Effects of Internet Diabetic Education on Glycosylated Hemoglobin and Self-Care in Type 2 Diabetic Patients over One Year (1년 간 인터넷 당뇨교육이 당뇨환자의 당화혈색소와 자가 관리 개선에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effect of web-based diabetic education on glycosylated hemoglobin(HbAlc) and self-care in people with type 2 diabetes. Method: Twenty-five patients were requested to input the blood glucose level weekly at http://www.biodang.com by cellular phone or wired Internet for one year. The researcher sent optimal recommendations weekly to each patient using the short message service for cellular phones and wired Internet. Results: Patients had a significant mean decrease in HbAlc, from 8.0% at pre-test to 6.9%, after three months and rested at 6.8% until 12 months. Patients had a significant mean increase in physical exercise adherence, from 3.3 days at pre-test to 4.1 days per week, after three months. Patients had a significant mean increase in foot care adherence, from 4.8 days at pre-test to 6.9 days per week, after one year. Conclusion: These findings indicate that Internet education could be used to improve HbAlc, physical exercise, and foot care adherence in type 2 diabetic patients.

Creatinine and microalbuminuria levels are increased in type 2 diabetic patients with hypertension (고혈압을 동반한 제2형 당뇨병 환자에서의 creatinine과 미세 단백뇨 증가)

  • The purpose of this study was to identify clinical characteristics of type 2 diabetic patients with hypertension. The subjects were 209 type 2 diabetic patients who visited at the endocrine center at Kangnam St. Mary's Hospital of Catholic University in Seoul from beginning of March through the end of April in 2001. The patient's clinical laboratory data were assessed at medical record review. The data were analyzed using for t-test, $x^2$ test. The results were as follows: 1) There were no significant differences in age, body mass index, sex, family history of diabetes and oral hypoglycemic agents between hypertensive group and normotensive group, However, percentage of patients receiving insulin treatment was higher significantly in the hypertensive group. 2) Creatinine and microalbuminuria levels were higher significantly in the hypertensive group. However, fasting blood glucose levels were lower significantly in the hypertensive group. There were no significant differences in $HbA_1c$, 2-hour postprandial blood glucose, total cholesterol, triglyceride, high density lipoprotein cholesterol, lipoprotein(a) and blood urea nitrogen between two groups. Our present study supports that Creatinine and microalbuminuria levels were higher significantly in the hypertensive group.

A Study on the Related Factors and the Change after Foot-reflexo-massage Education Program for Diabetic Patients (당뇨병 환자의 발반사요법 교육프로그램 후 발관리지식, 자가간호행위, 생리적지표의 변화 및 관련요인분석)

  • Purpose: The purpose of this study was to identify the change in foot care knowledge, self care behavior, and physiologic indexes after foot reflexomassage education program, and the related factors. Method: Data were collected from 20 patients who were visited out patients clinic. The change and difference were analyzed with non-parametric statistics. Result: There were significant differences in foot care knowledge(P=.001), self care behavior(P=.000), dosalis pedis blood flow volume (P=.011), skin temperature of foot(P=.001), dorsalis pedis pulse(P=.000), capillary filling time of foot (P=.000) between pre and post. The level of changes of foot care knowledge was significant differences according to admission experience (P=.049), and negative related to systolic blood pressure(P=.028). The level of changes of self care behavior was related to age(P=.049), that of dosalis pedis blood flow was significant difference according to smoking(P=.042), that of skin temperatureof foot(P=.002) and dosalis pedis pulse(P=.038) were significant difference according to weight. The level of changes of capillary filling time of foot was related to diagnosis period(P=.014). Conclusion: Foot-Reflexo-Massage education program is an effective nursing intervention to promote foot care in diabetic patients. And the related factors can be recommended for the management of diabetic patients.

A Study on Effects of Short-term Nutritinoal Education on Diabetic Pediatric Patient (당뇨 환아들의 단기 영양교육의 효과에 관한 연구)

  • The purpose of this study was to investigate the effect of knowledge and practice of diet therapy on diabetic control in Insulin-dependent Diabetics. The subjects were 31 patients (male 15, female 16) aged 7 to 16 years old who participated pediatric health camp. The average of diabetic of diabetic duration was4.2 years. 48.4 % of subjects has the motive to diabetics diagnosis because of 3-plentiful symptom in both group. The average of height was 151.5 cm for male and 141.4cm for female, the average weight was 43.1 kg for male and 38.6kg for female. In PIBW, underweight was 53.3 % for male, 37.5% for female, normal weight was 40.0% for male, 56.3% for female. The fasting blood glucose was 173.5mg/dl males, 202.9mg/dl for females, Postprandial blood glucose was 138.8mg/dl for males, 194.3mg/dl for females. Energy intakes were 2,100 -2.600 kcal for males and 1,800-2,600kcal for females. Energy intake of the subjects was higher than that of RDA. Carbohydrate, protein and fat ratio was 58 : 17: 25. The mean score of knowledge before education on diet therapy was 17.07$\pm$2.02 for males, 15.19$\pm$3.58 for females. The mean score of knowledge after education on diet therapy was 17.93$\pm$1.33 for males , 17.31 $\pm$1.49 for females.

