Effects of a mobile healthcare service provided by public health centers on practicing of health behaviors and health risk factors

  • BACKGROUND/OBJECTIVES: This study evaluated whether a mobile health (mHealth) application can instigate healthy behavioral changes and improvements in metabolic disorders in individuals with metabolic abnormalities. SUBJECTS/METHODS: Participants were divided into an mHealth intervention group (IG), which used a mobile app for 24 weeks, and a conventional IG. All mobile apps featured activity monitors, with blood pressure and glucose monitors, and body-composition measuring devices. The two groups were compared after 24 weeks in terms of health-behavior practice rate and changes in the proportion of people with health risks, and health behaviors performed by the IG that contributed to reductions in more than one health risk factor were analyzed using multiple logistic regression. RESULTS: Preference for low-sodium diet, reading nutritional facts, having breakfast, and performing moderate physical activity significantly increased in the mHealth IG. Furthermore, the mHealth IG showed a significant increase of eight items in the mini-dietary assessment; particularly, the items "I eat at least two types of vegetables of various colors at every meal" and "I consume dairies, such as milk, yogurt, and cheese, every day." The proportion of people with health risks, with the exception of fasting glucose, significantly decreased in the mHealth IG, while only the proportion of people with at-risk triglycerides and waist circumference of females significantly decreased in the control group. Finally, compared to those who did not show improvements of health risks, those who showed improvements of health risks in the mHealth IG had an odds ratio of 1.61 for moderate to vigorous physical activity, 1.65 for "I do not add more salt or soy sauce in my food," and 1.77 for "I remove fat in my meat before eating." CONCLUSIONS: The findings suggest that the additional use of a community-based mHealth service through a mobile application is effective for improving health behaviors and lowering metabolic risks in Koreans.

A Case Study of Taeeumin Patient with Edema Treated with Gunyuljejo-tang (태음인 부종 환자의 건율제조탕 치험 1례)

  • Objectives Edema occurs in various disorders, such as heart failure, nephrotic syndrome, renal failure, liver cirrhosis, and cancer. The excessive accumulation of interstitial fluid in limb tissue will often manifest with ankle and knee swelling and lowers patient's quality of life. But the conventional treatment includes restricting dietary sodium and using diuretics. The purpose of this study is about significant improvement of a patient with edema who had suffered from dependent gait accompanied by mild pain, after treatment by herbal medicine based on SCM(Sasang constitutional medicine). Methods The pattern identification of the patient was diagnosed with a superficial disease of Taeeumin. So after the admission, Gunyuljejo-tang was used, almost two times per day during 2 weeks. Also, we applied acupuncture routinely every day and western medicine as needed. We measured the circumferences of Lt. thigh and followed up the bioelectrical impedance analysis and checked the appearance of lower limb. Results and Conclusions The therapy was effective, and the circumferences, which was 59.69cm at onset time, decreased to 52.07cm almost same to the healthy side of her thigh. There was no more edematous sign on the body, either. This case showed that management of Taeeumin's healthy energy(呼散之氣, the energy of exhalation) could be effective in treating edema.

  • Ascites is the accumulation of fluid in the peritoneal cavity, and it is generally allied with liver disease like cirrhosis of the liver. The ascites is commonly associated with liver cirrhosis, malignancy, or cardiovascular disease today. The liver cirrhosis is the most significant cause of ascites in developed countries as per the available data. In the modern medicine system, the treatment includes the restriction of sodium intake, use of diuretics, and paracentesis done in severe cases. There are many side effects of synthetic chemical diuretics, and their long-term use is not recommended. The alternative medicine like Unani system of medicine provides natural diuretics and drugs which also have protective effects on kidney, liver and other internal organs. It is well known in all drug systems that the prognosis is poor in ascites, but the use of Unani herbal medicines can minimize the side effects caused by synthetic chemical diuretics. The natural herbs used for the management of ascites have fewer side effects as compared to synthetic drugs. The natural drugs can be used for longer duration and provide some dietary supplement which improves the quality of life. This review addressed the thorough treatment of ascites by natural diet and medications.

Comparison of the Nutrient Intake and Health Status of Elderly Koreans According to their BMI Status: Focus on the Underweight Elderly Population (우리나라 노인의 체질량지수에 따른 영양소 섭취 수준과 건강 상태 비교: 저체중 노인을 중심으로)

  • Objectives: With an increase in the population of the elderly in Korea, their nutritional status has become a cause for concern. This study was designed to compare the nutritional intake and health status of the Korean elderly according to their body mass index. Methods: The subjects were 3,274 elderly people aged 65 and above who had participated in the 2016-2018 Korea National Health and Nutrition Examination Survey. The subjects were divided into four groups: underweight, normal, overweight, and obese, based on their BMI. The general characteristics, daily energy, and nutrient intakes, nutrient intakes compared to the recommended nutrient intake, percentage of participants whose nutrient intake was lower than the estimated average requirement (EAR), index of nutrient quality, the mean adequacy ratio (MAR), intakes by food group, and health status of the four groups were compared. Results: Underweight elderly people showed lower energy, lipids, dietary fiber, vitamin C, riboflavin, niacin, phosphorus, sodium, and potassium intake and MAR score (P < 0.001) compared to the normal or obese elderly. The mean protein, riboflavin, niacin, vitamin C, phosphorus, and iron intake of the underweight elderly was lower than the EAR (P < 0.05). Underweight elderly people also had a lower intake of vegetables and fats, oil and sweets food groups than the other groups (P < 0.001). The prevalence of diabetes and dyslipidemia was higher in the obese group, but the percentage of anemia was higher in the underweight group. Conclusions: Underweight elderly people were vulnerable to undernutrition and were at a higher risk of anemia.

