부산지역 일부 병원 입원환자의 치료식에 대한 인식 및 급식만족도 (Patients' Perception of and Satisfaction on Therapeutic-diets at Hospitals in Busan)

  • The purpose of this study was to investigate the inpatients' perception of therapeutic-diets and of explanation about those diets provided by hospitals and satisfaction on therapeutic-diets at hospitals in Busan. The subjects consisted of 155 inpatients at five hospitals, which all had over 400 beds. The research was performed through the interviewing process using questionnaires. Seventy five percent of patients had received an explanation for their therapeutic-diet and 57.4% of respondents were given a manual that explained the reason for the therapeutic-diet. The professionals who explained the therapeutic-diet was 61.7% dietitians and 25.6% doctors. 59.4% of the patients considered the dietitian to be suitable for explaining the diet and 25.6% patients believed the doctor to be suitable for explaining the diet. In terms of the patients' perception of the therapeutic-diet explanation, 74.5% of the patients understood very well, 78.9% of them perceived this explanation as very important, and 67.5% of them were satisfied. On a scale of 5.00 for therapeutic-diet satisfaction, the average scores were 2.95 for meal characteristics and 3.06 for service characteristics. The items that scored low in therapeutic-diet satisfaction were taste, seasoning and appearance of meals, provision of selective menu and consideration of personal preference. In terms of the perception of understanding the therapeutic-diet, patients who were provided a manual and an explanation gave high scores to 'taste', 'variety of diet', 'meeting opportunity with dietitians', and 'prompt dealing with meal complications'. There was a significant (p<0.05) positive correlation between satisfaction for the explanation of the therapeutic-diet and the degree of perceived benefits of the explanation to the nutrition-management and the satisfaction on the therapeutic-diet satisfaction. Therefore, the results of this study suggest that hospitals should increase support for explaining the therapeutic-diet by dietitians and develop menus based on the patients' preference and the taste of the meal.

경기도지역 학교급식 영양사의 영양교사 후 업무 및 조직환경 변화에 대한 인식 분석 (Analysis of Recognized Changes in Performance and Organizational Environment by Dietitians Transposed to Nutrition Teachers in Gyeonggi Province)

  • This study was conducted to gather basic data on the tasks and roles of nutrition teachers by analyzing the changes inwork importance, performance frequency, and organizational environment after dietitians were transposed to nutrition teachers. E-mail surveys were sent to 100 newly assigned nutrition teachers who previously served as dietitians in Gyeonggi province. A total of 72 teachers responded to the survey. The respondents were asked the importance and frequency of 9 work-related duties performed as a dietitian and nutrition teacher, respectively. They were also surveyed regarding their satisfaction perceptions for 5 organizational changes. The majority of respondents were 36 to 40 years old (48.6%) and had 10$\sim$20 years of work experience (58.3%). Significant differences were found for work importance after changing to the role of nutrition teacher (p<0.001). In addition, work performance frequency differences were found for the categories of nutrition education, nutrition counseling, and dietary guidance. These results were attributable to the differences in work importance and performance frequency by now being a nutrition teacher rather than a dietitian. The respondents had increased satisfaction levels after changing from dietitian to nutrition teacher (p<0.001) in terms of 'pride for change in work', 'satisfaction level for pay', and 'satisfaction level for change in the organizational environment'. In order for nutrition teachers to have effective work performance, 'personal capabilities' and 'school manager support' should be coordinated. Overall, the results of this study suggest that effective training programs should be developed for nutrition teachers they can effectively complete newly assigned work tasks.

영양사의 직무능력 향상을 위한 계속교육의 참여동기 분석 (Analysis of Reasons for Continuing Education in Dietitians)

  • This study analyzed the reasons for continuing education among dietitians. An internet-based survey of 622 dieticians was conducted from August 31, 2012 to September 12, 2012. Based on data from the Participation Reasons Scale (PRS) questionnaire, factor analysis was conducted by principle component analysis for the extraction of major factors. Subsequent reliability analysis was performed by assessing Cronbach's ${\alpha}$. The ANOVA-test was performed to compare the participation reason scores for each factors according to general characteristics. Statistical analysis was performed using SPSS ver.17.0, and P<0.05 was considered significant. Factor analysis for the participation reasons revealed four types of factors. These factors were "maintenance and development of professional competencies", "interaction and responsibility of professionals", "job stability and personal profits" and "services for customers" respectively. Among the participation reasons, "maintenance and development of professional competencies" was the first reason with a 29.34% variance. In addition, the participation reasons for continuing education differed according to age (P<0.05), the highest level of education (P<0.05), workplace (P<0.01), and work experience in dietetic area (P<0.001). In conclusion, continuing education programs for dietitians should focus on effectively developing and promoting professionalism.

