서울시내 사립국민학교 아동의 비만요인에 관한 연구 (A Study on the Etiology of Childhood Obesity)

  • To study the factors related to childhood obesity, 75 subjects were selected as an obese group and 75 subjects were selected as a control group according to their obesity index and BMI among the 583 children of 5th and 6th grade in elementary school in Seoul. For each subject, skinfold thickness was measured, information on the characteristics of family, nutrient intake and activity time of the subjects were obtained by questionnarie. The results were summarized as following: 1) Parents' obesity indices of obese group were significantly higher than those of control group(Father's Obesity Index : P<0.01, Mother's Obesity Index : P<0.05). 2) Questions about eating rate and behavior showed that subjects in obese group were significantly faster in their eating rate(p<0.05) and always more eager to eat food(p<0.001) compared to control group. 3) Dietary intake data showed that average daily intake of all the nutrients exceeded Korean recommended dietary allowances for the age group. In female children, fat intakes of obese group were significantly higher than control group(P<0.05). There was no such significant difference in male subjects. 4) Activity time record of subjects reveal that there was a trend for males to spend more time in moderate and high activity and less time in light activity compared to females. Subjects in obese group spent significantly more time for light activity(P<0.001) and significantly less time for moderate activity (P<0.001) compared to subjects in control group. The difference of activity was more pronounced in male subjects. 5) Correlation matrix of the variables showed that obesity index, BMI, and skinfold thickness of the subjects were highly correlated with each other. These indices were positively correlated to parent's obesity indices and the time spent for light activity, but negatively correlated to the time spent for moderate activity. Nutrient intakes were not significantly correlated to obesity indices. Form the results shown above, although both obese and control groups are from high socioeconomic class, obese children tended to come from family with fat parents. They eat fast and are always eager to eat. They tend to eat more fat but are very different from controls in nutrient intake. Obese children are significantly lower in their actinity compared to controls. So, for the prevention and treatment of childhood obesity, it seems to be more important to increase their activity levels.

광주지역 일부 초등학교 3학년 아동의 식습관의 영양소 섭취량 (The Eating Behaviors and Nutrient Intakes of Third Grade Primary School Children in Kwangju)

  • The purpose of this study was to determine eating behaviors and nutrient intakes of 256 third grade primary school children(134:boys, 122:girls) in Kwangju. There were no significant differences in the heights, weights, physical circumferences measurements between the boys and girls, but the skinfold thickness measurements were significantly different. Most nutrient intakes were higher than the Rec ommended Dietary Allowances but the intake of energy, calcium, and vitamin E were lower than the RDA. The intakes of calcium, niacin, and vitamin E of the girls were significantly lower than those of the boys. With regard to meal volume, 13.6% of the subjects has been 'much'. With regard to meal speed, 24.6% of the subjects has been 'fast'. The study also found that 50% of the subjects skipped breakfast, 67.6% of the subjects had irregular meal times, 64.6% of the subjects ate snacks after dinner and 85.9% of the subjects ate snacks. These results show that the nutrient intake of children was related to snack eating more than regular meals. With regard to dietary behavior and nutrient intakes, meal volume, meal speed, skipping breakfast, eating snacks and eating snacks after dinner were major influences. But regularity of meal time was not influence on nutrient intakes. These results suggest that children's eating behaviors are in needs of correction. This reasserts the importance of nutrition education to children through teachers, parents and dietitians. The findings of this study should be applied to nutrition education to ensure better physical fitness of children.

농업인의 식습관과 건강상태의 관련성 연구 (The farmers consciousness of Health status and food habits in Korea)

