A Study on the Diagnostic Detection Ability of the Artificial Proximal Caries by Digora$\textregistered$ (Digora$\textregistered$ 영상시스템을 이용한 인접면 인공 치아우식병소의 진단능에 관한 연구)

  • Digora system is an intraoral indirect digital radiography system utilizing storage phosphor image plate. It has wide dynamic range which allows it to decrease the patient s exposure time and may increase diagnostic ability through image processing (such as edge enhancement, grey scale conversion, brightness change, and contrast enhancement). And also, it can transmit and storage image information. The purpose of this study was to evaluate the diagnostic ability of artificial proximal caries between Conventional radiograph and Digora images(unenhanced image, brightness & contrast controlled image, and edge enhanced image). ROC(Receiver Operating Characteristic) analysis, paired t-tests, and F-tests were done for the statistical evaluation of detectability. The following results were acquired: 1. In Grade I lesions, the mean ROC areas of Conventional radiograph, Digora unenhanced image, Digora controlled image, and Digora edge enhanced image were 0.953, 0.933, 0.965, 0.978 (p>0.05). 2. In Grade II lesions, the mean ROC areas of Conventional radiograph, Digora unenhanced image, Digora controlled image, and Digora edge enhanced image were 0.969, 0.964, 0.988, 0.994. Among theses areas, there was just statistical significance between Diagnostic abilities of Digora edge enhanced image and Conventional radiograph (p<0.05). 3. In the Interobserver variability, the ROC curve areas of Digora edge enhanced image was lowermost in these areas, regardless of the Carious lesion depths. In conclusion, intraoral indirect digital system, Digora system, has the potential possibility as an alternative of Conventional radiograph in the diagnosis of proximal caries.

Incidental findings of temporomandibular joint osteoarthritis and its variability based on age and sex

  • Purpose: This study investigated the prevalence of temporomandibular joint osteoarthritis (TMJ-OA) using the Research Diagnostic Criteria for Temporomandibular Disorders image analysis criteria, assessed the severity of incidental osteoarthritic changes affecting the TMJ, and evaluated the correlations of sex and age with the prevalence and severity of TMJ-OA. Materials and Methods: This retrospective study assessed 145 randomly selected cone-beam computed tomography scans (261 TMJs) from the authors' institutional maxillofacial radiology archive following the application of inclusion and exclusion criteria. The criteria described by Ahmad et al. were used to determine whether each TMJ was affected by OA, and the severity of the osteoarthritic changes was scored for each joint based on the method described by Alexiou et al. The chi-square, McNemar, Bhapkar chi-square, and Stuart-Maxwell chi-square tests were applied to evaluate the significance of the relationships between variables(age and sex). Results: Sixteen TMJs (6.1%) had no OA, 74 (28.6%) were indeterminate for OA, and 171 (65.5%) had OA. Flattening and sclerosis were observed in 86.6% and 12.3% of cases, respectively, while resorption was observed in 7.3% of the joints. Only 21 (8.1%) of the examined TMJs had subchondral cysts. Erosion of the articular eminence was observed in 58 (22.1%) cases, while sclerosis and resorption were found in 68 (25.9%) and 16 (6.1%) TMJs, respectively. Conclusion: Female patients had a higher prevalence and severity of TMJ-OA than male patients. The prevalence and severity of TMJ-OA increased with age, with peaks in the fifth and seventh decades of life.

  • 1. Objectives Functional medicine is a system which utilizes certain Investigative and treatment methods that are primarily oriented toward the recognition of functional disorder. The 7-zone-diagnostic system(VEGA-DFM722 and ABR-2000, etc) is a diagnostic device which applies pulse signals to predetermined bodily locations. We think that we can discriminate between Soeumin, Soyangin and Taeumin with this system. 2. Methods The subject of our study is no disease men and women who are decided the same constitution both survey of the QSCC II and diagnosis of specialist of the Sasang Constitution. All subject are 76(Soeumin(N=24), Soyangin(N=17), Taeumin(N=35)) cases. We make an analysis of a distinctive feature on the result of the VEGA-DFM722. 3. Results and Conclusions 1) Soeumin or Taeumin women had that the red bar graphs of and 1, 2 and 3 are lower than the red bar graphs of zone 4, 5, 6 and 7 in factor AA on the result of the VEGA-DFM722 with the naked eye. Soyangin or Taeumin men had that the red bar graphs of zone 1,2 and 3 are higher than the red bar graphs of tone 4, 5, 6 and 7 in factor AA on the result of the VEGA-DFM722 with the naked eye. 2) The typical discrimination between Soeumin and Soyangin showed statistical significance(p<0.05) in Factor PF 4(red bar) on the result of VEGA-DFM722.

