한국 성인의 영양위험군 진단을 위한 식생활진단표의 개발과 타당성 검증에 관한 연구 (Development and Validation of Mini Dietary Assessment Index for Koreans)

  • Improving dietary patterns, na, in turn, improving nutritional status, is now viewed as a key to improve public health and to prevent chronic diseases. There has been a peat needs to assess diet quality to identify nutritional risk group, however, little research has been done on methods to assess overall diet quality. The purpose of this study was to develope a mini dietary assessment (MDA) index for evaluation of overall dietary quality. A 10-component system was devised based on dietary guidelines and food Tower for Koreans. The system contained 4 food elements of which use is encouraged, such as milk, meat, vegetables, fruits, and 3 food elements of which use is limited, such as fat, salt, or sugar. Also the included were elements on dietary regulation and variety. A subject is to check one of ‘always' 'generally'seldom', which has score of 5, 3, and 1, respectively, so the total possible index score is 50. This index is to use without dietary survey and is to use even by non-nutritionist. A sample of 432 healthy males and females in their 30's and 40's contributed diet intake data based on 24-hour recall for the validation of MDA. The mean MDA score was 31.34 of a possible 50 points. The main nutrients for each MDA component was correlated very well with the results of 24-hour recall. Also, very good correlation was found between healthy eating index (HEI) score and MDA score. However, some of MDA components were needed a modification of term or/and statement. So the Uh was revised and another effort for validation was made with new sample of 169 subjects and even better correlation was found. The revised MDA could be used with minor modification to assess diet quality and to screen nutritional status. (Korean J Nutrition 36(1): 83-92, 2003)

  • Obesity is a state in which there is an over-accumulation of subcutaneous and/or abdominal adipose tissue. This adipose tissue is no longer considered inert and mainly devoted to storing energy; it is emerging as an active tissue in the regulation of physiological and pathological processes, including immunity and inflammation. Adipose tissue produces and releases a variety of adipokines (leptin, adiponectin, resistin, and visfatin), as well as pro- and anti-inflammatory cytokines (tumor necrosis factor-${\alpha}$, interleukin [IL]-4, IL-6, and others). Adipose tissue is also implicated in the development of chronic metabolic diseases such as type 2 diabetes mellitus or cardiovascular disease. Obesity is thus an underlying condition for inflammatory and metabolic diseases. Diet or dietary patterns play critical roles in obesity and other pathophysiological conditions. A healthy diet and some nutrients are generally considered beneficial; however, some dietary nutrients are still considered controversial. In this article, dietary factors that influence inflammation associated with obesity are discussed.

어머니의 종합적 식사지도와 유아의 식생활 실천과의 관계 (Associations between maternal comprehensive feeding practices and dietary practices in preschool children)

  • 본 연구는 어머니의 종합적 식사지도와 유아의 식생활 실천과의 관련성을 알아보기 위해 실시되었다. 서울과 경기 지역의 어린이집과 유치원에 재학 중인 만 3세부터 만 5세의 유아의 어머니 227명을 대상으로 CFPQ를 이용하여 어머니의 식사지도 내용을 조사하였으며, 유아의 영양지수 (NQ-P)를 측정하여 식생활 실천 정도를 평가하였다. 유아의 영양지수 중 균형 요인은 어머니의 종합적 식사지도의 모델링, 체중조절을 위한 섭취제한, 균형 및 다양성 권장, 음식강요 및 식사준비 참여와 양의 상관관계를 나타내었다. 유아의 영양지수의 절제 요인은 어머니의 종합적 식사지도의 모니터링, 모델링, 영양정보 전달, 균형 및 다양성 권장, 건강한 식생활 환경, 식사준비 참여와 양의 상관관계를, 정서조절, 음식보상, 아동통제와는 음의 상관관계를 나타내었다. 유아의 영양지수 환경 요인은 어머니의 종합적 식사지도 중 모니터링, 모델링, 영양정보 제공, 균형 및 다양성 권장, 건강한 식생활 환경, 식사준비 참여와 양의 상관관계를 보였으며, 정서조절과는 음의 상관관계가 나타났다. 유아의 식생활 실천정도를 나타내는 지표인 영양지수 각 요인에 대한 어머니의 종합적 식사지도의 영향을 평가하기 위해 다중회귀분석을 실시한 결과, 영양지수 중 균형 요인을 양의 방향으로 유의하게 예측가능한 식사지도 항목은 체중조절을 위한 음식섭취제한, 음식강요, 식사준비 참여였다. 어머니의 식사지도 항목 중 영양지수 절제 요인에 대한 양적 연관성이 있는 요인은 균형 및 다양성 권장, 건강한 식생활 환경이었으며, 음적으로 연관성이 있는 요인은 음식보상, 아동통제였다. 또한 영양지수 환경 요인과 양적 연관성이 있는 어머니의 식사지도 항목은 모니터링, 건강한 식생활 환경이었다. 따라서 어머니는 자녀에게 모니터링, 모델링, 영양정보 전달, 균형 및 다양성 권장, 건강한 식생활 환경, 식사준비 참여와 같은 식사 지도를 실천하고, 정서조절, 음식으로 보상, 아동통제, 음식강요 와 같은 부정적인 식사지도 방법을 지양해야 할 것이다.

