무지 피부 결손에 신경 분리-문합을 이용한 이종 수지 신경혈관 도서형 유경 피판술의 결과 (Results of Thumb-pulp Loss Reconstruction with a Heterodigital Neurovascular Island Flap Using a Nerve Disconnecting-Reconnecting Technique)

  • Purpose : The problems of late deterioration of sensation and a double sensibility phenomenon are a known deficiency of the original neurovascular island flap. To eliminate this problems, the authors was performed to assess the results of Thumb-pulp loss reconstruction with a heterodigital neurovascular island Flap Using a Nerve Disconnecting-Reconnecting Technique Material and Methods : Between July. 1987 and November. 2000, we have used a disconnecting-reconnecting technique for heterodigital island flaps in which divided nerve of the transferred island flap is sutured to original nerve of the thumb in 9 patients. 9 patients with a mean follow-up period of 3.6 years were examined, while 7 patients were follow-up for 2years or more. Evaluation of results was based on flap quality, donor site quality, classic two-point sensory discrimination(2PD), and location of sensation(double sensibility), donor site cold intolerance, neuroma formation, scar contractures, total active movement(TAM) of the thumb and donor digit and subjective results and objective results. Results : The two-point discrimination was average 6.7 mm. Donor site paresthesia was observed more frequently in a short postoperative period. Sensation was referred to the thumb in all cases and the recovery occurred over a period of a two months. There were no flap complication or failures and the length of the thumb tip was preserved in all cases. Conclusion : This technique was found to provide well-vascularized pulp of the thumb with proper cortical representation in one operative stage.

퍼지 추론법을 적용한 OFDM 시스템의 LS(Least Square) 채널추정 기법 (Least Square Channel Estimation Scheme of OFDM System using Fuzzy Inference Method)

  • 본 논문에서는 최근 여러 분야에서 불확실성에 대한 예측을 위해 사용되는 Fuzzy 추론법을 OFDM(Othgonal Frequency Division Multiplexing)의 채널추정 방식에 적용함으로써 향상된 성능과 낮은 복잡도를 갖는 새로운 채널추정 방식을 제안하였다. 제안된 방식은 LS(Least Square) 채널추정 이전에 Pilot을 이용하여 Fuzzy추론법에 의하여 채널의 통계적 특성을 계산하고 이에 대한 보간을 해 줌으로써 채널추정 성능을 향상시키는 방식이다. QPSK(Quadrature Phase Shift Keying)를 적용한 시뮬레이션 결과 제안된 채널추정 방식은 MSE(Mean Square Error)가 $10^{-3}$인 지점에서 MMSE(Mimimum Mean Square Error) 채널추정 방식보다는 약 1.3 dB 정도 성능이 열화되는 것으로 나타났지만 LS 채널 추정 방식과 비교하면 약 5.5 dB 정도의 성능 이득이 있는 것으로 분석되었으며, SER(Symbol Error Rate)은 SNR(Signal to Noise Ratio)이 20 dB인 지점에서 MMSE 채널추정 방식과 제안된 채널추정방식이 각각 $10^{-1.96}$, $10^{-1.93}$ 정도로 유사한 성능을 보이는 것으로 분석되었고, LS 채널추정 방식 보다 약 $10^{-0.35}$ 정도 제안된 방식의 성능이 향상된 것으로 분석되었다.

근근막 발통점의 활성도에 따른 초음파 영상구조 및 전기생리학적 특성 (Ultrasound Imaging and Electrophysiological Characteristic According to Activity Levels of Myofascial Trigger Point)

  • Purpose : This study is to offer clinical primary data that examines the change of imaging structure and the quantitative evaluation of muscle activity on myofascial trigger points. This study examines neuromuscular physiological characteristic by comparing the differences in physical findings, pressure pain threshold, imaging, and electrophysiological characteristics in latent and active myofascial trigger points muscle and normal muscle through the following experimental procedures. Methods : The participants for the study were thirty-three adults in their twenties. We divided three groups into normal, latent and active myofascial trigger points groups by physical findings. We analyzed the results of measured pressure pain, threshold for pain, ultrasound imaging perform for structure characteristic of muscle, surface EMG according to type of muscle contraction for function of muscle contraction. Results : Significant differences were indicated in pressure pain threshold (p<0.05). Significant differences were discovered in the ultrasound imaging analysis. There were increases in muscle Echogenicity white area index (p<0.001). There were significant differences that decrease in %MVIC (p<0.05), increase in MDF (p<0.05). Conclusion : From these results, active rnyotascial trigger points muscle showed quality deterioration on ultrasound imaging and decreased function of muscle contraction, increased motor unit action potential of II type fiber, and electrophysiologically. Imaging structure and neuromuscular physiological characteristic can be diagnostic and quantitative analytical techniques for myofascial pain syndrome and a primary factor that reflected in physical therapy intervention.