  • Hepatoscintigram has been a useful diagnostic method for the liver diseases since 1953, but reasonable diagnostic criteria for parenchymal liver diseases are not yet accurately established. For the purpose of searching for more advanced diagnostic criteria for various types of live disease by the liver scan, a retrospective study was made of 272 cases who underwent both hepatoscintigram with 198 Au colloid and liver biopsy in Hanynag University Hospital from Jan., 1978 to Dec., 1981. The results were as follows: 1. Fuzzy margin (irregular indentation of the liver margin) in the hepatoscintigram was noted in 226 cases (97.79%). 2. Of 35 cases with fuzzy margin only, 28 cases (80%) revealed mild parenchymal liver disease, such as acute hepatitis or chronic persistent hepatitis by the liver biopsy. 3. Mottling change (209 cases) was always accompanied by fuzzy margin except only one case, and 31 cases (86.1%) of fuzzy and mottling cases (36 cases) showed mild parenchymal liver disease. 4. Configuration change (193 cases) was usually accompanied with other changes and espicially 104 cases had configuration change with fuzzy and mottling changes. 73 cases (88.44%) of 86 cases with severe configuration change revealed advanced parenchymal liver disease on biopsy. If liver scan showed mild configuration change, we could not decide the type of liver disease only liver scan, and so further studies are needed. 5. Splenic uptake was noted in 34 cases (40.48%) of 84 cases with advanced parenchymal liver disease, and the degree of splenic uptake was for the most part morderate or severe; whereas splenic uptake was noted in 18 cases (16.51%) of the mild parenchymal liver disease (109 cases), and the degree of splenic uptake was largely mild.

Should Korean Medicine doctors use modern diagnostic medical devices? Survey result of public perspectives (국민인식을 기초로 한의사의 현대 진단의료기기 사용 법제화 필요성에 대한 제언 : 설문조사를 중심으로)

  • Objectives: This study investigated public opinion on the use of modern diagnostic medical devices by Korean Medicine doctors. Methods: A questionnaire looking into perceptions on the use of modern medical devices was developed. It was distributed by a third party and data was collected. Results were analyzed through frequency analysis, chi-square analysis (χ2-test), frequency and cross analysis using R program. The measurement variable in the study was the respondent's perspectives and expectations on the use of modern diagnostic medical devices by Korean Medicine doctors. The maximum significance level was 0.05. Results: There were a total of 3,000 responses and 80.8% of the respondents replied that there is a need for government-level discussion on endowing rights to Korean Medicine doctors as supervisors for safety management of radiation generating devices. Also, 83.3% of the respondents agreed the use of ultrasound imaging equipment in Korean Medicine clinics should be legalized. Conclusions: According to this study, respondents strongly support the use of modern diagnostic medical devices by Korean Medicine doctors. This is the first study to investigate public opinion in this area and it provides a significant insight into the public needs and desires for a wider scope of practice for Korean Medicine in the healthcare system.

Multivariate Analysis of Predictive Factors for the Severity in Stable Patients with Severe Injury Mechanism (중증 손상 기전의 안정된 환자에서 중증도 예측 인자들에 대한 다변량 분석)