제2형 당뇨병 환자의 식사요법 실천 정도, 식이섭취 상태, 신체활동과 대사증후군 유병률과의 관계 (The Relations between Diabetic Dietary Compliance, Dietary Intake, and Physical Activity and the Prevalence of Metabolic Syndrome (MS) in Type 2 Diabetic Patients)

  • Objectives: The purpose of this study was to investigate the relations between diabetic dietary compliance and dietary intake, physical activity and prevalence of metabolic syndrome (MS) in type 2 diabetic patients. Methods: Seventy five subjects diagnosed with type 2 diabetes visiting the D hospital in Dongducheon from May 2014 to Dec 2014 were included in this study. The subjects were divided into two groups according to their diabetic dietary compliance score (median 39); low diabetic dietary compliance (LDDC) group (n=44) and high diabetic dietary compliance (HDDC) group (n=31). Survey data collection was carried out by direct interview method. The nutrient intake, food intake and KDDS (Korean's dietary diversity score), DVS (dietary variety score) and GMVDF (grain, meat, vegetable, dairy and fruit) were analyzed using data from the 24-recall method. Metabolic parameters were obtained from the hospital records. Data was analyzed using Chi-square test and general linear model adjusted for sex. Results: The prevalence of MS was 59.7% in the participating diabetic patients. The prevalence of MS of the HDDC (39.3%) was significantly lower than that of the LDDC (72.7%). The distribution of physical activity showed a significant difference between the groups (p=0.006). The intakes of seeds and nuts of the HDDC were significantly lower than those of the LDDC. Fat and vegetable fat intakes and % fat of energy in the HDDC were significantly lower than those in the LDDC. But, carbohydrate (CHO) and potassium intake and %CHO of energy in the HDDC were significantly higher than those in the LDDC. KDDS and GMVDF showed significant differences between groups (p=0.033; p=0.012). Conclusions: Continuous intervention by specialized nutritionists for long-term self-regulation is needed for diabetic patients, and the high compliance to diabetic diet and increasing physical activity may be effective in the prevention of metabolic syndrome.

Regulation of Nrf2-Mediated Phase II Detoxification and Anti-oxidant Genes

  • The molecular mechanisms by which a variety of naturally-occurring dietary compounds exert chemopreventive effects have been a subject of intense scientific investigations. Induction of phase II detoxification and anti-oxidant enzymes through activation of Nrf2/ARE-dependent gene is recognized as one of the major cellular defense mechanisms against oxidative or xenobiotic stresses and currently represents a critical chemopreventive mechanism of action. In the present review, the functional significance of Keap1/Nrf2 protein module in regulating ARE-dependent phase II detoxification and anti-oxidant gene expression is discussed. The biochemical mechanisms underlying the phosphorylation and expression of Keap1/Nrf2 proteins that are controlled by the intracellular signaling kinases and ubiquitin-mediated E3 ligase system as well as control of nucleocytoplasmic translocation of Nrf2 by its innate nuclear export signal (NES) are described.

