship between the 5 deprivation levels and the cerebrovascular mortality. The SMR was increased in lower education level. Each 246 areas had different values in SMR, Predicted SMR and area-based deprivation. The area-based deprivation and the SMR of the whole country was not correlated in both sexes. The education level of an individual was associated the risk of cerebrovascular mortality in men. The risk of cerebrovascular mortality increased with age compared to the reference(<30). The area-based deprivation was not associated with the risk of cerebrovascular in both sexes. The findings of this study suggest that the SMR had positive and negative correlations with area-based deprivation depending on the metropolitan cities or province. It also suggests that the individual education level and age were related with mortality and finally that the area-based deprivation was not associated to the cerebrovascular mortality in Korea.
Attractiveness of stink bugs to various colors, heights and locations of fish-net traps incorporated with aggregation pheromones was determined. Bean bug, Riptortus pedestris Fabricius, was most attracted to yellow color trap, followed by white, black, green, blue and brown. R. pedestris and one-banded stink bug, Piezodorus hybneri Gmelin, were most attracted to fish-net traps placed 80 cm above the ground regardless of sexes of two species of stink bugs. Between the sexes, however, R. pedestris females were more attracted to 40 and 80 cm above the ground than the males at those heights although the attraction was not significantly different at 120 and 160 cm heights. P. hybneri females tended to be attracted to 40 and 80 cm height traps but the attraction was the opposite at 120 and 160 cm heights. The highest attraction of R. pedestris was observed in a perilla field with no significant difference in the soybean field and border area in which no crop were cultivated between the perilla and soybean fields. R. pedestris was also more attracted to the sweet persimmon orchard than the soybean and medicinal crops fields, which was a significant difference between the two fields.
Background and Objectives: In patients with atrial fibrillation (AF), females taking vitamin K antagonist are at higher risk of stroke or systemic embolism (SSE), bleeding and all-cause death than males. This study investigated the relationship between sex and adverse clinical events in a contemporary AF patient cohort taking anticoagulation. Methods: This prospective multicenter AF registry study comprised 6,067 patients with AF (mean age, 70±9 years; men, 59%) with intermediate to high risk of stroke (CHA2DS2-VAscore ≥1) and receiving oral anticoagulation therapy. Adverse clinical outcomes, including SSE, bleeding, death were evaluated in patients stratified by sex and anticoagulation patterns. Results: Women were older and used more direct oral anticoagulants (85% vs. 78%, p<0.001) than men. During a median (25th and 75th percentiles) follow-up of 30 (24, 38) months, the incidence rate and risk of SSE (0.7 in women vs. 0.7 in men per 100 person-years) and major bleeding (0.1 in women vs. 0.1 in men per 100 person-years) were not different between the sexes. However, women had a lower all-cause death rate (0.4 in women vs. 0.6 in men per 100 person-years, hazard ratio: 0.48, 95% confidence interval: 0.25-0.91, p=0.025) than men. Conclusions: In contemporary anticoagulation for AF, SSE and major bleeding risks did not differ between sexes. However, women showed a lower risk of all-cause death rate than men, indicating that the use of oral anticoagulants for treating AF in females does not appear to be a risk factor for adverse clinical events.
Parathyroid hormone (PTH) regulates calcium (Ca) and phosphate transport. Secondary hyperparathyroidism usually occur in chronic renal failure (CRF), and it is a common complication of CRF in dogs and cats. Twenty one dogs with renal failure were enrolled in this study. Plasma Ca, inorganic phosphorus (iP) and PTH concentrations were measured. Dogs were divided into two groups by the ratio of Ca to iP (Ca/iP$\leq$1; Group I, Ca/iP$\leq$1.5; Group II). The PTH level between sexes ages were not significantly different. The PTH level and Ca/iP between two groups were significantly different (p < 0.0001). In conclusion, The ratio of Ca to iP may be considered as useful surrogator to predict PTH level of dogs with renal failure.
The purpose of this study is to help the understanding the characteristics in the costumes of men and women throughout the overall acceptance process of 'Naewaebub' in the late 'Chosun Dynasty' in which the notion of 'Naewae' is more prosperous than any other period in Korean history. Originally the distinction between man and woman was not intended to display the high and the low but to show the mutual respect according to each duty. As time goes on, that ideal became gradually changed to heighten the man and lower the women especially in 'Han' period of China. There was the ideal of distinction in the ancient times in Korea. Until 'Koryo Dinasty' the community has the system of blood-tied that put together maternal and paternal. 'Karye' was introduced in the late 'Koryo Dynasty' and accepted to the stereotype of morale in the 'Chosun Dynasty' and there seemed to be 'Samgang' and 'Oryun' at once. Many restrictions was imposed to behavior in women like rules of prohibition in attending the temple and concealment of woman's face and was recognized to rule of distinction between man and woman. Confucian life custom has been settled to Korean society throughout the late 16th century and 17th century and there were some appearances in the housing construction which divide the residences of man and woman. The characteristics in the costume from the avoidance of opposite sexes are clear in the structure of clothes changed from similar style to different style. The examples of those characteristics are as fellows. The costume in man was developed to advanced 'Pyoun-bokkwan' and 'Pyounbokpo' as social action of man prospered. Meanwhile the trousers which had been the same in the man and woman were separated to different trousers between man and woman. The costume life style of woman was changed to using the a hair whirl, hiding the face in the street and overlapping the innerwear under the skirt which was extension of woman's closed life style in late 'Chosun Dynasty'.