장애인 생활시설 급식관리 및 위생관리 실태조사 (Foodservice Management and Food Sanitation Management in the Welfare Institutions for the Disabled in Korea)

  • The purpose of this study was to investigate the characteristics of the foodservice management practices, the equipment ratio of sanitary facility/equipment, and dietitians' perceptions of (i) the barriers to sanitary management and (ii) the sanitary management performance level in the welfare institutions for the disabled in Korea. The survey was conducted during the period from September 7 to October 15, 2006. A total of 91 institutions(response rate 74.6%) were analyzed by using SPSS(windows ver. 14.0). The average number of meals served per day per an institution was 379. The majority(93.4%) of dietitians made decisions in procurement. The major part of the purchase was made through private contract. The factors affecting menu planning were nutrition, food preference, and cost, in the order of importance. Among the food items, fruits were infrequently served, while protein source foods and green leaf vegetables were almost daily served. The equipment ratio of sanitary facilities/equipment was 45%, which was relatively low. Most dietitians perceived 'limited availability of facilities and equipment' and 'the lack of support from financing department' as the major barriers in implementing a desirable sanitary system. Sanitary management performance in 'the food ingredient' was perceived as the lowest, while that in 'the uniform' showed the highest. The results of this study suggest that a proper supporting program on securing the facility/equipment and adequately trained employees are needed for successful sanitary management. Also, a more frequent supply of fruits for the disabled is recommended.

노인복지시설 영양·급식관리자의 교육 참여동기 및 저해 요인 분석 (Analysis of the Participation Reasons and Deterrents on Welfare Facility Dietitians for the Elderly)

  • Objectives: This study analyzed the education participation reasons and deterrents of dietitians who work in welfare facilities for the elderly. Methods: The survey was completed by 144 dietitians working at welfare facilities for the elderly in Korea. The survey was conducted in October, 2018, both on-line and off-line, based on the demographic characteristics, work status on welfare facilities for the elderly, Participation Reasons Scale (PRS) and Deterrents to Participation Scale (DPS-G). The data were analyzed using frequency analysis, descriptive analysis, factor analysis, reliability analysis, regressive analysis using SPSS ver. 25.0. Results: The reason for participation were divided into three factors: 'Responsibility of professional and self-development ($5.76{\pm}1.04$)', 'Job stability and personal benefits ($4.98{\pm}1.28$)', and 'Interaction and development of professional competencies ($5.85{\pm}1.00$)'. 'Interaction and development of professional competencies' was the highest motivation factor. Also, the deterrents for participation were divided into four factors: 'Dispositional barrier ($2.70{\pm}1.29$)', 'Dissatisfaction of education usability ($3.39{\pm}1.38$)', 'Institutional barrier ($4.21{\pm}1.45$)', and 'Situational barrier ($2.36{\pm}1.30$)'. 'Institutional barrier' showed the highest deterrents factor. In addition, 'Responsibility of professional and self-development' and 'Interaction and development of professional competencies' were negative attributes for 'Dispositional barrier' (p<0.001). Conclusions: These results provide basic data to promote participation in education and contribute to the improvement of their job ability and education capacity of the food and nutrition management of welfare facilities for the elderly.

