Development and Effects' Analysis of Nutrition Education Program for Diabetes Mellitus at Community Health Center - Focused on Individual Daily Energy Requirements and Food Exchange Units - (당뇨병 성인 대상 보건소 영양교육 프로그램 개발 및 효과 분석 - 개인별 맞춤 하루 필요 에너지 및 식품군 단위수 교육을 중심으로 -)

  • The purpose of this study was to investigate effects of the developed nutrition education program focused on individual daily energy requirements and food exchange units using Food Exchange System for diabetes mellitus at a community health center. Developed the nutrition education program, four weeks' nutrition education including provided twice individual meal as diet therapy (2 hour/lesson/week, 4 week), was provided to 20 diabetic elderly (12 male, 8 female, 50-75 yrs): 1st lesson "Introduction: management of diabetes mellitus", 2nd lesson "6 Food groups and sources of 6 food groups", 3rd lesson "Individual daily energy requirements and food exchange units", and 4th lesson "Food choice for diabetes mellitus". For effects' analysis of the developed program, we assessed the changes in anthropometric characteristics; biochemical characteristics and nutrient intakes using 24 hr recall method. Effects of the developed nutrition education program were as follows: weight was significantly decreased, blood urea nitrogen (BUN) and glycosylated hemoglobin (HbA1c) were significantly decreased, and distribution of subjects in BUN and HbA1c was significantly changed. In protein : fat : carbohydrate (PFC) ratio, it was significantly changed from 15.98 : 16.30 : 66.69 to 17.51 : 18.94 : 64.10. In evaluation of nutrient intakes by Dietary Reference Intakes for Koreans (KDRI), protein, fiber, fat, vitamin E, niacin, folic acid, calcium and zinc were shown significantly positive changes in distribution of subjects according to intake level. The index of nutrition quality (INQ), nutrition adequacy ratio (NAR) and mean nutrition adequacy ratio (MAR) were significantly increased. In conclusion, the developed 4 weeks' nutrition education program focused on individual daily energy requirements and food exchange units using Food Exchange System for diabetes mellitus at community health center may improve the symptom of diabetes mellitus.

Study on Relationship between the Obesity Index and the Refractive Power in Adolescent (청소년기 비만도 지수와 굴절력과의 상관성 연구)

  • Purpose: In this study, nutrient intake during the adolescent period is a critical time for the development of the vision. Therefore we analyzed the relationship between the obesity index and the refractive power. Methods: We used the Korean National Health and Nutrition Examination Survey 2010 document. The obesity index is classified as Broca index (less than 80.0% is low weight, 80 to 89.9% is underweight, 90.0 to 109.9% is normal, overweight is from 110.0 to 119.9%, 120.0% or more is obesity) and body mass index (BMI)(less than 18.5 is underweight, 18.6~22.9 is normal, 23.0 to 24.9 is overweight, and obesity is higher than 25.0). We analyzed correlation with the body mass index and refractive error in adolescent. Results: As shown in the statistics, according to Broca index and body mass index (BMI), the refractive power and the obesity index showed a statistically significant correlation in the ages 15 to 18. Under weight subjects are ingested 43.84% dietary fiber, vitamin C 56.55%, 35.20% iron, 31.84% of, and 38.03% potassium less than the average food intake by the standard group. Conclusions: Moderate weight with a broad variety of taking nutrition and good eating habits seems to have an effect to the good growth and the good vision.

Studies of nutrient composition of transitional human milk and estimated intake of nutrients by breast-fed infants in Korean mothers (한국인 수유부의 수유초기 이행유의 모유성분 분석과 영아의 섭취량 추정 연구)