The Influence of Lipids on Exocrine Pancreatic Secretions in Pigs - Review -

  • The characteristics of the exocrine pancreatic secretions in pigs and its hormonal regulation as influenced by dietary lipids are reviewed. There is clear evidence that the secretion of lipolytic enzymes is positively correlated with the amount of fat consumed by the pig. For example, there was an increase in the specific lipase activity by 83% after the dietary fat content was increased from 5% to 25%. Moreover, it was shown that also the quality of fat has an influence on exocrine pancreatic secretions. Peroxidized canola oil stimulated total lipase secretion much more than non-peroxidized oil. The influence of fatty acid composition on exocrine pancreatic secretions is discussed equivocally. Some authors showed that saturated fats stimulated the exocrine pancreatic secretions more than unsaturated. Others showed that the chain length of fatty acids had a strong influence on pancreatic secretions as well. Due to the different surgical methods used for sampling of pancreatic juice and wide variety of fats and oils used in these studies, direct comparisons between studies are extremely difficult to make. Plasma levels of hormones such as cholecystokinin (CCK), neurotensin (NT) and peptide YY (PYY) are influenced by the nutrient composition of the diet. With increasing amounts of fat present in the small intestine, the release of these hormones was stimulated. There is evidence that CCK release is dependent on the chain length of the fatty acids. Medium chain triglycerides stimulated the CCK release more than long chain triglycerides. Neurotensin was released more by unsaturated than by saturated fatty acids; similar results were observed for the PYY release. However, results are contradictory and further investigations are warranted that focus on the underlying mechanisms involved in the regulatory response of the exocrine pancreas to lipids of different origin.

Evaluation and Categorization of Commercially Prepared Enteral Nutrition Formulas

  • In order to investigate the types of enteral nutrition formulas currently used in hospitals and evaluate and categorize the commercially prepared enteral nutrition formulas formulas available in the domestic market, we asked dietitians working in 6 hospitals in Seoul to complete the questionnaire and obtained compositional characteristics of 12 commercially prepared enteral nutrition formulas. The average proportion of patients receiving the commercially prepared enteral nutrition formulas(60.6%) was greater than that of patients receiving the in-hospital preparations(31.9%). In the group of patients receiving the in-hospital prepared formulas, the enteral feeding was mainly administered orally, whereas, in the group of patients receiving the commercially prepared formulas, tube feeding was the primary route of formula administration. In both groups, however, a greater proportion of patients received the formulas as total replacements of their meals and for the purpose of dietary supplementation. On the basis of major criteria for evaluation of the commercially prepared enteral nutrition formulas, the 6 products out of the 9 nutritionally complete products formulated for the purpose of dietary supplementation were grouped into the same category(standard protein, caloric density of 1kcal/ml, and tube/oral), so they were considered therapeutically comparable. However, the remaining 3 products were different in protein content(high protein) or route of administration(tube only). Of the 3 nutritionally complete products formulated specifically for the purpose of dietary therapy, 2 products were formulated for patients with renal disease, and the one product was formulated for diabetic patients. Therefore, the data in this study showed that the commercially prepared enteral nutriton formulas became an important part of the enteral nutrition for hospitalized patients in Korea, but the domestic market has not yet generated a wide variety of the formulas, not providing many choices for clinicians to manage the diets for their patients. The results of this study would be helpful for clinicians in choosing appropriate products for their patients, for manufactures in developing new products, and for regulatory authorities to establish the regulation for the broad group of heterogeneous products that are marketed and will be developed as medical foods. In addition, the process of maintaining the categories for evaluation of the commercially prepared enteral nutrition formulas should be dynamic because new products may not reasonably fit any of the existing categories.

Protective effect and mechanism of ginsenoside Rg2 on atherosclerosis

  • Background: Ginsenoside Rg2 (Rg2) has a variety of pharmacological activities and provides benefits during inflammation, cancer, and other diseases. However, there are no reports about the relationship between Rg2 and atherosclerosis. Methods: We used 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) to detect the cell viability of Rg2 in vascular smooth muscle cells (VSMCs) and human umbilical vein endothelial cells (HUVECs). The expression of inflammatory factors in HUVECs and the expression of phenotypic transformation-related marker in VSMCs were detected at mRNA levels. Western blot method was used to detect the expression of inflammation pathways and the expression of phenotypic transformation at the protein levels. The rat carotid balloon injury model was performed to explore the effect of Rg2 on inflammation and phenotypic transformation in vivo. Results: Rg2 decreased the expression of inflammatory factors induced by lipopolysaccharide in HUVECs-without affecting cell viability. These events depend on the blocking regulation of NF-κB and p-ERK signaling pathway. In VSMCs, Rg2 can inhibit the proliferation, migration, and phenotypic transformation of VSMCs induced by platelet derived growth factor-BB (PDGF-BB)-which may contribute to its anti-atherosclerotic role. In rats with carotid balloon injury, Rg2 can reduce intimal proliferation after injury, regulate the inflammatory pathway to reduce inflammatory response, and also suppress the phenotypic transformation of VSMCs. Conclusion: These results suggest that Rg2 can exert its anti-atherosclerotic effect at the cellular level and animal level, which provides a more sufficient basis for ginseng as a functional dietary regulator.