Normal haematological and biochemical indices along with thyroid hormone status were studied in healthy Kathiawari horses of different age groups (yearling, young stock, adults and old stock) belonging to either sex. Effect of both age and sex was observed on thyroid hormone levels, haematological and biochemical indices. In females, hemoglobin levels was significantly lower in yearlings than adult animals while total leukocyte counts were higher in yearlings than equids of other age groups. Sex had effect only on total erythrocyte counts, mean corpuscular hemoglobin concentration and mean corpuscular hemoglobin in horses of 1-3 years age group (young stock) and on packed cell volume in adult female and male equids. Among biochemical indices, activities of enzymes were observed to be influenced both by age and sex. Creatine kinase, gamma glutamyl transferase, glutamate pyruvate transaminase, glutamic oxaloacetate transaminase and lactate dehydrogenase activities were significantly higher in young and adult equids than animals of other age groups in Kathiawari horses while activity of alkaline phosphatase was significantly higher in yearlings than equids belonging to other age groups in both male and females. However, activity of sorbitol dehydrogenase was unaltered due to both sex and age factor. Albumin, bilirubin direct, bilirubin total, cholesterol, creatinine, protein, triglyceride and uric acid were statistically different in various age and sex groups of horses. Calcium, magnesium and chloride contents were almost same in various age groups of male horses. Significantly higher levels of $T_3$ and $T_4$ were observed in both male and female yearlings as compared to equids of other age groups in both the sexes.
The pyrrolizidine alkaloids (PAs), contained in a number of traditional remedies in Africa and Asia, show wide variations in metabolism between animal species but little work has been done to investigate differences between animal strains. The metabolism of the PA senecionine (SN) in Fischer 344 (F344) rats has been studied in order to compare to that found in the previously investigated Sprague-Dawley (SO) rats (Drug Metab. Dispos. 17: 387, 1989). There was no difference in the formation of ($\pm$) 6,7-dihydro-7-hydroxy-1-hydroxymethyl-5H-pyrrolizine (DHP, bioactivation) by hepatic microsomes from either sex of SO and F344 rats. However, hepatic microsomes from male and female F344 rats had greater activity in the Noxidation (detoxication) of SN by 88% and 180%, respectively, when compared to that of male and female SD rats. Experiments conducted at various pH showed an optimum pH of 8.5, the optimal pH for flavin-containing monooxygenase (FMO), for SN N-oxidation by hepatic microsomes from F344 females. In F344 males, however, a bimodal pattern was obtained with activity peaks at pH 7.6 and 8.5 reflecting the possible involvement of both cytochrome P450 (CYP) and FMO. Use of specific inhibitors (SKF525A, 1-benzylimidazole and methimazole) showed that the N-oxide of SN was primarily produced by FMO in both sexes of F344 rats. In contrast, SN N-oxide formation is known to be catalyzed mainly by CYP2C11 rather than FMO in SD rats. This study, therefore, demonstrated that there were substantial differences in the formation of SN N-oxide by hepatic microsomes from F344 and SD rats and that this detoxification is catalyzed primarily by two different enzymes in the two rat strains. These findings suggest that significant variations in PA biotransformation can exist between different animal strains.
Purpose: The purpose of this study was to compare the difference in compression quality and fatigue levels in a rescuer for three different hand techniques used in cardiopulmonary resuscitation (CPR). Methods: The participants were paramedic students at the basic life support provider level. The hands-only CPR was performed for 10 minutes for each of the three hand techniques without disruption, and the quality of chest compressions and fatigue levels were analyzed. Results: There was no difference between the sexes in the chest compression quality and the physiologic parameters before and after compression. Among the quality indexes of chest compression with each of the techniques performed for 10 minutes, the mean depth (p<.01) and mean accuracy (p=.000) of the compression were found to be higher in the five finger fulcrum technique, while the mean compression rate and relaxation accuracy showed no significant differences. Regarding fatigue levels, the five finger fulcrum technique caused lesser subjective fatigue as compared to other techniques (p<.05), although the heart rate and blood pressure revealed no difference. Conclusion: The five finger fulcrum technique was found to be better than the other techniques in terms of chest compression quality and subjective levels of fatigue, indicating that it should be used in CPR education.