  • Purpose: This study was conducted to examine the concentration of nutrients in transitional breast milk from Korean lactating mothers and to evaluate daily intakes of their infants based on the Dietary Reference Intakes for Koreans 2010 (KDRIs 2010). Methods: Breast milk samples were collected at 5~15 days postpartum from 100 healthy lactating Korean mothers. Macro- and micro-nutrients, and immunoglobulin (Igs) concentrations in breast milk were analyzed. Results: The mean energy, protein, fat, and carbohydrate concentrations in breast milk were $59.99{\pm}8.01kcal/dL$, $1.47{\pm}0.27g/dL$, $2.88{\pm}0.89g/dL$, and $6.72{\pm}0.22g/dL$. The mean linoleic acid (LA), a-linolenic acid (ALA), arachidonic acid (AA), and docosahexaenoic acid (DHA) concentrations were $181.44{\pm}96.41mg/dL$, $28.15{\pm}8.89mg/dL$, $5.67{\pm}1.86mg/dL$, and $5.74{\pm}2.57mg/dL$. The mean vitamin A, vitamin D, vitamin E, vitamin $B_1$, vitamin $B_2$, vitamin $B_{12}$, and folate concentrations were $2.75{\pm}1.75{\mu}g/dL$, $2.31{\pm}1.12ng/dL$, $0.74{\pm}1.54mg/dL$, $3.02{\pm}1.84mg/dL$, $7.51{\pm}20.96{\mu}g/dL$, $61.78{\pm}26.78{\mu}g/dL$, $63.71{\pm}27.19ng/dL$, and $0.52{\pm}0.26{\mu}g/dL$. The mean concentrations of calcium, iron, potassium, sodium, zinc, and copper were $20.71{\pm}3.34mg/dL$, $0.59{\pm}0.86mg/dL$, $66.71{\pm}10.35mg/dL$, $27.72{\pm}10.16mg/dL$, $0.44{\pm}0.41mg/dL$, and $70.48{\pm}30.41{\mu}g/dL$. The mean IgA and total IgE concentrations were $61.85{\pm}31.97mg/dL$ and $235.00{\pm}93.00IU/dL$. The estimated daily intakes of infants for protein, vitamin D, vitamin E, vitamin $B_2$, vitamin $B_{12}$, iron, potassium, sodium, zinc, and copper were sufficient compared to KDRIs 2010 adjusted by transitory milk intakes. The estimated infants' intakes of energy, fat, carbohydrate, vitamin A, vitamin C, vitamin $B_1$, folate, and calcium did not meet KDRIs 2010 adjusted by transitory milk intakes. Conclusion: In general most estimated nutrient intakes of Korean breast-fed infants in transitory breast milk were sufficient, however some nutrient intakes were not sufficient based on KDRIs 2010. These results warrant conduct of future studies for investigation of important dietary factors associated with nutrients in breast milk to improve the quality of breast milk, which may contribute to understanding nutrition in early life and promoting growth and development of breast-fed infants.

Dietary Factors and Life Style Affecting the Causal Infection on the Gastric Cancer of Adults in Rural Korea (위암발증 원인균에 대한 생활습관 및 영양섭취에 있어서 농촌지역 주민조사)

  • To examine the relationship between the causal infection of gastric cancer and lifestyle and nutritional status, we surveyed 183 persons, 102 female and 81 male, aged 40 years and over from August to October, 1996, in Chunchon area, Kang Won Do, Korea. For this purpose, we investigated Helicobacter pylori infection in the agricultural Korean adult and identified the effect of sex, age, smoking, alcohol consumption, economic status, occupation and lifestyle. Helicobacter pylori immunoglobulin G(IgG). Helicobacter pylori infection was present in 64.5% of total subjects, and 63.7% of female, 65.4% of male and its prevalence was increased with age, smoking, low education level, low monthly income, high childbirth. Some nutrients intakes were significantly difference between Helicobacter pylori infection positive(+) group and those negative(-) group. The negative Helicobacter pylori infection group were much more intaked energy, total protein, total fat(p<0.05), animal fat(p<0.05), Iron(p<0.001), vitamin A(p<0.01), vitamin C(p<0.05) than Helicobacter pylori positive infection group. But the calcium intakes were lower than positive(+) group.

Nutrient Density of Fast-Food Consumed by the Middle School Students in Cheongju City (청주지역 중학생의 패스트푸드 섭취량에 대한 영양소 밀도 평가)

  • The purpose of this study was to investigate fast-food consumption and it's nutrient density, and the correlation between subject's characteristics and consumption frequency of fast-foods among middle school students. The questionnaires were distributed to 150 male and 190 female students in Cheongju city. The data were analyzed using SAS and statistics used were percent of frequency, $X^2$-test, t-test, and pearson's correlation coefficient. The main reasons for using fast-food restaurants were 'good taste' and 'convenience'. The major concerns when the subjects choose fast-foods were 'taste ($65.9\%$), price ($20.3\%$) and 'nutrition'($6.0\%$). The consumption pattern of fast-foods go as follows: twice a month ($42.3\%$), once a week ($31.5\%$), and less than once a week. The number of fast food items that the subjects ate for one meal was two ($46.0\%$) or three ($33.2\%$). The most frequently chosen combination of foods for number of two choices was beefburger and cola. The mean average energy intake from fast foods for one meal was 620.7 kcal for male, 504.5 kcal for female. The energy ratio of carbohydrate : protein fat from fast foods was 49 : 14 : 43, which means fat intake is much higher than recommended level ($20\%$). Fiber was appeared to be the lowest on the nutrient density which was $17.7\%$ of the recommended level for Koreans, vitamin C was next ($22.8\%$ for male, $20.1\%$ for female). In mineral, iron was the lowest ($71.8\%$ for male, $67.1\%$ for female), and protein was over $100\%$ for both males and females. Frequency of fast food intake was positively correlated with eating frequency of the salty, the sweets, him, caffeine containing foods, instant noodles, and cookies. In conclusion, frequent consumption of fast foods can lead unbalanced nutrient intakes for middle school students, and those who consumed fast foods frequently showed undesirable food habits in their daily meal. Therefore, nutrition education for middle school students should be needed to encourage them to choose more nutritious food and have healthier dietary pattern.