요추 운동효과 향상을 위한 그래픽 기반 유도점 표시 시스템 설계 (Graphic-based guidance point display system design for obtaining and improving lumbar exercise effect)

  • 컴퓨터 활용을 통한 문화도 기존의 문화 형태를 변화시킬 만큼 다양하고 빠르게 변화되고 있다. 앉아서 생활하는 시간이 증가하면서 인간은 요통의 질병에 더 많이 노출 되었으며, 요통은 허리디스크를 중심으로 허리와 관련된 통증을 포함한다. 요통의 대부분은 척추와 몸통을 지지해주는 근육과 인대 이상에 따른 원인이 크다. 이에 허리 요통을 포함한 건강한 허리를 유지하기 위한 효과적인 대책이 필요하다. 본 논문에서는 요통을 감소하거나 치료에 도움을 줄 수 있도록 요추 운동효과를 얻을 수 있는 앱 개발을 위한 전반적인 설계를 제시한다. 이를 효과적으로 사용하기 위해서 요추 운동효과를 얻기 위한 방안을 제시하고, 이를 그래픽기반 유도점을 활용하여 효과적인 요통 치료에 도움일 될 수 있는 앱 설계 제시를 목표로 한다.

영상처리기법을 이용한 다중 변위응답 측정 알고리즘의 검증 (Verification of Multi-point Displacement Response Measurement Algorithm Using Image Processing Technique)

  • 최근 토목, 건축 구조물의 유지관리 기술에 대한 관심이 커지고 있으며 구조물의 성능저하 및 노후화 등으로 구조적 안전성의 검토가 요구되는 구조물의 수가 급증하고 있는 실정이다. 그리고 구조물의 노후화 및 부재의 균열 등으로 인하여 강성이 저하되면 구조물의 동특성에 변화가 나타나게 되며 구조물의 실제 거동상태에서 동특성을 분석하여 손상부위와 손상정도를 정확히 판단하는 것은 중요한 문제이다. 구조물 모니터링에 사용되는 대표적 계측장비가 동적계측기이다. 기존의 동적계측기는 측정 센서와 장비를 연결하는 케이블 길이가 길어질 경우 신뢰할 수 있는 데이터를 얻기 힘들고 각 센서와 계측기를 1:1로 연결하는 방식을 취하고 있어 비경제적이다. 따라서 센서를 부착하지 않고 원거리에서 진동을 측정하는 방법이 필요하다. 구조물의 진동을 계측하기 위하여 적용 가능한 비접촉식 방법으로는 레이저의 도플러효과, GPS를 이용하는 방법 및 영상처리기법 등이 대표적이다. 레이저의 도플러효과를 이용하는 방법은 정확도가 상대적으로 높지만 비경제적이며, GPS를 이용하는 방법은 장비가 고가이고 신호 자체의 오차와 데이터 취득속도의 제약이 있는 단점이 있다. 그러나 영상신호를 이용하는 방법은 간편하고 경제적이며 접근이 어려운 구조물의 진동 및 동특성 추출에 적합하다. 기존에도 센서를 대신하여 카메라의 영상신호를 이용하는 연구가 수행되기도 하였으나, 기존의 방법은 구조물에 부착된 표적의 한 지점을 기록한 후 영상처리기법을 이용하여 진동을 측정하는 방법으로서 측정 대상이 비교적 국한적일 수 있다. 그러므로 본 연구에서는 영상처리기법을 이용하여 구조물의 다중 변위응답을 측정할 수 있는 방법의 타당성을 검증하기 위하여 진동대 실험 및 현장재하실험을 수행하였다.

이중차이분석을 활용한 노인장기요양보험제도의 방문간호서비스 효과 (The Effects of Visiting Nursing Services in Long-term Care Insurance: A Difference-in-Difference Analysis)

  • Purpose: The purpose of this study was to demonstrate the effects of visiting nursing services in long-term care (LTC) insurance on changes in health status and physical functions among the elderly. Methods: We analyzed survey data on the living status and welfare needs of the elderly from 2008 and 2011 using the difference-in-difference method with propensity score matching. Results: The subjects were 76 elders including 38 visiting nursing service users(intervention group) and 38 home-based LTC service users (control group). Results from DID analysis confirmed that the subjective health status level of the intervention group was 2.2 points higher (p=.044) and the depression level was 0.3 point lower (p=.039) than the control group. In addition, the intervention group's ADL score was 1.9 (p=.027) and IADL 3.9 (p=.030), showing that their health was deteriorated less. Conclusion: Visiting nursing service in LTC insurance was associated with delayed deterioration of subjective health status, depression, ADL and IADL. These findings suggest the need of rebuilding visiting nursing service programs focusing on prevention services, which will be more contributive to elderly health care and the reduction of social costs.