  • The purpose of this study was to compare the consciousness health status and food habit for farmers in Korea. Eight hundred persons were surveyed by rural leader of RDA, composed with 35.1% of male and 64.9% of female. Data collection includes height weight and blood pressure by measuring method and personal & family situation by interview for influential factor of health status and food habit. The questionnaire was included dietary habit of salt, sugar, dietary fiber and fat intake and other behavior questions related with diet. The subjects were composed 32.5% of elementary school graduated and 50% of middle size farming. Of the all subjects, 54% of were rice planted farming mainly and 19 were over 65 year-aged group. In the concept of disease prevalence. gender difference was exist, liver and heart complains were more frequency in male and waist pain and urinary complain in female, shoulder pain and nervousness were frequently prevalent to all group of farmers. The hypertension prevalence of farmers were 16% by measuring blood pressure, but consciousness of hypertension was very low (5%). The correlation of health status with personal factors were analysed, body weight, number of family, education level, and working level were significant with CMI score and Farmers' syndrome score. The food habit status was negatively correlated with weight and BMI, age was negatively correlated with CMI score and farmers' syndrome. The consciousness of health status and food habit score were significantly correlated with smoking and alcohol drinking. It concludes that good food habit contribute wellness of health. Nutrition education for better food habit would be helpful on health status.

Development and application of the sodium index to estimate and assess sodium intake for Korean adults

  • BACKGROUND/OBJECTIVES: The purpose of this study was to develop a sodium index, which is a tool for estimating and assessing sodium intake easily and quickly, to assist in the prevention of various diseases induced by excess sodium intake in Korean adults. SUBJECTS/METHODS: The 24-h urine collection and dietary behavior surveys were performed on 640 healthy people in 4 regions of South Korea, and an equation for the estimation of 24-h sodium intake was developed. The validity and reliability of the equation were verified with 200 adults. The sodium index was developed by converting the estimated sodium intake using the equation. Finally, the sodium intake status of 1,600 adults was assessed using the sodium index. RESULTS: The equation included sex, age, body mass index, eating habit and dietary behaviors related to sodium intake. In validity test of the equation, the mean bias between sodium intake using 24-h urine analysis and using the equation from the Bland-Altman plots was -1.5 mg/day. The sensitivity and specificity of the equation for estimation of sodium intake were 80.5% and 64.4%, respectively. In the reliability test of the equation, there was no significant difference between the first and second sodium intakes calculated using the equations, and Spearman's correlation coefficient between the 2 sodium intakes was 0.98. Sodium intake can be assessed as 'very moderate' for 75-100 on the sodium index, 'moderate' for 100-150, 'careful' for less than 75 or 150-200, and 'severe' for 250 or more. When sodium intake was assessed using the sodium index in 1,600 subjects, 54.3% and 24.3% of the subjects were assessed to be in the 'careful' and 'severe' categories, respectively. CONCLUSIONS: Using a simple questionnaire, the sodium index can be used to monitor and assess sodium intake status, assisting in nutrition education and counseling in a large population.

식행동, 식생활 환경을 반영한 청소년의 식생활·영양 실천 평가 항목 개발 (Development of evaluation items for adolescents' dietary habits and nutritional practices reflecting eating behaviors and food environment)

  • 본 연구를 통해 개발된 청소년 식생활·영양 실천 평가 항목은 총 22개의 문항으로 청소년들이 개인의 전반적인 식사의 질 및 식행동, 식환경을 영역별로 평가 가능하며, 향후 청소년의 식생활 개선을 위한 평가 도구로 활용될 수 있다. 한국인을 위한 식생활 지침을 기반으로 청소년의 다양한 식생활, 식문화와 관련된 선행연구, 국민건강영양조사자료, 국가 정책 및 지침에 대한 검토와 전문가 대상 타당도 평가를 진행하여 58개의 수도권단위 예비 조사용 체크리스트 평가항목을 우선 선정하였다. 수도권단위 조사는 중·고등학생 403명을 대상으로 진행되었고, 조사 자료의 탐색적 요인분석 결과 식단의 질 평가항목을 식품섭취, 식행동, 식생활 환경 세 영역으로 분류하였다. 식사섭취실태와의 상관관계 분석을 통해 수렴타당도를 분석하고 최종 모형으로 총 22개의 평가항목이 도출되었다. 최종적으로 청소년 식단의 질 평가도구는 식품섭취 영역 10개 항목 (채소류, 황색채소, 생과일, 계란 및 육류, 콩과 견과류, 우유·유제품, 다양한 식품군, 가당음료, 가공육류, 튀김류), 식행동 영역 4개 항목 (유산소, 체중유지, 끼니섭취 횟수와 규칙적인 식사), 식생활 환경 영역 8개 항목 (유통기한, 덜어먹기, 로컬푸드, 친환경식품, 가정 내 신선한 과일채소, 유제품의 비치와 부모님의 식사준비 여부 및 건강한 간식 섭취의 격려)으로 구성되었다. 본 연구의 청소년 식생활·영양 실천 평가 항목은 22개의 간단한 문항으로 청소년들이 개인의 전반적인 식사의 질 및 식행동, 식환경을 영역별로 점수 평가하는데 용이하다는 점에서 청소년의 식생활 개선을 위한 도구의 활용이 기대된다.