Dietary Effect of Puer Tea Extract on the Body Weight in Rats

  • Puer tea is a traditional beverage originating from Yunnam area of China. It supplies basic nutrients such as vitamin C. It has been well reported that daily drinking of Puer tea can help the digestion and ease the stomachache after food intake. Puer tea also contains various polyphenols which may exert antibacterial activity against Staphylococcus aureus and Listeria monocytogenes. Because of these functional effects on digestive system we suspected if Puer tea can display any dietary effect or decrease the obesity after long-term drinking. We employed 6-week old SD rats as experimental animal and treated them with extract of Puer tea in relation to the body weights. Rats were divided into 5 groups (NC, PC, E, E+P, E+P5). NC group was experimental control and rest of them are as follows: water only (PC), water with exercise (E), water with exercise and Puer tea extract (E+P), water with exercise and 5X extract of Puer tea (E+P5). Feeding was carried out every day for 5 weeks by oral administration. Reduction rate of body weights was highest in E group. Relative ratio of losing weight was as follows: PC group (100.78%), E group (95.57%), E+P group (94.53%) and E+P5 group (74.22%), respectively. Exercise was more helpful to control the body weight. The result strongly suggests that Puer tea is highly effective to control the body weight and could be used for pharmaceutical purpose to treat obesity without side effects.

Changes in Lymphocyte DNA Damage and Antioxidant Status after Supplementing Propolis to Korean Smokers: A Placebo-Controlled, Double-Blind Cross-Over Trial (프로폴리스 섭취 후 흡연자의 임파구 DNA 손상도 및 항산화 상태의 변화: 이중맹검 교차 인체시험)

  • Smoking has been known to exacerbate the initiation and propagation of oxidative stresses. Efforts have been made to reduce the smoking-induced oxidative stresses using commercial dietary supplements. Propolis is the resinous substance collected by bees from the leaf buds and bark of trees, especially poplar and conifer trees. In this trial, we examined whether a daily supplementation of 800 mg propolis can protect endogenous lymphocytic DNA damage and modulate antioxidative enzyme activities and the level of antioxidant vitamin in smokers using a placebo-controlled, doubleblinded cross-over trial. After two weeks of running-in period, 29 smokers (mean age 34.38 ${\pm}$ 1.73) received 6 tablets/day of either propolis or placebo pills for 4 weeks. After 2 weeks of washout period the subjects switched they pills for cross-over study. The degree of DNA damage (assessed by tail DNA, tail length and tail moment) was not significantly changed with propolis intake or placebo intake. Similarly, total antioxidant status (TAS) remained at the same level regardless of the treatment. Erythrocyte catalase, glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), plasma vitamin C and tocopherol level did not differ before and after propolis treatment, and did not differ between treatments. Putting all these results together, we would suggest that it is still too early to claim that propolis possess antioxidative activities.

A Study on the Dietary Behaviors, Physical Development and Nutrient Intakes in Preschool Children (학령 전 아동의 식습관, 신체 발달 및 영양 섭취상태에 관한 연구)