  • Purpose: For determining the prognosis of critically injured patients, transporting patients to medical facilities capable of providing proper assessment and management, running rapid assessment and making rapid decisions, and providing aggressive resuscitation is vital. Considering the high mortality and morbidity rates in critically injured patients, various studies have been conducted in efforts to reduce those rates. However, studies related to diagnostic factors for predicting severity in critically injured patients are still lacking. Furthermore, patients showing stable vital signs and alert mental status, who are injured via a severe trauma mechanism, may be at a risk of not receiving rapid assessment and management. Thus, this study investigates diagnostic factors, including physical examination and laboratory results, that may help predict severity in trauma patients injured via a severe trauma mechanism, but showing stable vital signs. Methods: From March 2010 to December 2011, all trauma patients who fit into a diagnostic category that activated a major trauma team in CHA Bundang Medical Center were analyzed retrospectively. The retrospective analysis was based on prospective medical records completed at the time of arrival in the emergency department and on sequential laboratory test results. PASW statistics 18(SPSS Inc., Chicago, IL, USA) was used for the statistical analysis. Patients with relatively stable vital signs and alert mental status were selected based on a revised trauma score of more than 7 points. The final diagnosis of major trauma was made based on an injury severity score of greater than 16 points. Diagnostic variables include systolic blood pressure and respiratory rate, glasgow coma scale, initial result from focused abdominal sonography for trauma, and laboratory results from blood tests and urine analyses. To confirm the true significance of the measured values, we applied the Kolmogorov-Smirnov one sample test and the Shapiro-Wilk test. When significance was confirmed, the Student's t-test was used for comparison; when significance was not confirmed, the Mann-Whitney u-test was used. The results of focused abdominal sonography for trauma (FAST) and factors of urine analysis were analyzed using the Chi-square test or Fisher's exact test. Variables with statistical significance were selected as prognostics factors, and they were analyzed using a multivariate logistics regression model. Results: A total of 269 patients activated the major trauma team. Excluding 91 patients who scored a revised trauma score of less than 7 points, 178 patients were subdivided by injury severity score to determine the final major trauma patients. Twenty-one(21) patients from 106 major trauma patients and 9 patients from 72 minor trauma patients were also excluded due to missing medical records or untested blood and urine analysis. The investigated variables with p-values less than 0.05 include the glasgow coma scale, respiratory rate, white blood cell count (WBC), serum AST and ALT, serum creatinine, blood in spot urine, and protein in spot urine. These variables could, thus, be prognostic factors in major trauma patients. A multivariate logistics regression analysis on those 8 variables showed the respiratory rate (p=0.034), WBC (p=0.005) and blood in spot urine (p=0.041) to be independent prognostic factors for predicting the clinical course of major trauma patients. Conclusion: In trauma patients injured via a severe trauma mechanism, but showing stable vital signs and alert mental status, the respiratory rate, WBC count and blood in the urine can be used as predictable factors for severity. Using those laboratory results, rapid assessment of major trauma patients may shorten the time to diagnosis and the time for management.

Clinical Significance of Creatine Kinase MB mass and Cardiac Troponin I as a Marker of Perioperative Myocardial Infarction After Coronary Artery Bypass Grafting (관상동맥 우회술 후 심근경색의 표지자로서 Creatine Kinase MB 농도와 Cardiac Troponon I의 임상적 의의)

  • Background: A perioperative myocardial infarction(PMI) is one of the major complications after CABG. Among diagnostic methods of PMI, CK-MB activity assays have been increasingly replaced by CK-MB mass assays, which have more sensitive, simple measurement. Also, new cardiac-specific and -sensitive marker, cardiac troponin I(cTnl), has been shown to be a marker of myocardial infarction. We report our evaluation of clinical significance of CK-MB mass and cTnl as a marker of PMI after CABG. Material and Method: We studied 32 patients who underwent CABG at Kangdong Sacred Hospital between April 2000 and April 2001. Postoperative serum CK-MB activity level, serum CK-MB mass, cTnl, electrocardiogram, echocardiogram, and clinical data were recorded prospectively The diagnosis of PMI was defined as positive 2 among 3 or all of the following , by a new Q wave on the electrocardiogram, by serum CK-MB activity higher than 200 lU/L within 72 hours after operation, and by new regional wall motion abnormality on the echocardiogram. Result: After CABG, 3 patients had sustained a PMI according to current diagnostic criteria. As serum CK-MB activity time course, a level of CK-MB activity 12 hours after CABG had very linear correlated significance with serum CK-MB mass 24hours(R=0.946) and cTnl 48 hours(R=0.933) after CABG(p=0.000). As we used a receiver operating characteristics curve(ROC curve) for a diagnostic cutoff value in patients with PMI, serum CK-MB mass levels higher than 30.05 ug/L 24 hours after CABG detected the presence of PMI with an area under the ROC curve of 1.0, a sensitivity of 100%, a specificity of 100%, a positive predictive value of 100%, and a negative predictive value of 100%. Also serum cTnl levels higher than 17.15 ug/L 48 hours after CABG detected the presence of PMI with an area under the ROC curve of 0.98, a sensitivity of 100%, a specificity of 96.6%, a positive preclictive value of 75%, and a negative predictive value of 100% Conclusion: We concluded that both the measurement of CK-MB mass and cTnl are the easier, accurate methods as a diagnostic marker of PMT after CABG, also as a proposal of diagnostic cutoff value enables to an early detection of PMI. However, a 1arger number of patient will be needed because of statistic limitation that a small number of participating patients, a small number of PMI.