경북 일부지역 여자 중·고등학생의 체형인식도 및 식생활 행동 비교 (A Comparison of Body Image and Dietary Behavior in Middle and High School girls in Gyeongbuk Area)

  • 연구는 경북지역 여자 중 고등학교 학생의 체형인 식도와 식생활 행동 비교를 알아보기 위하여 경북의 여자중학생 194명과 여자고등학생 170명을 대상으로 2014년 9월에 설문 조사를 실시하였다. 설문지는 총 364부를 배부한 후 364부(100% 회수)를 회수하여 최종 연구 분석에 사용하였다. 1. 조사 대상자의 평균 BMI는 21.29로 정상 수준이었으며, 여고생이 여중생에 비해 신장, 체중, BMI가 모두 높은 수치를 보였고 신장(p<0.001)과 체중(p<0.01)은 유의한 차이를 나타냈으나 BMI는 유의한 차이를 보이지 않았다. 2. 조사대상자의 체형인식에서, 조사대상자 전체 중 51.4%가 보통체형, 37.4%가 뚱뚱한 체형, 11.3%가 마른 체형 순으로 응답하였으며, 여고생(43.9%)이 여중생(31.6%)에 비해 뚱뚱한 체형이라고 응답한 비율이 높게 나타나 유의한(p<0.05) 차이를 보였다. 체형에 대한 만족도에서, 조사대상자 전체 중 52.2%가 '불만족이다'고 응답하였으며 32.7%가 '보통이다', 15.1%가 '만족이다'고 응답하였으며, 특히 '불만족이다'고 응답한 비율이 여고생(64.1%)이 여중생(44.0%)에 비해 유의하게(p<0.001) 높게 나타났다. 3. 조사 대상자의 체형인식과 BMI를 비교 분석한 결과, 조사대상자 전체에서 자신을 마른 체형으로 응답한 군의 63.4%가 저체중으로, 보통 체형으로 응답한 군의 70.0%가 정상체중으로, 뚱뚱한 체형으로 응답한 군의 53.3%가 과체중과 비만으로 나타나 뚱뚱한 체형으로 응답한 군이 다른 두군에 비해 올바른 체형인식 비율이 낮았다. 특히 자신을 뚱뚱한 체형으로 인식하는 대상자들 중 5.2%가 저체중, 41.5%가 정상체중으로 조사되어 자신을 뚱뚱하다고 인식하는 여학생의 절반이 왜곡된 체형인식을 하는 것으로 나타났다. 여중생과 여고생의 차이를 비교한 결과, 자신을 뚱뚱한 체형으로 응답한 여학생 중 여중생 39.3%, 여고생 53.3%는 정상체중과 저체중으로 나타났으며, 자신을 마른 체형이라고 응답한 여학생 중 과체중 이상이 여중생은 0.0%, 여고생은 18.8%로 나타나 여고생이 여중생에 비해 자신의 체형에 대해 잘못 인식하는 비율이 유의하게(p<0.001) 높은 것으로 조사되었다. 4. 조사대상자 전체 중 63.7%가 체중조절 경험이 있는 것으로 나타났으며, 여고생(67.3%)이 여중생(60.6%)에 비해 체중조절 경험이 많은 것으로 조사되었으나 유의한 차이를 보이지 않았다. BMI가 높을수록, 자신을 뚱뚱한 체형으로 인식할수록 체중조절 경험이 유의하게(p<0.001) 많은 것으로 나타났다. 체중조절 방법으로는 '음식의 조절과 운동을 병행한다'(22.0%), '식사량을 조절한다'(20.9%), '운동을 한다'(18.7%), '간식과 야식을 줄인다'(17.4%) 순으로 나타났다. 5. 조사대상자의 비만 관련 식행동 평가를 15항목에 대해 5점 척도로 조사한 결과, 조사대상자들의 총 평균 점수는 3.19/5.00점으로 조사되었다. 평균점수보다 낮은 평가점수를 나타낸 항목은 총 7개로 '음식의 영양소를 따져 가면서 먹는다'(2.15), 'TV나 컴퓨터를 보면서 식사를 한다'(2.94), '자주 과식을 한다'(2.98), '맛있는 음식이 있으면 배가 불러도 계속 먹는다'(3.03), '다른 식사보다 저녁을 많이 먹는다'(3.03), '밤 9시 이후에 간식을 먹는다'(3.07), '배가 고프지 않아도 뷔페의 음식을 모두 먹는다'(3.14) 순으로 나타났다. 여중생과 여고생의 비만관련 식행동을 비교한 결과, 총평균 점수는 여고생(3.06)이 여중생(3.33)에 비해 유의하게(p<0.001) 낮은 점수를 보여 비만관련 식행동이 나쁜 것으로 나타났다. 이상을 살펴볼 때, 조사대상 여중생과 여고생의 BMI는 유의한 차이가 없었으나 여고생이 여중생에 비해 뚱뚱한 체형이라고 응답한 비율과 자신의 체형에 대해 잘못 인식하는 비율이 높고 체형만족도가 낮으며, 비만관련 식행동이 나쁜 것으로 나타났다. 부적절한 식습관이 체중증가로 이어지고 사춘기에 가지는 예민한 감수성으로 단지 날씬해져야 한다는 생각에 스스로 체중감량을 시도하는 학생들이 많아지고 있는 것으로 여겨진다. 학교에서 양질의 급식이 이루어지고 있음에도 불구하고 학교 안에서 이를 해결할 수 있는 체계적인 영양교육이 이루지지 않고 있는 실정이다. 무엇보다 체형에 관심이 많은 여학생들이 불충분한 지식과 잘못된 상식으로 바람직하지 않은 방법의 체중조절을 하지 않도록 학교 안에서 체계적인 영양교육 프로그램을 운영하는 것이 중요하다. 특히 본 연구결과에서 도출된 바를 토대로 여고생들에 대한 체형인식 및 체중조절에 관한 영양교육 프로그램을 실시하여 올바른 체형인식과 식행동이 이루어질 수 있도록 하는 것이 시급한 것으로 사료된다.