Higher Protein Digestibility of Chicken Thigh than Breast Muscle in an In Vitro Elderly Digestion Model

  • This study investigated the protein digestibility of chicken breast and thigh in an in vitro digestion model to determine the better protein sources for the elderly in terms of bioavailability. For this purpose, the biochemical traits of raw muscles and the structural properties of myofibrillar proteins were monitored. The thigh had higher pH, 10% trichloroacetic acid-soluble α-amino groups, and protein carbonyl content than the breast (p<0.05). In the proximate composition, the thigh had higher crude fat and lower crude protein content than the breast (p<0.05). Sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) of myofibrillar proteins showed noticeable differences in the band intensities of tropomyosin α-chain and myosin light chain-3 between the thigh and breast. The intrinsic tryptophan fluorescence intensity of myosin was lower in the thigh than in the breast (p<0.05). Moreover, circular dichroism spectroscopy of myosin revealed that the thigh had higher α-helical and lower β-sheet structures than the breast (p<0.05). The cooked muscles were then chopped and digested in the elderly digestion model. The thigh had more α-amino groups than the breast after both gastric and gastrointestinal digestion (p<0.05). SDS-PAGE analysis of the gastric digesta showed that more bands remained in the digesta of the breast than that of the thigh. The content of proteins less than 3 kDa in the gastrointestinal digesta was also higher in the thigh than in the breast (p<0.05). These results reveal that chicken thigh with higher in vitro protein digestibility is a more appropriate protein source for the elderly than chicken breast.

Survey of the Food Preferences and Nutrient Intakes of Elderly People Hospitalized in a Nursing Hospital in Daegu (대구지역 요양병원 입원노인의 식품선호도 및 영양소 섭취 실태 조사)

  • The number of elderly in care facilities is increasing in parallel with a rapid increase in the elderly population in Korea. We analyzed the meal satisfaction ratings, food preferences, and nutritional intakes of 74 patients aged ≥65 years living in a nursing hospital in Daegu. Food preferences were high for cooked rice, rice gruel, meat soup, soybean paste stew, beef, frozen pollack, squid, egg, spinach, sea mustard, cabbage kimchi, apple, peanut, drinking yogurt, pickled perilla leaves, and salted squid. Preferences for duck (P<0.01) and vegetable (P<0.01) cooking method were significantly different for men and women, and preference for soybean paste stew increased with age (P<0.05). Regarding nutrient intakes, men had higher energy (P<0.01), carbohydrates (P<0.05), and zinc (P<0.05) intakes than women, while women had higher vitamin C (P<0.05) intake than men. Furthermore, energy (P<0.001), carbohydrates (P<0.001), protein (P<0.05), dietary fiber (P<0.01), vitamin B6 (P<0.01), phosphorus (P<0.05), and zinc (P<0.001) intakes significantly decreased with age. Men had a significantly higher nutrient adequacy ratios (NARs) for carbohydrates (P<0.05) and cal- cium (P<0.01) than women, while women had a significantly higher NARs for protein (P<0.05), vitamin A (P<0.01), vitamin C (P<0.05), thiamine (P<0.01), riboflavin (P<0.001), iron (P<0.001), and zinc (P<0.01). Mean adequacy ratios were 0.69 for men and 0.75 for women. In particular, NARs for carbohydrates (P<0.01), vitamin B6 (P<0.01), and calcium (P<0.05) decreased significantly with age. In summary, the study shows that supplementing vitamin C, vitamin B6, calcium, and iron, reducing sodium, and providing meals that reflect preferences will improve the nutritional statuses of elderly residents in a nursing hospital.

Extraction of Liberated Reducing Sugars from Rapeseed Cake via Acid and Alkali Treatments (산 및 알칼리 처리에 의한 유채박의 유리당 추출)

  • Rapeseed cake, which is the organic waste remaining after rapeseed oil production, is readily available and considered an ecologically-friendly resource with very low cost and high dietary fiber content. This research was carried out for two reasons. First, it was done to analyze the liberated reducing sugar content of rapeseed cake. Second, it was done to investigate the effects on the sugar yield of the various concentrations of acidic and alkaline catalysts used for the hydrolysis of rapeseed cake and the concentrations of rapeseed cake in each catalyst. Several amounts of ground rapeseed cake, 0.5 g, 1 g, and 2 g, were put into 100 mL of catalysts such as sulfuric acid (0.5~2%), hydrochloric acid (0.5~2%), and sodium hydroxide (0.5~2%). Then they were hydrolyzed for 5 min at 121$^{\circ}C$. After hydrolysis, HPLC equipped with an RI detector was used to analyze liberated reducing sugars such as sucrose, glucose, galactose, fructose, and arabinose separated from rapeseed cake. The degradation rate of rapeseed cake was the highest in hydrochloric acid. As the catalyst concentrations used for hydrolysis of rapeseed cake increased, the degradation rate of rapeseed cake also significantly increased. Total reducing sugar content was the highest in hydrochloric acid, and it increased with the increase of catalyst concentrations. However, as the amount of rapeseed cake increased, the total reducing sugar content decreased, exceptionally sucrose in the case of sodium hydroxide.