Application of draft nutritional standards for school lunches in the Republic of Korea: a feasibility study from the supplier perspective

  • BACKGROUND/OBJECTIVES: The purpose of this study was to evaluate the feasibility of application of the draft nutritional standards for school lunches in the Republic of Korea (South Korea) from the supplier perspective. SUBJECTS/METHODS: In November 2018, a pilot operation was conducted at 104 participating schools for 2 weeks each by applying the draft standards A and B. To evaluate the ease and appropriateness of application of the standards, we analyzed the nutritional values and food cost of the school lunches for 2 weeks before and for 4 weeks under the pilot operation. We conducted an online survey of school dietitians or nutrition teachers of the participating schools to evaluate the feasibility of application of the standards in school lunch sites. RESULTS: The proportions of schools of which lunches met the reference values of the school's own nutritional standards were 19.2% for the standards A and 21.2% for the standards B. The food cost of school lunches applying the standards A (KRW 2,880) was significantly lower than that of school lunches applying the standards B (KRW 3,030) or the existing standards (KRW 2,980). As a result of the survey, the mean score for the ease of application was significantly higher for the standards B (2.75) than for the standards A (2.24). The proportions of school dietitians or nutrition teachers reporting no problem in meeting an existing food cost standard were 72.5% for the standards A and 61.8% for the standards B, respectively. The overall appropriateness of application was higher for the standards B, because almost two-thirds (58.8%) of the respondents answered that the standards B were more appropriate than the standards A. CONCLUSION: The standards B were more feasible to apply as nutritional standards for school lunches than the standards A from the supplier perspective.

학교 급식 관리 영양사의 근로가치관이 조직효과성에 미치는 영향 : 영양교사 제도에 대한 기대감의 조절효과를 중심으로 (Influence of School Foodservice Dietitians' Work Value on Organizational Effectiveness: Moderating Effect of Expectation to the Institution of nutrition Teachers)

  • The purpose of this study was to identify the influence of school foodservice dietitians' work value on job satisfaction, organizational commitment, and turnover intentions. Also, this study explored the role of the expectation to the institution of nutrition teachers as a moderating variable in three relationships: 1) between work value and job satisfaction, 2) between work value and organizational commitment, and 3) between work value and turnover intentions. This study surveyed dietitians who worked at school foodservice in Kyunggii and Daegu${\cdot}$Gyeoungbuk province. A total of 509 responses were collected using on-site survey and online survey (response rate 59.3%). This study conducted hierarchical regression analysis to test the moderating effect of the expectation to the institution of nutrition teachers. Results of hierarchical regression showed that work value of status upgrade had a positive effect on job satisfaction to promotion opportunity and salary, and such effect was more significantly apparent in high expectation to new work. In addition, work value of status upgrade had a positive effect on turnover intention, and such effect was more significantly apparent in low expectation to new work environment improvement. Work value of work activity preference also appeared to have a positive effect on organizational commitment, and such effect was more significantly apparent in high expectation to social status and professionalism.

단체급식소 영양사의 육가공품에 대한 인식 및 이용실태 조사 (A Survey on the Perception and Usage Status of Dietitians in Food Service Business for Meat Products)