유당 및 유지방을 중심으로 한 한국 성인 남성의 우유 섭취 행태 (Milk Intake Patterns with Lactose and Milk Fat in Korean Male Adults)

  • Objectives: This study examined the milk intake patterns with lactose and milk fat in Korean male adults using the following variables: milk intake level, awareness of lactose, and milk fat, health problems, and necessity of milk intake. In addition, the factors affecting milk intake were analyzed by multiple regression analysis. Methods: The subjects were 532 males aged 20 years or older among the nationwide milk purchasing group. The subjects were 223 (41.9%) in the 20-29 year age group, 188 (35.3%) in the 30-49 year age group and 121(22.7%) in the over 50 year age group. The survey was conducted using ANOVA and multiple comparative analysis to examine the differences in age and multiple regression analysis was performed to investigate the factors affecting the intake of milk. Results: The intake of milk in the subjects was $538.14{\pm}494.23ml$ per week. There were statistically significant differences in the subjects' age according to processed milk, low fat, nonfat milk, cheese, and ice cream. The perception of milk and lactose and milk fat was recognized as a good food for skeletal health when milk was consumed. Among the milk nutrients, lactose was highly recognized at the age of 20-29, and milk fat was recognized in those over 50 years. In addition to lactose and milk fat, calcium was the most highly recognized among the milk nutrients. Health problems associated with milk were skeletal health, obesity, and lactose intolerance. The perception of lactose intolerance was related to lactose intolerance and fatness, and the dietary behavior was unaffected. Conclusions: This study examined the milk intake patterns of adult Korean males. Many variables were found to be related to the intake of milk. In this study, the milk intake was high when there was no problem with the perception and dietary behaviors of milk nutrition (lactose and milk fat). This study focused on lactose and milk fat, which are major nutrients in milk, and it is a new perspective study among milk-related research.