Monitoring of Forbidden Medicines as Adulterants in Dietary Supplements Marketed Online by HPLC and ESI-tandom Mass Spectrometry (HPLC 및 ESI-tandom Mass Spectrometry를 이용한 인터넷에서 판매되는 기능성표방식품 중 부정유해물질 모니터링)

  • It was carried out to detect anti-impotence drug-like compounds, sibutramine and their analogues in dietary supplements, which are doubtful whether they include illegal compounds. A total of 51 food products were bought online and have been investigated. The separation was achieved on a C18 column, with the mobile phase made up of water (5 mM sodium hexanesulfonate and 0.1% phosphoric acid) and 95% acetonitrile, at a flow rate of 1.2 mL/min with gradient elution using by HPLC-DAD. The UV signals were monitored at 220 nm and 291 nm. LC-ESI-tandom MS was utilized to confirm that detected compounds in samples are the same as the reference materials. Two nutrient supplement foods and ginseng products were found to contain 1.3-82.1 mg of sildenafil, dimethylthiolsildenafil and pseudovardenafil per serving size. In addition, two other processed products were detected to contain 1.7 and 2.2 mg of didesmethylsibutramine, derived from sibutramine per serving size.

Comparison of Nutritional Compositions of Green Vegetables (나물의 영양성분 비교)

  • This study examined the nutritional compositions of seven raw and blanched green vegetables: Amaranthus mangostanus L. (AM), Aster scaber Thunberg, Taraxacum platycarpum H. Dahlstedt (TP), Oenanthe javanica (Blume) DC, Allium tuberosum Rottler, Ligularia fischeri (Ledeb.) Turcz (LF), and Euonymus alatus (Thunb.) Siebold (EA). Proximate compositions (moisture, protein, ash, dietary fiber, and fat), minerals (calcium, phosphorus, iron, potassium, sodium, and magnesium), and vitamins (riboflavin and vitamin C) were analyzed in the study. In this study, moisture contents of raw and blanched green vegetables were 71.28~92.68 g/100 g and 76.90~92.09 g/100 g, respectively. The protein and fat contents of raw LF (33.38 g/100 g and 4.50 g/100 g dry weight basis, respectively) were higher than those of other cultivars. Ash contents of AM were significantly higher than those of their respective raw samples. Potassium, magnesium, calcium, and sodium contents of AM were 7,710.63 mg/100 g, 1,786.67 mg/100 g, 1,534.44 mg/100 g, and 743.18 mg/100 g, respectively. In the case of raw green vegetables, iron content of TP was 55.23 mg/100 g, which was the highest value among tested green vegetables. Total dietary fiber content of EA was 59.75 g/100 g, which was the highest value. Riboflavin content was highest in the raw and blanched LF (3.10 mg/100 g and 3.05 mg/100 g, respectively). Vitamin C contents of raw and blanched EA were 468.28 mg/100 g and 471.42 mg/100 g, respectively, which were higher than the other values. There were differences in nutritional ingredients according to different types of green vegetables. Therefore, it is advisable to evenly ingest various green vegetables.

A Study on Correlations between Dietary Nutrients and Body Composition of College Students (남녀 대학생의 영양소 섭취 상태와 체성분 조성의 상관관계)

  • A survey was conducted for 221 male and 191 female college students to study correlations between dietary food intakes and body composition. Among the subjects 75 male and 82 female students were analyzed body protein mass, body mineral mass, percent body fat, waist-hip ratio (WHR), body mass index (BMI) and fitness score using Inbody 3.0. The results show that 94.6% of the males and 24.7% of the females were observed at high level in body protein mass. As for body mineral mass, 86.4% of the males were observed below the normal range. As for WHR, 32.1% of the males and 43.5% of the females were evaluated as abdominal obesity. As for BMI, 34.8% of the males were observed above the normal range. In female students, correlation coefficient between sodium intakes and age showed significantly negative 0.232 while correlation between calcium intakes and self-reported health status showed significantly positive 0.234. In males, correlation coefficient between fiber intakes and self-reported health status resulted in significantly positive 0.237 and that between BMI and age showed significantly positive 0.276. In females, correlation coefficient between type of living and WHR showed significantly positive 0.253 and that between self-reported health status and percent body fat showed significantly positive 0.230. Also correlation coefficient between body protein mass and sodium intakes showed significantly negative 0.276 in female students.