  • 본 연구는 서울과 강원지역의 단체급식소 영양사 305명을 대상으로 하는 설문조사 결과를 기초로 국내 육가공품에 대한 인식 및 이용실태를 파악하였다. 그 결과 단체급식소 영양사는 육가공품에 대한 품질수준(Likert 5점 척도, 평균 3.4점, 표준편차 0.6), 선호도($3.1{\pm}0.7$), 식단반영률 79.6%와 고품질화(개선 요구도 26%)는 높은 수준의 긍정적인 인식도를 보였다. 그러나 육가공품 섭취 시 영양과 건강에 미치는 영향성($2.9{\pm}0.8$), 위생 안전성(개선 요구도 31.5%)은 낮은 수준의 부정적인 인식도를 보였다. 특히 영양사의 선호도($3.1{\pm}0.7$)는 육가공품의 사용빈도, 영향범위, 근무경력, 결혼여부, 품질수준과 연령 등과 각각 양(+)의 상관관계를 보였다. 즉, 영양사의 선호도는 육가공품의 구매 및 소비확대를 위한 육가공품 식단반영(식단반영율 79.6%)에 영향을 주는 가장 중요한 요인이라 할 수 있다. 또한, 영양사는 육가공품의 선택과 구매 결정권(86.9%)을 가지고 있으며, 식단 작성 시 학생 기호도(32.8%), 육함량(21.9%)과 조리 편리성(19.5%)을 고려하고, 검수 시 가장 유의하는 것은 제조일자(89.2%)인 것으로 조사되었다. 햄, 프레스햄과 소시지의 차이점에 대해 '약간 알고 있다' 47.5%, '전혀 모른다' 36.7%, '거의 알고 있다' 11.8%, '확실히 알고 있다' 3.9%로 응답하였다. 단체급식에 이용되는 육가공품은 햄(33.9%), 소시지(21.7%)로 볶음(34.5%), 국(18.6%)요리의 빈도가 높은 것으로 조사되었다. 단체급식에서 육가공품은 월 1회(41.6%), 주 1회(36.4%) 제공되며 식단 제공자인 영양사가 판단하는 피급식자의 만족도는 '만족'(67.9%), '보통'(22.3%)으로 응답하였다. 또한 국내 육가공품의 위생안전성 수준에 대하여 '보통'(60.9%), '우수'(29.8%)로 응답하였고, '나쁘다'(9.2%)는 이유로는 '본인 전공 판단'(62.7%), '언론보도'(17.1%), '첨가물 과다' (14.2%) 순으로 나타났다. 그리고 육가공품을 섭취 시 영양과 건강면에 미치는 영향에 대하여는 '보통'(50.8%), '해가 된다'(24.9%), 품질수준에 대해서는 '보통'(54.1%), '우수'(39.7%)로 응답하였다. 현재 시판되고 있는 육가공품의 가격 수준은 '비싸다'(53.8%), '적당하다'(41.6%)로 응답하여 비싸다는 인식이 높으나, 고품질제품은 '10-20%가 비싸져도 구매 하겠다'가 93.1%로 영양사의 고품질제품에 대한 높은 욕구를 알 수 있었다. 영양사의 육가공품에 대한 품질 개선점은 위생 안전성(31.5%), 고품질화(26%) 및 성분표기의 정확성(17.8%) 순으로 제안되었다. 이는 육가공품의 조리 편리성 보다는 위생안전성과 고품질화를 중요시 한다는 것을 응답 결과로 알 수 있다. 특히 영양사의 선호도와 인식도는 육가공품의 구매 및 소비확대에 직접적인 영향을 주는 것으로 통계분석 결과로 알 수 있었다. 그러므로 앞으로 단체급식소와 B2B시장의 확대를 위해서는 위생안전성과 건강뿐 아니라 영양과 기능성을 고려하여 품질수준을 높이고 이에 대한 홍보를 강화하는 것이 최선일 것으로 판단된다.

경남지역 학교급식 HACCP 시스템 적용 장애요인에 대한 영양(교)사의 인지도 분석 (Nutrition Teachers (Dietitians)' Perceptions of Barriers to Implementation of HACCP System in School Foodservices in the Gyeongnam Area)

