Massive hydrothorax complicating continuous ambulatory peritoneal dialysis (CAPD) is relatively rare. A 67-year-old male and a 23-year-old female patients during CAPD presented massive pleural effusion. They have been performing peritoneal dialysis due to end-stage renal disease for 8 months and 2 weeks respectively. We injected $^{99m}Tc$-labelled radiopharmaceutiral (phytate and MAA, respectively) into peritoneal cavity with the dialysate. The anterior, posterior and right lateral images were obtained. The studies reveal visible radioactivity in the right chest indicating the communication between the peritoneal and the pleural space. After sclerotherapy with tetracycline, the same studies reveal no radioactivity in the right chest suggesting successful therapy. We think nuclear imaging is a simple and noninvasive method for the differential diagnosis of pleural effusion in patients during CAPD and the evaluation of therapy.
A hollow fiber device was utilized to perform a dialysis culture for E. coli. Acetic acid inhibition on the growth of E. coli was relieved by dialyzing the acid from broth into a dialysate reservoir. The rate of acetic acid formation was very sensitive to the concentration of glucose and dissolved oxygen. Therefore it was found that the glucose permeation rate should be balanced with the oxygen supply rate. Specific growth rate of E. coli was determined by the glucose permeation rate through membrane. Under a low permeation rate, acetic acid formation was depressed in accordance with high dissolved oxygen concentration as well as low glucose concentration.

We have determined the urea concentration in an aqueous solution using Raman spectroscopy by incorporating a Teflon tube as an effective intensity correction standard as well as sample container. A non-overlapping Teflon band was used as the reference peak to correct Raman intensity variations that occasionally resulted from changes in laser power. To increase the sensitivity, we positioned a copper reflector inside the Teflon tube to maximize the collection of Raman scattering. The obtained accuracy using Raman spectroscopy was 0.53 mM, close to the range of accuracy of previous NIR studies (0.15-0.52 mM).
It was found that water extract of Panax ginseng strongly inhibited adrenaline-induced lipolysis in isolated fat cells of rat epididymal adipose tissue. An antilipolytic action of the water extract was easily inactivated by treatment with pronase, suggesting that the active principle might be a protein or a peptide. Experiments were designed to purify the antilipolytic substance, or insulin-like substance, of the water extract. The water extract was dialyzed against disti'led water. The outer dialysate was subjected to DEAE-cellulose column chromatography, gelfuaration on sephadex G-50 column, avicel cellulose column chromatography and phospho-cellulose column chromatography, successively. The finally purified substance gave one spot on thin layer chromatography. The molecular weight was found to be around 1000. Experiments are now in progress to elucidate the structure of this insulin-like peptide.
Purpose : Genetic and clinical factors can influence the permeability of the peritoneal membrane. The peritoneal equilibration test (PET) is helpful in measuring peritoneal permeability in peritoneal dialysis (PD). We investigated the influence of genetic polymorphism of vascular endothelial growth factor (VEGF) on the PET parameters. Methods : Pediatric patients who underwent PET within 12 months of initiating PD at Seoul National University Children's Hospital and Samsung Medical Center were selected. The patients with positive history of peritonitis before PET were excluded. The VEGF -2578C/A, -14978T/C, -1154G/A, -634G/C, and +936C/T single-nucleotide polymorphisms were genotyped. Results : The mean 4-hour dialysate-to-plasma ratio for creatinine (D/P creatinine) and the mean 4-hour dialysate glucose to baseline dialysate glucose ratio (D/$D_0$ glucose) were $0.56{\pm}0.13$ and $0.43{\pm}0.11$, respectively. The patients with haplotype CTGGC showed higher 4-hour D/P creatinine ($0.67{\pm}0.12$ vs $0.50{\pm}0.09$, P=0.007) and lower 4-hour D/$D_0$ glucose ($0.35{\pm}0.12$ vs $0.47{\pm}0.08$, P=0.037) than those without haplotype CTGGC. Conclusion : The VEGF genetic polymorphism may influence the peritoneal solute transport.