Study of Sasang Constitution Classification Method Based on Elasticity Coefficient of Hand Doral Skin of Elderly People (고령자의 손등피부 탄성 특성을 활용한 체질분류 방법 연구)

  • This paper presents a novel diagnostic method to determine Sasang Consitution (SC) by elasticity coefficient of hand dorsal skin of elderly people. For this purpose, we carried out a clinical test, in which Our SC diagnostic tool and one Oriental medical doctor with SC medicine expertise participated to categorize 458 healthy subjects into four constitutions. After excluding heavy-noise, we extracted 295 subjects' elasticity coefficient feature data and statistically analyzed their SC relevance with ANOVA test. It resulted in that, Taeum-in has high elasticity coefficient compared to Soeum-in in a significance level of 0.05. There are no differences of elasticity coefficient in genders. This result is supported by the theory of Longevity and Life Preservation in Eastern Medicine(Donguisusebowon, 동의수세보원) which suggested that Taeum-in's flesh is solid and Soeum-in's one is tender. This is the first work which reports the relatedness of skin elasticity coefficient and SC by an objective clinical test.

A Fault Diagnostic Method for Position Sensor of Switched Reluctance Wind Generator

  • Fast and accurate fault diagnosis of the position sensor is of great significance to ensure the reliability as well as sensor fault tolerant operation of the Switched Reluctance Wind Generator (SRWG). This paper presents a fault diagnostic scheme for a SRWG based on the residual between the estimated rotor position and the actual output of the position sensor. Extreme Learning Machine (ELM), which could build a nonlinear mapping among flux linkage, current and rotor position, is utilized to design an assembled estimator for the rotor position detection. The data for building the ELM based assembled position estimator is derived from the magnetization curves which are obtained from Finite Element Analysis (FEA) of an SRWG with the structure of 8 stator poles and 6 rotor poles. The effectiveness and accuracy of the proposed fault diagnosis method are verified by simulation at various operating conditions. The results provide a feasible theoretical and technical basis for the effective condition monitoring and predictive maintenance of SRWG.

A Case Report of migraine and a case report of restless legs syndrome treated with Osuyu-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 오수유탕(吳茱萸湯)을 투여한 편두통 증례 및 하지불안증후군 증례 각 1례)

  • Objective : This study was intended to evaluate if treatment with Osuyu-Tang following the disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS) worked analyzing the progress of patients with migraine and restless legs syndrome and to reinterpret the provision of 309 Soyinbing Osuyu-Tang based on the analysis of whether it worked. Methods : Two cases treated with Soyinbing Osuyu-Tang were analyzed using DPIDS. The migraine and restless legs syndrome in both cases were evaluated using the migraine disability assessment (MIDAS) and the Korean versions of the international restless legs scale (K-IRLS), respectively. Results : In the first case, during 45 days of treatment, the MIDAS decreased from 10 days to 3 days. In the second case, during 30 days of treatment, the K-IRLS decreased from 30 to 9. In both cases, Soyinbing was associated with the disease, although the activity was less; and Leng (冷) was associated with numbness in the kneeling position. Conclusions : It was confirmed that treatment with Osuyu-Tang was effective against migraine and restless legs syndrome and that Leng could be associated with numbness in the kneeling position. The etymological analysis of the Shanghanlun characters suggested the possibility of further clarifying the clinical significance and interpretation of Shanghanlun.