  • 본 연구는 경남지역의 초 중 고등학교 영양(교)사를 대상으로 HACCP 시스템 적용 시 HACCP 영역별 장애 요인과 CCP 단계별 장애 요인에 대한 인지도를 분석하고자 수행되었다. 2009년 11월부터 12월까지 경상남도교육청 소속 초 중 고등학교 영양(교)사 350명을 대상으로 설문지를 배부하고 214부를 회수하여 최종 분석에 사용하였다. 본 연구의 결과는 다음과 같다. 첫째, HACCP 영역별 장애 요인의 분석 결과 '시설 설비 현황(3.38)'> '모니터링(3.35)'> '업무만족도(3.11)'> '조리종사자(3.00)'> 'HACCP팀 협력(2.95)'> '급식종사자 외 관계인 협조(2.80)'> 'HACCP 시스템 이해(2.39)'의 순으로 인지도가 높은 것으로 나타났다. '시설 설비현황' 영역에서는 '학교설립형태(p<0.05)', '급식형태(p<0.05)', '조리장 신개축 여부(p<0.01)', '조리장 작업구역 구분 여부(p<0.01)', '전처리실 유무(p<0.01)', '식기세척실 유무(p<0.01)'에 따라 유의한 차이를 보여 시설이 잘 갖추어진 학교에서는 시설 설비 현황 영역에 대한 장애요인 인지도가 낮은 것으로 조사되었다. 둘째, HACCP 단계별 장애 요인에 대한 인지도 분석 결과 'CCP 1(식단의 구성)'> 'CCP 3(검수)'> 'CCP 2(잠재적으로 위험한 식단의 공정 관리)', 'CCP 6(식품취급 및 조리과정)'> 'CCP 4(냉장 냉동의 온도 관리'> 'CCP 8(식품 접촉 표면 세척 및 소독)'> 'CCP 7(운반 및 배식 과정)'> 'CCP 5(채소 과일의 세척 및 소독)'의 순으로 높게 나타났다. 이상의 결과를 종합하여 보면, 경남지역 영양(교)사들은 학교급식 HACCP 시스템 적용 시 HACCP 영역별에서는 '시설 설비 현황' 영역을, 단계별에서는 'CCP 1(식단의 구성)' 단계를 가장 큰 장애 요인으로 인식함을 알 수 있었다. 경남지역 영양(교)사들은 HACCP 시스템에 대한 이해도는 높으나 실질적으로 식단 작성에 있어서는 높은 수준의 장애 요인을 인지하고 있는 것을 알 수 있었다. 본 연구 결과를 근거로 하여 다음과 같은 제언을 하고자 한다. 첫째, HACCP 시스템 적용에 따른 가장 큰 장애요인인 '시설 설비 현황' 다음으로 많은 장애를 느끼는 것은 '영양(교)사의 업무 과중으로 인한 많은 시간의 소모', '실질적인 모니터링의 부족', 'CCP의 관리 및 기록을 위한 시간과 노력의 투입'이 조사되었으므로 영양(교)사의 업무 분석 및 적정 인력 배치에 대한 대책을 수립하여야 할 것이다. 둘째, 향후 영양(교)사를 위한 위생적이고 안전한 식단 구성을 위한 구체적인 가이드라인 및 교육 훈련 프로그램이 마련되어야 할 것이다. 셋째, 학교급식에서 HACCP 시스템의 성공적인 적용을 위해서 무엇보다 관련 당사자들 즉 급식 현장에 있는 영양(교)사, 조리종사자 뿐만 아니라 학교장, 예산관련 부서, 교사, 학생, 학부모, 지역 사회의 적극적인 협조가 우선적으로 필요하다. 관련 당사자들의 적극적인 협조를 이끌어 내기 위해 교육청 및 지방자치단체 차원에서의 적극적인 교육 및 홍보를 실시하여 많은 지원과 관심을 이끌어 내야 할 것이며, 위생의 중요성에 대한 인식 수준을 높여야 할 것이다.

전국 요양병원에서의 임상영양서비스 실태 조사 (Clinical Nutrition Services of a Long-term Care Hospital in Korea)

  • Objectives: The purpose of this study was to investigate how clinical nutrition services is provided at a long term care hospital in Korea and to investigate job satisfaction levels of the clinical dietitians. Methods: Survey questionnaire was sent to dietitians working at a long term care hospital in Korea. The participating hospitals (n=240) were randomly selected from 1,180 long- term care hospitals using a stratified sampling method. A total of 134 long term care hospital s and 223 dietitians completed the survey of clinical nutrition service s and job satisfaction questionnaires The job satisfaction questionnaire included 27 job satisfaction questions on task, stability vision, working conditions, and relationship areas. Results: The average nutritional screening rate was 17.9% and the rate of computerized nutritional screening system was 9.7% in the participating hospitals. Nutritional intervention rate was only 3.2% of all patients. KOIHA (Korea Institute for Healthcare Accreditation) accreditated hospitals showed only 50% performance rate of nutrition service evaluation area. This shows that after achieving KOIHA accredition, many hospitals do not emphasize the performance of nutritional services. The job satisfaction scores in all four areas ranged from 2/5 to 3/5, implying generally low job satisfaction level in hospital dietitians. Linear regression analysis results showed that the "hospital adequacy grade" type was a significant predictor of job satisfaction level for two areas (working conditions & relationship). Conclusions: There is a need to provide proper standardized clinical nutrition services as a primary treatment and we observed large variations in the quality of nutritional service s in long term care hospitals. Therefore, government and local hospitals have to work on implementing nutritional programs and policies for improved service and care.