  • The purpose of this study was to investigate the health status of preschool children using the questionnaires about dietary behaviors and anthropometric indices. And also nutritional status was investigated using questionnaires for 24-hr recall method. The study was conducted in 145 children aged 3 to 6 years and questionnaires for dietary behaviors and dietary intakes were performed by mothers of children in Ulsan. Just nine percent of children were graded as good in terms of having healthy eating habits, this means that the nutrition education for the dietary behaviors should be more focused on preschool children. With regard to the frequency of food intake, children consumed green & yellow vegetables less frequently, meanwhile consumed high protein source food (meat, egg and bean) and milk and its product more frequently. Children almost never consumed fried foods as often as 1-2 times a weak. In assessment of the health status, children have the highest prevalence of colds and allergy, but lower prevalence of clinical symptoms due to the nutritional deficiency. The mean height was $103.6\;{\pm}\;6.4\;cm$ and significantly different among age (p < 0.05), but was not significantly different between sex. The mean weight was $17.8\;{\pm}\;3.0\;kg$ and significantly different in 5, 6years old among age. By the WLI criteria, 11.1% of children were underweight and 17.4% of children were overweight or obese. By the Rohrer index criteria, any children were not underweight and 86.8% of children were overweight or obese. By the Kaup index criteria, 2.8% of children were underweight and 29.2% of children were overweight or obese. And Obesity Index criteria, 2.1% of children were underweight and 20.8% of children were overweight or obese. The results of obesity rate by all criteria except Rohrer index indicated similar level, were significantly high in age 3 with all criteria, and decreased with age increased. The energy intake of children was lower than EER (Estimated Energy Requirements) of Dietary Reference Intakes for Koreans (KDRIs) by as much as 85.7%. Acceptable Macronutrient Distribution Ranges (AMDR) was 62.6:21.5:15.7 as carbohydrate:protein:lipid, so children consumed protein more, but consumed lipid less compared with those of KDRIs. Vitamin A intake was 133% of recommended intakes (RI) and calcium intake which was identified as the nutrient most likely to be lacking in diets was 98.9% of RI. The intakes of all minerals and vitamins except folate were higher than KDRIs. 33.3% of children were distributed in insufficiency of energy intake, 42.7% of children were distributed in insufficiency of lipid intake. These results indicate that the need of developing of nutrition education program and further concern of a public health center, university and children care center about dietary life for preschool children.

Evaluation of the Nutrition Status and Metabolic Syndrome Prevalence of the Members according to the Number of Household Members based on the Korea National Health and Nutrition Examination Survey (2013-2014) (국민건강영양조사(2013-2014년) 자료에 근거한 가구원수별 구성원의 영양상태 및 대사증후군 유병율 평가)

  • Objectives: This study evaluated the nutritional status and prevalence of metabolic syndrome of the people who participated in the KNHANES according to the number of household members. They were assessed by using information from the 2013~2014 Korea National Health and Nutrition Examination Survey (KNHANES). Methods: A total of 6,088 persons aged 19 years and over participated in 2013~2014 KNHANES, and they were classified into three groups according to the number of household members (single-person, two-person, three-person & over). The dietary behavior, nutritional status, health-related factors and the prevalence of metabolic syndrome of the subjects were investigated with using information from the survey questionnaires of KNHANES. The nutrient intake data of the subjects were obtained by the 24-hour recall method and this was analyzed for evaluating the nutrition adequacy ratio and the index of nutritional quality. The prevalence of metabolic syndrome among the subjects, and according to the study groups, was estimated using the blood and physical measurement data of the subjects. Results: As for EQ-5D index available for all the health states generated by the EQ-5D descriptive system, the single-person household member was the lowest among all the household types. The index of nutrition quality for protein, crude fiber, calcium, phosphorus, potassium, riboflavin and vitamin C in the single-person household was lower than that of the two-person or the three-person and over households (p<0.001). The mean adequacy ratio of single-person households was significantly decreased compared with that of the other types of households (p<0.001). The prevalence of metabolic syndrome was higher in the single-person households than that in the multiple-person households (p<0.001). Conclusions: These results showed that dietary behaviors, nutrition status and health status might be influenced by the number of household members. The results from this study would be useful for improving Korean people's dietary life and health status by implementing evidence-based, specialized intervention for the members of diverse types of households.

Effects of Dietary Quercetin on Growth Performance, Blood Biochemical Parameter, Immunoglobulin and Blood Antioxidant Activity in Broiler Chicks (사료 내 Quercetin의 첨가가 육계의 생산성, 혈액 생화학 특성, 혈액 내 면역글로불린과 혈액 내 항산화 인자에 미치는 영향)

  • This study was conducted to investigate the effects of dietary quercetin on growth performance, blood biochemical parameters, immunoglobulin, and blood antioxidant activity in broiler chickens. Three hundred twenty one-day old Ross broilers were divided 8 treatments (C(-), basal diet; C(+), basal diet with antibiotics; vitamin E 20 IU; vitamin E 200 IU; quercetin 20 ppm; quercetin 200 ppm; methoxylated quercetin 20 ppm; methoxylated quercetin 200 ppm) with 4 replicates and 10 birds per replicate. Birds were reared for 35 days and their feed intake and weight gain were measured weekly. At 35d, eight birds of average weight from each replicate were selected for blood collection and analysis. Weight gain of birds in the groups fed quercetin was higher when compare to NC but there was no significant difference. In the serum, creatinine, BUN and AST in quercetin groups significantly decreased compared to those of control (NC and PC) (P<0.05). The contents of IgA and IgM were significantly lower in quercetin groups than those of NC (P<0.05). SOD like activity and MDA content tended to decrease in quercetin groups, however, there was no significant difference among treatments. In conclusion, supplemental quercetin to poultry diet could be positive aspect on performance and blood metabolites. Optimum adding levels was more than 20 ppm.