The studies were carried out to investigate the effect of recovery in rewarming using the esophageal thermal tube in the deep hypothermia($25{\pm}1^{\circ}C$ ; rectal temp) in rabbits. Fifteen rabbits were divided into control group(n=6), peritoneal dialysis group(n=5) irrigated with dialysate at $42{\pm}1^{\circ}C$, and esophageal rewarming group(n=4) perfused with circulating water at $38{\pm}1^{\circ}C$. Rewarming of the rabbits was performed for 5 hours. MAP, HR, RR, esophageal temp, rectal temp, pH, $pCO_2$, $pO_2$, $Na^+$, and $K^+$ were observed. The results obtained in these experiments were summarized as follows : Esophageal rewarming group($38{\pm}1^{\circ}C$) had more effect on esophageal temperature than other groups. Peritoneal dialysis group($42{\pm}1^{\circ}C$) had more effect on rectal temperature and $pO_2$ than other groups. The both groups also had more effects on MAP, HR, RR, and $pCO_2$ than control group. Three groups had no significant effect on pH, $Na^+$, and $K^+$. In conclusion, we found that the simple, safe, and non-invasive esophageal rewarming method had an effect on the treatment of profound hypothermia as well as the peritoneal dialysis method in spite of the temperature difference between the dialysate and the circulating water, and the circulating water at $38{\pm}1^{\circ}C$ for esophageal rewarming also had an effect on the recovery of deep hypothermia.
Purpose : Regional anticoagulation with trisodium citrate for continuous renal replacement therapy(CRRT) is an effective and safe method, with lower bleeding risk. However it is not widely used because of complex current protocols used to prevent anticipated metabolic derangements. We evaluated simplified regional anticoagulation protocols with ACD-A(R) solution and commercially available calcium-containing dialysis solution. Methods : The medical records of twenty-eight patients who underwent CRRT were reviewed. Hemofilter life span according to the anticoagulation method used was compared, and laboratory findings at Pre- and 48 hours post-CRRT initiation were compared in the citrate-based CRRT group. Results : Of the twenty-eight Patients, five patients underwent citrate-based CRRT Hemofilter life span was 1.60 $\pm$ 0.72 days, showing no significant differences with the hemofilter life span in the heparin based and LMWH based CRRT group. No patients experienced hemorrhagic complications. PT, aPTT, sodium, t$CO_{2}$, iCa levels showed no difference in pre- and post-CRRT. Total calcium levels were increased. At the recommended postfilter iCa level, j.e., 0.25-0.39 mmol/L, all five patients needed increased amount of citrate infusion, and Ca infusion requirement was decreased. Conclusion : Simplified regional citrate anticoagulation with calcium-containing dialysate is an effective and safe method, and is not associated with increased hemofilter clotting. However, increased postfilter iCa level is recommended.
The purpose of this study was to investigate the effects of a protein-free, standardized dialysate/ultrafiltrate (HD) derivatives from calf blood (Solcoseryl$^{(R)}$) for second-intention full-thickness skin wound healing in dogs. Three $2{\times}2cm$ area-matched full-thickness skin wounds were created bilaterally on the dorsolateral aspect of the trunk of nine dogs. In each dog, two wounds were treated with HD, cemella asiatica extract (Centrasol$^{(R)}$) and normal saline, respectively. For six weeks, the wounds were evaluated grossly for contraction, epithelialization and healing and were examined histopathologically. In the first week of the wound healing period, HD stimulated wound contraction and healing more significantly than centasol and normal saline (p<0.05). Neutrophils were more increased in the HD-treated wounds than those in centasol or normal saline treated wounds. In the second week, HD stimulated epithelialization more significantly than centasol or normal saline (p<0.05), and neovascularization and granulation more increased in the HD-treated wounds than those in centasol and normal saline treated wounds. In conclusion, HD was the most effective on early wound contraction, epithelialization and healings among three experimental drugs in full-thickness skin wound.
In order to elucidate a mechanism responsible for the development of the odor characteristics of cooked, desirable-flavored shellfish, oysters were extracted using various solvents and the resulting extracts were evaluated organoleptically after cooking. The 80% aqueous methanol extract was found to produce a desirable cooked flavor. This oyster extract was fractionated using ion-exchange column chromatography and dialysis, and each of the fractions was subjected to cooking, followed by organoleptic evaluation. The outer dialysate fraction such as acidic and amphoteric water-soluble fractions produced a cooked oyster flavor. The volatile flavor compounds identified from cooked oyster included 29 hydrocarbons, 20 alcohols, 16 acids, 12 aldehydes, nine nitrogen-containing aromatic compounds, eight ketones, five furans, three esters, three phenols, and one benzene.

Pleuroperitoneal communication (PPC) is a rare mechanical complication of peritoneal dialysis (PD), which causes dialysate to move from the peritoneal cavity to the pleural cavity, resulting in pleural effusion. Typically, PPC is discovered through pleural effusion in PD patients who are not in volume overload status. A unique characteristic of the pleural effusion caused by PPC is that it is not resolved by increasing ultrafiltration by dialysis. In this report, we present a 7-year-old girl with PD after birth with the history of various infectious PD-related complications, presenting with fever ongoing for 6 months. PPC-associated pleuritis was suspected as the cause of fever, which eventually developed after long-term PD and induced complicated pleural effusion, lung inflammation, and prolonged fever for 6 months.