Torsional behaviour of reinforced concrete beams retrofitted with aramid fiber

  • Retrofitting is an alteration of existing member or component of the structure. In civil engineering point of view, it is called strengthening of the old structure. Deterioration of structures may be due to aging, corrosion, failure of joints, earthquake forces, increase in service loads, etc. Such structures need urgent repair, retrofitting and strengthening to avoid collapse, cracking and loss in strength or deflection. Advanced techniques are required to be developed for the repair of structural components to replace conventional techniques. This paper focuses exclusively on torsional behaviour of Reinforced Concrete (RC) beams and retrofitted RC beams wrapped with aramid fiber. Beams were retrofitted with aramid fiber by full wrapping and in the form of 150 mm wide strips at a spacing of 100 mm, 150 mm, 200 mm respectively using epoxy resin and hardener. A total 15 numbers of RC beams of 150 mm×300 mm×1300 mm in size were cast, 3 beams are tested as control specimens, and 12 beams are tested for torsion up to the failure and then retrofitted with aramid fiber. Experimental results are validated with the help of data obtained by finite element analysis using ANSYS. The full wrapping configuration of aramid fiber regains 105% strength after retrofitting. With the increase in spacing of fabric material, torsional strength reduces to 82% with about 45% saving in material.

Characteristics of the Economic Repair Time of the Components in Public Rental Housing

  • Building has required the repair money to improve or maintain the decent living condition continuously after construction. It needs to grasp the building deterioration to decide the scope and contents before it is repaired. Under various conditions such as physical, social and financial constraints, the repair plan would be prepared. Among constraints, the cost is indispensible to specify the repair time, repair scope and target. The required cost would be planned to preparation over the years. In this paper, it aimed at providing the repair strategy of the public rental housing in repair time, using the cumulative cost model which is $3^{rd}$ function. In the $3^{rd}$ function, the inflection point should exist in the line. And there are two types in the cumulative model, First, if the maximum cost be shown, the repair time would be provided. Second, if the maximum cost not be shown, the cumulative function should be proportionally increased and the repair strategy is properly provided with a short cycle. In results of this study, 11 items would provide the repair time. These cumulative function would be repaired about 4 years after constructed, and after about 4 years, the cumulative function would be continuously increased.

Estimation of Thickness of Concrete Structures using the Impact Echo Method and Ultrasonic Pulse Velocity Method

  • The structure must be periodically checked and measures must be taken to prevent deterioration in building construction. From this point of view, a nondestructive test is essential to estimate whether the construction of buildings is proper, and whether the dimension of depositing concrete is consistent and without damage. This study estimated the thickness of the concrete component of construction framework using the ultrasonic velocity method and the impact echo method, in order to investigate reliability of the estimation of the thickness of normal strength concrete and high strength concrete, leading to the following conclusions. In the estimation of the thickness of the concrete structures, specimens of normal strength of 24MPa and specimens of high strength of 40MPa demonstrated an average error rate of 5.1% and 2.2%, respectively. The impact-echo method, one of the non-destructive tests, is verified as an efficient diagnostic technique. With this information, we will determine specific standards for the maintenance of structures, and the re-creation of lost building blueprints.

예조건화된 Navier-Stokes 방정식에서의 풍상차분법의 수치특성 (Numerical Characteristics of Upwind Schemes for Preconditioned Navier-Stokes Equations)

  • Numerical characteristics of implicit upwind schemes, such as upwind ADI, line Gauss-Seidel (LGS) and point Gauss-Seidel (LU) algorithms, for Navier-Stokes equations have been investigated. Time-derivative preconditioning method was applied for efficient convergence at low Mach/Reynolds number regime as well as at large grid aspect ratios. All the algorithms were expressed in approximate factorization form and von Neumann stability analysis was performed to identify stability characteristics of the above algorithms in the presence of high grid aspect ratios. Stability analysis showed that for high aspect ratio computations, the ADI and LGS algorithms showed efficient damping effect up to moderate aspect ratio if we adopt viscous preconditioning based on min-CFL/max-VNN time-step definition. The LU algorithm, on the other hand, showed serious deterioration in stability characteristics as the grid aspect ratio increases. Computations for several practical applications also verified these results.