Smoking, Meat Intake and Exercise related to Alcohol Use

  • This study was conducted to investigate the relationship between drinking, smoking, meat intake, and exercise. The participants were 1,060 males and females who voluntarily responded to the survey by visiting the health promotion booth at the H Festival held in C city in 2014. Research data were collected in interview form using questionnaires on alcohol use, smoking, exercise, and meat intake. The data were analyzed by descriptive analysis, cross tabulation analysis, multinominal logistic regression after stratification of gender using the SPSS 24 program. The results of the study are as follows: Smokers had a significantly higher frequency of drinking ($x^2=163.33$, p<.001) than non-smokers. Meat intake was high when alcohol use was high ($x^2=35.13$, p<.001). The risk rates for smoking, meat consumption, and exercise related to alcohol use are as follows. The smoking was higher as the frequency of drinking increased. For men, smoking OR(odd ratio) was 6.26 (p=.001) and for women, smoking risk OR was 16.82 (p=.002). Meats intake showed a higher OR as the alcohol use increased. The OR of almost daily ingestion of meat at frequent drinking days was 4.40 (p=.002) for males and 4.52(p=.009) for females. As the frequency of drinking increased, the OR of days of exercise tended to decrease. In men, the OR was 0.36 (p=.003), the lowest in the probability of exercising more than 5 days when drinking high. In the case of women drinking less than once a week, the OR was 0.43 (p=.027), which was the lowest for exercise for 5 days or more. In conclusion, the higher the frequency of drinking, the higher the smoking and meat intake, and the less exercise. If drinking, smoking, and meat intake are high but there is a lack of exercise, a chronic disease can occur and cause premature death unless there is a change in the new health behavior. The results of this study suggest researches to understand the motives related to alcohol abstinence, smoking cessation, dietary control and exercise, and to develop programs.

Risk of all-cause mortality is associated with multiple health-related lifestyle behaviors and does not differ between urban and rural areas in Korea

  • BACKGROUND/OBJECTIVES: Urban-rural inequities in health and mortality exist in Korea, a highly centralized developed country. The potential impact of multiple health-related lifestyle behaviors on mortality and difference between urban and rural areas is not fully understood. This study aimed to investigate the effect of high-risk health behaviors on all-cause mortality among residents living in urban and rural in Korea. SUBJECTS/METHODS: Cross-sectional analyses were conducted on 8,298 adults aged 40 yrs and older from the Korea National Health and Nutrition Examination Survey 2013-2015. High-risk behaviors were defined as having poor diet quality, current smoking, high-risk drinking, or insufficient physical activity. Mortality status was linked to the Cause of Death data followed up to December 31, 2019. The associations between all-cause mortality and high-risk behaviors were evaluated using Cox proportional hazard regression models adjusted for age, sex, education, income, and survey year. Population attributable fractions (PAFs) were calculated, and effect modification analysis was conducted. Participants were stratified by residential area (urban or rural). RESULTS: During the follow-up (median: 5.4 yrs), 313 deaths occurred. A higher proportion of rural residents than urban residents engaged in multiple high-risk behaviors (28.9% vs. 22.6%; P < 0.0001). As individual factors, a greater risk of mortality was associated with poor diet quality, current smoking, and inadequate physical activity, and these tendencies persisted in rural residents, especially for diet quality. Multiple high-risk behaviors were positively associated with a higher risk of mortality in Koreans living in urban and rural areas. PAF (95% confidence interval) was 18.5% (7.35-27.9%) and 29.8% (16.1-40.2%) in urban and rural residents, respectively. No additive or multiplicative effect of the region was observed. CONCLUSION: The higher prevalence of multiple high-risk lifestyle behaviors in rural residents may explain the higher mortality in rural areas compared to urban areas. Comprehensive public health policies to improve health-related behaviors in rural populations may be needed.