  • Inevitable immobilization after surgery on lower extremities can induce chronic constipation. Elderly diabetic women usually express ambiguous gastrointestinal symptoms and signs. We present here a case of panperitonitis developed from severe fecal impaction in an elderly diabetic woman after hand reconstruction using material harvested from the lower extremities. A 68-year-old diabetic female underwent anterolateral thigh free flap and wound revision twice on the left thumb. Three weeks after surgery, she complained about mild abdominal pain though she had daily defecation. Despite encouraging ambulation, her compliance was low. Resection of the sigmoid colon and colostomy were performed after diagnosis with bowel perforation. However, the patient went into septic shock and died with multiorgan failure after the guardians issued a DNR (do not resuscitate) order. For preventing bowel perforation, increased uptake of dietary fiber and early ambulation postoperatively should be encouraged, after even hand surgeries.

  • The current SARS-CoV-2 coronavirus disease 2019 (COVID-19) pandemic has been a particular challenge for diabetes patients. Since these patients are at a higher risk of COVID-19, they have been prioritized for vaccination. In this report, we describe the case of a patient scheduled for diabetic foot amputation who received the first dose of ChAdOx1 nCov-19 vaccine and subsequently developed severe purpura in his genitalia and both of his hands and feet, accompanied by acute renal failure. The operation had to be postponed as severe limb purpura appeared just before the operation. With adequate management for acute renal failure and topical steroid application for the severe purpura lesions, a successful outcome could be obtained after the delayed first ray amputation. We recommend that COVID-19 vaccination should be carefully administered in patients with a diabetic foot requiring amputation.

  • We herein present the unique case of a 68-year-old male diabetic patient who developed sterile necrosis of the sternum 1 month after myocardial revascularization with the use of bilateral internal thoracic artery grafts. The sternum had been closed by the bilateral Robicsek wiring technique. The sternum was removed, and bilateral pectoralis major flaps were used to cover the defect. The patient had an uneventful recovery.

  • In this paper, we have developed a Retina Healthcare Service System through which the patient himself/herself can manage his/her retina health. In the case of conventional portable ophthalmic cameras, patients cannot check their eye health on their own because most of them are used by doctor in environments where ophthalmography cannot be performed properly. This system consists of web, app and camera modules, and when a patient mounts a camera module for fundus photography on his / her smart phone and then photographs his / her fundus through the app, the image is transmitted to a server, and the transmitted image reads the fundus the patient's fundus image status in the fundus image reading model learned using deep learning. When the doctor expresses his/her opinions about the patient 's eye condition based on the reading result and the fundus photograph, the patient can check through the app and judge whether to receive ophthalmologic treatment.

Effect of Critical Pathway on Diabetes Mellitus (당뇨병 표준진료지침 적용 효과에 관한 연구)

  • Background : The aim of this study was to examine the effectiveness of the critical pathway to diabetic patients who were admitted in the hospital. Methods : For establishment of critical pathway for diabetic patients, we organized the multidisciplinary care team. During 5 months, 31 diabetic inpatients were applied the critical pathway and the results were compared with 11 diabetic inpatients who were treated with conventional way. We assessed the patients satisfaction, knowledge on the disease, compliance of medical regimen, length of stay(LOS) and hospital cost. We used the computer program SAS for statistical analysis. Data were summarized with mean, and analyzed using t-test repeated measures ANOVA and Wilcoxon rank sum test. Results : Length of stay(LOS) was remarkably shortened in critical pathway group compared with the conventional treatment group($7.6{\pm}1.23$ vs $12.0{\pm}4.73$, p<0.000). Although LOS was significantly shortened in critical pathway group, patients satisfaction was much higher than conventional treatment group. There were no significant difference of knowledge on the disease, compliance of medical regimen and blood glucose level between two groups at discharge. Hospital cost of each patients also significantly decreased in critical pathway group(890,000 won vs 1,280,000 won, p<0.05). Conclusion : These results showed that establishment of critical pathway for diabetic inpatients with team approach is the one of the way to improve the quality of diabetic patients management and to enhance the efficiency of hospital management.

  • Evaluation of clinical usefulness of current perception threshold test and vibration sense perception threshold test in diagnosing the diabetic poly-neuropathy patients is one of the diagnosis methods for diabetic poly-neuropathy. Up to the present, some diagnostic methods were used for diabetic poly neuropathy patients. For example, there are neuropathy impairment score test of lower limbs, nerve conduction test, cooling detection threshold test, heat-pain threshold test and so on. However, most of the above tests require very expensive cost and take a lot of time in test. In this paper, a new apparatus estimating vibration sense ability is introduced. For this purpose, the VCM(voice coil motor) stimulating patient's peripheral nerve and current amplifier were manufactured. Also, softwares sensing and driving the vibration detection threshold test in order to measure the quantitative vibration sensory levels in diabetic poly-neuropathy patients were developed.