  • 대학생들의 신체활동도를 국제신체활동설문(IPAQ)을 통하여 조사하고 그에 따른 식행동, 건강관련 생활습관 및 식이섭취 상태를 알아보고자 인천지역 대학교에서 체중조절과 영양 관련 교양강의를 듣는 수강생들을 대상 (남자 243명, 여자 195명)으로 설문조사와 신체계측 및 3일간 식이섭취조사를 실시한 결과는 다음과 같다. 남학생과 여학생의 평균 연령은 각각 23.7세, 21.0세이었으며, 자택에서 거주하는 비율은 남녀 각각 59.3%, 73.3%이며 자취, 기숙사, 하숙이나 친척집에 거주하는 순이었다. 조사대상자의 50% 이상이 21${\sim}$40만원의 용돈을 받고 있었고, 여학생이 남학생에 비해 아르바이트를 하는 경우와 체중감소를 원하는 경우가 유의적으로 많았다. 대학생의 신체활동도를 조사해 본 결과 남학생은 저 신체 활동군이 11.5%인데 반해 여학생은 30.8%나 되었고, 고 신체 활동군의 경우 남학생은 38.7%, 여학생은 22.6%로 남학생이 여학생에 비해 신체활동도가 높았다. 여학생의 경우에는 고 신체 활동군의 신장이 저 신체 활동군의 신장보다 유의적으로 높았고, 근육량과 기초대사량의 경우 고 신체 활동군이 다른 군들에 비해 유의적으로 높은 값을 보였다. 남학생과 여학생 각각 46.3%, 58.5%가 평소에 아침식사를 하고 있었으며 남학생의 59.9%와 여학생의 31.8%는 식사를 빠르게 하는 것으로 나타났다. 남학생의 경우 저 신체 활동군이 다른 군들에 비해 외식 횟수의 비율이 낮았으며 남녀 모두 외식할 때 기호를 가장 많이 고려하였다. 간식은 주 3${\sim}$7회 섭취하는 비율이 높았으며 남학생은 저녁 이후(55.6%)에, 여학생은 오후(57.2%)에 많이 먹는 것으로 조사되었다. 남학생의 41.2%, 여학생의 3.6%가 흡연을 하며 남학생의 78.6%, 여학생의 68.2%가 음주를 하는 것으로 조사되었고, 대부분이 주 2회 이상 음주를 하고 있었다. 컴퓨터사용 및 TV 시청 시간은 남녀 각각 46%, 34.9% 가‘하루에 3시간 이상’이라고 대답하였으며 남학생의 경우 고 신체 활동군에서 자신이 건강하다고 지각하는 비율이 높았다.영양소 섭취량을 분석해 본 결과 신체활동도에 따른 차이는 보이지 않았으나 열량은 한국인 영양섭취기준의 필요추정량의 남녀 각각 74.4%, 80.1% 수준으로 섭취하고 있었으며 단백질의 섭취량은 남녀 모두 권장섭취량의 150%보다 높았다. 칼슘은 권장량의 40% 밖에 섭취하고 있지 않았으며, 철도 여학생은 섭취 부족의 심각성을 보이는 등균형 잡히지 못한 식생활 양상을 보여 주었다. 식사의 질을 살펴본 결과에서도 가장 NAR이 낮은 영양소는 남녀 모두 칼슘이었으며 영양밀도 지수 (INQ)의 결과 남학생은 엽산, Vit C, 칼슘이, 여학생은 엽산, Vit C, 칼슘, 철이 1보다 낮았다. 여학생의 경우 저 신체 활동군이 30% 이상이나 되었으나 평균 열량섭취는 필요추정량의 80.1%밖에 섭취하고 있지 않았다. 또한 평균 BMI는 정상이고 체지방율은 평균 28% 이상으로 높은 편에 속해 최근 논란화 되고 있는 마른비만의 문제를 보이고 있었다. 이와 같은 생활은 비타민과 무기질 등의 섭취 불균형으로 인한 각종 질병을 초래할 수 있고, 낮은 기초대사량으로 인하여 나이가 들어감에 따라 비만이 될 확률이 높아질 수 있다. 그러므로, 신체활동의 증가를 동반한 올바른 식습관과 정확한 영양지식을 바탕으로 한 식사섭취를 통해 건강한 생활을 유지할 수 있도록 영양교육이 더욱 활성화 되어야겠다.
